Claims (1)
- 【特許請求の範囲】 1.プロテーゼを患者の体内の組織と接触するように配置するシステムにして 、 基端及び末端と、組織に係合するその間の本体とを有するプロテーゼを備え、 前記プロテーゼが、患者の体内に導入するための半径方向にコンパクトな形態 を有し、組織に係合し得るようその本体に沿って半径方向に膨張可能であり、該 プロテーゼの長さが、前記本体の膨張程度に依存して変化し、 前記システムが、患者の体内に導入する間に前記プロテーゼを前記コンパクト な形態に支持する部分を有するカテーテルであって、前記プロテーゼを組織と接 触するように膨張させる構造とされた前記カテーテルを更に備え、 前記部分が、前記プロテーゼの対応する部分を所定の軸方向位置にて半径方向 にコンパクトに保ち得るように末端付近にて前記プロテーゼに係合し得るように 配置された部材を備える一方、前記プロテーゼの基端部分が半径方向に膨張して 組織に係合するようにし、 前記部材が係合する前記プロテーゼの前記部分が、前記部材と前記プロテーゼ の相対的な軸方向動作により、記所定の位置に略対応する軸方向位置にて前記カ テーテルから解放可能とされ、従って、前記部材の自由端が前記プロテーゼを非 係合状態とするようにしたことを特徴とするシステム。 2.請求の範囲第1項に記載のシステムにして、前記プロテーゼが前記組織に 係合する本体に形成された開口部をその末端付近に備え、 前記部材が、カテーテルの略軸線に沿って自由端まで伸長し、前記開口部を貫 通して伸長することにより、前記プロテーゼに係合し、 前記開口部に対応する前記プロテーゼの前記部分が、軸方向への動作により解 放可能であり、前記部材の自由端が前記開口部から除去されるようにしたことを 特徴とするシステム。 3.請求の範囲第2項に記載のシステムにして、前記開口部が、前記プロテー ゼの外周の廻りで配置された一連の開口部を備え、 前記部材が、前記一連の開口部を貫通して伸長する対応する一連の細長の部材 であることを特徴とするシステム。 4.請求の範囲第2項に記載のシステムにして、前記部材が、前記自由端まで 末端方向に伸長し、前記プロテーゼの前記カテーテルからの解放が、前記部材を 前記プロテーゼに関して基端方向に動かすことにより行われることを特徴とする システム。 5.請求の範囲第2項に記載のシステムにして、前記部材が前記カテーテル上 に固定され、前記プロテーゼの前記カテーテルからの解放が、前記カテーテルを 前記プロテーゼに関して動かすことにより行われることを特徴とするシステム。 6.請求の範囲第2項に記載のシステムにして、前記細長の部材が、前記カテ ーテルの軸線に関してある角度にて伸長し、前記部材と前記カテーテルとの間に 、前記プロテーゼに係合する所定の楔状スペースを形成することを特徴とするシ ステム。 7.請求の範囲第2項に記載のシステムにして、前記部材が、半径方向への力 に応答して外方に撓んで、前記部材の前記自由端を前記開口部から解放する可撓 性材料で出来ていることを特徴とするシステム。 8.請求の範囲第2項に記載のシステムにして、前記部材が超弾性ワイヤーで あることを特徴とするシステム。 9.請求の範囲第2項に記載のシステムにして、前記プロテーゼが、前記半径 方向にコンパクトな形態にて前記支持部分の周りで同軸状に配置された管状の形 態のプロテーゼであることを特徴とするシステム。 10.請求の範囲第2項に記載のシステムにして、前記プロテーゼが、偏平なフ ィラメントで形成され、前記開口部が前記パターンにより形成されることを特徴 とするシステム。 11.請求の範囲第10項に記載のシステムにして、前記プロテーゼが、編み型 であり、前記開口部が前記編みパターンの端部ループであることを特徴とするシ ステム。 12.請求の範囲第2項に記載のシステムにして、前記プロテーゼが自己膨張型 であることを特徴とするシステム。 13.請求の範囲第12項に記載のシステムにして、前記部材に対応する前記プ ロテーゼの部分が、前記部材によりコンパクトな状態に保たれ、前記部材から離 れた部分が拘束部材によりコンパクトな形態に保たれることを特徴とするシステ ム。 14.請求の範囲第13項に記載のシステムにして、前記拘束部材が軸方向に退 却可能なシースであることを特徴とするシステム。 15.請求の範囲第14項に記載のシステムにして、前記プロテーゼが、前記シ ースが退却したとき、顕著な摩擦力にて前記シースに係合して、前記プロテーゼ を張力状態下に置くことを特徴とするシステム。 16.プロテーゼを患者の体内の組織と接触するように配置するシステムにして 、 基端及び末端と、組織に係合するその間の本体とを有する管状プロテーゼを備 え、 前記プロテーゼが、患者の体内に導入するための半径方向にコンパクトな形態 を有し、組織に係合し得るようその本体に沿って半径方向に膨張可能であり、該 プロテーゼの長さが、前記本体の膨張程度に依存して変化し、 前記プロテーゼが、偏平なフィラメントで形成され、該プロテーゼの外周の周 りで前記プロテーゼの組織に係合する本体部分に形成された一連の開口部を備え 、 前記システムが、患者の体内に導入するために前記部分の周りでカテーテルを 前記コンパクトな形態に支持する部分を有するカテーテルであって、前記プロテ ーゼを組織と接触するように膨張させる構造とされた前記カテーテルを更に備え 、 前記部分が、前記カテーテルの外周に配置され、前記カテーテルに固定された 一連の細長の部材であって、前記カテーテルの略軸線に沿って自由端まで伸長す る前記一連の細長の部材を備え、前記自由端が、前記プロテーゼの組織係合本体 の外周の周りで対応する開口部を貫通して伸長するように配置され、 前記部材が、前記プロテーゼの対応する部分を所定の軸方向位置にて半径方向 にコンパクトな状態に保つ一方、前記プロテーゼの基端部分が組織に係合し得る ように半径方向に膨張するようにし、 前記開口部に対応する前記プロテーゼの前記部分が、前記カテーテルを基端方 向に動かすことにより、前記所定の位置に略対応する軸方向位置にて前記カテー テルから解放可能であり、前記部材の自由端が前記開口部から除去されるように したことを特徴とするシステム。 17.請求の範囲第16項に記載のシステムにして、 前記プロテーゼが自己膨張型であり、前記プロテーゼの他の部分が半径方向に 膨張した後、前記開口部内の前記細長の部材が、前記プロテーゼの末端をコンパ クトな状態に保ち、 前記部材の基端方向の前記プロテーゼが退却式シースによりコンパクトな状態 に保たれ、 前記プロテーゼが、前記シースが退却したとき、顕著な摩擦力にてシースに係 合し、前記プロテーゼを張力状態下に置くようにしたことを特徴とするシステム 。 18.自己膨張型プロテーゼを体内に配置するシステムにして、 基端及び末端と、組織に係合するその間の本体とを有するプロテーゼを備え、 プロテーゼを半径方向にコンパクトな形態にて支持する部分を有するカテーテ ルと、 前記プロテーゼを支持する前記カテーテルの前記部分が、前記プロテーゼの末 端に係合して、プロテーゼの対応する部分を前記カテーテルに関して所定の軸方 向位置に保つように配置された部材を備える一方、前記プロテーゼの他の部分が 半径方向に膨張して組織に係合し得るようにされ、 前記プロテーゼがコンパクトな形態であるとき、前記プロテーゼの上方に亙っ て配置され且つ該プロテーゼと接触する退却型シースと、 前記シースを退却させて前記プロテーゼを露出させる間に、前記プロテーゼに 軸方向力を付与して前記シースと前記プロテーゼとの間の摩擦力を軽減する緊張 要素とを備えることを特徴とするシステム。 19.請求の範囲第18項に記載のシステムにして、前記シースが、前記プロテ ーゼの半径方向への膨張力に抗して、前記プロテーゼの部分をコンパクトな状態 に保つ拘束式シースであり、 前記緊張要素が、前記シースにより形成され、前記シースが退却したとき、前 記プロテーゼにより顕著な摩擦力にて係合されて、前記プロテーゼを張力状態下 に置くようにしたことを特徴とするシステム。 20.自己膨張型プロテーゼを体内に配置するシステムにして、 請求の範囲第16項によるシステムを提供する段階と、 該プロテーゼの所望の末端方向への伸長距離に対応する内腔壁の軸方向位置に 略隣接する位置に、前記プロテーゼの末端が配置された、前記コンパクトな形態 にて前記システムを体腔内に配置する段階と、 前記末端の基端方向の前記プロテーゼ部分を膨張させて、前記内腔壁に係合さ せる段階と、 前記部分の自由端が前記開口部から除去されるように、前記カテーテルを基端 方向に引き出す段階と、を備えることを特徴とする方法。
