Abstract

본 발명은 안정성이 향상된 경추 추체간 케이지에 관한 것이다. 본 발명의 경추 추체간 케이지에 따르면, 양측 구상돌기간을 가로지르는 대들보 역할을 하는 날개 모양의 돌기부가 구상돌기와 접촉하여 케이지의 안정성을 향상시키고, 경추 종판에 전달되는 하중을 넓은 면적의 종판에 분산시키도록 케이지의 골접촉 면적을 최대화하였는바, 경추 질환의 수술적 치료를 위한 전방 경추 추간판 절제술 및 유합술시 유용하게 이용될 수 있을 것으로 기대된다.

본 발명은 경추 추체간 케이지에 관한 것으로서, 보다 구체적으로는 골접촉 면적과 안정성이 향상된 경추 추체간 케이지에 관한 것이다.
퇴행성 경추 질환이나 경추의 외상의 수술적 치료 목적으로 전방 경추 추간판 절제술 및 유합술이 최근 흔히 사용되고 있다. 전통적으로는 추체간 유합을 위해 자가골 이식이 사용되고 있으나, 초기 안정성이 낮고 이식골의 붕괴 및 탈출과 같은 자가골 자체의 문제, 자가골 공여부의 통증 또는 염증과 같은 자가골 체취로 인한 합병증 등으로 인해, 최근에는 경추 추체간 케이지가 흔히 사용되고 있다. 케이지를 사용한 경추 유합술시 대부분 문헌에서 좋은 임상결과가 보고되고 있으나, 케이지 침강, 불유합과 같은 주요 합병증이 보고되면서 케이지 추체간 안정성을 향상시키기 위한 노력으로 금속판으로 추가 고정하거나 케이지를 나사로 고정하는 방법 등이 사용되고 있다. 케이지 디자인 측면에서는 케이지 침강과 불유합 등을 예방하기 위해서는 골접촉 면적을 최대화하는 것이 중요한데, 경추의 해부학적 특성을 고려할 때 이에 어려움이 있다. 경추의 추체는 실린더 모양에 윗면과 아랫면이 두꺼운 치밀골로 종판을 형성한다. 척추뼈 중 경추에서만 특징적으로 볼 수 있는 중요한 3차원적 골구조물로 구상돌기가 있으며, 상부 종판의 후외측에서 상방으로 솟은 모양으로 좌우 한쌍이며 대칭형으로 위치한다. 구상돌기가 전방에서 후방으로 갈수록 양측 모두 내측으로 넓어짐으로 인해서 케이지가 위치하게 되는 비교적 평평한 상부 종판 부위의 너비가 제한되고, 전방에서 후방으로 갈수록 좁아지게 된다. 이러한 경추의 해부학적 특성으로 인해 전방이 넓고 후방이 좁은 사다리꼴 모양의 케이지 디자인이 많이 사용되고 있으나, 구상 돌기는 각 경추 분절과 개인별로 그 모양과 크기가 다양하여 케이지가 접촉할 수 있는 접촉면이 제한되므로, 충분한 케이지-골 접촉면을 확보하는데 어려움이 있다. 더욱이, 다양한 너비, 깊이의 케이지를 제작할 수 있으나, 대부분의 척추외과의사들이 적절한 케이지 높이의 선택에만 신경쓸 뿐 수술 편의상 케이지 너비는 고려하지 않는 경향이 있어 현재의 케이지 디자인으로는 안정성 확보에 한계가 있으며(한국등록특허 10-1225006), 안정성 확보를 위한 새로운 케이지 디자인 개념은 최근 제시된 바가 없다.
본 발명은 상기와 같은 문제점을 해결하기 위해 안출된 것으로서, 본 발명은 케이지의 안정성을 향상시키고, 골 접촉면적을 최대화시킨 경추 추체간 케이지를 제공하는 것을 그 목적으로 한다.
그러나 본 발명이 이루고자 하는 기술적 과제는 이상에서 언급한 과제에 제한되지 않으며, 언급되지 않은 또 다른 과제들은 아래의 기재로부터 당업자에게 명확하게 이해될 수 있을 것이다.
상기와 같은 본 발명의 목적을 달성하기 위하여, 본 발명은 경추 추체 사이에 삽입되며, 중앙에 관통공 (110)이 형성되어 있는 본체부 (100); 및
상기 본체부 (100)의 양 측면에 대향하게 돌출 형성된 돌기부(200)를 포함하는, 경추 추체간 케이지를 제공한다.
바람직하게 상기 돌기부 (200)의 측면 및 하면이 구상돌기 및 하위추체와 접촉함으로써 접촉면적을 넓히고 경추 추체간 케이지를 고정시켜 안정성을 향상시킬 수 있다.
바람직하게 상기 돌기부 (200)는, 날개 형상으로 이루어질 수 있다.
바람직하게 상기 돌기부(200)의 측면은, 오목부와 볼록부가 순차적으로 형성된 요철 형상으로 이루어질 수 있다.
바람직하게 상기 경추 추체간 케이지는, 폴리에스테르케톤 (PEEK: Polyetheretherketone), 탄소 섬유, 세라믹 또는 티타늄 합금 (Titanium alloy) 재질로 이루어질 수 있다.
본 발명에서 제안하고 있는 경추 추체간 케이지에 따르면, 양측 구상돌기간을 가로지르는 대들보 역할을 하는 날개 모양의 돌기부가 구상돌기와 접촉하여 케이지의 안정성을 향상시키고, 경추 종판으로 전달되는 하중이 구상 돌기와 함께 넓은 면적의 종판에 분산되도록 케이지의 골접촉 면적을 최대화하였는바, 경추 케이지를 사용한 추간판 절제술 및 유합술 후 발생할 수 있는 케이지 침강현상과 불유합 등의 합병증을 최소화할 수 있다.

이하, 첨부된 도면을 참조하여 본 발명이 속하는 기술분야에서 통상의 지식을 가진 자가 본 발명을 용이하게 실시할 수 있도록 바람직한 실시예를 상세히 설명한다. 다만, 본 발명의 바람직한 실시예를 상세하게 설명함에 있어, 관련된 공지 기능 또는 구성에 대한 구체적인 설명이 본 발명의 요지를 불필요하게 흐릴 수 있다고 판단되는 경우에는 그 상세한 설명을 생략한다. 또한, 유사한 기능 및 작용을 하는 부분에 대해서는 도면 전체에 걸쳐 동일한 부호를 사용한다.
덧붙여, 명세서 전체에서, 어떤 부분이 다른 부분과 '연결' 되어 있다고 할 때, 이는 '직접적으로 연결' 되어 있는 경우뿐만 아니라, 그 중간에 다른 소자를 사이에 두고 '간접적으로 연결' 되어 있는 경우도 포함한다. 또한, 어떤 구성요소를 '포함' 한다는 것은, 특별히 반대되는 기재가 없는 한 다른 구성요소를 제외하는 것이 아니라 다른 구성요소를 더 포함할 수 있다는 것을 의미한다.
도 1 내지 도 3은 본 발명의 일 실시예에 따른 경추 추체간 케이지 (10)의 사시도, 평면도, 및 저면도이며, 도 1 내지 도 3에 도시된 바와 같이, 본 발명에 따른 경추 추체간 케이지 (10)는 본체부 (100) 및 돌기부 (200)를 포함하여 구성될 수 있다.
본 발명의 일시예에 따른 경추 추체간 케이지 (10)는, 제거된 추간판 결손부위에 삽입 및 고정되어 상위경추체와 하위경추체의 사이에 골 유합을 형성하고, 추체간 간격을 복원, 유지시키기 위한 인공 구조물로, 경추 종판과 구상돌기의 3차원적 특성을 이용하여, 경추의 양측 구상돌기간을 가로지르는 대들보 역할을 하는 날개 모양의 돌기부가 추가된 경추 추체간 케이지를 고안하였다. 이와 같은 구성을 채택함으로써, 경추 추체간 케이지 전체의 안정성을 증진시키고, 경추 종판으로 전달되는 하중을 구상 돌기를 포함하여 넓게 분산시킴으로써, 케이지를 사용한 전방 경추 추간판 절제술 및 유합술 후 발생할 수 있는 케이지 침강과 불유합 등의 합병증을 최소화시킬 수 있을 것으로 기대된다.
이하에서는, 본 발명의 일시예에 따른 경추 추체간 케이지 (10)를 구성하는 각각의 구성요소에 대하여 상세히 설명하기로 한다.
본체부 (100)는 제거된 추간판의 결손부위에 삽입 및 고정되는 경추 추체간 케이지 (10)의 주요한 프레임을 형성하는 구성으로, 도 1에 도시된 바와 같이, 전체적으로 제거된 추간판에 대응되는 형상으로 이루어질 수 있다. 또한, 상기 본체부 (100)의 중앙에는 상하방향으로 관통된 구조인 관통공이 형성되어 있으며, 이는 제거된 추간판의 상하에 위치하는 상위경추체와 하위경추체 사이에 골 유합이 이루어질 수 있도록 연계하는 역할을 한다.
경추 내 구상돌기의 높이는, 경추 6-7번 전방부를 제외한 모든 분절에서 추간판 높이와 비교하여 평균 1mm 이하의 차이를 보여, 좌우 구상돌기 상방 내측에서 지지되는 대들보 형태의 돌기부를 포함한 경추 케이지로 추간판 높이의 회복이 가능한 근거가 될 수 있다. 또한, 후방으로 갈수록 좁아지는 종판 너비에 비해 구상돌기 상부 내측간 너비는 전후방에서 2mm 이내의 비교적 일정한 측정값을 보이는바, 이러한 해부학적 특성을 이용하여 구상돌기와 접촉되는 돌기부를 포함한 케이지로 안정성 및 골과의 접촉 면적을 최대화하고자 하였다.
돌기부 (200)는 케이지와 추체간 접촉 면적, 보다 구체적으로 하위경추체와의 접촉면적을 증진시키기 위한 구성으로, 도 1에 도시된 바와 같이, 상기 본체부 (100)의 양 측면에 대향하게 돌출 형성될 수 있고, 목적하고자 하는 기능을 효율적으로 구현하기 위해 바람직하게 날개 형상으로 이루어질 수 있다.
상기 돌기부(200)의 측면 및 하면은, 도 3 및 도 4에 도시된 바와 같이, 구상돌기와 하위추체와 접촉함으로써 접촉면적을 넓히고 경추 추체간 케이지를 고정시켜 안정성을 향상시키며, 상위경추체로부터 전달되는 하중을 분산시키는 역할을 수행한다. 한편, 구상돌기와 접촉하는 접촉면의 미끄러짐을 방지하고, 구상돌기 너비에 맞추어 돌기부 너비 조정을 위한 절단을 용이하게 하기 위하여, 상기 돌기부(200)의 측면은, 도 3에 도시된 바와 같이, 오목부와 볼록부가 순차적으로 형성된 요철 형상으로 구현하는 것이 바람직하지만, 이에 제한되는 것은 아니다.
본 발명의 일시예에 따른 경추 추체간 케이지 (10)는 스테인레스스틸과 같은 내식성금속 재질로 제조될 수도 있지만, 폴리에스테르케톤 (PEEK: Polyetheretherketone), 탄소섬유, 세라믹 또는 티타늄 합금 (Titanium alloy) 재질로 제조되는 것이 바람직하다.
도 4는 본 발명의 일 실시예에 따른 경추 추체간 케이지 (10)가 경추 추체간 삽입된 상태를 나타낸 도이다. 도 4에 도시된 바와 같이, 경추 추체간 케이지 (10)는 상위경추체와 하위경추체 사이에 삽입되며, 돌기부(200)의 측면 및 하면은 구상돌기에 고정되어 케이지와 추체간 안정성을 향상시키고 하위추체 종판에 가해지는 하중을 분산시킴으로써, 케이지를 사용한 전방 경추 추간판 절제술 및 유합술에 따른 합병증을 최소화시킬 수 있다.
전술한 본 발명의 설명은 예시를 위한 것이며, 본 발명이 속하는 기술분야의 통상의 지식을 가진 자는 본 발명의 기술적 사상이나 필수적인 특징을 변경하지 않고서 다른 구체적인 형태로 쉽게 변형이 가능하다는 것을 이해할 수 있을 것이다. 그러므로 이상에서 기술한 실시예들은 모든 면에서 예시적인 것이며 한정적이 아닌 것으로 이해해야만 한다.

Claims (5)

  1. 경추 추체 사이에 삽입되며, 중앙에 관통공 (110)이 형성되어 있는 본체부 (100); 및
    상기 본체부 (100)의 양 측면에 대향하게 돌출 형성된 돌기부(200)를 포함하는, 경추 추체간 케이지.
  2. 제1항에 있어서, 상기 돌기부 (200)의 측면 및 하면이 구상돌기 및 하위추체와 접촉함으로써 접촉면적을 넓히고 경추 추체간 케이지를 고정시켜 안정성을 향상시키는 것을 특징으로 하는, 경추 추체간 케이지.
  3. 제1항에 있어서, 상기 돌기부 (200)는, 날개 형상으로 이루어지는 것을 특징으로 하는, 경추 추체간 케이지.
  4. 제1항에 있어서, 상기 돌기부(200)의 측면은,
    오목부와 볼록부가 순차적으로 형성된 요철 형상으로 이루어지는 것을 특징으로 하는, 경추 추체간 케이지.
  5. 제1항에 있어서, 상기 경추 추체간 케이지는,
