Abstract

1. Стабильная фармацевтическая композиция, содержащая: ! а. терапевтически эффективное количество кортикостероида, ! b. антиоксидант, ! с. буфер, ! d. поверхностно-активное вещество, ! е. консервант, ароматизирующее средство, подсластитель или их комбинацию, ! f. по меньшей мере, один дополнительный эксципиент, включающий декстрозу в количестве 0-500 мг на мл композиции, и ! g. жидкий носитель, ! где композиция является физически и химически стабильной, и где композиция является подходящей для однократного или многодозового введения. ! 2. Стабильная фармацевтическая композиция по п.1, содержащая: ! а. терапевтически эффективное количество кортикостероида, ! b. эдетат, ! с. цитрат, ! d. полисорбат 80 и ! е. воду. ! 3. Стабильная фармацевтическая композиция по любому из пп.1 или 2, где фармацевтическая композиция предназначена для применения в лечении или предотвращении воспаления пищевода или ассоциированных с ним симптомов. ! 4. Стабильная фармацевтическая композиция по п.3, где воспаление пищевода ассоциировано с эозинофильным эзофагитом или гастроэзофагеальной рефлюксной болезнью (GERD). ! 5. Стабильная фармацевтическая композиция по п.1, где композиция остается, по существу, однородной после хранения в течение, по меньшей мере, одного дня. ! 6. Стабильная фармацевтическая композиция по п.1, где композицией является препарат, состоящий из множества доз. ! 7. Стабильная фармацевтическая композиция по п.1, где кортикостероидом является местно действующий кортикостероид. ! 8. Стабильная фармацевтическая композиция по п.7, где местно действующим кортикостероидом является будесонид, флутиказон, мометазона фуроат, циклесонид, триамци�

Claims (40)

1. Стабильная фармацевтическая композиция, содержащая:
а. терапевтически эффективное количество кортикостероида,
b. антиоксидант,
с. буфер,
d. поверхностно-активное вещество,
е. консервант, ароматизирующее средство, подсластитель или их комбинацию,
f. по меньшей мере, один дополнительный эксципиент, включающий декстрозу в количестве 0-500 мг на мл композиции, и
g. жидкий носитель,
где композиция является физически и химически стабильной, и где композиция является подходящей для однократного или многодозового введения.
2. Стабильная фармацевтическая композиция по п.1, содержащая:
а. терапевтически эффективное количество кортикостероида,
b. эдетат,
с. цитрат,
d. полисорбат 80 и
е. воду.
3. Стабильная фармацевтическая композиция по любому из пп.1 или 2, где фармацевтическая композиция предназначена для применения в лечении или предотвращении воспаления пищевода или ассоциированных с ним симптомов.
4. Стабильная фармацевтическая композиция по п.3, где воспаление пищевода ассоциировано с эозинофильным эзофагитом или гастроэзофагеальной рефлюксной болезнью (GERD).
5. Стабильная фармацевтическая композиция по п.1, где композиция остается, по существу, однородной после хранения в течение, по меньшей мере, одного дня.
6. Стабильная фармацевтическая композиция по п.1, где композицией является препарат, состоящий из множества доз.
7. Стабильная фармацевтическая композиция по п.1, где кортикостероидом является местно действующий кортикостероид.
8. Стабильная фармацевтическая композиция по п.7, где местно действующим кортикостероидом является будесонид, флутиказон, мометазона фуроат, циклесонид, триамцинолон, беклометазон или их фармацевтически приемлемый сложный эфир, или их комбинация.
9. Стабильная фармацевтическая композиция по п.1, где композиция содержит консервант.
10. Стабильная фармацевтическая композиция по п.1, где, по меньшей мере, один дополнительный эксципиент включает один или более мальтодекстрин, гидроксиэтилцеллюлозу (НЕС), гидроксипропилметилцеллюлозу (НРМС), карбоксиметилцеллюлозу (CMC), микрокристаллическую целлюлозу (МСС), карбомер или их комбинации.
11. Стабильная фармацевтическая композиция по п.1, где композиция представляет собой суспензию кортикостероидов, и где композиция, по существу, свободна от частиц, отличных от кортикостероидных частиц.
12. Стабильная фармацевтическая композиция по п.1, где композиция представляет собой суспензию кортикостероидов, и где композиция содержит менее чем 5% мас./мас. нерастворенных частиц.
13. Стабильная фармацевтическая композиция по п.1, где фармацевтическая композиция является неньютоновской текучей средой.

20. Стабильная фармацевтическая композиция по п.18, где, по меньшей мере, 95% частиц кортикостероида являются микрочастицами, имеющими диаметр менее чем приблизительно 10 мкм.
21. Стабильная фармацевтическая композиция по п.1, где кортикостероид содержится в количестве приблизительно от 0,01 мг до 1 мг на мл композиции.

23. Стабильная фармацевтическая композиция по п.1, где фармацевтическая композиция содержит приблизительно от 0,1 мг до 20 мг кортикостероида.
24. Стабильная фармацевтическая композиция по п.1, где, по меньшей мере, один дополнительный эксципиент включает CMC, и где CMC содержится в количестве приблизительно от 1 мг до 30 мг на мл композиции.
25. Стабильная фармацевтическая композиция по п.1, где, по меньшей мере, один дополнительный эксципиент включает МСС, и где МСС содержится в количестве приблизительно от 5 мг до 30 мг на мл композиции.
26. Стабильная фармацевтическая композиция по п.1, где, по меньшей мере, один дополнительный эксципиент включает комбинацию CMC и МСС, где комбинация СМС-МСС содержится в количестве приблизительно от 1 мг до 75 мг на мл композиции, и где массовое отношение смешанных компонентов СМС/МСС составляет приблизительно 11/89.
27. Стабильная фармацевтическая композиция по п.1, где, по меньшей мере, один дополнительный эксципиент включает мальтодекстрин, и где мальтодекстрин содержится в количестве выше чем 8% мас./мас.
28. Стабильная фармацевтическая композиция по п.27, где мальтодекстрин имеет декстрозные эквиваленты (DE) приблизительно от 13 до 18.
29. Стабильная фармацевтическая композиция по п.27, где мальтодекстрин имеет декстрозные эквиваленты (DE) больше чем 5.
30. Стабильная фармацевтическая композиция по п.1, где антиоксидантом является эдетат, и эдетат содержится в количестве приблизительно от 0,05 мг до 25 мг на мл композиции.
31. Стабильная фармацевтическая композиция по п.1, обладающая вязкостью, по меньшей мере, 100 сП при комнатной температуре со скоростью сдвига 15 с-1.
32. Стабильная фармацевтическая композиция по п.1, где стабильная фармацевтическая композиция является химически стабильной, содержащей, по меньшей мере, 90% первоначального количества местно действующего кортикостероида после хранения композиции в течение 3 недель.
33. Стабильная фармацевтическая композиция по п.1, содержащая:
а. будесонид в количестве приблизительно от 0,02 мг до 0,75 мг на мл композиции,
b. эдетат в количестве приблизительно от 0,05 мг до 25 мг на мл композиции,
с. цитрат в количестве приблизительно от 0,1 мг до 30 мг на мл композиции,
d. полисорбат 80 в количестве приблизительно от 0,05 мг до 1 мг на мл композиции,
е. консервант, ароматизирующее средство, подсластитель или их комбинацию,
f. по меньшей мере, один дополнительный эксципиент и
g. водный жидкий носитель.
34. Набор, включающий упаковку для множества доз и множество доз стабильной фармацевтической композиции, где каждая доза фармацевтической композиции является химически и физически стабильной и содержит:
а. терапевтически эффективное количество кортикостероида,
b. антиоксидант,
с. буфер,
d. поверхностно-активное вещество,
е. консервант, ароматизирующее средство, подсластитель или их комбинацию,
f. по меньшей мере, один дополнительный эксципиент, включающий декстрозу в количестве 0-500 мг на мл композиции, и
g. жидкий носитель.
35. Набор по п.34, где набор содержит от 2 до 60 доз фармацевтической композиции.
36. Набор по любому из пп.34 или 35, где набор также включает дозирующее устройство для введения композиции индивидууму.
37. Стабильная пероральная фармацевтическая композиция, содержащая:
а. терапевтически эффективное количество кортикостероида,
b. повышающее вязкость средство,
с. декстрозу в количестве 0-500 мг на мл композиции и
d. жидкий носитель,
где композиция является подходящей для однократного или многодозового введения, и где, по меньшей мере, 10% композиции находится в пищеводе, по меньшей мере, 15 с после перорального введения стабильной пероральной фармацевтической композиции.
38. Стабильная пероральная фармацевтическая композиция для лечения воспаления пищевода или ассоциированных с ним симптомов, содержащая:
а. терапевтически эффективное количество кортикостероида и
b. по меньшей мере, один дополнительный эксципиент,
где композиция представляет собой текучую среду и имеет объем 2-20 мл, где композиция является физически и химически стабильной, и где композиция является подходящей для введения одной или множества доз.
39. Стабильная твердая или полутвердая пероральная фармацевтическая композиция для лечения воспаления пищевода или ассоциированных с ним симптомов, содержащая:
а. терапевтически эффективное количество кортикостероида;
b. необязательный антиоксидант;
с. необязательный буфер;
d. необязательное поверхностно-активное вещество;
е. необязательный консервант;
f. необязательное ароматизирующее средство;
g. необязательный подсластитель;
h. по меньшей мере, один дополнительный эксципиент и
i. необязательный носитель.
40. Пероральная композиция по п.39, где композицию получают в виде таблетки, пилюли, драже, капсулы, мягкой жвачки, крема, пасты, жевательной таблетки, геля, гелевого матрикса, леденцов, шипучей таблетки или пастилки.
