Endodontics has begun to take advantage of the scientific improvements touching other aspects of dental care. Precision instrumentation allows the practitioner to develop better instrumented canals, making obturation easier and more predictable. Treatment predictability from precision-based instrumentation has allowed a leap forward in how we provide endodontic therapy.

Traditionally, endodontic treat- ment requires instrumentation of the root canal to remove infectious debris, leaving clean canal walls. After the instrumenta- tion and irrigation of debris from the can- al is complete, the root canal is filled with gutta-percha inserted with an endodontic sealer. These sealers to be used with gutta percha have included calcium hydroxides, zinc-oxide and eugenol cements, and glass ionomer cements. When Resilon™ (Pen- tron® Clinical Technologies, LLC, Walling- ford, CT) was introduced 4 years ago, much concern was expressed for this obturation material, ranging from how would it geo- metrically fill canals compared to gutta- percha to whether it could provide a seal equal to zinc oxide/eugenol (ZOE) or AH- 26 (epoxy resin root canal sealer). The technique was used in the treatment of a failed implant-supported fixed bridge that was unable to be reinserted due to fractured components and a lack of passive fit. This technique may also be used for the treatment of a failing natural dentition as well as an implant prosthesis.

Perio treatment has evolved beyond simple scaling with hand instruments. Ultrasonics and diode lasers have improved both the efficiency of treatment as well as treatment prognosis to arresting the disease process and gaining clinical attachment and decreasing pocket depth. Add to this the benefits of adjunct medicaments both at time of treatment via site placement and during routine home care by the patient, and we are able to tip treatment outcome in a more favorable direction. Periodontology has been closely linked to systemic health both as a causative agent to health issues and as a secondary site for some medical conditions. Dentistry has truly--and finally--become a part of total healthcare.

La instrumentación por sí sola no prepara al sistema de canales para la obturación, y la desinfección es la clave para aumentar y optimizar el proceso de obturación. Pero, ¿a qué nos referimos cuando mencionamos la desin-fección del sistema de canales? La desinfección comprende la eliminación del tejido residual en el sistema de canales y las bacterias asociadas a través de lavado del sistema de canal con una solución de irrigación. La clave es eliminar la mayor cantidad de tejido residual como sea posible, y entre más exhaustivo sea el proceso de irrigación, más bajo es el nivel de bacterias restantes. Establishing nontension primary wound closure of various soft-tissue flaps is paramount for optimal postsurgical wound healing. Surgical procedures that require clinical flap manipulation such as those used with traditional periodontal therapy, periodontal plastic cosmetic surgery, hard- and soft-tissue regeneration, and the excision of pathologic tissue also require excellence in execution and thorough understanding of the various techniques of surgery, suturing, and the materials currently available to ensure the desired clinical results. This article will discuss the rationale of specific suturing techniques and suture materials to aid the clinician in obtaining optimal wound closure.

Fiber obturators have been introduced into endodontics but few reports exist regarding their efficacy compared to standard obturation materials. This study evaluated the leakage resistance of fiber obturators compared with other obturation materials. All specimens showed a progressive increase in leakage with time. The data indicate that fiber obturation and a new cone/sealer system provide the best canal obturation as evidenced by the leakage behavior.
