Tooth injuries

Many articles have detailed local accidents and complications in dental implant treatment. Comparisons of the data they report are not always easy because different criteria have been followed in the various classifications and there is confusion between the terms accident and complication. The aim of this paper is to propose a classification that considers the timing of the events and makes a distinction between the two terms. Accidents are events that occur during surgery, and complications are any pathologic conditions that appear postoperatively. The proper diagnostic procedures and surgical techniques for their prevention and treatment are also described.

The aim of the present investigation was to assess the prevalence of dental traumatic injuries in children referred to Queen Fabiola Children's Hospital, Free University of Brussels, Belgium. This study was based on the clinical data of 457 traumatized teeth in 214 children. Most injuries involved 2 teeth (51%). Of these, maxillary central incisors were the most affected teeth (89%). The highest frequency of trauma occurred between 2 and 4 years of age. Falls were the most common cause of injury in both girls and boys. The most common type of injury in the primary and permanent teeth was subluxation. Gingival and mucosal laceration was the most common type of soft tissue lesion (61%). Only 42% of children came for dental treatment on the same day that they were injured.

Abstract –  The objective of the present study was to determine the association between attention-deficit/hyperactivity disorder (ADHD) and traumatic dental injuries in a child psychiatric clinical population in Istanbul. A total of 475 consecutive children aged 8–17 years were enrolled in the study. Children and their parents were interviewed about the presence of traumatic dental injury and if a positive response was obtained further questions were inquired regarding the time and cause of the injury, and whether treatment had been sought. Child psychiatric diagnosis was based on Diagnostic and Statistical Manual of Mental Disorders, 4th edition (DSM-IV) criteria. Of the 475 children, 32 reported a traumatic dental injury (6.7%) with statistically significant association with ADHD (χ2 = 26.90, d.f. = 1, P = 0.0001; odds ratio = 17.41, 95% CI: 4.11–73.55). On the Conners Teacher Report Questionnaire, children with dental trauma group differed significantly from the non-ADHD group but not from the ADHD group (F = 17.02, d.f. = 2, P = 0.0001). Among children with ADHD and dental fracture, the most common subtype was combined ADHD (90%). This is the first study describing a significant association between dental fractures and ADHD which contributes to an explanatory model on dental traumas among children. According to us, awareness of this association is essential at child psychiatry and pediatric dentistry settings and we recommend preventive efforts to be directed at the treatment of ADHD.

Abstract The purpose of this retrospective study was to analyze, by age, the prevalence of traumatic injuries to primary and permanent incisors, and their distribution according to type in a group of boys from private schools in Monterrey, Mexico. The sample consisted of 1010 boys aged 3-13 years enrolled in 4 private schools in the city. Garcia-Godoy's classification for traumatic injuries was used. The prevalence of injuries was 28.4%; 72.2% of the 4-year-olds examined presented signs of traumatic dental injuries. Thirty-seven percent of the 11-year-olds examined had evidence of traumatic dental injuries. The most common type of injury in the primary and permanent dentitions was enamel fracture (58.5% and 69.6%, respectively).

Abstract –  A systematic review of the literature was undertaken to evaluate the criteria used for the diagnostic classification of traumatic dental injuries from an epidemiological standpoint. The methodology used was that suggested by the Cochrane Collaboration and the National Health Service. A total of 12 electronic bibliographical databases (BBO, BioMed Central, Blackwell Synergy, Cochrane, DARE, EMBASE, HighWire, LILACS, MEDLINE, PubMed Central, SciELO, SciSearch) and the World Wide Web were searched. There was no attempt to specify the strategy in relation to date, study design, or language. The last search was performed in May 2003. Two reviewers screened each record independently for eligibility by examining titles, abstracts, keywords and using a standardized reference form. Disagreements were resolved through consensus. The final study collection consisted of 164 articles, from 1936 to 2003, and the population sample ranged from 38 to 210 500 patients. 54 distinct classification systems were identified. According to the literature, the most frequently used classification system was that of Andreasen (32%); as regards the type of injury, the uncomplicated crown fracture was the most mentioned lesion (88.5%). Evidence supports the fact that there is no suitable system for establishing the diagnosis of the studied injuries that could be applied to epidemiological surveys.

This article addresses the incidence of specific complications and, where possible, offers a preventive or management strategy. Injuries of the inferior alveolar and lingual nerves are significant issues that are discussed separately in this text. Surgical removal of third molars is often associated with postoperative pain, swelling, and trismus. Factors thought to influence the incidence of complications after third molar removal include age, gender, medical history, oral contraceptives, presence of pericoronitis, poor oral hygiene, smoking, type of impaction, relationship of third molar to the inferior alveolar nerve, surgical time, surgical technique, surgeon experience, use of perioperative antibiotics, use of topical antiseptics, use of intra-socket medications, and anesthetic technique. Complications that are discussed further include alveolar osteitis, postoperative infection, hemorrhage, oro-antral communication, damage to adjacent teeth, displaced teeth, and fractures.

The current diversity of opinions in endodontic diagnosis has been a source of interest and academic debate by clinicians and researchers. Currently, no single pulp testing technique can reliably diagnose all pulpal conditions neither it has been proven to be superior in all aspects. Despite improvements of various aspects of this process, there are no historically dramatic changes, or consensus for pulpal status in health or disease in addition to a lack of relative systematic reviews. In this review, the past, present and future most debated and critically questioned issues of endodontic diagnosis are discussed. The aim of this review is to provide insights in future diagnostic modalities and areas for further study in endodontic practice pertinent to diagnosis.

Maxillofacial and traumatic dentoalveolar injuries can cause pain and inflammation, and reduce function of the mouth, impacting a cat's quality of life. Many traumatically induced feline fractures have been reported to involve the mandible or skull and, in cats with maxillofacial trauma, traumatic dentoalveolar injuries are particularly prevalent. Traumatic dentoalveolar injuries can also often be found in otherwise healthy cats. Some traumatic dentoalveolar injuries require emergency treatment; timely recognition and managment is therefore important for achieving the optimal outcome. Abstract This study analyzed traumatic injuries in the primary and permanent dentition in children treated from 1990 to 1992 at the Children's Dental Traumatology Service in Valparaiso, Chile. A total of 227 records from patients 2 to 21 years old was studied with regard to the cause of injury, location of injury, time elapsed before treatment, history of previous trauma, type of injury, number of injured teeth., and how patients seek treatment. A total of 73 patients had injuries to the primary dentition (PD) and 154 to the permanent dentition (PD) with a total of 357 injured teeth (115 primary, 242 permanent). Ten to 12-year-old children had the highest number of injuries (33%). Falling was the most common cause of injury in both groups (82% pD, 58% PD), followed by striking against objects (13% pD, 19% PD) and bicycle accidents (9% PD). Most injuries in children with primary dentition (68%) occurred at home, while children with permanent dentition had most accidents at school (38%). Most children (61%) sought treatment after 24 h (52% pD, 65% PD), 61 children had suffered previous trauma (36% pD, 23% PD). The most common injuries in primary teeth were luxation (26%), intrusion (21%) and subluxation (18%). Uncomplicated crown fracture (34%), followed by complicated crown fracture (21%), were the main injuries in permanent teeth. There was no difference in the number of teeth involved for either dentition. A single tooth injury was found in 54%. Two teeth were involved in 35%, and three or more teeth in 11%. Tooth developmental disturbances were seen in 10 children with 14 permanent teeth involved, due to traumatic injuries that were unattended in the primary dentition.

The purpose of this study was to evaluate the awareness of sports as risk factor of dental injuries, the emergency management when a tooth avulsion occurs and the compliance about mouthguards. Two hundred children and youngsters 8- to 15-year-old (147 boys and 53 girls) attending Sports Societies in Isernia, a town in Southern Italy, participated to the investigation. The sports involved were soccer, martial arts, tennis, swimming, volleyball, basketball and cycling. The questionnaire was structured into three parts: 1) questions about age, sex, type and time of sports practice; 2) questions about dental injuries, particularly personal experience, awareness of first aid and procedure about tooth avulsion; 3) questions about knowledge and use of mouthguards. Objectives: This study investigated the prevalence of traumatic dental injury (TDI) and associated factors in the permanent incisors of Brazilian schoolchildren.Methods: A cross-sectional survey was carried out with 1,612 male and female children aged 11 to 14 attending public and private elementary schools in Belo Horizonte, Brazil. A multistage sampling technique was adopted to select the children. Oral examinations were performed by calibrated examiners for the diagnosis of TDI (criteria proposed by Andreasen) and dental caries [Decayed, Missing and Filled Teeth Index (DMFT)]. The Social Vulnerability Index was used for socioeconomic classification. Data analysis involved descriptive statistics and the Poisson regression model.Results: The prevalence of TDI was 17.1%. Falls (43.6%) were the most common cause of TDI, mainly at home (41.8%). Boys were more affected than girls. There was no statistically significant association between TDI and socioeconomic status. The adjusted results revealed that TDI was significantly associated with DMFT [1.11, 95% confidence interval (CI): 1.06 to 1.16] and overjet (1.15, 95% CI: 1.00 to 1.31).Conclusions: TDI was associated with dental caries and overjet and was not influenced by socioeconomic status.
