NO COMPREHENSIVE CAR INSURANCE

I, ANABELLE M. MAGSIPOC, of legal age, Filipino, and with office address at


Unit 2601 Antel Global Corporate Center Building, Doña Julia Vargas Avenue, Ortigas
Center, Pasig City after having sworn to in accordance with law hereby depose and state:

1. That I am the Administrative Officer of Shogun Ships Co., Inc. with office
address at Unit 2601 Antel Global Corporate Center Building, Doña Julia
Vargas Avenue, Ortigas Center, Pasig City.

2. That as such officer, I am in charge, control and in custody over corporate


papers/ matters of the Corporation’s vehicle including its insurances details.

3. That Shogun Ships Co., Inc. is the registered owner of a dark steel mica
colored Toyota Hilux pick up, year model 2013 with Plate No. UOG 781.

4. That as of the present time, the vehicle above described, which our company
owned, is not under any comprehensive vehicle insurance by any insurance
company.

5. That on November 15, 2019, the above-said company car got involve with a
vehicular accident at Roman Highway, Brgy. Lamao Limay, Bataan last with
Police Report, specifically appearing on Limay MPS Blotter Entry No. 2019-
11-0315, Page No. 0460 and dated November 16, 2019 (attached herewith).

6. That because of the vehicular accident, the company’s vehicle sustained


damages and has been agreed by both parties to shoulder all expenses
incurred for the repair of damages by the counterpart’s vehicle insurance.

7. That I hereby further attest to the foregoing as a requirement for insurance


claim.

8. That I am executing this Affidavit to attest to the truth of the foregoing and
for any other purpose this affidavit may serve.

December 2019 at Pasig City.
