Description
Claims (17)
- 患者の乳房のトモシンセシス撮像を利用してバイオプシニードル位置特定を容易にするシステムであって、
X線撮像のために患者の乳房を支持するように構成された乳房プラットフォームと、
バイオプシニードルを保持するニードル組立体と、
トモシンセシス画像形成ユニットであって、
トモシンセシススキャンの過程でX線源を所定の経路に沿った複数の撮像位置を通って移動させ、前記複数の撮像位置で撮像用X線ビームを放出させ、前記患者の乳房を通過した前記撮像用X線ビームが照射されるX線画像レセプタが取得した画像データを受け取るように構成され、
さらに、前記画像データを計算処理することで、トモシンセシス画像として、(i)前記乳房プラットフォームに対し異なった角度で取られた前記乳房の複数のトモシンセシス投影画像、および(ii)それぞれが前記患者の乳房のスライスを表す複数の再構築されたトモシンセシススライス画像を得るように構成され、
さらに、前記ニードル組立体の少なくとも一部が前記X線源の一部の撮像位置と前記患者の乳房との間に位置することによる前記トモシンセシス画像への影響に、前記トモシンセシス画像の計算において前記一部の撮像位置についての画像データの使用が削減または回避されるようにすることによって対処するよう構成された、
トモシンセシス画像形成ユニットと、
前記トモシンセシス画像形成ユニットから前記トモシンセシス画像を受け取って前記トモシンセシス投影画像および前記再構築されたトモシンセシススライス画像を表示し、撮像された前記トモシンセシス画像の少なくとも1つにおける前記患者の乳房内の対象エリアの識別情報を受け取り、前記乳房内の前記識別された対象エリアの3次元座標を決定するように構成されたユーザインタフェースと、
前記ニードル組立体を移動するために、前記ニードル組立体に結合する位置決めデバイスと、
バイオプシコントローラと、
を備え、
前記バイオプシニードルは、前記乳房内の前記対象エリアの前記3次元座標に基づいて位置決めされ、
前記位置決めデバイスは電動式であり、
前記バイオプシコントローラは、前記位置決めデバイスを制御して、前記ユーザインタフェースによって決定された前記3次元座標に基づいて前記ニードル組立体を特定の位置に移動させることが可能であり、前記トモシンセシススキャンの最初のスキャンの前に、前記ニードル組立体が前記トモシンセシススキャン中に前記対象エリア内に影を落とさない非干渉位置に前記ニードル組立体を移動させることを特徴とするシステム。 - 前記画像形成ユニットは、前記トモシンセシススキャン中に、前記ニードル組立体が前記対象エリア内に影を落とさないX線源の画像位置から得られた画像データのみを処理して前記トモシンセシス画像とするよう構成されていることを特徴とする請求項1に記載のシステム。
- 前記患者の乳房内における前記対象エリアの前記識別情報は、前記トモシンセシス画像上のマークとして記録され、前記マークは、前記患者の乳房の1つまたは複数の後続の画像上の対応するロケーションに自動的に設置されることを特徴とする請求項1に記載のシステム。
- 前記トモシンセシスコントローラは、前記X線源およびトモシンセシス画像再構築手段を制御して、前記バイオプシニードルのファイア前に、前記対象エリアに対するニードルロケーションを確認するために前記患者の乳房の1つまたは複数のプレファイア画像を取得することを特徴とする請求項1に記載のシステム。
- 前記トモシンセシスコントローラは、前記X線源およびトモシンセシス画像再構築手段を制御して、前記バイオプシニードルのファイア後で、かつ、前記患者の乳房の組織がサンプリングされる前に、適切なニードルロケーションを確認するためにポストファイア画像を取得することを特徴とする請求項1に記載のシステム。
- 前記バイオプシニードルは実質的にX線透過性であり、前記ニードル先端はX線不透過性であることを特徴とする請求項1に記載のシステム。
- 前記ニードル組立体は導入器をさらに含み、前記導入器はX線透過性であり、X線不透過性バンドを前記導入器の遠端に有することを特徴とする請求項1に記載のシステム。
- 前記ニードル組立体の本体部はX線透過性であり、前記ニードル先端はX線不透過性であることを特徴とする請求項1に記載のシステム。
- オペレータは、前記位置決めデバイスを利用して、前記ニードル組立体を手作業で位置決めすることを特徴とする請求項1に記載のシステム。
- 前記対象エリアの前記3次元座標は、前記ユーザインタフェースを介して表示され、ニードル先端座標は、前記バイオプシニードルが手作業で位置決めされるにつれて、前記ユーザインタフェースを介して動的に表示されることを特徴とする請求項9に記載のシステム。
- 前記位置決めデバイスに機械的にまたは電気的に結合した手操作式デバイスをさらに備え、
オペレータは、前記手操作式デバイスを手作業で操作して、前記ニードル組立体を位置決めするように前記位置決めデバイスを制御することを特徴とする請求項9に記載のシステム。 - 前記バイオプシニードルによって取得されたサンプルを収集するように構成された収集チャンバと、
前記バイオプシニードルと前記収集チャンバとの間のサンプル送出経路内の自動化された石灰化検出器とをさらに備え、
前記自動化された石灰化検出器は、前記サンプルが石灰化を含むか否かを判定することを特徴とする請求項1に記載のシステム。 - 前記自動化された石灰化検出器は、前記石灰化の量を推定することを特徴とする請求項12に記載のシステム。
- 麻酔薬および/または造影剤を導入するための、前記ニードル組立体に結合する付加的なラインをさらに備え、
前記麻酔薬および/または造影剤の前記導入は、前記撮像シーケンスに同期することを特徴とする請求項1に記載のシステム。 - 前記画像再構築は、フィルター逆投影を使用して実施されることを特徴とする請求項1に記載のシステム。
- 前記画像再構築は、アーチファクト低減法を使用することを特徴とする請求項1に記載のシステム。
- 乳房バイオプシを実施する装置であって、
請求項1に記載のシステムと、
患者の乳房のサンプルを取得するように構成されたバイオプシニードルと、
サンプル送出経路を介して前記バイオプシニードルに結合した収集チャンバと、
前記サンプルが石灰化を含むかどうかを判定し、前記石灰化の量を推定するための、前記サンプル送出経路に結合した石灰化検出器とを備えることを特徴とする装置。
