Level 1
• Initially the MHO answers the
self assessment part and invites
the PHO AHDP coordinator to
assess the facility and provide
technical assistance for the
facility to comply with level 1
requirements
• After the RHU is labeled by the
PHO as level 1, PHO and PDOHO
will provide technical assistance
on how to be a level 2 facility
Level 2
• SB should pass a resolution
declaring their RHU as
adolescent friendly and express
what services they are willing to
offer based on AO 2017-0012
• MHO/AHDP Coordinator shall
form policies for the AHFF and
have the written policy approved
by the LCE
Level 2
• For facilities wanting to request
technical assistance from PDOHO
or DOHRO1 :
- Resolution declaring the
intention to be an AHFF and
resolution allowing the LCE to
enter into a MOA with DOHRO1
for ta (50K)
- MOA with program of works
- Cover letter addressed to RD
Level 3
• DOH RO1 with DOH CO will do
the final assessment for level 3
