Description
本発明の一特徴によれば、自己拡張性ステントの運搬システム用のプッシャ組立体であって、プッシャ組立体が、端方向圧縮力を受ける近位プッシャ端部及び運搬されるべきステントに端方向圧縮力を送る遠位プッシャ端部を備えたカテーテルシャフトを構成し、プッシャ組立体が、近位プッシャ端部から、近位プッシャ端部よりも遠位プッシャ端部の近くに位置する遠位ストランド端部まで延びるプッシャストランドと、使用中、ステントに当接して力をステントに送るプッシャ要素とを有し、プッシャ組立体が更に、近位端部及び遠位端部を備えた移送シャフトを有し、近位端部が、遠位管端部に連結され、遠位端部が、プッシャ要素に連結されているプッシャ組立体において、プッシャ要素は、ガイドワイヤ経路を構成し、移送シャフトは、ガイドワイヤ経路の一方の側に位置していることを特徴とするプッシャ組立体が提供される。
図2では、ガイドワイヤ2が、運搬システムの遠位部分の遠位端部を越えて延び、ステント運搬システムをこのガイドワイヤに沿って患者の体内の狭窄の部位まで前進させる。ステント6は、外側シース4によって半径方向圧縮状態に保持され、外側シースの遠位端部は、ステント運搬システムの遠位端部を構成している。図2に示すように外側シース4の遠位先端部4Aは、体内管腔に沿ってステント運搬システムを前進しやすいようにするためにテーパしている。さらに、外側シース4は、放射線不透過性マーカ27を有し、この位置は、ステントの配備までステントの遠位端部を表す。ガイドワイヤ2は、外側シースルーメンを完全に貫通して近位側ガイドワイヤポート24のところでステント運搬システムの遠位部分から出る。
それ自体プッシャ要素8で支持された遠位マーカキャリヤ10が、ステント6を越えて遠位側へ突き出るのに十分な長さを備え、ガイドワイヤ2のためのルーメンを備えている。使用にあたり、ガイドワイヤ2は、移送シャフト12と並んで位置する軸方向経路に沿って延び、この移送シャフト12は、外側シース4の軸線から外れている。遠位マーカキャリヤ10の近位端部は、有利にはグルーによってプッシャ要素8の内面に取り付けられ、その軸方向位置を固定している。遠位マーカキャリヤ10の近位端部は、末広がり端部を有し、又は幾分チューリップ形になっており、それにより、運搬システムのプッシャ組立体を通るガイドワイヤ2の遠位側への前進が容易になる。グルー及び末広がり端部によって得られる固定方式も又、プッシャ要素8からのキャリヤ管10の分離の恐れを軽減させる。
図7では、ステンレス鋼シャフト116の内側に接着され、ステント6の遠位側に位置した遠位先端部ゾーン142まで遠位側に延びるポリマー材料の内側カテーテル140が設けられている。遠位マーカ112がこの遠位先端部ゾーンの周りにスエージ加工され、この遠位マーカは、ステント6の遠位端部のすぐ遠位側に位置している。遠位マーカ112の遠位側に位置する内側カテーテル140の残りの遠位先端部分142に関し、直径は、図8で最もよく分かるように僅かに大きくなっており、それによりマーカ112を内側カテーテルシャフト140上に保持する安全性が高くなり、これに対応して、カテーテル140の遠位端部から滑り落ちることによるマーカ112の損失の恐れが減少する。理解できるように、ガイドワイヤ20は、このルーメンに比較的ぴったりと嵌まった状態でシャフト116と内側カテーテル140を貫通して延びている。
カテーテルシャフト管116、プッシャ要素108及びガイドワイヤ20は全て金属で作られているので、ガイドワイヤとステント運搬システムとの間の摩擦は小さく、したがってPTFE又は他の特定の低摩擦皮膜を省くことができ、それにより製造費が節約される。
本発明は、迅速交換運搬システムの既に知られている利点、例えばガイドワイヤが患者の体内の所望の位置に位置したままの状態で運搬システムを容易に取り出して交換できることを十分に考慮している。
Claims (14)
- ガイドワイヤ(2)上でこれに沿って送り進められ、迅速交換形態を持つ自己拡張性ステント配備システムであって、
ステント(6)を配備まで半径方向内方に閉じ込めるキャビティを備えた管状のシース(4)を有し、このシース(4)が、遠位端部及び近位端部を備え、配備システムの軸線を定め、
上記配備システムが、
上記シース(4)を近位側へ引いてステント(6)を解放するようになっていて、造影剤(F)をシース(4)のキャビティに運搬するルーメンを備えたパイプ(18)と、
使用中、ステント(6)に当接するプッシャ要素(8)、ロッド(16)、及び、移送シャフト(12)を備えたプッシャ組立体と、を更に有し、上記プッシャ組立体の移送シャフト(12)の遠位端部は、上記プッシャ要素(8)の近位端部に連結され、上記移送シャフトの近位端部は、上記ロッド(16)の遠位端部に連結され、上記移送シャフト(12)及び上記ロッド(16)は、上記プッシャ要素(8)の近位側への運動を止めるよう上記プッシャ要素(8)を拘束し、上記プッシャ組立体のロッド(16)は、この遠位端部から近位側へ上記パイプ(18)のルーメンに沿って延びて上記ロッド(16)の近位端部が上記シース(4)の近位端部を越え、上記ロッド(16)が、ステント(6)の配備中、上記プッシャ組立体の受ける端方向圧縮応力(C)を支持でき、
上記配備システムが、
配備システムの軸線と平行に並置した関係に配置されている第1のルーメン(24)及び第2のルーメン(22)を備えたアダプタ(20)を更に有し、上記第1のルーメン(24)が、ガイドワイヤ(2)ルーメンであり、上記第2のルーメン(22)が、上記パイプ(18)のルーメン及び上記シース(4)のキャビティと連通し、
上記シース(4)は、上記ステント(6)のキャビティの遠位側に位置するテーパした先端部を有し、このテーパした先端部は、上記ガイドワイヤ(2)が上記シース(4)の遠位端部を越えて遠位側に延びるようにすることができ、
上記配備システムは、更に、上記ステント(6)のルーメン内に位置するよう上記プッシャ要素(8)上に支持されて遠位側に片持ちされると共に、上記プッシャ要素(8)によって包囲されている遠位マーカキャリヤ(10)を有し、
上記遠位マーカキャリヤ(10)の近位部分は、上記プッシャ要素(8)から近位側へ延びていて、上記プッシャ組立体の移送シャフト(12)の遠位端部が上記プッシャ要素(8)の近位端部に連結可能となるように外方近位側へ末広がりになっていることを特徴とする配備システム。 - プッシャ要素(8)は、ステント(6)の近位端部に当接する遠位側に向いた端面を備えた筒体であることを特徴とする請求項1記載の配備システム。
- プッシャ要素(8)は、放射線不透過性マーカであることを特徴とする請求項1又は2に記載の配備システム。
- 上記移送シャフト(12)は、上記プッシャ要素(8)及び上記ロッド(16)のうち少なくとも一方に溶接されていることを特徴とする請求項1記載の配備システム。
- 上記移送シャフト(12)は、第1の移送シャフト部分及び第2の移送シャフト部分を備え、上記第1の移送シャフト部分及び上記第2の移送シャフト部分は、連結片(14)によって互いに接合されていることを特徴とする請求項1又は4に記載の配備システム。
- 上記連結片(14)は、カラーであることを特徴とする請求項5記載の配備システム。
- 上記移送シャフト(12)の第1の移送シャフト部分及び第2の移送シャフト部分の隣り合う端部は各々、連結片(14)にはんだ付けされていることを特徴とする請求項5記載の配備システム。
- 上記アダプタ(20)は、金属で作られていることを特徴とする請求項1〜7のいずれか1項に記載の配備システム。
- 上記シース(4)と上記アダプタ(20)との間の連結は、接着剤で行われていることを特徴とする請求項1〜8のいずれか1項に記載の配備システム。
- 上記シース(4)は、封入状態の金属編組体(43)を有していることを特徴とする請求項1〜9のいずれか1項に記載の配備システム。
- 上記金属編組体(43)は、上記アダプタ(20)の遠位端部の円周方向面の周りに配置されたスタッド(20A)と協働することを特徴とする請求項10記載の配備システム。
- 上記第1のルーメン(24)は、2つの弧状セグメント(23A,23B)によって構成されていることを特徴とする請求項1〜11のいずれか1項に記載の配備システム。
- 上記パイプ(18)は、上記第1のルーメン(24)を構成する壁に設けられた凹部の端面に当接するまで上記アダプタ(20)の上記第1のルーメン(24)内へ挿入されることを特徴とする請求項1〜12のいずれか1項に記載の配備システム。
- 上記遠位マーカキャリヤ(10)はそれ自体、上記ステント(6)の遠位端部についての放射線不透過性マーカ(26)を支持していることを特徴とする請求項1記載の配備システム。
