International Patient Safety Goals


1- Improving the accuracy of patient :‫ حتسني عوليه التعرف علي ادلرضي‬- 1
identification

1. All patients receive an identification card


that lists their names and health record number
2. ensures the proper identification of patients
through two identifiers {names and health record
number. And the patient should not be identified
by the clinic number or diagnosis
3. When a patient is admitted to the
emergency department unconscious and/ or
unable to identify self and unaccompanied by a
relative ,registered as unknown and will be
provided a permanent health record

2- Improving the effectiveness of


‫ حتسني التواصل بني كل هقذهي اخلذهه الطبيه‬- 2
communication among care givers

Implement a process/procedure for taking


verbal or telephone orders, or for the reporting
of critical test results that requires a verification
“read-back” of the complete order or test result
by the person receiving the information.
3- Improving safety in using medication ‫ حتسني سالهه استخذام االدويت‬- 3
 Verbal Orders may be accepted and transcribed 
in an emergency situation for emergency drugs
except Narcotics & Controlled Drugs
 Telephone Orders may be accepted and
transcribed the patient's condition does not
require physician's presence to make a medical 
assessment. Telephone Orders include all drugs
except Narcotic & Controlled Drugs.
 List of prohibited abbreviation is available and
known 
 High alert medications labeled segregated from

other medications
 Multi dose vial policy is known and applied 
 Dispensing
o pharmacist ensures the accuracy of

the physician order
o Minimize confusion between look- alike -
,sound-alike medications list of -
confused drug names
 Identifying and handling of expired medications
as per policy requirement 
 Remove concentrated electrolytes (including,
but not limited to, potassium chloride, potassium 
phosphate, sodium chloride >0.9%) from patient
care units

review by process owners

associated infections :

 Comply with current published and 
generally accepted hand hygiene
guidelines 
 Safe waste disposal
 Safely manage needle stick injury

6- Reducing the risk of patients : ‫ تقليل اخلطوره ادلتسببة هن سقوط ادلرضي‬- 7


harm resulting from falls:

Assess and periodically reassess each


patient’s risk for falling, including the
potential risk associated with the
patient’s medication regimen, and take
action to decrease or eliminate any
identified risks.
