[발명의 상세한 설명]
본 발명은 제약학적 배합물로부터 약제의 방출을 조절하는 것에 관한 것이다. 현재 천식 및 기관지염에대한 유용한 치료는 일반적으로 효과적이라해도, 빈번한 약제 투여의 필요성 및/또는 불쾌하거나 무기력하게하는 부작용의 가능성에 의해 제한된다. 흡입(iIlhalation)에 의해 기관지 확장제(bronchodilating drugs)를 폐에 직접 적용하는 것은 천식 및 기관지엄 증상에 대해 빠른 경감을 제공한다고 증명되었다. 그러나 이러한 경로에 의해 투여될때 약제의 매우 빠른 전신계(systemic) 흡수는 요구되는 임상적 효과의 비교적 짧은 지속기간을 유발한다. 결과적으로 투여의 이러한 경로는 급성 천식 발작을 막기위한 시도에 있어서 기관지 확장제의 예방적 투여에 대해 적합한 것이 아니었다. 특정 경구 투여 형태가 이 목적에 유용하다. 그러나, 작용부위에 직접 투여되지는 않는다. 그러므로 요구되는 투여량이 매우 다량이고 불쾌한 부작용의 유발도 훨씬 많다.

기관지 확장제가 흡입을 위해 캡슐화되는 바람직한 실시양태에서, 천식, 기관지염 및 기타 호흡기 계통의 질병들이 치료될 수 있다. 투여되는 배합물의 양은 치료되고 있는 특수한 질병 또는 장애(disorder) 및 투여되고 있는 기관지 확장제의 유형에 따라 달라진다. 그러나 일반적으로, 배합물은 하루에 단지 1회 또는 2회 흡입될 수 있다. 따라서, 배합물은 하루에 2회 흡입될 수 있으며, 두번째 흡입에 8-12시간 간격을 두고 1회에 50μg-2mg, 더 바람직하게는 100-500μg을 폐에 운반하는데 충분한 양의 기관지 확장제가 흡입될수 있다. 예컨대, 살부타몰의 복용량은 1회 100μg으로 2회 흡입될 수 있다. 이것은 3-4시간 마다 각각100μg으로 1-2회, 즉 20시간당 최고 8회 흡입시키는 통상의 살부타몰 에어로졸 흡입법에 대해 권장된 투여량과 대조를 이룬다(Martindale, The Extra Pharmacopoeia, 28판, 1982년) 터부탈린의 투여량은 각각 250μg으로 2회 흡입될 수 있다.

(터부탈린 마이크로캡슐의 제조)
0.7mm 분무 노즐이 장치된 부키 190 미니 분무 건조기(브린크만 인스트루먼츠사, 뉴욕주 웨스트버리시)를 사용하여 터부탈린 설페이트 마이크로캡슐을 제조한다. 분무-건조 공정은 하기와 같이 실시된다.
용매로서 염화 메틸렌을 사용하여 1.25중량% 폴리락티드-글리콜리드 공중합체(DL-PLG) 용액을 준비한다. 중합제를 완전히 용해시킨 후에, 터부탈린 설페이트를 DL-PLG 중량과 동일한 양으로 중합체 용액에 첨가한다. 그 후에 터부랄니 설페이트 입자들을 브린크만 폴리트론 군질화기(homogenizer)(브린크만 인스트루먼츠사, 뉴욕주 웨스트버리시)를 사용하여 균질화시킴으르써 현탁시킨다. 균질화가 완결되면 즉시,약제/중합체 혼합물을 약 1m1/분의 유속으로 분무 건조기의 노즐내로 펌핑한다. 분무주입법을 실시하기 위해, 또한 노즐을 통해 질소를 도입시킨다. 질소압은 약 103kPa(15lbs/in2)으로 유지된다. 분무-건조기실의 온도는 약 70℃에서 유지된다. 모든 약제/중합제 혼합물을 가공처리한 후에, 분무 건조기를 서서히 실온으로 식히고 마이크로캡슐 생성물을 수집한다.
[실시예 1]
마이크로캡슐로부터 터부탈린 시험관내 방출에 미치는 소르비탄 트리올레이트의 효과
소르비탄 트리올레이트의 용액(스판 85)을 하기와 같이 제조한다. 113g(4oz) 유리병내로 가스실린더를통해 150-160g의 프레온을 첨가한다. 다음에, 각각의 병에 적절한 양의 스판 85를 첨가한다. 프레온내1% 스판 85를 제조하기 위해, 약 1.5g의 스판 85를 청량 보우트에서 무게를 재고 프레온을 함유한 병내에 붓는다. 보다 희석된 계면활성제 용액을 제조하기 위해 피폣을 통해 스판 85 계면활성제를 첨가하는 것이 필요하다. 스판 85 10방울이 약 150mg인 것으로 측정된다. 그러므로 1방울은 약 15mg이다.

마이크로캡슐을 플라스틱 신틸레이션 바이알 내에서 무게를 재고 각각의 바이알에 개별적으로 서로 다른 계면활성제 용액을 첨가한다. 그것에 따라서 바이알 라벨을 붙이고 각각의 바이알에 교반 막대를 가한다. 견본을 약 4시간동안 실온에서 덮개를 하지 않은채 교반한다. 그 후에 견본을 후드 아래에 놓고 약 30분간 연속적인 공기 흐름에 노출시켜 모든 잔류 프레온을 제거한다. 그 후에 마이크로캡슐/스판 85 혼합물을 병으로부터 모은다.
약 15mg의 터부탈린 설페이트를 함유한 마이크로캡슐의 양을 3배로 해서 무게를 재고 작은 나일론 주머니 내에 넣는다. 이 주머니들은 5μm의 나일론 매쉬(스몰 파트사, 플로리다주 마이아미시)로 이루어지며 3.81cm×3.81cm(1.5in×1.5in)이다. 이 주머니들은 가장자리를 고온 스파튤라 블레이드로 가열 밀봉함으로써 만들어진다. 또한 이 방법에 의해 마이크로캡슐이 주머니내로 밀봉된다.

[실시예 2]
(액체 경구용 터부탈린 배합물)
터부탈린 설페이트의 마이크로캡슐을 참조실시예에 따른 분무 건조에 따라 제조한다. 마이크로캡슐의 입자 크기는 1-10μm이다. 마이크로캡슐을 소르비탄 트리올레이트 계면활성제와 혼합한 후, 오일인 에틸올레이트와 혼합하고, 마지막에, 오일과 마이크로캡슐의 혼답물에 비-이온성 계면활성제 그레모포어 EL를 먼저 첨가한 후 격렬히 혼합하면서 적당량의 물을 첨가하여 물속에서 유화시킨다. 터부탈린 마이크로캡슐 소르비탄 트리올레이트의 중량비율은 약 1:5이다. 하기 양적인 배합을 사용한다.

제조 후 1일,5일 및 14일째에 수성 상층액의 터부탈린 함량을 분석한다. 14일 후에, 이 혼합물의 20ml 견본을 50회전/분의 교반 빈도로, 37℃의 pH 6.8 완충액내에서 표준 USP 용해 시험을 한다. 결과를 제1도와 제2도에 나타내었다.

[실시예 3]
(마이크로캡슐을 함유한 에어로졸의 제조)
빈 알루미늄 통에 적절한 양의 터부탈린 설페이트의 마이크로캡슐을 가하여 실험적인 에어로졸을 제조한다. 참조실시예에 따라 분무-건조시킴으로써 마이크로캡슐을 생산한다. 그들은 D743-055-1로 표시된 뱃치이다. 터부탈린 설페이트의 중심 하중은 26.6중량%이다. 계면활성제의 부재시에 마이크로캡슐로부터 방출된 터부탈린 설페이트의 양은 실시예 1에 기재된 공정에 의해 시험관내에서 측정된다. 5,10 및 30분 후에방출된 터부탈린 설페이트의 중량%는 각각 84.5%,98.6% 및 98.8%이다.

[실시예 4]
(터부탈린 마이크로캡슐을 수반한 생체내 연구)

표 2와 3에서 나타낸 자료는 마이크로캡슐화된 터부탈린을 함유한 에어로졸 1과 2가 sGaw에 미치는 약제의 효과를 연장할 수 있다는 것을 명백하게 나타낸다. 1차 및 2차 실험 결과의 비교는 에어로졸 혼합물의 소르비탄 트리올레이트 성분이 또한 결과에 중요한 영향을 미친다는 것을 나타낸다 에어로졸 1에서 터부탈린에 대한 최초의 반응이 실질적으로 저하되며 반응은 6시간까지 일정하다. 비교로, 연무질 2의 최대 약리학적 효과는 시약을 마이크로캡슐화한 결과 약 1시간까지 연장된다.
반응은 일정하지 않으며 4시간까지는 급격하게 저하된다. 이 데이타은 흡입된 마이크로캡슐 배합물의효과를 계면활성제가 연장시킨다는 것을 확실하게 나타낸다.
[실시예 5]
(계면활성제가 벽 물질에 합입된 마이크로캡슐의 제조)
0.07mm 분무 노즐이 장치된 부키 190 미니 분무-건조기(브린크만 인스트루먼츠사, 뉴욕주 웨스트버리시)를 사용하여 스판 85를 함유한 살부타몰 설페이트 마이크로캡슐을 제조한다. 분무-건조 공정을 하기와같이 수행한다.
용매로서 염화 메틸렌을 사용하여 DL-PLG 용액(0 80중량%)를 제조한다. 중합체를 완전히 용해시킨후에 스판 85를 첨가한다. 10중량%의 계면활성제의 중심 하중을 얻도록 하는 양만큼 스판 85를 첨가한다. 스판 85를 전체 용액에 분산시킨 후에, 살부타롤 설페이트를 첨가한다. 20 1중량%의 원하는 약제의 중심하중을 얻도록 하는 양만큼 살부타몰 설페이트를 첨가한다. 그 후에, 브린크만 폴리트론 균질화기(브린크만인스트루먼츠사, 뉴욕주 웨스트 버러시)로 균질화 시킴으로써 살부타몰 설페이트 입자들을 현탁시킨다. 이 균질화 공정을 실행하기 위해, 균질화기의 탐침을 중합체/약제 혼합물에 담근다. 균질화기를 약 6으로 세팅하여 조작한다. 혼합물을 작동중간에 30초씩 쉬면서 3회에 걸쳐 30초간씩 속력내어 작동시켜 균질화시킨다. 균질화 공정직후에, 중합체/약제 혼합물을 교반 플레이트에 놓고 격렬하게 교반하여 살부타몰 설페이트입자들이 현탁액내에 있도록 한다. 그 후 길슨 미니펄스 2펌프(길슨 메디칼 일렉트로닉스사, 위스큰신주미들톤시)를 사용해 혼합물을 분무-건조기의 노즐내로 펌핑한다. 크기-14비톤 관(콜-파머 인스트루먼트＆ 이큅먼트사, 일리노이주 시카고시)을 사용해 약 1ml/분의 흐름 속도를 얻는다. 작은 조각의 알루미늄호일로 용매병의 입구와 관 주위를 견고하게 감싸서 증발을 통한 염화 에틸렌의 손실을 방지한다.
분무 주입법을 실시하기 위해, 분무-건조기의 노즐을 통해 질소를 주입시킨다. 질소압을 질소 탱크상에서 조절기로 측정하여 약 103kPa(15lb/in2)으로 유지한다. 분무-건조기의 주입실의 온도를 약 50℃로 유지하는 한편, 수집실을 약 40℃에서 유지한다. 도든 혼합물을 가공처리한 후에, 분무-건조기를 서서히 실온으로 냉각시키고 마이크로캡슐 생성물을 수집한다.
[실시예 6]
(계면활성제가 벽 물질에 함입된 마이크로캡슐로부터의 시험관내 방출속도의 측정)
폐에서의 시약의 방출을 도방하기 위해, 단순한 용해/확산 기구를 사용한다. 이 시스템에서, 순수 살부타몰이나 스판 85가 없는, 실시예 5에 따라 제조된 살부타몰-함유 마이크로캡슐의 정확히 무게를 잰 양을 투석관의 밀봉된 부분 안쪽에 놓는다.
그 후에 관부분을 pH 7.4의 완충용액 150ml을 함유하는 비이커내에 현탁시킨다. 완충용액을 일정 속도로 교반한다. 작은 견본들을 규칙적인 간격으로 비이커내 유체로부터 취하여 하기 공정을 사용하여 활성 약제를 분석한다.
노보팍 컬럼과 가이드팍 예비-컬럼을 갖는 2-모듈 및 방출필터업시 230nm 요기에서의 형광 검출기 세트가 설치된 HPLC상에 완충용액의 견본을 직접적으로 주입한다. 사용된 용매는 오르토인산을 사용하여 pH 3으로 조절된 TEA가 항유된 4:96 아세트니트릴/물이며, 흐름 속도를 2.5ml/분이다.
하나는 계면활성제가 있고 다른 하나는 계면활성제가 없는 두 종류의 상이한 마이크로캡슐의 뱃치로부터의 살부타몰의 방출은 하기 표 4에 기재되었다. 분명하게, 마이크로캡슐내에 스판 계면활성제의 존재는 본질적으로 시약의 방출을 연장한다.

Claims (15)

  1. 하기 (ⅰ)과 (ⅱ)로 구성되는 흡입에 적합한 제약학적 배합물: (ⅰ) 약제를 캡슐화하는 생적합성,생분해성 벽을 형성하는 중합체로 필수적으로 이루어진 0.1-10μm의 평균직경을 갖는 마이크로캡슐 및 (ⅱ') 마이크로캡슐과 혼합되거나, 마이크로캡슐의 벽을 형성하는 중합체내에 함입되거나 그것을 피복시키는 지용성 계면활성제.
  2. 제1항에 있어서, 에어로졸 통내에 함유된 흡입용 에어로졸 형태인 배합물.
  3. 제 2 항에 있어서, 에어로졸 통내에 있는 계면활성제의 통로가 0.01-1중량%인 배합물.
  4. 제1항에 있어서, 흡입용 건조 분말형태이며, 계면활성제 1-10중량%가 마이크로캡슐의 벽내에 함입된 배합물.
  5. 하기 (ⅰ')과 (ⅱ')로 구성되는 경구 섭취에 적합한 제약학적 배합물 (ⅰ') 약제를 캡슐화하는 생적합성 벽을 형성하는 중합체로 필수적으로 이루어진 0.1-2.0μm의 평균 직경을 갖는 마이크로캡슐 및 (ⅱ')마이크로캡슐과 혼합되거나, 마이크로캡슐의 벽을 형성하는 중합체내에 함입되거나 그것을 피복시키는 지용성 계면활성제.
  6. 제5항에 있어서, 계면활성제로써 피복되는 마이크로캡슐이, 물과 섞이지 않는 제약학적으로 허용가능한 오일내에 현탁된 후 수성 매질내에 유화되는, 경구투여에 적합한 조성물 형태의 배합물.
  7. 제1항 또는 제5항에 있어서, 계면활성제가 소르비탄 지방산 에스테르인 때합물.
  8. 제7항에 있어서, 계면활성제가 소르비탄 트리올레이트인 배합물.
  9. 제1항또는 제5항에 있어서, 약제가 항생물질, 심장혈관성 약제, 기침 또는 감기제제용 약제, 펩티드 약제, 기타 천연산 약제 또는 그의 유도체, 항-비루스성 약제, 진경제 또는 암치료를 위한 화학요법제인 배합물.
  10. 제1항 또는 제5항에 있어서, 약제가 기관지 확장제 또는 기타 항-천식성 약제, 예컨대 코르티코스테로이드, 이나트륨 크로모글리케이트 및 항히스타민제인 배합물.
  11. 제10항에 있어서, 기관지확장제가 β-아드레날린성 아고니스트, 크산티, 항-콜린효능성 약제, 칼슘안타고니스트 또는 류고트리엔 또는 그의 유도체인 배합물.
  12. 제11항에 있어서, 약제가 살부타몰 또는 터부탈린인 배합물.
  13. 마이크로캡슐을 계면활성제와 혼합하거나, 마이크로캡슐의 벽을 형성하는 중합체내에 계면활성제를 함입시키거나, 약제를 캡슐화한 마이크로캡슐을 계면활성제에 노출시켜 계면활성제가 마이크로캡슐의 벽 물질을 피복시키는 것으로 구성되는, 제1항 또는 제5항에 따른 제약학적 배합물의 제조방법.
  14. 제13항에 있어서, 계면활성제에 노출된 마이크로캡슐을 물과 섞이지 않는 제약학적으로 허용가능한 오일내에 현탁시키고, 결과 현탁액을 수성 매질내에 유화시키는 것을 포함하는 방법.
  15. 제14항에 있어서, 벽을 위한 중합체 물질을 용매내에 용해시키고. 계면활성제 및 약제를 중합체 용액내에 분산시키고, 용매를 증발시키는 것을 포함하는 방법.
