A resident of Mwinyi Ward, Kigoma road, within Tabora municipality


(entrepreneur)

That, SAMSON JAMES MASANJA the Defendant is the biological father of the
children subject of this application one “VICTORIA SAMSON MASANJA”.

The defendant is legally liable to maintain the said VICTORIA SAMSON


MASANJA born on 7/11/2012 at Kitete Hospital in Tabora municipality.

That the child is under my custody and care.

The Defendant is legally liable to maintain the child named above for the
reasons of being the biological father of the child.

The Defendant has supported the said child since 22/3/2019 and has made no
contribution towards maintenance.

I the Applicant, request that the Defendant be ordered to make the following
contribution towards maintenance: A 60,000/= monthly contribution of
VICTORIA SAMSON MASANJA.
The first payment shall be made on ........................................... and after that
on or before the 27th day of each succeeding month. All payments should be
made in favour of VICTORIA SAMSON MASANJA

Other contributions includes payment of Tshs 20,000/= per month being


contribution for medical expenses, tuition fees, uniforms, expenses for sport
and cultural activities.
