KNOW ALL MEN BY THESE PRESENTS:

This Agreement made and entered into this _______________in the City of

MARILYN P. COLITA, of legal age, Filipino, widow and a resident of Zone 4,


Bayabas, Cagayan de Oro City;

KENT IAN P. COLITA, of legal age, Filipino, single and a resident of Zone 4,
Bayabas, Cagayan de Oro City;

KARL LLOYD P. COLITA, of legal age, Filipino, single, and a resident of Zone
4, Bayabas, Cagayan de Oro City;

W I T N E S S E T H:

WHEREAS, the above-named HEIRS are the legitimate, surviving and sole heirs
of the deceased VICTOR C. COLITA who died on October 18, 2021 at
Polymedic Plaza Hospital Cagayan de Oro City , copy of his death certificate is hereto
attached as Annex A;

WHEREAS, said deceased died intestate, without any Last Will or Testament
and without any outstanding debts in favor of any person or entity;

WHEREAS, the deceased, at the time of his death, left a savings account
amounting to _____________ at Banco de Oro - Osmena Branch, Cagayan de
Oro City, with the Account No. 003110031242.

WHEREAS, pursuant to Rule 74, Sec. 1 of the Revised Rules of Court of the
Philippines, and being with full capacity to contract, we do hereby adjudicate
unto ourselves the abovementioned personal properties/deposits of money
in equal shares pro indiviso.
