Abstract
본 발명은 하이드록시클로로퀸(hydroxychloroquine)을 함유하는 치핵 치료를 위한 국소투여용 주사제 조성물에 관한 것이다.
구체적으로는 하이드록시클로로퀸(hydroxychloroquine)을 주사용 생리식염액에 용해하고 국소마취제와 항산화제를 함유하는 국소투여용 주사제 조성물에 관한 것이다.

Description
본 발명은 하이드록시클로로퀸(hydroxychloroquine)을 함유하는 국소투여에 의한 치핵 치료용 주사제 조성물에 관한 것으로, 본 발명은 치핵부분에 직접 주사하여 치핵조직을 경화시켜 대사를 차단함으로써 치핵조직을 불활성화하여 궁극적으로는 치핵조직을 괴사시켜 치료하는 치핵 치료용 주사제 조성물에 관한 것이다.
치질환(hemorrhoids)은 우리나라 입원진료 1위를 차지하고, 50대 이상의 50% 이상이 경험하고 있고(서울대 병원), 미국에서도 전인구의 5%가 이 병을 가지고 있다.
일반적으로 치질이라고 칭하는 치질환은 주로 치핵, 치열, 항문 주위 농양 및 치루 등이 있고, 주요 증상으로는 출혈(bleeding), 치핵 돌출(prolapse), 통증(pain), 소양증(itching), 직장 기능 이상(dysfunction), 변실금(soiling)이 있다. 그 중 대부분 치질이라 함은 주로 치핵을 의미한다.
치핵은 항문의 피부와 점막 밑의 혈관조직이 늘어나고 확장되어 생기는데 혈 관을 덮고 있는 피부와 점막이 같이 늘어나서 생긴 살덩어리이다. 내치핵과 외치핵, 혼합치핵으로 구분하며 항문 안쪽 1.5cm 정도에 위치한 치상선을 경계로 그 위쪽에서 생겼으면 내치핵, 아래쪽에서 생겼으면 외치핵, 위아래에 다 생겼으면 혼합치핵이라고 한다. 초기엔 내치핵만 있거나 외치핵만 있는 경우도 있지만 시간이 지나면서 대개 혼합치핵으로 진행되는 경우가 많다.
내치핵의 진행 정도를 탈항의 정도에 따라 분류되는데, 1기는 출혈로 치핵이 진단되었으나 탈항이 없는 상태이고, 2기는 변을 볼 때 탈항이 되나 곧 다시 들어가는 상태이며, 3기는 변을 볼 때 탈항이 되어 배변 후 시간이 지나야 들어가거나 밀어 넣거나 누워있어야 들어가는 상태이고, 4기는 변을 볼 때 탈항된 것이 잘 들어가지 않거나 금방 다시 나오는 상태이다.
외치핵의 진행 정도는 누구나 인정하는 객관적인 분류가 아직 없거나 내치핵의 진행 정도와의 형평성을 고려하여 혈전의 발생 기간을 기준으로 외치핵을 분류할 수 있다. 즉, 1기 외치핵은 혈전이 생겨 부어 있는 기간이 일년에 10일 미만이고, 2기 외치핵은 일 년에 11~20일 정도이며, 3기 외치핵은 혈전이 생겨 부어 있는 기간이 일년에 3주 이상일 때이고, 4기 외치핵은 혈전이 수시로 발생할 때이다.
이와 같은 치질환의 치료를 위해 일반적인 방법으로는 치질환에서 내치핵의 출혈을 가라앉히는 방법이거나, 외치핵에서 생긴 혈전과 이로 인한 부종과 통증을 가라앉히는 치료로는 내복약과 좌약, 그리고 좌욕 등이 있다.
내복약의 주성분은 혈액순환개선제로 만들어져 있고, 시중에 10여 가지의 비슷한 약제가 판매되고 있다. 좌약은 진통소염과 항균, 수렴 작용 등을 하는 성분으 로 구성되어 있으며, 20 가지의 약제가 판매되고 있다.
상기의 치질환 치료제로서 연고 및 좌약은 주로 국소 마취제, 스테로이드 제제 및 항히스타민제 등을 함유하는 제제가 사용되고 있다. 그러나 국소 마취제나 스테로이드 제제의 투여는 일시적인 불편함을 완화시키기는 하나 상처의 치유를 지연되게 하고, 여러가지 전신성, 국소적 부작용을 일으킬 가능성이 많다. 게다가 상처주위에 비스테로이드성 소염제를 투여하는 것은 병의 치료를 늦춤으로써 금지되어 있다.
또한, 외과적인 수술방법은 환자들이 기피하는 방법일 뿐 만아니라 외형적으로 돌출되어 있는 것만 제거하기 때문에 치핵의 뿌리 제거에 실패하는 경우도 종종 있다.
공개특허공보 공개번호 10-2005-0102663 에는 활성성분으로 아세틸살리실산 또는 그의 염을 포함하는 치핵,항문열창으로 인한 치질환 외용제가 개시되어 있다.
등록특허공보 등록번호 10-0863758 에는 테오필린 또는 디필린으로 이루어진 군으로부터 선택된 아데노신 수용체 길항제를 함유하는 급성 또는 만성 항문열상,혈전성 내치핵 또는 외치핵, 치핵성 질환 등에 국소 적용하는 약학조성물이 기재되어 있다.
공개특허공보 공개번호 10-2008-0105857 에는 복어알추출물과 복어간 추출물과 난황유 및 천궁 추출물을 함유하는 항문 직장 질환치료활성을 갖는, 국소적인 표면에 직접 적용하는 조성물이 기재되어 있다.
그러나, 본 발명과 같이 하이드록시클로로퀸을 함유하는 주사제 조성물을 환 부에 직접 주사하여 치핵조직을 경화시켜 대사를 차단함으로써 치핵조직을 불활성화하고 궁극적으로는 치핵조직을 괴사시켜 치료하는 치핵 치료용 주사제 조성물은 현재까지 알려진 바 없다.
본 발명은 하이드록시클로로퀸(hydroxychloroquine)을 함유하는 국소투여용 주사제 조성물을 환부에 직접 투여하여 치핵의 뿌리까지 침투시켜 치핵을 이루는 조직세포를 전체적으로 괴사시키고, 조직의 괴사가 일어나면 환부조직이 1/3정도로 위축되어 결과적으로 박리되어 탈락하게 된다.
본 발명의 작용기전을 살펴보면, 본 발명의 하이드록시클로로퀸을 함유하는 국소투여용 주사제 조성물을 환부에 직접 투여하면 환부 조직을 경화시켜 대사를 차단하고, 궁극적으로 조직의 불활성화가 일어나 환부조직의 괴사가 일어나는 것이다.
본 발명은 하이드록시클로로퀸(hydroxychloroquine)을 함유하는 치핵 치료를 위한 국소투여용 주사제 조성물에 관한 것으로, 구체적으로는 하이드록시클로로퀸 5~10g 을 주사용 생리식염액 10 ml에 용해하고 국소마취제와 항산화제를 함유하는 국소투여용 주사제 조성물에 관한 것이다.
본 발명에 있어서, 하이드록시클로로퀸은 주사용 생리식염액에 대해 1~60 % w/w함유하는 것이 바람직하며, 보다 바람직하게는 50% w/w함유하는 것이다.
본 발명에 있어서, 하이드록시클로로퀸을 주사용 생리식염액에 대해 50% w/w 로 용해하여 제조할 경우, 잘 용해되지 않아 실온에서 현탁액 상태로 존재하며, 사용시 따뜻한 물에 중탕하여 용해시켜 사용하는 것이 바람직하다.
하이드록시클로로퀸이 함량이 1 % w/w 이하이면 치료효과를 기대하기 어렵고, 60 % w/w 이상이 되면 쉽게 용해되지 않아 제제화 하기 어렵고 조직의 괴사가 빨리 진행되어 바람직하지 않은 결과를 초래할 수도 있다.
본 발명에 사용되는 국소마취제는 환부에 주사로 직접 투여시 통증제거 목적으로 사용되며, 바람직하게는 리도카인 1~2% w/v 를 함유한다.
본 발명에 사용되는 항산화제는 제제 안정성을 위해 배합되며, 바람직하게는 리보플라빈 0.1% w/v 를 함유한다.
본 발명의 국소투여용 주사제 조성물은 상기와 같은 처방으로 통상의 방법에 따라 제조할 수 있다.
치핵치료를 위해 본 발명은 환부에 직접 주사제로 투여하며, 환자의 상태에 따라 3~4일 간격으로 2~3회 투여하는 것이 바람직하다.
환자의 치핵(치질)상태가 크고 통증이 심한 경우에는 매일 소량씩 투여 할 수도 있으며 투여 기간도 일주일 이상 할 수도 있다.
본 발명은 하이드록시클로로퀸(hydroxychloroquine)을 함유하는 국소투여용 주사제 조성물을 환부에 직접 투여하여 치핵의 뿌리까지 침투시켜 치핵을 이루는 조직세포를 전체적으로 괴사시키고, 조직의 괴사가 일어나면 환부조직이 1/3정도로 위축되어 결과적으로 환부가 박리되어 탈락됨으로써 치핵을 외과적 수술 없이 간편하게 근원적으로 치료할 수 있다.
이하, 실시예 및 실험예에 의해 본 발명을 구체적으로 설명한다.
실시예 1
본 발명(이하 "Haemsol 50" 라고 칭함)의 국소투여용 주사제 조성물의 제조
주사용 saline용액 10 ml에 hydroxy chlorquine 10 g 을 용해하고 리도카인 2% w/v, 리보플라빈 0,1% w/v을 첨가하여 제조한다.
치핵투여방법
3~4일 간격 2~3회 헴솔50용액을 0.2~0.3ml용량으로 치핵을 삼각뿔로 간주 꼭지점을 포인트로 투여함.
실험예 1
홍 0 0 82세 여
60여년간 치질로 고생하다 갖가지 방법으로 치료해오다 최근 치질전문 병원을 찾았으나 치핵이 너무 크고 나이가 많은 관계로 수술이 불가한 상태로 내원
내원당시 치핵크기는 3.5x5.0cm로 심한고통을가지고옴
첫날 헴솔50 용액 1.2ml를 광범위하게투약 4일후 2차투약
투약 8일째 치핵크기 4.5x6.5cm 커짐 통증은 줄어들음
14일째부터 조직이 괴사 되기시작
21일째 치핵 완전분리 괴사된조직 실바딘 크림으로 도포
조직 재생 염증방지 투여 36일후 완치(도 1 참조)
실험예 2
김 0 0 36세 여
오랫동안 출혈과 치핵 고통으로 대변 보기 어려워하며 지나옴
내원당시 치핵이 아래위로 나뉘어 위치핵 직경 1.5x1.0cm 아래치핵 직경2.0x1.0cm로 1차 투약후 치핵이 다소 커짐
투약 14일후 치핵이 괴사 완전 박리되고 크게 구멍이 생김
실바딘 크림으로 환부에 도포 하고 염증이 생기지 않도록 하고
조직 재생을 도움 투약 26일후 조직 완전 회복됨(도 2 참조)
실험예 3

처음 내원할 당시는 치핵크기 1.0x2.0cm로 크지않은 것으로 간주 1차투여
투약 5일후 잠복해 있던 치핵 돌출 치핵크기3.5x5.0cm로 크게 돌출
2차 투약후 19일째 치핵 완전 괴사 박리됨
괴사된 조직 실바딘으로 도포 투약 30일만에 조직회복(도 3 참조)
실험예 4
김 0 0 56세 남
오랫동안 병원으로 민간요법으로 치료하다가 실패하고 고통중에 있었음
처음 내원한날 치핵크기 2.0x2.5cm
처음 투약후 조금씩 치핵이 커지다가 6일째 되는 날 치핵크기가 3.5x4.5cm로 커짐
투약 23일째 치핵괴사 완전 박리됨
약 2주간 실바딘으로 외용치료 하고 완전 조직재생(도 4 참조)

치질로 여러해 고생 치핵과 출혈로 고통을 받음
내원시 치핵크기 2.5x3.0cm로 헴솔50약물 투여후 3일만에 치핵이 3.0x4.0cm로 커짐
투약 13일만에 치핵완전 괴사 박리됨
실바딘으로 외용치료 2주일만에 완전 회복 조직재생됨(도 5 참조)

수년 동안 치질로 고생하다가 수술후 재발하여 포도송이처럼 치핵이 발달함(큰 치핵이 4개정도 분화)
투약 5일후 치핵이3.5x5.0cm로 크게 부풀음
투약 후 2l일만에 치핵 괴사 완전분리
실바딘으로 외용치료후 투약 31일만에 조직 완전회복(도 6 참조)

오랫동안 치질로 고생 치료하다 완전히 없어지지 않아 내원
내원초기 치핵 1.5x2.0cm로 크지않은 것으로 생각 투약
3일후 치핵이 2.5x3.0cm로 커짐

투약 30일후 완전 조직회복(도 7 참조)

평소 큰 치핵으로 대변보기 불편함 내원시 치핵크기 3.5x5.0cm로 힘들어 했음
투약 10일째부터 조직괴사 박리되기 시작
투약후 16일째 치핵이 완전괴사 박리되고 투약 6주후 완전회복됨(도 8 참조)

Claims (5)
- 하이드록시클로로퀸(hydroxychloroquine)을 함유하는 치핵 치료를 위한 국소투여용 주사제 조성물.
- 제1항에 있어서, 주사용 생리식염액 10 ml 에 대해 하이드록시클로로퀸을 1 내지 10 g 함유하는 것을 특징으로 하는 치핵 치료를 위한 국소투여용 주사제 조성물.
- 제2항에 있어서, 주사용 생리식염액 10 ml 에 대해 하이드록시클로로퀸을 10 g 함유하는 것을 특징으로 하는 치핵 치료를 위한 국소투여용 주사제 조성물.
- 제1항 내지 제3항 중 어느 한항에 있어서, 국소마취제와 항산화제를 추가로 함유하는 것을 특징으로 하는 치핵 치료를 위한 국소투여용 주사제 조성물.
- 제4항에 있어서, 국소마취제는 리도카인으로 2% w/v 함유하며, 항산화제는 리보플라빈으로 0.1% w/v 함유하는 것을 특징으로 하는 치핵 치료를 위한 국소투여용 주사제 조성물.
