Description

組織を保護する方法であって、動脈の内腔内の位置に流体を導入することであって、位置は動脈内の閉塞よりも下流にあり、流体は動脈内の血液よりも低温である、導入することを含む方法。
導入することの前に、閉塞よりも上流の位置から閉塞よりも下流の位置まで、長尺部材の一端を内腔内に通すことを含む、方法1に記載の方法。
長尺部材は誘導ワイヤを備える、方法2に記載の方法。
流体を導入することは、閉塞よりも下流の位置に配置された遠位端を有するカテーテルを通す、方法2又は3に記載の方法。
長尺部材の一端を、閉塞の周辺を通過させることによって、閉塞よりも下流の位置まで通す、方法2、3、又は4に記載の方法。
さらに、下流の位置に導入された流体が閉塞よりも上流に流れることを制限する膨張可能な要素を膨張させることを含む、方法2乃至5のいずれか1つに記載の方法。
カテーテルは、流体が下流の位置に導入されるまで、流体を低温に維持する熱絶縁要素を備える、方法4乃至5のいずれか1つに記載の方法。
閉塞は血栓を含み、さらに、動脈から血栓の少なくとも一部分を取り出すことを含む、方法1乃至7のいずれか1つに記載の方法。
流体は、約2℃~約35℃の温度を有する、方法1乃至8のいずれか1つに記載の方法。

流体は、約15℃の温度を有する、方法1乃至12のいずれか1つに記載の方法。
流体が導入される位置は、内頸動脈(ICA)内、又は内頸動脈の分岐(例えば、MCA、ACA、及びそれらの分岐)内にある、方法1乃至13のいずれか1つに記載の方法。
さらに、血栓が除去された後に、動脈内に冷却流体を導入することを含む、方法1乃至14のいずれか1つに記載の方法。
組織冷却デバイスであって、長尺部材であって、長尺部材を通って遠位ポートまで延伸する流体送達内腔を有し、血管内に延伸するようにサイズ設定及び構成された長尺部材と、熱部材であって、長尺部材の少なくとも一部分に沿って延伸し、流体チャンバであって、使用時に循環する第1の流体が通って流れ、これにより、流体送達内腔を通って流れて遠位ポートに出る第2の流体を冷却する流体チャンバを有する、熱部材と、(i)第1の流体を冷却する冷蔵要素と、(ii)第1の流体を流体チャンバに循環させるポンプとを備える冷却ユニットとを備える組織冷却デバイス。
さらに、長尺部材の遠位部分に結合された膨張可能な部材であって、血管内で膨張して血管を少なくとも部分的に閉塞するように構成された膨張可能な部材を備える、装置1に記載の組織冷却デバイス。
熱部材は膨張可能な部材を備え、循環する第1の流体は膨張可能な部材を膨張させる、装置2に記載の組織冷却デバイス。
Claims (12)
- 組織を保護する方法において使用するための組織冷却デバイスであって、前記組織冷却デバイスは、
長尺部材(300)であって、前記長尺部材(300)を通って遠位ポートまで延伸する流体送達内腔(301)を有し、血管内に延伸するようにサイズ設定及び構成された長尺部材(300)と、
熱部材(303)であって、前記長尺部材(300)の少なくとも一部分に沿って延伸し、流体チャンバであって、使用時に循環する第1の流体が通って流れ、これにより、前記流体送達内腔(301)を通って流れて前記遠位ポートに出る第2の流体を冷却する流体チャンバを有する、熱部材(303)と、
前記組織冷却デバイスの近位領域で前記流体チャンバと流体連通している冷却ユニット(313)であって、(i)前記第1の流体を冷却する冷蔵要素と、(ii)前記第1の流体を前記流体チャンバに循環させるポンプ(315)とを有する冷却ユニット(313)とを備え、
前記流体チャンバは、外側内腔(310)と、前記外側内腔(310)と流体連通する内側内腔(311)とに分割されており、前記内側内腔(311)は、前記長尺部材(300)の長さに沿って延在し、前記外側内腔(310)よりも前記流体送達内腔(301)により近くにあり、
前記組織冷却デバイスは、使用時に、循環する前記第1の流体が、前記冷却ユニット(313)から、近位から遠位への方向に沿って前記外側内腔(310)を通って流れ、次に、遠位から近位への方向で前記内側内腔(311)を通って流れ、前記流体送達内腔(301)と熱的に連通するように構成されており、
前記組織冷却デバイスは、前記遠位ポートが動脈の内腔にあるとき、前記長尺部材(300)の近位端から前記遠位ポートを通して前記第2の流体を導入するように構成されており、前記第2の流体は、前記動脈内の血液よりも低温である、組織冷却デバイス。 - さらに、前記長尺部材(300)の遠位部分に結合された膨張可能な部材であって、前記血管内で膨張して前記血管を少なくとも部分的に閉塞するように構成された膨張可能な部材を備える、請求項1に記載の組織冷却デバイス。
- 前記熱部材(303)は前記膨張可能な部材を備え、前記循環する第1の流体は前記膨張可能な部材を膨張させる、請求項2に記載の組織冷却デバイス。
- 前記長尺部材(300)は、前記閉塞の部分を通って通過する、又は前記閉塞の部分の周辺を通過する貫通要素を備える、請求項1~3のいずれか一項に記載の組織冷却デバイス。
- 前記長尺部材(300)は、前記閉塞と動脈又は静脈の血管壁との間を通過するガイドワイヤを備える、請求項1~4のいずれか一項に記載の組織冷却デバイス。
- 前記長尺部材(300)が、カテーテルであり、前記カテーテルは、前記導入の前に、前記カテーテルの一端が、閉塞よりも上流の位置から前記閉塞よりも下流の位置まで、前記動脈の前記内腔内を通るように構成されている、請求項1に記載の組織冷却デバイス。
- 前記カテーテルはガイドワイヤを備える、請求項6に記載の組織冷却デバイス。
- 前記第2の流体の前記導入は、前記閉塞よりも下流の位置に配置された遠位端を有するカテーテルを通る、請求項6又は7に記載の組織冷却デバイス。
- 前記カテーテルの前記一端が、前記閉塞の周辺を通過し、前記閉塞よりも下流の前記位置まで通る、請求項6~8のいずれか一項に記載の組織冷却デバイス。
- さらに、下流の前記位置に導入された前記第2の流体が前記閉塞よりも上流に流れることを制限する膨張可能な要素を膨張させるように構成されている、請求項7~9のいずれか一項に記載の組織冷却デバイス。
- 前記流体送達内腔(301)は、前記第2の流体が下流の前記位置に導入されるまで前記第2の流体を低温に維持するように熱絶縁されている、請求項8~10のいずれか一項に記載の組織冷却デバイス。
- 前記第2の流体が導入される前記下流の前記位置は、内頸動脈内、又は内頸動脈の分岐内にある、請求項6~11のいずれか一項に記載の組織冷却デバイス。
