Abstract

맞물린 모듈형 디스크 핵 보형물{INTERLOCKED MODULAR DISC NUCLEUS PROSTHESIS}

발명의 요약
본 발명은 모듈형 디스크 핵 보형물을 사용하여 최소로 침습적인 방식으로 손상된 척추간 디스크 핵을 치료하는 방법 및 장치를 제공한다. 모듈형 디스크 보형물은 바람직하게는 세 개 이상의 모듈형 분절을 포함한다. 한 실시양태에서, 각 모듈형 분절은 내부 코어 및 외부 셀을 포함한다. 모듈형 분절은 서로 제자리에서 선택적으로 맞물린다. 모듈형 분절은 디스크 핵 공동의 기하구조를 매우 잘 모방하는 이식된 일체형 장치를 형성한다.

보형물은, 장치가 완전히 형성될 때까지 외과 의사에 의해 직접적으로 제어되는 예비형성된 부품으로 이루어지므로, 부품이 삽입되는 동안 부품이 이탈될 가능성이 거의 없다. 삽입 동안 부품을 제어할 수 있기 때문에, 일단 삽입되고 나면 외과 의사에 의해 제어되지 않는, 빈 디스크 공간 내에서 움직일 수 있는 비드와 같은 개별 지지체를 사용하는 장치보다, 유리하다. 본 발명의 부품은 외과 의사에 의해 직접적으로 제어되기 때문에, 장치의 삽입 동안에 부품이 바람직하지 못한 위치로 움직일 가능성이 덜 하면서, 외과 의사가 빈 디스크 공간 내에 부품들을 보다 정확하게 배치할 수 있다.
추가로, 모듈형 디스크 보형물의 크기를 예정할 수 있기 때문에, 핵 공동을 보다 완전하게 채울 수 있고, 기타 종류의 최소 침습적 이식물에 비해 이식물의 스트레스 반응의 균일성을 보다 잘 제어할 수 있다. 심지어는 후벽이 무력할 경우에도, 이식물의 스트레스 반응을 제어할 수 있기 때문에, 보형물은 안정하게 유지된다. 보형물의 크기를 조정하고 핵 공동을 완전히 채울 수 있기 때문에, 몇몇 히드로겔 블록 이식물의 경우에 일어날 수 있는 것과 같은, 장치가 완전히 이식된 후에 보형물이 핵 공동으로부터 이탈되는 것을 방지할 수 있다.
이제 도 9 및 도 10에서, 모듈형 디스크 보형물(200)의 대안적 실시양태를 볼 수 있다. 모듈형 디스크 보형물(200)은 장치를 정렬시키는 것을 돕는 밧줄(216, 218)을 갖는 3-피스 이식물이다. 모듈형 디스크 보형물(200)은 슬롯(208)을 갖는 제 1 모듈형 분절(202), 제 1 고정 슬라이드(210) 및 제 2 고정 슬라이드(212)를 갖는 제 2 모듈형 분절(204), 및 슬롯(214)을 갖는 제 3 모듈형 분절(206)을 포함한다. 모듈형 디스크 보형물(200)은 제 1 모듈형 분절(202) 및 제 3 모듈형 분절(206) 내의 구멍(220, 222)을 통해 꿰어진 밧줄(216, 218)을 추가로 포함한다. 해당 분야의 숙련자라면, 밧줄(216, 218)의 개수 및 위치는 다양할 수 있고, 밧줄을 상이한 개수의 모듈형 분절을 갖는 모듈형 디스크 보형물에 사용할 수 있다는 것을 알 것이다.
대안적인 실시양태에서, 모듈형 디스크 핵 보형물의 외부 셀은 약물을 용출시키도록 개조될 수 있다. 이러한 약물은 진통제, 항생제, 항신생물제 또는 바이 오오스테올로직(bioosteologic), 예를 들면 뼈성장제를 포함할 수 있다. 핵 대체에서는 운동 보존이 일반적으로 근본 목표인 반면에, 어떤 적응증에서는 몇몇 뼈 융합을 촉진시키는 것이 바람직할 수 있다. 이러한 적응증은 경부 척추에서의 핵 대체를 포함할 수 있다.

Claims (17)

  1. 각각 서로 마주보는 측부들, 근위 말단 및 원위 말단에 의해 한정되는 너비를 갖는 세 개 이상의 모듈형 분절을 포함하는, 빈 디스크 핵 공간 내에 이식되도록 적응된 모듈형 디스크 핵 보형물로서,
    여기서 보형물이, 외부 모듈형 분절의 한 측부의 근위 말단은 중간 모듈형 분절의 한 측부의 원위 말단에 가깝게 작동가능하게 배치되며, 또다른 외부 모듈형 분절의 한 측부의 원위 말단은 중간 모듈형 분절의 또다른 측부의 근위 말단에 가깝게 작동가능하게 배치된 확장 상태, 및 모듈형 분절의 총 너비가 일반적으로 빈 디스크 핵 공간의 너비에 상응하는 일반적으로 연속적인 원주를 갖는 일체형 본체를 한정하도록, 모듈형 분절이 서로 인접한 각 모듈형 분절의 근위 말단 및 서로 인접한 각 모듈형 분절의 원위 말단과 일반적으로 사이드 바이 사이드(side by side) 배열로서 빈 핵 디스크 공간 내에 배치된 이식 상태를 갖도록 모듈형 분절들이 서로 선택적으로 맞물리는 모듈형 디스크 보형물.
  2. 제 1 항에 있어서, 두 개 이상의 중간 모듈형 분절을 추가로 포함하고, 여기서 중간 모듈형 분절의 한 측부의 근위 말단이 또다른 중간 모듈형 분절의 한 측부의 원위 말단에 가깝게 작동가능하게 배치된 것인 모듈형 디스크 보형물.
  3. 제 1 항에 있어서, 각 모듈형 분절이 외부 셀 및 내부 코어를 포함하는 것인 모듈형 디스크 보형물.
  4. 제 3 항에 있어서, 내부 코어가 흡습성 물질로 이루어지고 외부 셀이 중합체로 이루어진 것인 모듈형 디스크 보형물.
  5. 제 4 항에 있어서, 외부 셀이 보다 낮은 듀로메터의 중합체로 이루어진 제 1 부분 및 보다 높은 듀로메터의 중합체로 이루어진 제 2 부분으로 구성된 것인 모듈형 디스크 보형물.
  6. 제 1 항에 있어서, 각 모듈형 분절이 내부 부분 및 외부 부분을 포함하고, 내부 부분은 하나 이상의 측부에서, 인접한 모듈형 분절의 하나 이상의 측부에서 상응하는 구조와 선택적으로 맞물리는 구조를 포함하고, 외부 부분은 인접한 모듈형 분절과 선택적으로 맞물리는 구조를 갖는 측부를 제외하고 내부 부분을 실질적으로 둘러싸는 것인 모듈형 디스크 보형물.
  7. 제 6 항에 있어서, 인접한 모듈형 분절과 선택적으로 맞물리는 구조가 모든 모듈형 분절에 있어서 유사한 것인 모듈형 디스크 보형물.
  8. 제 6 항에 있어서, 인접한 모듈형 분절과 선택적으로 맞물리는 구조가 한 측부에 래치트 및 또다른 측부에 상응하는 톱니멈춤쇠를 포함하는 것인 모듈형 디스 크 보형물.
  9. 제 6 항에 있어서, 외부 부분의 하나 이상의 층이, 보형물이 이식된 후에 용출되는, 외부 부분에 의해 작동가능하게 운반되는 하나 이상의 약물을 추가로 포함하는 것인 모듈형 디스크 보형물.
  10. 제 1 항에 있어서, 모듈형 분절의 너비가 실질적으로 유사하고, 확장 상태에서, 보형물을 빈 디스크 핵 공간에 삽입하기 위한 개구의 최소 너비를 결정하는 보형물의 너비를 한정하는 것인 모듈형 디스크 보형물.
  11. 제 10 항에 있어서, 보형물을 삽입하기 위한 개구의 최소 너비가 약 1 ㎝ 미만인 모듈형 디스크 보형물.
  12. 제 1 항에 있어서, 하나 이상의 모듈형 분절에서, 이식 동안에 모듈형 디스크 보형물을 정렬시키는 것을 돕는, 개구를 통해 꿰어진 밧줄을 추가로 포함하는 모듈형 디스크 보형물.
  13. 제 1 외부 모듈형 분절을 빈 디스크 핵 공간 내에 부분적으로 삽입하고;
    중간 모듈형 분절의 한 측부의 원위 말단을 외부 모듈형 분절의 근위 말단의 내측에 선택적으로 부착하고;
    중간 모듈형 분절이 빈 디스크 핵 공간 내에 부분적으로 존재할 때까지 중간 모듈형 분절을 제 1 외부 모듈형 분절과 나란히 배치하고;
    제 2 외부 모듈형 분절의 내측의 원위 말단을 중간 모듈형 분절의 근위 말단의 한 측부에 선택적으로 부착하고;
    중간 모듈형 분절의 원위 말단이 제 1 외부 모듈형 분절의 원위 말단과 정렬될 때까지, 중간 모듈형 분절을, 제 1 외부 모듈형 분절과 사이드 바이 사이드 배열로서 빈 디스크 핵 공간 내에 완전히 배치시키고;
    제 2 외부 모듈형 분절의 원위 말단이 중간 모듈형 분절의 원위 말단과 정렬될 때까지 제 2 외부 모듈형 분절을 빈 디스크 핵 공간 내에 완전히 배치시키는 것
    을 포함하는, 모듈형 디스크 보형물을 빈 디스크 핵 공간 내에 이식하는 최소 침습적 방법.
  14. 제 13 항에 있어서,
    추가의 중간 모듈형 분절의 한 측부의 원위 말단을 중간 모듈형 분절의 근위 말단의 내측에 선택적으로 부착하고;
    추가의 중간 모듈형 분절이 빈 디스크 핵 공간 내에 부분적으로 배치될 때까지, 추가의 중간 모듈형 분절을 중간 모듈형 분절과 나란히 배치시키는 것
    을 추가로 포함하는 방법.
  15. 제 13 항에 있어서, 선택적으로 부착하는 단계를 모두 배치 단계 이전에 수 행하는 방법.
  16. 제 1 외부 모듈형 분절을 빈 디스크 핵 공간 내에 완전히 삽입하고;
    중간 모듈형 분절의 한 측부의 원위 말단을 외부 모듈형 분절의 근위 말단의 내측에 선택적으로 부착하고;
    중간 모듈형 분절의 원위 말단이 제 1 외부 모듈형 분절의 원위 말단과 정렬될 때까지, 중간 모듈형 분절을, 제 1 외부 모듈형 분절과 사이드 바이 사이드 배열로서 빈 디스크 핵 공간 내에 완전히 배치시키고;
    제 2 외부 모듈형 분절의 내측의 원위 말단을 중간 모듈형 분절의 근위 말단의 한 측부에 선택적으로 부착하고;
    제 2 외부 모듈형 분절의 원위 말단이 중간 모듈형 분절의 원위 말단과 정렬될 때까지 제 2 외부 모듈형 분절을 빈 디스크 핵 공간 내에 완전히 배치시키는 것
    을 포함하는, 모듈형 디스크 보형물을 빈 디스크 핵 공간 내에 이식하는 최소 침습적 방법.
  17. 제 13 항에 있어서,
    추가의 중간 모듈형 분절의 한 측부의 원위 말단을 중간 모듈형 분절의 근위 말단의 내측에 선택적으로 부착하고;
    추가의 중간 모듈형 분절의 원위 말단이 중간 모듈형 분절의 원위 말단과 정렬될 때까지, 추가의 중간 모듈형 분절을 중간 모듈형 분절과 나란히 배치시키는 것
    을 추가로 포함하는 방법.
