REMARKS
History of Don Jose NHS
Don Jose National High School was first established in 1972 through Barangay Captain
Herminio Villar with the help of late Pililia B. San Agustin, Elementary Principal. Its first operation
had only 28 first year students who came from the nearby barangay mostly out of school youths
who’s utilizing one of its rooms with Mrs. Simplicia P. Silva as the first teacher.
In 1974, first and second year used the Industrial Arts building of the elementary. It was in
1976 when this school held its first graduation rite with 32 graduate students.
In 1979, The Don Jose was headed by assistant Principal Asuncion C. Cendaña, followed by
Mr. Celso Ramos in the year 1985.
In 1988, a significant increase in population motivated Don Jesus Miguel Yulo to donate a
land approximately 1,035 square meters in size. It was also the time when this school got its name
Don Jose Barangay High School with Mrs. Luz A. Catindig as Teacher-In-Charge.
It was nationalized and had its name Don Jose National High School in 1992.
In 2010, the high school was administered by Mrs. Dionisia l. Alvarez and in 2011 Miss Amor C.
Constantino replaced her. Then Mrs. Edwinda P. Talavera became the Head of the school from
2012-2017.
At present, Don Jose National High School have 59 personnel headed by Dr. Alvin D. Sta.
Maria, EdD. including (49) nationally funded teachers , ADAS (1) and four (4) utility and (5)
security guards.
Curriculum
The K-12 Basic Education Program is the
working guide of the school implemented last
School Year 2012 – 2013 with four (4) years
in Junior High School and two (2) years in
Senior High School.

Every quarter, the Advisers give
recognition to the students that shows
great performance in all subject areas. At
the end of the school year, the students will
get recognition for showing excellent
performance in academic, behavior and
attendance. Special awards are also
given.
Selection of Honors
GOLD – the student will be given Gold medal if she/he got a
general average of 98-100 % (Highest Honors)
SILVER - the student will be given Silver medal if she/he got a
general average of 95-97 %(High Honors)
BRONZE - the student will be given Bronze0 medal if she/he
got a general average of 90-94 %(with Honors)
Note: Honor students may come from any section per grade
level.
Code of Discipline
1. School Uniform
-Boys should wear white polo patch on the left
pocket, worn with plain white undershirt,
black long pants of any material except
denims and plain black leather shoes with
white socks
- Girls should wear blouse with necktie with
patch on left collar, checkered skirt and plain
black shoes with plain white socks.
- P.E. uniform should consists of white/violet jogging
pants, white/gray shirts and rubber shoes, it should wear
on scheduled day and/ or any school function as may be
prescribed.
2. Grooming
-Boys and girls should observe proper grooming.
-Boys should maintain 2x3 clean-cut, no fancy hair
style, length not touching collar of polo shirt nor
ears, while bald haircut are not permitted
- No make-up, long nails, or artificial colored hair and
nails
3. Jewelry/Accessories
- Wearing of jewelry is generally discouraged.
-Tattoos, body paints, anklets, and
fashionable accessories are not permitted
-No earrings for boys
-No dangling, long, big, colored earrings for
girls
3. Identification (ID)
- Student ID is worn and displayed at all times in the
school beginning entrance, and also during outside
school activities and function.
-Lost ID is reported to the Principal’s
Office/Adviser/Guidance Office by the student
with excuse letter from parent/guardian.
Replacement of the lost ID will be paid by the
parent.
4. Bell/Ring Signals
- The regular rings in the school and their meanings:
• FIRST BELL (Students proceed to the line area)
• SECOND BELL ( Students stop talking and maintain
the silence)
• THIRD BELL ( Start of Flag Ceremony)
1.Morning Ceremony/ Assembly
1. Everyday the bell ring at 6:45 for the assembly
2. Students must proceed to their respective area for the
flag ceremony , prayer and announcement of
reminders.

2. Tardiness/Absences
- Tardiness means arrival after the first bell/ring which is 6:45
everyday.
- Late / absent students who present excuse slip signed by the
parents/ not later than the following day whether or not
tardiness/absences is considered EXCUSE: the slip is presented
to the concerned subject teachers for initial and submitted to the
class advisers.
- Medical certificate is to be attached to the excuse slip if absence
is due to sickness of three days or more.
- Students suffering from sore eyes, chicken pox and similar
contagious diseases should stay home and will not go to school
until the duration of ailment as advised by the doctor.
- A student must not incur total absences of 40 days, or
more than 20% of the required school days, whichever is
higher.
- Tardiness/absence during school activities on Saturday
or Sunday as prescribed are considered regular school
days.
- Absent student should inquire from classmates about
the lessons and assignments taken up/given during
absence.
- For excused absences student must see his/her
subject teachers for the missed lesson and
quizzes/examination
- The class secretary records tardiness and absences
- Utilize excuse slip
Cutting Classes/Skipping school
A student is considered to have cut a class / classes if
she/ he:
a. Is known to be on campus but is not in the classroom
during a class period.
b. Is not in the classroom ten (10) minutes after the
schedule time, unless have important matter with the
teachers or administration.
The number of times a student has cut classes shall be
counted and reflected in his attendance record.
If the student is habitually doing the act , disciplinary
measures may be given.
3. Holidays / Suspension of Classes
- No classes on announced non-working holidays and
signal no. 2 typhoons for the province of Laguna.
(Executive Order No.66 s,2012)
Section 1.“When signal No. 2 is raised by PAGASA, classes at the pre-
school, elementary and secondary levels in the affected area shall
AUTOMATICALLY CANCELLED or SUSPENDED.”
Section 2. “Classes in all levels in both public and private schools as well as
work in the gov’t offices may be cancelled or suspended…..”
“The concerned LDRRMO headed by the local chief executive shall be
responsible for announcing the suspension of classes or work in gov’t
offices…”
7. Holidays / Suspension of Classes
- Discretionary on the part of the parent/ guardian to
allow or not the child to go to school when the rain
or flood water is getting heavy.

SCHOOL DISCIPLINE POLICY
The disciplinary goal of DJNHS is for each student to
achieve the ability to govern themselves from within,
based on the example and model of a good law
abiding citizen.
As the student’s ability to govern themselves in this
way increases, there is less need for imposing rules and
regulations upon them.
Each student will be disciplined as needed
according to individual needs. A student must learn to
submit to authority. If a student learns to submit to
teachers, parents, and others in authority, then he will
learn to submit to God’s authority in his own life.
Each teacher is given the responsibility of enforcing
classroom and school rules. The teacher will maintain
order in the classroom by speaking to children about the
policy violation and deciding what consequences are
appropriate for the child and circumstance in
accordance to the discipline policy. The administrator will
be available to assist as needed.
Discipline and order are attained through a proper
balance of positive and negative consequences.
Teachers will use a variety of methods to promote good
behavior and aid in character training. The teacher will
use positive responses for appropriate behavior.
General Rules and Regulations
A. Student Conduct
Students are expected to conduct themselves in a manner that is
honoring to God, themselves, their parents, and the school. This
expectation applies to all student conduct regardless of whether
the students are in school or at school activities as well as the time
they are away from school. Expectations for general conduct that
have been adopted are:
1. Behave properly and abide by the rules, regulations, and policies
of the school.
2. Stand for the customary greetings of teachers and visitors
entering the classroom, be cordial to everyone
3. Stay inside the classroom at all times during class hours except
when class/school activities are held
STUDENT CONDUCT...
4. Students are allowed to eat snacks or lunch just make sure
to dispose their trash properly
5.Observe silence inside the classroom and along hallways, no
yelling, shouting or using of violent , indecent words
6.Keep personal belongings like notebooks, books and bags
orderly and safely
7.Carry personal valuables such as money, cell phone at all
times, valuables are to his/her own personal responsibility
8.Using of cell phone during classes is PROHIBITED
9.Maintain an orderly and clean classroom; put litters inside
the garbage bin and follow the school program on proper
garbage disposal
10.Do not vandalize walls, tables, chairs and other furniture
and fixture. Parents will pay the replacement of the
vandalized items.
11.Use with care the textbooks, school facilities and
equipment, replace items lost or destroyed if proven
liable, cover books with plastic
12.Switch off electric fans and lights when last to leave the
classroom during the day or in the afternoon, at the end
of classes before going home
13.Report to the class adviser any destroyed or damaged
items in the classroom that need repair or replacement
14.Do not leave the campus anymore once entered the
school gate for the day, unless taken by the
parent/guardian
15.Do not enter the faculty rooms or the school offices without
permission from the teachers or personnel who are inside the
room.
16.Do not bring , use any pornographic materials, do not bring
prohibited drugs, liquors, cigarettes, fire crackers, or deadly
weapons
17.Do not bring pets, skateboards, video cam, play stations, radios
or similar devices, unless to be used in class activities and
permitted by the subject teacher; or else such item will be
confiscated and can be claimed by the parent/guardian
personally.
18. Do not bring any items for sale inside the campus unless duly
authorized by the Principal/authorities.
Disregard for, or violation of, the Student Conducts and will be
dealt with according to the School Discipline Policy of DJNHS.
Inappropriate behavior includes :
1. Gross Misconduct;
2. Cheating and Stealing;
3. Disrespect a teacher or any school authority or his agent or students;
4. Smoking inside the school premises;
5. Vandalism, writing on or destroying school property like chairs, tables, windows,
book, laboratory equipment and others;
6. Gambling of any sort;
7. Stealing, Extortion or asking money from others;
8. Possession or use of cigarette, alcohol or other controlled substances/prohibited
drugs;
9. Fighting , causing injury to others;
10. Possession of knives, guns or other deadly weapons;
11. Hazing in any form or manner whether inside or outside the school premises;
12. Engaging in fraternities;
13. Any form of harassment or intimidation;
14. Telling a false statement;
15. Leaving school without permission from the teachers and school head;
16. Instigating , leading or participating in concerted activities leading to
stoppage of classes;
17. Mistreatment of other students;
18. Possession of nuisance items and gadgets, such as radios, CD players,
games, personal toys, Skate boards, pets, cellular phones, etc.
19.Immorality/ sexual harassment;
20. Falsification of school documents such as student’s I.D, Form 138 (Report
Card) and Form 137(Transcript of Record).
21. Preventing, threatening students or faculty members or school authorities from
discharging their duties or from attending classes or entering school premises
DISCIPLINARY ACTIONS
Disciplinary actions that may be used includes, but not limited to:
• Warning:
1st Warning - Teacher will talk to student and explain his/her
offense
2nd Warning- The teacher will refer to the Grade Level Key teacher
together with the parents
3rd Warning – The case will be sent to the guidance office. Filling
up the Intake Sheet together with the parents
4th Warning - The case will be sent to the principal’s Office for
counselling. Student conference with the
administrator and parents.
The imposition of suspension and expulsion of student is
based on DECS Service Manual IV Chapter 3 Section 1.
• School officials and teachers shall have the right to
impose appropriate and reasonable disciplinary
measures in case of minor offenses or infractions of good
discipline. However, no cruel or physically harmful
punishment shall be imposed or apply against any
student.
• Suspension for not more than three (3) days or
expulsion is imposed depending on the seriousness of the
offense.
• Expulsion for grave misconduct , possession, usage and
selling of prohibited drugs. The case will be forwarded
to the Higher Authorities
Duties and Responsibilities of Students
Students are expected to contribute to their own learning
success. A student's duty includes:
1. Participate in learning activities
2. Accept increasing responsibility for his or her learning.
3. Do homework regularly.
4. Attend school regularly and on-time.
5. Be part of a safe and positive learning environment.
6. Be responsible for his or her conduct at school and on
the way to and from school.
7. Respect the rights of others
8. Obey all school policies.
9. Preserve and maintain school properties and
facilities.
10. Always lived and practice the four core values set
by the DepEd
11. Uphold good self reputation, family and the school
12.Do not wear the school uniform in personal
gatherings, malls, or places not related to any school
function.
Duties of Students
1. To uphold the mission and vision, ideals and the integrity of the
school to give honor and glory to the school, community, family and
the country and show respect in God
2.To obey and observe all rules , regulations, policies and code of
conduct in the school
3.To give proper time and devotion to studies and achieve the
standard grade requirements of the school
4.To respect and maintain good relationship with peers, teachers and
staff, parents and family members, and the proper authorities in and
outside the school
5.To maintain the orderliness, maintenance of cleanliness, and
upkeep of school properties and facilities
GUIDELINES FOR PARENTS
Parents play a vital role in the education of their child,
whatever child’s age is, (either he read in college or in
school). If parents pay attention to their children, they make
more scholastic achievements than those who are ignored. If
parents take care of following things, they can improve the
education of their children.
We expect the following from you:
1. Daily supervision of your son’s/daughter’s Academic
progress.
2. Be supportive in all activities, programs and projects of
the school to give your son’s/daughter moral support.
3. Regular participation in the meetings, and conferences
set by the school.
4. Discussions regarding some issues on behavior
and progress of the students should be by
appointment set by the adviser or guidance
counselor
5. Designated receiving place for parents/guardians
or visitors is provided at the waiting area.
6. Come on the scheduled date for card
distribution.
7. Parents shall be actively involved in all school
activities or events that raise awareness on
children’s rights, positive discipline, and the
prevention of bullying
