AFFIDAVIT OF UNDERTAKING

I, _____, of legal age, Filipino, married and a resident of ________


Davao City, Philippines, after being duly sworn in accordance with law, hereby
depose and say THAT:

1. That I secured a loan from _____ in the amount of _____ and I


used the Original Owner’s Duplicate copy of Title with TCT No.
____, of which I am the VENDEE, registered in the name of
______, as security thereof; hence, the Original Owner’s
Duplicate copy of the Title is in the possession of ______;

2. That I am currently processing the transfer of the above-


mentioned title in my name;

3. That _____ temporarily turned over to me the possession of the


said title;

4. That therefore I hereby undertake to return the original copy of


Transfer Certificate of Title No. ____ to _______, once the
Transfer Certificate of Title No. ______, which is already
transferred in my name, is already in my possession;

5. That I further hereby undertake to inform _______, once the title


is already released in my name;

6. That I also undertake to return the possession of the Original


Owner’s Duplicate copy of Transfer Certificate of Title No.
_____to _____, in the event that I failed to process the Transfer
of title in my name after a period of 8 months, which shall
commence upon the execution of this affidavit;

7. I am executing this affidavit in order to attest to the truth of the


foregoing facts and for whatever purpose it may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 23 rd


day of June 2022, in Davao City, Philippines.
