Description
Innretning til innføring av kontrast-
middel i en kunstig tarmutgang.
Oppfinnelsen vedrører en innretning til inn-føring av kontrastmiddel i en kunstig tarmutgang i mage-
veggen ved hjelp av et i tarmeutgangen innført innløpsrør.
For røntgenologisk undersøkelse av tarmen,, spesielt tykktarmen må det innføres en kontrastmiddel i tarmen, som er forholdsvis tyntflytende, for at det også
fyller ut små fordypninger i tarmveggen, for om mulig å
gjøre alle detaljer i tarmveggen synlig på røntgenbildet.
Por innføring av flytende kontrastmiddel i
den naturlige tarmeåpning anvendes innløpsrør, som ved enden kan være omgitt med en" sirkelformet oppblåsbar ballong, såvidt lukkemusleåem åkke er helt suffisient. I området av denne ballong støttes tarmveggen av muskulatoren, således at det for ballongen gies en tilstrekkelig motstand til å tilveiebringe den ønskede tetningsvirkning.
Disse kjente forholdsregler er ikke egnet for innføring av kontrastmidlet i en kunstig tarmutgang (anus praeter), fordi her mangler ringmuskulaturen, hvortil en i tarmeutgangen oppblåsbar anordnet sirkelringformet ballong kunne avstøttes. Dessuten betinger innføring av en slik ballong at det siste avsnitt av tarmen umiddelbart før den kunstige tarmutgang ikke ville være fylt med kontrastmiddel, således at dette området, som muligens er sterkest truet og derfor må undersøkes nøyaktig, forblir utelukket fra den røntgenolo-giske undersøkelse. Dette er også tilfelle når man søker å hindre den utilsiktede uttreden av kontrastmiddel ved en kunstig tarmutgang ved at man fører innløpsrøret lengst mulig inn. Dertil kommer at dette er forbundet med ubehag og smerter for pasienten.
Derfor er det hittil uunngåelig at det i mange tilfeller ved innføring av kontrastmiddel i en kunstig t,armut- gang trer en stor kontrastmiddelmengde ut. Dette faller desto sterkere i vekt, da nettopp pasienter med kunstige tarmutganger må undersøkes spesielt hurtig og grundig røntgenologisk, fordi et tarmcarzinom har nødvendiggjort tarmoperasjonen av anlegg av en kunstig tarmutgang.
Hensikten med oppfinnelsen er å tilveiebringe en innretning til innføring av kontrastmiddel av den overnevnte type, som muliggjør ved en kunstig tarmutgang (anus praeter) sikkert å hindre uttreden av kontrastmiddel ved kolonkontrast-diagnostikk. Derved skal tarmen spesielt også i sitt siste avsnitt være fullstendig og overlagringsfritt fylt med kon-trast middel for å muliggjøre en fullstendig røntgenologisk undersøkelse.
Denne oppgave løses ved at innløpsrøret er omgitt
av en mot mageveggen åpen klokke, som ved sin omkrets ligger an mot mageveggen med en sirkelringformet klebeflate og at det i det indre av klokken er anordnet en i området mellom klebeflaten og innløpsrøret i bukveggen anliggende med overtrykk påkjennbar ringformet blære.
Med klebeflaten holdes innretningen fast til bukveggen i avstand til den kunstige tarmutgang. Mellom inn-løpsrør og tarmutgang finner det ikke sted noen kraftvirkning for oppnåelse av en tetning, således at det hindres en smerte-lig misdannelse av tarmutgangen, som ikke har noen lukkemuskel. Den med overtrykk påkjente ringformede blære legger seg i et forholdsvis bredt ringformet område tettende til bukveggen og hindrer uttreden av kontrastmiddel også ved plutselige bevegelser og/eller muskelspenninger hos pasienten.
I en videreutformning av oppfinnelsestanken er det foreskrevet at innløpsrøret er anordnet i et sentralt rør av klokken og at den ringformede blære i det minste ligger an på innløpsrøret i det nedre området som er naboplassert til tarmutgangen. Derved kan innløpsrøret utføres forskyvbart i forhold til klokken for å bringes i egnet stilling. Inne i det sentrale rør finner det ikke sted noen berøring mellom den ringformede blære og innløpsrøret. Den ringformede blære ligger imidlertid umiddelbart an utenfor tarmutgangen tettende på inn-løpsrøret, således at det uttredende kontrastmiddel også hindres i å strømme langs innløpsrøret.
I en videre utførelse av oppfinnelsestanken er
det foreskrevet at klebeflaten dannes av to konsentriske kanter av klokken, hvorimellom det ligger en sirkelringformet, med undertrykk påkjennbar sugeåpning. Derved kan klokker med den hver gang nødvendige klebekraft fastsuges til bukveggen før den ringformede blære påkjennes med overtrykk. Sugekraften kan derved hver gang velges så høyt at den overstiger den av b;læren frembragte trykkraft tilstrekkelig. Derved er det mulig, hvis nødvendig også å påføre forholdsvis høye trykkrefter med blæren.
Klebeflaten kan også dannes av en dobbeltsidig klebestrimmel, som er fastgjort ved en sirkelringflate på omkretsen av klokken. Derved fremkommer en spesiell enkel ut-førelsesform av klokken, som derfor med forholdsvis små omkost-ninger kan foreskrives for en -bare engangs anvendelse.
Istedenfor kan i en videre utførelsesform av oppfinnelsen klebeflaten også dannes av en omkretsvulst av klokken, idet det til klokken er fastgjort en bukgjord til påpresning av klokken til bukveggen. Også denne utførelsesform er meget enkel i sin oppbygning.
Ved en noe modifisert utførelsesform kan klebevirk-ningen til bukveggen dessuten forbedres ved at klebeflaten er en med konsentriske riller utstyrt ringflate av en på klokken anbragt gummielastisk ring, idet det likeledes er anordnet en bukgjord som imidlertid ved denne utførelsesform bare har å ut-bringe forholdsvis små krefter.
Oppfinnelsen skal forklares nærmere ved hjelp av noen utførelseseksempler av innretninger til innføring av kontrastmiddel i en kunstig tarmutgang. Innretningene er på tegningene hver gang vist forenklet i loddrett snitt. Figur 1 viser en innretning med dobbeltvegget klokke og ringformet sugeåpning. Figur 2 viser en innretning hvor den ringformede sugeåpning er utformet på en nedre hulring av klokken. Figur 3 viser en innretning med en dobbeltsidig klebestrimmel. Figur 4 viser en innretning med en nedre omkretsvulst på klokken og
figur 5 viser en innretning med en gummielastisk ring som har kleberiller.
De på figurene viste innretninger til innføring
av kontrastmidlet i en kunstig tarmutgang 1 har hver et inn-løpsrør 2, som er innført i tarmutgangen 1 i bukveggen 3 og hvorigjennom et tyntflytende kontrastmiddel 4 for kolonkontrast-diagnose innføres.
Innløpsrøret 2 er forskyvbart opptatt i en sentralt rør 5, som ender kort over tarmutgangen 1 i en vulst. Det sentrale rør 5 danner den midtre del av en klokke 6, som likeledes som røret 5 er utført av kunststoff og er forbundet med dette på vanlig måte ved sveising, klebing eller lignende eller kan være utført med dette i et stykke. Klokken 6 er i det vesent-lige halvkuleformet; den kan imidlertid også utføres i annen form, eksempelvis som flat skål.
I det mellom bukveggen 3 og klokken 6 liggende ringformede hulrom, hvorigjennom sentrisk rørene 2 og 5 går, befinner det seg en ringformet blære 7 av gummi, kunststoff eller et lignende material, som gjennom en utadført ledning 8 fylles med et trykkmiddel, eksempelvis luft og kan påkjennes med overtrykk. Blæren 7 ligger med en forholdsvis bred sirkelringflate på bukveggen 3 og berører også innløpsrøret 2 og alle sider i området umiddelbart utenfor tarmutgangen 1. Når klokken 6 fastholdes på bukveggen, hvortil i det følgende forklares forskjellige muligheter og blæren 7 oppblåses, legger den seg tettende til bukveggen og mot innløpsrøret 2, således at kontrastmidlet 4 kan innføres i tarmen uten å tre ut til siden.
Forskjellene på de i figurene 1 til 5 viste ut-førelseseksempler ligger hovedsakelig i den måte som klokken 6 fastholdes på bukveggen 3.
Ved utførelsen ifølge figur 1 har klokken et ytre skall 9- Mellom klokken 6 og dens ytre skall 9 er det dannet et hulrom 10, som ved den nedre kant;-av klokken utvider seg til en sirkelringformet sugeåpning 11. Suges det gjennom en til hulrommet 10 tilknyttet ledning 12 luft ut fra hulrommet 10,
så suger klokken 6 med sin sirkelringformede flate 11 seg fast til bukveggen 3• Ved utførelsen ifølge figur 2 er likeledes den sirkelringformede sugeflate 11 tilstede, imidlertid er den her bare forbundet med et ringformet hulrom 10, hvori det gjennom ledning 12 kan frembringes et undertrykk. Innretningen, eksempelvis pumper til frembringelse av overtrykk i blæren 7 og
undertrykk i hulrommet 10 er av enkelthetshensyn utelatt, fordi det her bare dreier seg om vanlige innretninger.
Ved utførelsen ifølge figur 3 har klokken 6 ved sin nedre kant som dannes ved hjelp av en innadrettet, eventuelt forsterket flens 13) en dobbeltsidig klebestrimmel 14 som be-står av en midtre bærestrimmel 14.1 og hver gang et øvre og nedre klebesjikt eller vedhengningssjikt 1<*>1.2, som er antydet stiplet på tegningen. Når innretningen ifølge figur 3 er be-regnet for flere gangers anvendelse kan klebestrimlene lH fjernes etter hver anvendelse og erstattes med en ny klebestrimmel. Hvis klebekraften av klebestrimmelen 14 ikke eller ikke med sikkerhet er tilstrekkelig, kan det på klokken 6 på haker 15 være fastgjort en bukgjord 16, som i tillegg presser klokken 6 mot bukveggen 3.
Ved utførelseseksemplene ifølge figurene 4 og 5 overtar bukgjorden 16 mest mulig fastlegging av klokken 6 mot bukveggen 3» Ved eksemplet på figur 4 har klokken 6 ved sin nedre kant en ringformet omkretsvulst 17, som presses mot bukveggen 3.
Figur 5 viser en utførelsesform, hvor det ved
den nedre kant av klokken 6 er fastgjort en gummielastisk ring 18, hvis underside som danner klebeflaten er en med konsentriske riller 19 utstyrt ringflate.
Det anvendte material må være mest mulig gjennom-trengelig for røntgenstråler for ikke å forstyrre eller påvirke undersøkelsen.
Fastholding av klokken 6 til bukveggen kan også bevirkes ved at det under huden implanteres en magnetring,
som tiltrekkes av en tilsvarende magnetring ved den nedre kant av klokken 6.
Claims (4)
1. Innretning til innføring av kontrastmidlet i en kunstig tarmutgang i bukveggen ved hjelp av et i tarmutgangen innført innløpsrør, karakterisert ved at inn-løpsrøret (2) er omgitt av en mot bukveggen (3) åpen klokke (6), som ved sin omkrets med en sirkelringformet klebeflate (11, 14, 17, 19) ligger an mot bukveggen og at det i det indre av klokken (6) er anordnet en i området mellom klebeflaten og innløpsrør (2) mot bukveggen liggende, med overtrykk påkjent ringformet blære (7).
2. Innretning ifølge krav 1, karakterisert ved at innløpsrøret (2) er anordnet i et sentralt rør.(5) av klokken (6) og at den ringformede blære (17) i det minste i det nedre område som er naboplassert i tarmutgangen (1) ligger an mot innløpsrøret (2).
3« Innretning ifølge krav 1, karakterisert ved at klebeflaten dannes av to konsentriske kanter av klokken (6), hvorimellom det ligger en sirkelformet, med undertrykk påkjent sugeåpning (11).
4. Innretning ifølge krav 1, karakterisert ved at klebeflaten dannes av en dobbeltsidig klebestrimmel (14), som er fastgjort til en sirkelringflate (13) ved omkretsen av klokken (6).
5- Innretning ifølge krav 1, karakterisert ved at klebeflaten dannes av en omkretsvulst (17) av klokken (6) og at det på klokken er fastgjort en bukgjord (16) til påpresning av klokken mot bukveggen (3)« 6'. Innretning ifølge krav 1, karakterisert ved at klebeflaten er en med konsentriske riller (19) utstyrt ringflate av en på klokken (6) anbragt gummielastisk ring (18) og at det på klokken er fastgjort en bukgjord (16) til påpresning av klokken mot bukveggen (3).
7« Innretning ifølge krav 1, karakterisert ved at det på klebeflaten av klokken (6) er anbragt en magnetring, som sammenvirker med en tilsvarende rundt den kunstige tarmutgang i bukveggen implantert magnetring.
