【発明の詳細な説明】 【産業上の利用分野】
本発明は、骨への固定装置を備えた股関節用ソケット補
綴具に関する。
【従来技術およびその課題】
例えば、金属器（ドイツ公開公報第２．９５０．５３６
号）の据え付は部の外周に形成された螺子のような、ソ
ケット補綴具をセメントを用いないで骨内に固定するた
めの従来の周知装置は、その補綴具の外周面全面を隙間
なく覆って該補綴具の周囲に存在する骨組織内へ直接に
噛み込めることが要求される。万一、再手術するような
場合には、ソケットの挿入面がもはや解剖学的に円形で
はなく楕円形となるように、ソケットの頭部が置き換え
られるへき位置へ置き換えられることによって寛骨臼の
開口部が拡大されてしまうので、本来の寛骨臼の凹状球
面形状は、くぼんだ形状が楕円形に変形されたソケ、７
　）の挿入面を下方へ広がるように変化させ、新規な寛
骨臼ソケットを安定して装着するには、どんな場合にも
問題が生じる。そして円形が修復されるまで寛骨臼の外
側へさらにドリリングを行うことにより、半球状凹部が
楕円状拡張の最も大きな寸法にまで適応させなければな
らず、さらにはこの半球状凹部がその窪んだ形状をも含
まなければならない。その長軸方向で、ソケットの挿入
面の最も大きな拡張方向へその窪んだ形状を含むことは
、この方向には必要な骨の実体が欠落しているので、最
も不可能なことである。正常な腸骨翼はその必要な拡張
を備えておらず、特に腹面の方向には、そのような処置
は腹面の方向の有効な領域を越えて行き、完全な腸骨翼
を尾部の方から頭部の方へ傷付けて、それを駄目にして
しまう。極端により多くの骨を失うことは、そのような
処置においてより多くの不利益になると見なされる。そ
の腸骨翼は、それによって安定性をもっと失い、同様に
、将来に必要とされる寸法もそれによってかなり制限さ
れる。 そのような場合には、度々の処置が空洞部分も含めて楕
円形状のソケットの「欠き」を骨セメントで満たし、且
つ人造ヒップソケットをもう一度埋め込む。そのような
処置において骨セメントは、骨の復元を妨げるばかりで
はなく、実際に証明されているように、さらに破壊へ導
く。同時に、達成され得る安定性がまさに制限されるこ
とになる。 さらに周知の処置は、寛骨臼の欠けている領域内へ各腸
する人造ヒップソケットを受は入れるための半身の貝殻
のような凹形状を呈するとともに、尾部方向および頭部
方向の縁に金属製クリップが取り付けられ、且つ骨螺子
によって腸骨翼に固定される金属製の埋め込み物を適用
することである。 可能な固定における安定性は、この場合においても問題
である。その中で骨の復元と同時に起こる人工ヒップソ
ケットの新しい接合を達成するために、空洞部分も含め
て楕円形状の「欠き」を白質もしくは同質の骨材でで満
たすような試みも時々行われる。 勿論、この処置に伴って、達成し得る元来の安定性は極
めて不確かなものとなり、たとえ患者が長期間動かなく
とも、残っている成功の可能性は極めて疑わしい。
【発明の目的】
本発明は上述のごとき従来技術の課題に鑑み、これを有
効に解決すべ（創案されたものである。 したがって本発明の目的は、十分な固定を確かなものに
するとともに、骨の復元を阻害しない股関節用ソケット
補綴具を提供することにある。
【発明の開示］ 本発明に係る股関節用ソケット補綴具は、上述のごとき
従来技術の課題を解決し、その目的を達成するために以
下のような構成を備えている。 即ち、骨の中で該補綴具を支持するために、ソケットの
表面から突出し、且つ空洞部分を完全に満たしてしまう
ことなく、ソケットと骨の組織との間のスペースを橋渡
し、その結果、その中での骨の自然な復元が徐々に許容
され、且つその骨の組織自体もその補綴具の支持に関与
し得るような、少なくとも一つの支持部材を備えている
。 支持部材は、ソケットへ挿入された後に、ソケットの内
側方向からの操作によってソケットの後方（外側）から
突出し、それ以降はもはや動かせなくするのに好都合な
ようにデザインされている。この目的のために、ソケッ
トおよび支持部材は互いに協働して保持装置を構成しな
ければならず、この保持装置は、その後の支持部材の転
移と固定力の変換を許容する。例えばバヨネット保持装
置のような、他の構成に加えて、螺子付き穴としての補
綴具側の保持装置のデザインは、上記螺子付き穴に対応
する螺子が少なくともソケット側端部に形成されたピン
としてデザインされた支持部材による連接を有する本発
明においては特に好適である。支持部材は、既にソケッ
トに挿入されて保持装置内に存在していてもよい。保持
装置もしくはソケット内の螺子付き穴が多数設けられて
いることは更に好都合であって、ソケットの挿入後に、
保持装置は支持のために必要な最も好ましい位置を選択
できる。そのような多数の保持装置は以下のような利点
もある。即ち、一般的には１本の支持部材では不十分で
あるが、それを幾本か用いることによって有効に作用さ
せ得る。 ソケットの内側から保持装置の開口に挿通されるピンと
しての支持部材のデザインは、その断面の大きさを制限
し、そのようにして骨の表面上で有効な支持面積の大き
さを制限する。もし大きな支持面積が必要な場合には、
ソケットの埋め込みの前にでも、骨の表面の適当な箇所
に、若しくは支持部材が進行して行ってその後方で骨の
表面に接触するように、保持装置がそのように用いられ
て有効な領域内にあるソケットの外側の支持部材の管側
端部に、それに固定される支持プレートを取り付けるこ
とも可能である。 支持部材のできるだけ大きな支持面積を得るために、そ
の後方は管側端部において中実にデザインされてもよい
。併し乍らそれは、骨螺子が通るのをガイドするために
、中空の支持部材を用いるのに有利である。 ソケットの挿入後、手術する外科医は利用する保持装置
のそれぞれを決定し、深さゲージを用いて骨の表面まで
の距離、選択すべき支持部材の長さを決定する。もしこ
れを回避する必要がある場合には、それぞれが互いにテ
レスコピックに螺合される各スリーブ部材よりなる、本
発明に係る支持部材を用いることも可能である。これら
テレスコピックに螺合された各スリーブ部材の他の特徴
によれば、縮められた状態では、螺子付き穴を備えるソ
ケット部分の厚さ寸法よりも実質的には短い共通の長さ
寸法となり、そのことによってソケットの挿入時に既に
その中に含んでおき、ソケットと骨との間の各距離に応
じてこれらを用いることも可能である。 【実施例】 以下に本発明の好適な一実施例にづいて、第１図ないし
第１１図を参照して説明する。第１図および第２図に示
す股関節の部分でスマッジングを施して示した領域にお
いて、図示の補綴具部分ｌは、図示しないプラスチック
挿入物によって補われたソケット補綴具の金、属製据え
付は部を形成し、その表面−面には螺子として形成され
た係止突起部２が配置されているが、再手術により新た
に装着されるソケット補綴具が、領域３および４の部分
で上記補綴具部分ｌが横方向へ骨組織まで直接に達する
範囲内で、患者の頭蓋の方へ移動することによって、寛
骨臼が大略楕円状に広げられており、このことは、セメ
ントを用いない固定の場合には不適当である。 上述のことは第２図および第３図に総て認められ、据え
付は部には、種々の法則に基づいて表面−面に規則的に
配置された多数の螺子穴５が形成されている。この螺子
穴は、ソケット軸に関して共通な平面内に位置して、そ
の中心軸が関節のほぼ中間点に向かうような角度に配置
されている。 ソケット軸に含まれるその角度は、開口に近い方の列で
は約５０°から９０°であり、開口から遠い方の列では
約３５°から５０’である。どんな場合でも６から１０
の、例えば８個の穴がその表面−面の各列に配置されて
いる。全体的には、支持部材の角度はソケット軸に対し
て３５°から９０’の範囲に設定されるのが良い。 支持部材としては頭なしのピン部材が与えられており、
第４図から第７図にかけて示すようにこれらのピン部材
は中空にも中実にもでき、種々の長さにされてその一端
には溝６や、あるいは回転道具のアタッチメントのため
の６角形のソケットもしくはその種の他のものを備えて
いる。第２図に示すように、三つの支持部材７は、どん
な場合にも補綴具の上下へ用いられ、一方では、それら
が骨の表面８にまで突出してその後ろ側へ僅かに押され
るように、且つ他方では、内側へ突出はするがその挿入
物を受は入れるために備えられた表面９を超えないよう
に、その支持部材の長さは選定される。 第２図に示すように、支持部材は骨の表面に対してその
他の補助なしに凭れ掛かってそこに固定されており、第
１図に見られる支持部材のうちの一つにあっては、中空
支持部材の内部を通って骨螺子１０が設けられている。 他の二つの支持部材にあっては、骨に作用する面圧を小
さくするために支持プレート１１が設けられており、そ
の支持プレートは第８図および第９図に示されたタイプ
のものであって、骨の表面８と支持部材７の間に単に挟
まれることによって保持されている。これらに替わって
、支持部材への接続を確実にするために特別な接続部材
を用いることも可能である。 補綴具と骨の表面８との間の空洞部１２は、この領域に
おける骨組織の復元を促すための骨の材質で裏打ちされ
ており、この骨組織は、成長の速度に依存するが、数週
間から数カ月後の補綴具に対する強度への変換に関係し
ている。支持部材は、その時までに本来の機能を確かな
ものにし、且つその安定性を高める。 テレスコピックに構成された支持部材は、第１０図およ
び第１１図に示すように、数個のスリーブ部材１３，１
４，１５．１６から構成され、そのうちの最も長いスリ
ーブ部材１３はソケットの螺子付き穴に螺合する雄螺子
と、次に長いスリーブ部材１４の雄螺子に螺合する雌螺
子を備えており、このようにして最も短いスリーブ部材
１６まで順次接続される。最終のスリーブ部材１６は、
雌螺子は持っていないが、骨螺子を、そして回転工具の
６角形ソケツトをも受は入れるための穴を備えている。 二つの大きなスリーブ部材は、それぞれの内側で、第１
０図中右側の端部に雌螺子付きカラー１７を有する一方
、他方端の外側で雄螺子付きカラー１８を有しており、
それぞれ次に大きなスリーブ部材からそれよりも外側へ
はざらに螺進できないように、互いのカラーが協ｍする
ように構成されている。 最初の段階で、全スリーブ部材は、これらを受は入れる
補綴具の穴５の曇さよりも十分に短く、図示の各スリー
ブ部材の長さに相当する全長骨だけの長さとなるように
、他のひとつの内側に配置される。そして、適当な工具
を用いることによって、スリーブ部材１６が回転させら
れ、骨の表面８に到達するまで矢印（第１０図）の方向
へ螺進する。 ボルト７およびテレスピック支持部材１３〜１６のため
に適当な周知の装置によって、これらを回転可能な位置
に到達させて固定できる。 補綴具部分１の固定後、関節の表面を形成する挿入部材
は挿入される。 支持部材のための保持装置５を形成する補綴具部分は、
好ましいどんなタイプにもできる。併し乍ら、本発明の
典型的な実施例から見られるように、本発明は、特に保
持装置および特別な挿入物を含んだ堅い材質の据え付は
部分１を備えたソケット補綴具に用いられるのに好都合
である。 実施例において支持部材は、補綴具の表面と骨の表面と
の間の空間を橋渡しするように用いられている。また、
それら支持部材はその空間が存在しない場合でも用いる
ことができる。即ち、骨の中により良い固定を行うのに
簡単であり、この接続において支持部材は、その端部を
鋭く尖らせることができるのであるが、骨螺子というか
たちで張力に変換させることなく、骨の表面に僅かに入

補綴具を確実に固定できる。

におけるテレスコピック支持部材の各部分解断面図およ

Claims (8)

    【特許請求の範囲】
  1. （１）．骨への固定装置を備えた股関節用ソケット補綴
    具において、 骨（８）の中でソケット（１）を支持するために、ソケ
    ット（１）の表面から突出して該ソケット（１）と骨と
    の間の空間を橋渡しするための少なくとも一つの支持部
    材（７，１３〜１６）を備えたことを特徴とする股関節
    用ソケット補綴具。
  2. （２）．上記支持部材（７，１３〜１６）は、上記ソケ
    ット（１）の内側から外側へ伸長できる請求項１記載の
    股関節用ソケット補綴具。
  3. （３）．上記ソケット（１）を保持する保持装置は、該
    ソケット（１）に形成された螺子付き穴（５）と上記支
    持部材（７，１３〜１６）により構成され、該支持部材
    （７，１３〜１６）は、少なくとも上記ソケット（１）
    側の端部に、雄螺子が螺刻されたピン部材である請求項
    ２記載の股関節用ソケット補綴具。
  4. （４）．上記螺子付き穴（５）は、多数設けられている
    請求項３記載の股関節用ソケット補綴具。
  5. （５）．上記支持部材（７，１３〜１６）は、その骨側
    端部に支持プレート（１１）が設けられている請求項１
    ないし４のいずれかに記載の股関節用ソケット補綴具。
  6. （６）．上記支持部材（７，１３〜１６）は、骨螺子（
    １０）を収容するために中空にされている請求項１ない
    し５のいずれかに記載の股関節用ソケット補綴具。
  7. （７）．上記支持部材は、互いにテレスコピックに螺合
    できるスリーブ部材（１３〜１６）を含む請求項１ない
    し６のいずれかに記載の股関節用ソケット補綴具。
  8. （８）．上記テレスコピック・スリーブ部材（１３〜１
    ６）は、その収縮状態において、上記螺子付き穴（５）
    を有するソケット部分（１）の厚さ寸法以下の共通な長
    さ寸法を有する請求項７記載の股関節用ソケット捕綴具
    。
