(ALLEGED FALSIFICATION OF ENTRIES IN THE LOGBOOK)

This is anent the Memorandum Order No. 2019-00016, dated 30 September 2019, copy
of which was received by the undersigned on 04 October 2019 requiring the employees to
explain in writing within a period of fifteen (15) days from receipt of the said Memorandum
relative to the alleged falsification of official entries in the logbook, specifically their
attendance on July 31, 2019.

That on the said date, 31 July 2019, at around 8:00 o’clock in the morning of said date,
Melody R, Ombao disembarked from their motorcycle at the front entrance of the hospital
building and her husband Brian R. Ombao the driver proceeded to park their motorcycle at
the back of the hospital building. In order to obviate the possibility of being late for office,
Mrs. Melody R, Ombao filled up the attendance of her husband in the logbook with no
malice aforethought considering that her husband came to office that day.

That the only reason why Melody R, Ombao filled up the attendance sheet of her
husband is for Brian Ombao not to hamper the traffic at the hospital entrance and to obviate
the possibility of being late in his office and no other. There was no criminal intention to
defraud the government of time but to come to office as promptly as he could.

Finally, they promise that the said incident of signing the attendance sheet in proxy
capacity would never happen in the future again. They apologize for whatever
inconvenience this incident may have caused the province.
