AFFIDAVIT OF CONSENT AND SUPPORT

We, Edman T. Tolosa and Zenny S. Tolosa, both of legal age, married, Filipino citizen and
residing at Orchidsville Subdivision Arevalo, Iloilo City respectively, after having been duly sworn
according to law, hereby depose and say:

That we are biological parents exercising parental authority of minor, Zyra Erin Sia Tolosa , born
on October 21, 2012 and residing at Orchidsville Subdivision Arevalo, Iloilo City, with passport number
P0815828A issued on October 29, 2016 and expiring on October 28, 2021, will be leaving on June 6, 2019
and returning on June 10, 2019 and will stay in Bali, Indonesia.

That we are giving full consent and authority for my child to travel to Bali, Indonesia and back to
the Philippines, with her grandmother, Minviluz P. Sia, with passport number P8215378A issued on Aug
04, 2018 and expiring on Aug 03, 2028.

That we shall be financially responsible for all expenses relating to said travel and stay in Bali,
Indonesia.

That we cannot personally travel with our child due to scheduled business meetings with clients.

That we are executing this Affidavit to attest to the truth of the fact stated above for what legal
purpose it may serve.

Done this 30th day of May 2019, Iloilo City, Philippines.
