Description
本発明は一般に医療処置の分野に関し、特に、患者の大脳動脈からの血栓塞栓の除去に関連する虚血性脳梗塞治療のためのシステム及び方法に関する。
脳梗塞は死亡び身体障害をもたらすので、保険医療にとっては世界的規模でますます深刻な問題となっている。アメリカだけでも毎年700,000人を越える者が重度の脳梗塞症を被っており、そのうち死亡する者は150,000人を上回る。更に憂慮すべきことには、この既存の厄介な状況は、「ベビーブーム」に生まれた人口世代が高齢になるにつれてますます深刻化することが予期されており、これは特に、質の悪い食生活や肥満及び/又は脳梗塞をもたらす他の要因に悩まされる人口に顕著である。脳梗塞後に存命した者の約90%は、運動、感覚、記憶又は論理的思考力について軽度から重度に及ぶ長期間に亘る障害を伴う。そのアメリカの医療保健制度に対する総コストは年間50億米ドルを越えるものと推定されている。
以下、本発明の例示的な実施例について説明する。明瞭化のために、本明細書では実際の実施の必ずしも全ての特徴については述べてはいない。勿論、そのような実際の実施例の発展においては、開発者の所定の目的を達成するために、実装時に固有の様々な決定がなされるのであって、例えばシステム関連及びビジネス関連の制約により整合を図ることは個々の実施によって異なる。更に、このような開発努力は複雑な上に多大な時間を要するにも拘らず、本開示事項の利点を享受する当業者には定型的な業務であることに留意されたい。本明細書に開示した血栓塞栓除去システムは、個々でも組み合わせでも特許で保護されるべき様々な独創的な特徴及び構成要素を備えている。
図1は血栓塞栓除去システム10の例示的な実施例を示す。血栓塞栓除去システム10は、案内及び咬合カテーテル12、搬送及び吸引カテーテル14、血栓塞栓細断器又は分離器16、及び吸引ポンプ18を含んでいる。以下に詳細に説明するように、血栓塞栓除去システム10は、従来技術の欠点及び制約を克服しつつ、有益にも患者の大脳動脈から血栓塞栓を除去する能力を与える。
血栓塞栓除去システム10を使用する方法について図12乃至28を参照して説明する。図12及び13に概略的に示すように、第1の例示的な方法では、血栓塞栓除去システム10が例えばSeldinger法を介して患者の脈管へ導入されている。図14はこの処置の第1段階を示し、これは案内ワイヤ104を血栓塞栓100に対して基端点へ前進させることに関する。案内ワイヤ104は多数の商業的に市販されている案内ワイヤからなり、その操作は当技術分野では良く知られている。しかしながら、一つの方法では、分離器16の細長い部材56(図11B)が案内ワイヤ104として働く。
Claims (14)
- 脳血管から血栓塞栓物質を除去するためのシステムであって、
末端及び一定の内径を有して内部に延伸する内腔を有し、且つ脳血管内へ挿入されるように寸法付けられた細長いカテーテルと、
前記内腔に流体接続された吸引源と、
前記内腔を通じて延伸可能であり、且つ末端部分とこの末端部分上の幅広くされた分離要素とを含む細長い部材とを備え、前記分離要素は円錐状の中実部材を含み、この中実部材は一定の最大径部分を有し、その最大径は前記内腔の前記一定の内径よりも小さく、前記分離要素の前記最大径部分と前記一定の内径との間の許容差は緊密であり、前記分離要素は、前記吸引源を用いる吸引中に前記内腔に対して前進可能且つ後退可能であるシステム。 - 請求項1に記載のシステムにおいて、前記吸引源を用いる吸引期間中に、前記分離要素は前記内腔に対して前進及び後退可能であるシステム。
- 請求項1に記載のシステムにおいて、前記分離要素の最大径は約0.0762mm乃至0.2032mm(0.003乃至0.008インチ)であり、前記内腔末端の前記内径よりも小さいシステム。
- 請求項1に記載のシステムにおいて、前記カテーテルは約0.8128mm乃至1.651mm(約 0.032乃至0.065インチ)の範囲の外径を有するシステム。
- 請求項1に記載のシステムにおいて、前記分離要素の最大径は、固定された径であるシステム。
- 請求項5に記載のシステムにおいて、前記分離要素は、基端方向へ対面する面を有する基端部分と、テーパー状の末端部分とを含むシステム。
- 請求項6に記載のシステムにおいて、前記基端方向へ対面する面が凹面であるシステム。

- 請求項1に記載のシステムにおいて、前記細長い部材は可撓な末端チップを含み、前記分離要素は前記末端チップから基端方向へ離間しているシステム。
- 請求項1に記載のシステムにおいて、前記内腔へ挿通されて拡張可能な細長いシャフト上で拡張可能な収容要素を更に備え、この収容要素は血管内で血栓塞栓物質に接触するように前進可能であり、且つ少なくとも部分的には血栓塞栓物質を潰すように該血栓塞栓物質に係合するシステム。
- 請求項11に記載のシステムにおいて、前記収容要素は中央内腔を有するスリーブを形成するように配置された複数の構造部材からなり、この複数の構造部材は前記内腔へ延出する係合要素を含み、この係合要素は、前記中央内腔へ延伸する頂点領域を有し、且つ前記複数の構造部材は、長軸方向に向き付けられた頂点を有するV字形状の複数の要素を備えると共に、前記係合要素はこの複数のV字形状要素からなるシステム。
- 請求項1に記載のシステムにおいて、前記細長い部材の前記末端部分は末端チップを含み、前記分離要素は、前記末端チップから基端方向へ離間した固定位置において、前記細長い部材上に位置しているシステム。
- 請求項13に記載のシステムにおいて、前記分離要素は、最遠位の要素であり、且つ基端方向へ対面する凸状壁と、テーパー状の分離要素チップを形成するように末端方向へ対面する円錐状の壁とを有し、前記テーパー状の分離要素チップは、前記細長い部材の前記末端チップから基端方向へ離間しているシステム。
