Nursing Skills Improvement Plan

This document outlines a nursing student's three learning goals: [1] To develop confidence in medication calculations, [2] To improve sterile technique skills during procedures, and [3] To enhance communication skills during patient handoffs. For each goal, the student identifies learning activities and timeframes to achieve the goal by the end of the course. Activities include practicing calculations daily, attending open labs, and observing nurses. The goals support the student's commitment to continuing competency and relate to competencies around safe medication administration, infection prevention, and effective patient reporting. The learning plan helped the student gain confidence in skills and understand the value of outlining steps to accomplish goals.

SILOS NURS 3021

Learning Needs

What learning needs did I identify through practice reflection and peer input? Identify a minimum of two
(2) to three (3) learning needs.

My learning needs are developing more confidence in medication calculation, sterile technique during
trach care, wound care, and catheterization. The last learning goal is

Learning Goal #1

My goal must be based on my learning needs.

My goal is to become confident in medication calculation for injection and weight based equations. I will
know my learning is complete when I feel confident performing these calculations in the lab and clinical
setting. I will be able to obtain this goal through practice, seeking guidance in clinical, labs, and open
labs. This is an important skill for patient safety. This goal will be achieved by the end of this course.

Goal #1: Activities and Timeframes

How am I going to achieve my goal? Identify a minimum of three (3) learning activities with time
frames/timelines.

1. I will practice a minimum of 5 to 10 questions involving medication calculation per day.

2. I will attend 4 open labs to seek clarification on any difficulties. During these labs I will also
demonstrate my abilities to the lab instructors.
3. In the clinical setting I will seek out opportunities to observe other nurses completing these
calculations and medication orders.

Learning Goal #2

My goal must be based on my learning needs.

I want to become confident in performing sterile techniques during sterile procedures such as
catheterization, trach care, and wound care. I will know this goal is achieved when I complete when I'm
able to perform these skills without error and during the practical exam. I will be able to obtain this skill
through practice in labs and clinical settings. This goal is important for patient safety in preventing
infections. This goal will be achieved by the end of this course.

Goal #2: Activities and Timeframes

How am I going to achieve my goal? Identify a minimum of three (3) learning activities with time
frames/timelines.

1. Attend 4 open labs to practice sterile technique. Have the clinical instructor observe my
technique.
2. Practice at least twice a week at home with roommate observation. My roommate is also in
nursing.
3. Complete the practice quizzes and read through the extra resources provided on blackboard.
Learning Goal #3

My goal must be based on my learning needs.

I want to become better in my communication skills during handoffs. I want to be able to prioritize
information and give the correct information to the nurse taking over my patient. I will know my learning is
complete when I'm able to give an effective report to the nurse taking over. I will be able to obtain this skill
through observation, practice, and seeking clarification. This goal will be achieved by the end of this
course. This goal is important in keeping my patient safe and making sure their needs are met.

Goal #3: Activities and Timeframes

How am I going to achieve my goal? Identify a minimum of three (3) learning activities with time
frames/timelines.

1. In the clinical setting observe other nurses giving a hand off report.
2. Discussing with my clinical instructor on the information that should be included in a handoff
report.
3. Practicing a hand off report in my clinical setting.

How do my learning goals support my commitment to continuing competency?

How does my learning relate to the competencies of my practice?

Having confidence in my medication calculations will help me be able to find the correct doses of
medication to give to patients. This is important in the 6 rights to medication administration and for patient
safety. Being able to maintain a sterile field helps decrease infection which provides correct and safe care
to patients. Giving an effective report to a nurse that is taking over allows for the nurse to know what is
going on with the patient, and what to look out for.

Evaluation of changes/outcomes to my practice

What did I learn?

I learned that I do know my drug calculation more then I thought. I also built more confidence in myself in
completing drug administration. After completing of OCE I feel more confident in performing sterile
technique and has provided me more practice in this skill. I found that having a will informed worksheet on
my patient helps with my handoffs.

What impact has my Learning Plan had on my practice?

The biggest impact this learning plan provided me was the importance of outlining steps to achieve my
goals. These steps gave me small goal to complete which worked towards a large goal.
