Description
本発明は患者の1つもしくはそれ以上の体組織構造の大きさおよび/または形状を変更するなどの体組織リモデリングに関連する。本件で詳細に記述した例のほとんどは心組織構造のリモデリングに関連するものであるが、本発明が患者の他の比較的軟らかい(すなわち非骨)組織構造のリモデリングにも適用可能であることが理解されるであろう。
本発明の本目的および他の目的は、本発明の原則に従い達成され、少なくとも2つのアンカー構造を患者の体組織に埋め込み、該2つのアンカー構造(および周囲組織)をアンカー構造間に延出する結合もしくはシンチング構造を使用して互いの方向に引き合う方法および/または装置を提供することである。これによりアンカー構造および周囲組織間の距離を短縮し、所望の組織リモデリングを提供する。
図1は本発明の第1の例に従い治療した心臓10を示している。図1に示した心構造は、左心房20、右心房30、冠状静脈洞40である。冠状静脈洞40の心門42は右心房30の内部に連通している。左心房20の底部は僧帽弁50である。僧帽弁50は前尖52、および後尖54(3つの区分P1、P2、P3を持つ)を含む。後尖区分P3は冠状静脈洞40の心門42の最も近くに位置している。後尖区分P2は冠状静脈洞40に沿って1つ分心門42より離れている。また後尖区分P1は僧帽弁50の輪56に沿って心門42よりさらに離れている。弁尖52および54(すなわち区分P1〜P3)の底部は僧帽弁輪56に接合している。弁尖52および54の底部は互いに交連部58a(冠状静脈洞40の心門42近く)および反対の交連部58b(心門42から離れた部分)の極めて周辺にある。冠状静脈洞40は僧帽弁輪56の水平面に近い水平面で左心房20の後部外側に沿って心門42から大きく延出している。(僧帽弁輪56は実際には鞍部に似た形状をしている傾向があり、そのため幾何学的な平面ではない。しかし上述した内容を簡略化するため、一般的には僧帽弁輪56は平面に近い平均的な水平面と仮定されることがある。僧帽弁輪の「上」もしくは「下」、もしくは僧帽弁輪の水平面もしくは該水平面として参照される他の構造は、輪の略平面における平均的な水平面の上もしくは下を表していることが理解されよう。)
Claims (22)
- 患者の僧帽弁の周辺部を短縮することに適した装置であって、
患者の心臓の冠状静脈洞の少なくとも一部を介した経皮導入および患者の組織への固定に適したらせんからなる第1のアンカー構造と、
患者への経皮導入と、第2のアンカー構造の第1のアンカー構造と連通する位置での患者の組織への固定とに適した第2のアンカー構造と、
第1のアンカー構造および第2のアンカー構造の間に延出することに適し、長さを調節することによって前記第1のアンカー構造および前記第2のアンカー構造が患者の組織に固定されている位置と位置との間の距離を短縮する可撓性結合構造と
を備える装置であって、
前記第1のアンカー構造および前記第2のアンカー構造は、らせんと、前記らせんから外側に向かって突出して、患者の組織内に向かってねじ込まれる方向とは反対の後ろ向きに傾斜している複数のとげとを備えてなる装置。 - 患者の心臓の冠状静脈洞への第1のアンカー構造の経皮導入に適した手段をさらに備える請求項1記載の装置。
- 第1のアンカー構造を患者の組織に固定するための第1のアンカー構造の経皮的操作に適した手段をさらに備える請求項2記載の装置。
- 患者への第2のアンカー構造の経皮導入に適した手段をさらに備える請求項1記載の装置。
- 第2のアンカー構造を患者の組織に固定するための第2のアンカー構造の経皮的操作に適した手段をさらに備える請求項4記載の装置。
- 結合構造の経皮的操作を行って結合構造の長さを調節する手段をさらに備える請求項1記載の装置。
- 長さ調節の後、結合構造が選択可能に嵌合可能である請求項1記載の装置。
- 結合構造が嵌合後に選択可能に開放可能でもあるため、新しい所望の長さへの調節も許容する請求項7記載の装置。
- 第1のアンカー構造は、拡大して周囲組織の組織輪と輪状に嵌合することに適した部分を備える請求項1記載の装置。
- 第2のアンカー構造は、拡大して周囲組織の組織輪と輪状に嵌合することに適した部分を備える請求項1記載の装置。
- 第1のアンカー構造は組織を貫通することに適した部分を備える請求項1記載の装置。
- 第2のアンカー構造は組織を貫通することに適した部分を備える請求項1記載の装置。
- 該部分が組織を通り抜けることに適した請求項11記載の装置。
- 該部分が組織を通り抜けることに適した請求項12記載の装置。
- 結合構造が第1および第2のアンカー構造からそれぞれ延出する第1および第2の可撓性部材を備える請求項1記載の装置。
- 結合構造が第1および第2の可撓性部材を嵌合し第1および第2の可撓性部材の少なくとも一方に沿って移動することに適した嵌合構造をさらに備える請求項15記載の装置。
- 嵌合構造が、可撓性部材の少なくとも一方に沿って、可撓性部材が延出するアンカー構造に向けて移動し、可撓性部材に沿った反対向きの移動に抗することに適した請求項16記載の装置。
- 嵌合構造が、反対方向への移動を抗しない選択的操作に適した請求項17記載の装置。
- 前記可撓性部材がらせん型のものであり、らせん上にとげが設けられており、前記とげはらせんから突出しらせんが組織に向かってねじ込まれる方向とは反対の後ろ向きに傾斜している請求項15乃至18記載の装置。
- 結合構造が第1および第2のアンカー構造にそれぞれ第1および第2の相補的な内部嵌合のラチェット構造を備える請求項1記載の装置。
- ラチェット構造が、第1および第2の支持構造が互いの方向に移動することを許容するが反対方向への移動に抗するよう構成された請求項20記載の装置。
- ラチェット構造が、反対方向への移動に抗しない選択的操作に適した請求項21記載の装置。
