Description
本発明は、血管等の身体内腔の完全閉塞または部分閉塞を再疎通するための装置に関する。本発明の装置は、ばね部材である近位セクションと、穿孔部品または穿孔ヘッドである遠位セクションとを備えたカテーテル先端部を含む。装置の閉塞に衝撃を加える部分は振動可能であり、この振動可能部材が穿孔部品である。ばね部材は、オープンコイル(コイルの間に隙間が空いている)部分を有し、穿孔部品は、クローズドコイル(隣接するコイルどうしが相互に接触する)部分を備えた巻線またはばねである。例えば、ばね部材は、少なくとも2つの隣接するコイルが隙間の開いた構造を有していてもよい。この新規な二重構造により、カテーテル先端部およびカテーテルのデリバラビリティ(到達性;deliverability)およびクロスアビリティ(通過性;crossability)を改良するため、穿孔に必要な縦方向の十分な硬さと優れた軸方向(横方向)の柔軟性とを併せ持つ穿孔ヘッドが、カテーテル先端部の遠位端に提供され、外部エネルギー源と振動の遠位点との間で機械的なエネルギー損失が起きるという従来技術の問題を回避した、穿孔部品を振動させる機構が提供される。
Claims (26)
- 近位端および遠位端を有するカテーテルと、
近位端および遠位端を有し、当該近位端が前記カテーテルの前記遠位端に取り付けられ、圧縮ばねを含むばね部材と、
前記ばね部材の前記遠位端と連続し、当該遠位端に機能可能に取り付けられ、少なくとも2つの隣接するコイルが隙間なく密着しているばねを含む穿孔部品と、
前記カテーテル内に収容され、近位端と遠位端を有し、当該遠位端が、前記ばね部材および前記穿孔部品からなる群より選択される遠位構造体に取り付けられている引張部材と、
を含むことを特徴とする装置。 - 前記引張部材は、引っ張られて解放されると、前記ばね部材を圧縮および解放して、前記穿孔部品に少なくとも1つの振動を発生させることを特徴とする請求項1に記載の装置。
- 前記穿孔部品は、遠位側にテーパーすることを特徴とする請求項1または2に記載の装置。
- 前記ばね部材は、遠位側にテーパーすることを特徴とする請求項1から3のいずれか一項に記載の装置。
- 前記穿孔部品は、外径が遠位側にテーパーし、内腔の内径が一定または略一定であることを特徴とする請求項1から4のいずれか一項に記載の装置。
- 前記穿孔部品は、外径が第1の割合で遠位側にテーパーし、内腔の内径が前記第1の割合よりも小さい第2の割合で遠位側にテーパーすることを特徴とする請求項1から4のいずれか一項に記載の装置。
- 前記ばね部材は、外径が遠位側にテーパーし、内腔の内径が一定であることを特徴とする請求項1から6のいずれか一項に記載の装置。
- 前記ばね部材は、外径が第1の割合で遠位側にテーパーし、内腔の内径が前記第1の割合よりも小さい第2の割合で遠位側にテーパーすることを特徴とする請求項1から6のいずれか一項に記載の装置。
- 前記引張部材が、前記穿孔部品に取り付けられていることを特徴とする請求項1から8のいずれか一項に記載の装置。
- 前記引張部材が、前記ばね部材に取り付けられていることを特徴とする請求項1から8のいずれか一項に記載の装置。
- 前記ばね部材は圧縮ばねであり、前記引張部材が、前記ばね部材の前記遠位端に取り付けられていることを特徴とする請求項1から8のいずれか一項に記載の装置。
- 前記ばね部材が、つる巻ばね、板ばね、ベローズ、圧縮性ポリマーからなる群より選択され、前記引張部材が、前記ばね部材の前記遠位端に取り付けられていることを特徴とする請求項1から8のいずれか一項に記載の装置。
- 前記引張部材の前記近位端に動作可能に接続され、前記引張部材の引張および解放を繰り返すことで前記穿孔部品を振動させる振動エネルギー源をさらに含むことを特徴とする請求項1から12のいずれか一項に記載の装置。
- 前記振動エネルギー源が、往復部材を有するエンジン、シェーカー、作動装置、ソレノイドからなる群より選択されることを特徴とする請求項13に記載の装置。
- カテーテル固定装置をさらに含むことを特徴とする請求項13または14に記載の装置。
- 請求項13から15のいずれか一項に記載の装置と、
前記振動エネルギー源を制御するように構成された制御ユニットと、
を含むことを特徴とするシステム。 - 前記振動は、少なくとも1つの振動数と少なくとも1つの振幅を含み、前記少なくとも1つの振動数および少なくとも1つの振幅は、前記制御ユニットを介して独立して調節可能であることを特徴とする請求項16に記載のシステム。
- 前記振動は、少なくとも1つの振動数と少なくとも1つの振幅を含み、前記システムは、前記振動数または振幅の制御を支援するセンサをさらに含み、前記制御ユニットは、前記センサに機能可能に接続されたプロセッサをさらに含み、前記プロセッサは、前記センサからの入力を分析することができることを特徴とする請求項16または17に記載のシステム。
- 前記制御ユニットは、オペレータインタフェースユニットをさらに含むことを特徴とする請求項16から18のいずれか一項に記載のシステム。
- 前記カテーテルが湾曲している場合、引張部材経路長の変動を補償する張力調整機構をさらに含むことを特徴とする請求項16から19のいずれか一項に記載のシステム。
- 隙間なく密着している前記少なくとも2つの隣接するコイルは、前記穿孔部品の最遠位端に位置することを特徴とする請求項1に記載の装置。
- 前記振動エネルギー源は、前記カテーテルの外側に位置することを特徴とする請求項13に記載の装置。
- 前記穿孔部品は、近位端および遠位端を有し、前記穿孔部品の前記遠位端の遠位縁までテーパすることを特徴とする請求項3に記載の装置。
- 前記穿孔部品は、恒久的に前記ばね部材に取り付けられることを特徴とする請求項1に記載の装置。
- 前記引張部材は、可撓性のひもであることを特徴とする請求項1に記載の装置。
- 前記引張部材は、高張力ポリマーであることを特徴とする請求項25に記載の装置。
