Chronic Venous Disease

Aim: Chronic venous disease (CVD) is very common in nurses. Noticeably, operating room (OR) nurses are predicted to have a major prevalence of varicose veins. We investigated whether the prevalence of CVD in OR nurses was more than non-OR nurses. Methods: Study populations were OR nurses and non-OR nurses at the Faculty of Medicine, Chiang Mai University. Information was compiled by questionnaire. Physical examination was operated by examiners for CVD based on clinical finding using Comprehensive Classification System for Chronic Venous Disorders classification. Results: 222 nurses were included. The prevalence of C0-C2 was notably different between the two groups (P < 0.001). The prevalence of C1 in OR nurses and non-OR nurses was 59.6% and 72.1% while the prevalence of C2 in OR nurses and non-OR nurses was 8.1% and 16.4%, respectively. Nevertheless, the quality of life was not remarkably different between the two groups. Conclusion: The results demonstrated that CVD in non-OR nurses appear to be higher than OR nurses.

AIM:

To explain the mode of using and the obtained results during EVLA procedures and sclerotherapies with support of Visioven®, a laser transillumination instrument.

MATERIALS AND METHODS:

205 patients suffering from Chronic Venous Insufficiency - CEAP-C stage 1-2 enrolled (103 females and 102 males) and divided into two groups. A Group: patients candidate for sclerotherapy; B Group: patients to be subjected to endovascular laser ablation (EVLA). In both groups patients were divided into two sub-groups on the basis of the use of Visioven® or not during the procedure. The analyzed outcome variables were the total number of cannulations necessary to treat a whole limb, and the total procedure time.

RESULTS:

There is a statistically significant difference between the sub-groups in which Visioven® was used and the ones in which the procedure has been performed without using of any tools. Both in the sclerotherapy group than in the EVLA one, there is not a statistically significant difference for the time required to complete the procedures carried out with Visioven® compared with the ones performed without the use of any transillumination.

DISCUSSION:

Complete closure of the vein was highlighted. Total number of cannulations in sclerotherapy and EVLA procedures was reduced, as a consequence of a "smart" and "targeted" treatment achieved with Visioven® CONCLUSION: The VISIOVEN® system leads to have a more rational approach to sclerotherapy or Laser Ablation of teleangectasias and reticular veins as we can immediately verify the effectiveness of the treatment and adapting it to the desired effects.

Chronic venous insufficiency (CVI) is a disorder that affects 5-35% of the United States population. CVI arises in a state of chronic venous hypertension that alters the permeability of the skin's microcirculation. Leakage of protein and other matter from the blood vessels leads to edema and inflammation- induced tissue injury. The end result is fibrotic and edematous skin, with damage to capillaries involved in nutrient supply and gas exchange. The mainstay of allopathic treatment of CVI is compression therapy to control edema and venous hypertension. In Europe, botanicals are commonly added to this as adjunctive therapy for CVI, and have shown great benefit. The botanicals used include Ruscus aculeatus (butcher's broom) root, Aesculus hippocastanum (horse chestnut) seed, Centella asiatica (gotu kola) herb, Vitis vinifera (grape) leaf, and oligomeric proanthocyanidins such as Pycnogenol.®

Duplex ultrasound investigation has become the reference standard in assessing the morphology and haemodynamics of the lower limb veins. The project described in this paper was an initiative of the Union Internationale de Phlébologie (UIP), The aim was to obtain a consensus of international experts on the methodology to be used for assessment of anatomy of superficial and perforating veins in the lower limb by ultrasound imaging. The Society for Vascular Surgery (SVS) and the American Venous Forum (AVF) have developed clinical practice guidelines for the care of patients with varicose veins of the lower limbs and pelvis. The document also includes recommendations on the management of superficial and perforating vein incompetence in patients with associated, more advanced chronic venous diseases (CVDs), including edema, skin changes, or venous ulcers. Recommendations of the Venous Guideline Committee are based on the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) system as strong (GRADE 1) if the benefits clearly outweigh the risks, burden, and costs. AIM:

To demonstrate the importance of preoperative ultrasound haemodynamic mapping of venous system in surgery of varicose veins.

MATERIALS AND METHODS:

Doppler ultrasound evaluation of haemodynamic features of lower limb venous system according to Franceschi's subdivision.

DISCUSSION:

The importance of Doppler mapping in varicose veins surgery shows a similarity with the Plebographic classification of venous system developed by W. Hach. The study also reassesses the role of varicectomy in the surgery of varicose veins.

CONCLUSIONS:

Preoperative Doppler ultrasound mapping is essential to avoid accident and prevent recurrence of varicose veins.

All patients underwent ligation and proximal avulsion of the incompetent tributaries from the great saphenous vein trunk without SFJ disconnection. The purpose of this study was to develop a minimum data set (MDS) registry for the prevention, diagnosis and treatment of chronic venous insufficiency (CVI) of the lower limbs. We designed the instrument in two phases, comprising a literature review and an e-Delphi study to validate the content. We obtained a total of 39 documents that we used to develop a registry with 125 items grouped in 7 categories, as follows: Patient examination, venous disease assessment methods, diagnostic tests to confirm the disease, ulcer assessment, treatments to manage the disease at all its stages, patient quality of life, and patient health education. The instrument content was validated by 25 experts, 88% of whom were primary healthcare and hospital nurses and 84% had more than 10 years' experience in wound care. Using a two-round Delphi approach, we reduced the number of items in the MDS-CVI to 106 items. The categories remained unchanged. We developed an MDS for CVI with seven categories to assist healthcare professionals in the prevention, early detection, and treatment history of CVI. This tool will allow the creation of a registry in the primary care setting to monitor the venous health state of the population.
