1344852 (1) 九、發明說明 【發明所屬之技術領域】 本發明係有關透明質酸與磷脂醯乙醇胺反應生成物之 透明質酸化合物,其水膠及關節治療用材料者。 【先前技術】 軟骨係活體內少數之無血管系組織之一者,不易於原 組織再行建造之。爲抑制基於外傷引起之軟骨缺損、折斷 性骨軟骨炎等局限之軟骨病變之變形性關節症的發症,被 嘗試各種治療方法。 採取由自體軟骨細胞或骨髓細胞之間葉系幹細胞,進 行分化之軟骨細胞於細胞、或培養基材(Scaffold )進行 培養後移植至軟骨缺損部之自體軟骨細胞移植者( Autologous Chondrocytes I mplantation ; ACI )被嘗試之 (N Engl J Me d. 331,889-95 ( 1994) ,J Bone Joint Surg Am. 76,5 79-92 ( 1 994 )及 Artificial Organs. 25, 1 72- 1 79 (2001))。 又,活體外培養自體軟骨細胞時,被積極嘗試更接近 活體內環境之3元培養’做爲培養基材者可使用膠原、褐 藻酸、血纖維蛋白、等被認定於體內爲安全之材料。其中 針對膠原利用Atelocollagen之方法被越智等進行開發, 開始進行臨床試驗(特開200 1 一 293〇81號公報)。 惟,膠原雖爲活體吸收性’卻不易完全去除抗原性, 因此,是否將出現未知之病毒感染等危險性尙無法斷定等 (2) (2)1344852 課題存在。 對於此,透明質酸係形成關節軟骨之細胞外基質構成 成份者,與軟骨之親和性高。更且,透明質酸乃未含源於 動物之原料,以發酵法進行生成者,因此,不同於膠原, 其未知病毒感染等危險性低者。因而,最近於再生醫療中 ’被硏討利用透明質酸之膝軟骨損傷治療。 美國專利第 593 9323號明細書,:L Biomed. Mater. Res. 42,1 72- 8 1 ( 1 998 ) ,J. Biomed. Mater. Res. 46,3 3 7-46 ( 1 999 )及 J. Orthop. Res. 18,773 -780 ( 2000 )中被揭 示苄酯化透明質酸者。又,特開平7 — 9740 1號公報中, 被揭示雙環氧化物交聯透明質酸者。更於美國專利第 4 5 8 2 8 6 5號明細書及美國專利第4605 69 1號明細書中被揭 示二乙烯硯交聯透明質酸者,特開昭60 — 1 3 060 1號公報 中被揭示甲醛交聯透明質酸者,特開昭60 - 1 3 060 1號公 報中被揭示甲醛交聯透明質酸者。其他之醯肼交聯透明質 酸亦爲公知者。 【發明內容】 本發明目的係爲提供一種活體適用性佳,且安全之透 明質酸化合物者。 本發明目的係爲提供一種即使活體內載重部位仍可使 用之附與充份強度水膠之透明質酸化合物者。 本發明更有其他目的爲提供一種不溶於水性媒體之該 透明質酸化合物所成之成型體。 -6 - (3) 1344852 本發明更有其他目的爲提供—種由本發明該透明質酸 化合物所成之關節治療用材料者。 由以下說明,可更瞭解本發明其他目的及優點。 藉由本發明’其本發明該目的及優點係於第一藉由下 式(1 )

-CH,CH,NH (1) 一 a 所不之基,—OH或—〇Na者, R1爲碳數10〜28之烷基或脂烯基者,n爲50〜50,000 之數者,惟,R之1〜100%爲該式(1) - a所示之基者, 所示之透明質酸化合物所達成者。 藉由本發明,其本發明該目的及優點係於第二藉由 本發明該透明質酸化合物所成之水膠所達成者。 藉由本發明,其本發明該目的及優點係於第三藉由 本發明該透明質酸化合物之成型體所達成者。 又,藉由本發明,其本發明該目的及優點係於第四藉 由本發明該透明質酸化合物所成之關節治療用材料所達成 者。 (4) (4)1344852 【實施方式】 〔發明實施之最佳形態〕 本發明透明質酸化合物係該式(1 )所示者。該式(】 )中,R代表式(I) 一 a所示之磷脂醯乙醇胺基者、或、 一(^或一〇1^者。惟’11之1〜1〇〇%務必爲磷脂醯乙醇胺 基者。磷脂醯乙醇胺基爲未達R之1%時,將無法達成本 發明之目的。R爲碳數10〜28者宜,較佳者爲碳數14〜20 之烷基或脂烯基者,η爲50-50,000者宜,較佳者爲300〜 30,000,更佳者爲 1,〇〇〇〜1〇,000。 做爲R碳數10~28之烷基例者如:癸基、十一基、 月桂基、十三基、十四基、十五基、十六基、庚癸基、硬 脂醯、廿十基例者。又,做爲碳數1 0~2 8之脂烯基例者如 :對應所示例之該烷基之具有1〜3個碳-碳不飽和鍵之脂 烯基例如:油烯基例者。 做爲該式(1)所示化合物者以式(1) 一 a中之2個 基RkO—爲油醯基者宜。 該式(1 )所示之透明質酸化合物可藉由如:透明質 酸與磷脂醯乙醇胺相互反應後製造之》 做爲透明質酸者如:由動物組織所萃取者,或以發酵 法所製造者均可利用之。做爲發酵法所使用之菌株者可使 用具有鏈球菌屬之透明質酸產生能之微生物者、鏈球菌、 equi FM — 100 (特開昭63 - 1 233 92號公報)、鏈球菌' equi FM— 300 (特開平2— 234689號公報)爲公知者。利 (5) (5)1344852 用此等變異株後’進行培養’精製者亦可使用之。又,透 明質酸之分子量以約1 X 1 05〜1 X 1 〇7者宜。另外,其中透 明質酸亦包含其鹼金屬鹽’如:鈉、鉀、鋰之鹽者。 更有做爲磷脂醯乙醇胺者可使用由動物組織所萃取者 、或合成所製造之任意者。磷脂醯乙醇胺例者如:二月桂 醯磷脂醯乙醇胺、二肉豆蔻醯磷脂醯乙醇胺、二棕櫚醯磷 脂醯乙醇胺、二硬脂醯磷脂醯乙醇胺、二花生醯磷脂醯乙 醇胺、二山榆基磷脂醯乙醇胺、二巴西棕櫚醯磷脂醯乙醇 胺、二蠟油磷脂醯乙醇胺、二褐媒磷脂醯乙醇胺、二月桂 油醯磷脂醯乙醇胺、二肉豆蔻油醯磷脂醯乙醇胺、二棕櫚 醯磷脂醯乙醇胺、二油醯磷脂醯乙醇胺 '二神經醯磷脂醯 乙醇胺、磷脂醯乙醇胺衍生物(Diximenoyl Phosphatidylethanolamine)、二亞麻醒磷脂醯乙醇胺、磷 脂醯乙醇胺衍生物(Dihiragonoyl
Phosphatidylethanolamine)、二花生酿磷脂醯乙醇胺 '二 廿六碳六醯磷脂醯乙醇胺之例者。其中又以二油醯磷脂醯 乙醇胺其溶解性面較爲理想者。 磷脂醯乙醇胺於活體中爲安全物質者,本發明做爲透 明質酸化合物之透明質酸利用氫結合或疏水性相互作用之 物理交聯等促進交聯。因此,本發明之透明質酸化合物可 藉由此等交聯後形成後述之水膠、不溶性成型體者。 磷脂醯乙醇胺之使用量針對每100當量之透明質酸之 羧基爲1〜100當量者宜。若少於1當量則所生成之透明質 酸化合物未能形成水膠。反之,多於5 0當量則所生成之 (6) (6)1344852 透明質酸化合物之疏水性變高’產生不溶物,而不易形成 水膠,特別是,5 1當量以上之使用量時,則對水性媒體 出現極不溶性者。其中水性媒體係指含有水、生理食鹽水 、緩衝液及醇等有機溶媒之水溶液之意者,又,不溶性係 指某一定期間透明質酸化合物滯留於活體內,隨後,漸漸 分解,最後被吸收於活體內之意者。 相較於注入化合物於動物如:人類膝關節類之載重活 體膝關節部位時不易於200Pa以下維持形狀者,而,本發 明由透明質酸化合物所成之水膠則具有2 0 0 P a以上之高彈 性率,因此適用做爲膝軟骨損傷治療用之材料者。 本發明透明質酸化合物於水性媒體中,出現極不溶性 時’此可呈成型體如:海綿類多孔體、不織布、薄膜等形 狀進行成型之。又,本發明透明質酸化合物即使於水性媒 體呈不溶性仍可於埋入成型體於體內時2〜3週後,藉由體 液進行膨潤後轉換呈凝膠者。 做爲製造成型體之方法者如:凍結乾燥法、乾式製膜 、濕式製膜、凝固紡紗、紡黏法、熔融吹燃法、快速紡紗 法等例。 此等成型體可適用於具一定形狀做爲成型體使用後務 必修復軟骨時之用途者,特別期待高度滯留性之用途,如 •關節治療、術後組織之癒合預防劑或皮膚之保濕劑等者 〇 本發明透明質酸化合物可適用於具有如上述之軟骨修 復能力之關節治療用材料者。 -10- (7) (7)1344852 〔實施例〕 藉由以下之實施例進行本發明更詳細具體之說明。惟 ’本發明未受限於此等實施例者。 以下實施例1 ~4所使用之透明質酸鈉係其源於鏈球菌 屬之平均分子量爲〗,〇〇〇,〇〇〇之透明質酸鈉者,此相當於 n = 3,50〇者。針對其他試藥四氫呋喃,0.1M HC1、0.1M NaOH、1—乙基—3_〔3—(二甲基胺)丙基〕一羧二_ 亞胺(EDC) 、1_羥基苯並三唑(HOBt) 、L —白胺酸 甲基酯氫氯化物爲和光純藥工業(股份)、L— -二油 醯磷脂醯乙醇胺(COATSOME ME— 8181)爲日本油脂( 股份),3%八161〇<:〇丨1”611爲高硏(股份)者使用之。 〔實施例1〕 將 110mg(0.000033mol)(針對每 100當量透明質 酸之羧基爲1〇當量)之L-α -二油醯磷脂醯乙醇胺溶 解於 200 m«之四氫呋喃/水=1/1( v/v)。此溶液中加入 5〇〇mg之透明質酸鈉,再添加0.1 MHC1/0.1M NaOH,調 整 pH 爲 6.8。將 30mg(0.000033mol)之 1-乙基—3 — 〔3—(—甲胺)丙基〕一殘二亞胺(EDC) 、25mg( 0.000033111〇1)之1—羥基苯並三唑(1^〇81)溶於10„1£之 四氫呋喃/水=1 /1之水溶液後,加入反應系,進行整夜攪 拌。攪拌後’進行透析精製,凍結乾燥後,取得目的物。 藉由1HNMR (日本電子JNM — ^:400)進行確定後,確定 -11 - (8) (8)1344852 目的物之生成。 將30mg此凍結乾燥品溶於970mg離子交換水後,調 整濃度3 wt%之水膠。爲測定此水膠之複彈性率及滑動降 伏應力,利用 Rheometer RFIII、SRV(TA Instrument) 於37 °C下進行測定之。其結果示於表1。其中複彈性率係 指彈性體之應力與變形之比定數者,滑動降伏應力係指懸 吊滑動應力時,顯示維持凝膠結構之最大應力。 〔實施例2〕 除使用440mg( 0.00012mol)(針對每100當量透明 質酸之羧基爲40當量)之L—α -二油酿碟脂酿乙醇胺 ,120mg( 0.000132mol)之 1 一乙基一 3_ 〔3 —二甲胺) 丙基〕一羧二亞胺(EDC ) 、1 OOmg ( 0.000 1 32mol )之】 —羧基苯並三唑(HOBt)之外,與實施例1同法進行之 。結果示於表1。 〔比較例1〕 於50mg之透明質酸鈉中加入5 J離子交換水進行攪 拌後’取得水膠。針對物性評定,與實施例1同法進行之 。結果示於表1。 〔比較例2〕 參考用再追加試驗美國專利第4,9 3 7,2 70號明細書。 其詳細內容如下。 -12- (9) 1344852 將400mg之透明質酸鈉溶於40 4水中,藉由0.1M HC1進行調整pH爲4.75。 添力口 153mg(0.80mmol)之 1-乙基一3- 〔3-(二 甲胺)丙基〕-羧二亞胺(EDC) 、182mg(1.0mmol)之 L -白胺酸甲酯氫氯化物,進行攪拌5小時。攪拌後,進 行透析精製,凍結乾燥後取得目的物。與實施例1同法進 行物性評定。其結果示於表1。 〔比較例3〕 除使用A t e 1 〇 c ο 11 a g e η之外,與實施例1同條件下測 定Atelo collagen。其結果示於表1。 表1 \ 材料 複彈性率 (Pa) 滑動降伏應力 (Pa) 實施例1 透明質酸-L- α ·油烯磷 脂醯乙醇胺 (1 0 m ο 1 %) 42 1 1,083 實施例2 透明質酸-L- α -油烯磷 脂醯乙醇胺 (4 0 m 〇 1 %) 902 1,734 比較例1 透明質酸鈉 5 18 比較例2 交聯透明質酸 5 13 5 15 比較例3 Atelocollagen 293 9 19 -13- (10) 1344852 由表1證明’透明質酸—L一 二油醯磷脂醯乙醇 胺相較於透明質酸鈉,Ate】o collagen後,其複彈性率、 降伏滑動應力均較理想者。又,相較於交聯透明質酸,亦 顯示同等或其以上之機械特性。 〔實施例3〕
將100mg透明質酸鈉溶於40J之四氫呋喃1水=1/1 (v/v)水溶液。此水溶液中加入 154mg ( 0.00021mol)
(針對每100當量透明質酸之羧基爲70當量)之L—α 一二油醯磷脂醯乙醇胺,添加0.1M HC1/0.1M NaOH進行 調整 pH 爲 6.8。使 42mg(0.000231mol)之 1—乙基一3 —〔3—(二甲胺)丙基〕—羧二亞胺(EDC) ' 35mg( 0.000231mol)之1_羥基苯並三唑(HOBt)溶於10 之 四氫呋喃/水=1/1 ( V/V )後,加入反應系。此時,添加 0.1 M NaOH使反應系維持於pH 6.8。隨後,進行整夜攪 拌,攪拌之後,進行透析3天後,凍結乾燥後取得目的物 (海綿)。藉由WNMR (曰本電子JNM— α400)進行確 定,確定目的物之生成。 藉由以下方法進行溶解性試驗。將20mg取得之目的 物浸漬於5 之磷酸緩衝生理食鹽水後,針對室溫靜置狀 態下之溶解性,進行試驗4週,以目測進行確定。溶解性 試驗之結果示於表2。 -14 - (11) 1344852 〔實施例4〕 除取223 mg ( 0.0003mol )(針對每100當量透明質 酸之羧基爲100當量)之L— α —二油醯磷脂醯乙醇胺' 60mg ( 0.0003 3mol )之 1—乙基一3— 〔3—(二甲胺)丙 基〕-羧二亞胺(EDC) ,50mg(0.00033mol)之 1—羥 基苯並三唑(HOBt )之外’與實施例1同法取得目的物 (海綿)。溶解性試驗之結果如表2所示。 〔比較例4〕 將2 〇mg之透明質酸鈉浸漬於$ d之磷酸緩衝生理食 鹽水後’針對室溫靜置狀態下之溶解性進行試驗4週,以 目測進行確定。結果示於表2。 表2
\ --- .... ... 一 L-a -二油醯磷脂醯乙 醇胺含有量(當量) 1曰 1週間 2週間 4週間 實施例3 70 〇 〇 〇 〇 實施例4 100 〇 〇 〇 〇 比較例4 〇 X X X X 〇·未溶解:部份溶解,X:完全溶解 U針對每1〇〇當量透明質酸之羧基的量 由表2證明透明質酸—L— α 一二油醯磷脂醯乙醇胺 含量.’ 100當量)相較於透明質酸鈉,其對於水性 -15- (12) (12)1344852 液體較可促使透明質酸之不溶化者。 使用以下實施例5之消毒用乙醇,1 0%中性緩衝甲醛 水溶液,藏紅0溶液爲和光純藥工業(股份)之市販品 ’ Fast Green FCF爲polyscience (股份)之市販品、乙嫌 二胺一 Ν,Ν’ Ν',Ν·~四醋酸、四鈉鹽、四水解物(以 下EDTA )爲同仁化學硏究所(股份)之市販品、戊巴比 妥(以下Nembutal )爲大日本製藥(股份)之市販品、 1%利多卡因爲Astrazrnrka (股份)之市販品、結晶青黴 素G鉀(以下青黴素)爲萬有製藥(股份)之市販品、 優碘爲吉田製藥(股份)之市販品者。又,實施例5所使 用之紐西蘭白色家兔(以下NZW兔)爲雄性者,由日本 SLC (股份)購入後’以計表進行一般飼養至體重爲 3.0〜3.5kg爲止。手術後之週齡爲24〜28週齡。 〔實施例5〕 藉由以下方法進行實施例1所調整之濃度3 wt %之透 明質酸水膠之生物學評定。於一般飼養之NZW兔之耳介 靜脈投入戊巴比妥後,全身麻醉下進行以下手術。進行兩 側後肢膝關節週邊部份之剃毛後,進行乙醇消毒,將利多 卡因分數次局部性進行肌肉內投入。切開膝關節內側,使 膝蓋骨脫臼後露出大腿骨膝蓋溝。由內側側副韌帶5mm 之上部滑車溝部份以手術用鈷頭製作內徑5mm、深度 5mm之圓筒形缺損部,使膝關節軟骨全層缺損之。將上 記取得之水膠埋入其缺損部後’使膝蓋骨歸回原位進行手 -16- (13) (13)1344852 術用縫合肌肉。爲預防感染,於患部滴入青黴素,縫合皮 膚。最後以優碘進行消毒,回復計表進行一般飼養。術後 第8週,進行屠殺摘出缺損部份,於1 〇%中性緩衝甲醛水 溶液中進行浸漬,固定後,供於組織學評定。將固定之組 織進行脫脂,EDTA脫灰後,包埋於石蠟,使缺損部中心 部附近進行薄切呈矢狀面,製成標本。於所製成之標本上 進行藏紅0之染色,針對以下項目藉由分級化進行組織 學之評定。 組織學評定所使用之分級度數係藉由Wakitani S et al‘,J Bone J o i nt S ur g Am · 7 6 , 5 7 9 - 9 2 ( 1 9 9 4 )之變法之 Makino T et al., kobe J Med Sci. 48 : 97- 1 04 ( 2002 )進 行實施。表3顯示組織學評定時所利用之項目及得點。 總計爲1 4點,依項目以3〜5階段進行評定。組織修 復度愈高,亦即,愈接近正常組織之修復,接近14點者 。亦即,項目爲被修復組織之形態(0點〜4點)、基質染 色性(〇點〜3點)、表面狀態(〇點~3點)、軟骨組織之 厚度(〇點〜2點)'非缺損部相互結合度(0〜2點)者, 本方法中愈接近正常組織,其得點愈高者。 圖1顯示術後第8週摘出缺損部位所進行組織學評定 之結果者。術後第8週被修復之軟骨組織幾乎呈玻璃軟骨 樣,呈產生良好基質樣者。又,與正常部之結合亦良好者 ,出現組織連續性。 〔比較例5〕 -17- (14) 1344852 與實施例5同法製作於大腿骨滑車溝之缺損部份後, 埋入Atelocollagen (註冊商標)(I型,(股份)高硏) ’進行整復後,於術後第8週摘出缺損部份進行組織學評 定之結果示於圖1。 相較於實施例5,其基質之染色雖與其他條件未有出 入,惟,表面不平滑、軟骨下骨完全無法再重建。 -18- (15)1344852 表3 針對軟骨欠損之組織學評定 A ·細胞之形態 4以玻璃軟骨構成、正常 3多半以玻璃軟骨修復 1幾乎以非軟骨組織修復 0完全無軟骨組織 B.藉由藏紅0之基質染色性 3與(正常部位比較)同等之 染色性 2稍降低 1極低 〇無染色性 C.表面形態1 3 平滑(>3/4 ) 2 中程度(1/2— 3/4 ) 1凹凸不平(1/4- 1/2 ) 0凹凸嚴重(<1/4) D.軟骨組織之厚度2 2 >2/3 1 1/3 - 2/3 0 <1/3 E.針對接近正常部分之修 復部分的一例 2兩端均呈一體化 1單端一體化 〇兩端均未呈一體化 合計A — E 0-14
-19- 1 針對全體缺損部分之平滑部分比例 2 * 1修復部與正常部位比較後之平均厚度 (16) (16)1344852 由以上結果,證明實施例5與比較例5其基質染色性 所修復之軟骨組織厚度爲相同者’惟’實施例5之表面狀 態與正常組織之組織學性連續性之修復接近正常組織 '整 體顯示良好修復能者。 由此證明,本發明之透明質酸與磷脂醯乙醇胺二油醯 所成之透明質酸化合物相較於比較例(Atelocollagen )其 做爲軟骨治療用材料爲較理想者。 【圖式簡單說明】 圖1係代表實施例5及比較例5中,對比術後8週兔 子膝關節之組織學評定者。
