Description
椎骨の圧迫骨折は、1つ又はより多くの椎骨に対する潰れ損傷である。椎骨の骨折は、概して骨粗鬆症(「脆弱な骨」の疾患)転移及び/又は外傷と関係している。骨粗鬆症は、骨密度を低下させて骨を弱体にし且つ折れ易くする。
脛骨プラトー骨折は、関節丘の関節面が凹むことによって生じる脛骨関節丘の一方又は双方の圧潰損傷であり、圧迫骨折と共に脛骨プラトーの分裂骨折が生じることがある。圧迫骨折の適切な治療法は骨折の重大さに依存する。最小程度に変位した圧迫骨折は、外科的介入無しでギブス又は支持器にて安定化させることができる。変位骨折を伴い又は伴わない比較的酷く変位した圧迫は、骨折観血的整復法及び内固定によって治療が行われる。
脊椎固定術は、低侵襲性の治療法に応答しない不安定な又は変形した椎間板の疾患と関係した慢性的な腰の痛みを治療するために最も頻繁に採用される。固定術はまた、外傷及び先天的奇形を治療するためにも処方される。脊椎固定術は、椎間板を除去し且つ2つの隣接する椎骨を固定し又は接続するステップを含む。不安定さに悩む患者に対する主たる目的は、脊椎の動きを整復させることにより患者の痛みを軽減することである。
上述した色々な外科的方法における課題に対処する一つの方策は、2003年7月22日に発行されて、本発明の譲受人が所有する米国特許明細書6,595,998号に記載されている。米国特許明細書6,595,998号の開示内容は、参考として引用し明示的に本明細書に含めてある。米国特許明細書6,595,998号の装置は、一時的に又は長期間に亘って植え込み可能な装置と、装置を配置する器具との組み合わせを提供し、この場合、組織表面は、軸線に沿って伸延させ、組織間の空間へアクセスすることを可能にする。概して、米国特許明細書6,595,998号には、次々と積層して軸方向に伸びる柱を提供し、組織表面を伸延させ且つ支持するための中実なウェーハが開示されている。本発明の主たる用途は、椎骨の圧迫骨折を整復し且つ安定化させることであるが、開示された装置及び方法は、2つの組織表面を伸延させることが望まれる任意の状況にて使用することができる。組織は、骨、皮膚、柔軟な組織又はそれらの組み合わせとすることができる。更に、表面は、隣接する要素の対向面又は対向要素の表面とすることができる。米国特許明細書6,595,998号には、椎骨の圧迫骨折の治療、椎体間固定装置として椎間板の置換、楔状開放の脛骨高位骨切除術、脛骨粗面挙上、及び非限定的に、脛骨プラトーの骨折、踵骨、末端脛骨の骨折、又は末端橈骨(手首)の骨折を含むその他の圧迫骨折の治療のため使用可能な装置及び方法が開示されている。
Claims (8)
- 椎体の高さを回復する装置において、
椎体の内部にアクセスし得る形態とされ、幅を有する内側作用通路カニューレを含んでいる作用通路カニューレと、
末端と基端とを有し且つ当該末端と基端との間にある長さを有する細長い伸延構成要素と、
当該伸延構成要素を前記作用通路を介して椎体の内部に挿入する細長い挿入器と、
前記挿入器を取り外したときに前記作用通路カニューレを通って椎体の内部に伸びる形態とされた注入カニューレとを備え、
当該注入カニューレは、流動性材料の供給源と接続し得る形態とされた基端部と、流動性材料を排出する少なくとも1つの開口部を画成する末端部とを有しており、外径が前記内側作用通路カニューレの幅よりも狭く且つ前記内側作用通路内で前記作用通路カニューレの幅の一方の側部へとずらされ且つ前記注入カニューレの末端が前記細長い伸延構成要素の側部のうちの一つに隣接して位置するように配置される構造とされている椎体の高さを回復する装置。 - 請求項1に記載の椎体の高さを回復する装置において、
前記注入カニューレはその基端部分に方向マーカを有し、当該マーカは少なくとも1つの開口部と整合され、椎体の内部の開口部の向きを視覚的に表示し得るようにされている装置。 - 請求項1に記載の椎体の高さを回復する装置において、
前記注入カニューレが前記末端に複数の開口部を有している装置。 - 請求項3に記載の椎体の高さを回復する装置において、
前記複数の開口部が前記伸延構成要素の長さに実質的に等しい前記注入カニューレの末端部分に沿った距離に亘っている装置。 - 請求項1に記載の椎体の高さを回復する装置において、
前記流動性材料の供給源として作動する注射器を更に備えている装置。 - 請求項1に記載の椎体の高さを回復する装置において、
前記注入カニューレが末端の先端に1つだけ開口部を有し、当該開口部は前記注入カニューレの長手方向軸線に対して角度が付けられている装置。 - 請求項1に記載の椎体の高さを回復する装置において、
前記挿入器は前記通路の幅よりも狭い挿入器の幅を有し、
前記伸延構成要素が、その挿入方向に沿ったある長さと該長さに対し交差する横方向寸法とを有し、該横方向寸法は前記挿入器の幅よりも小さくなされている装置。 - 請求項7に記載の椎体の高さを回復する装置において、
前記注入カニューレが、該注入カニューレが前記作用通路カニューレ内を伸びるときに、当該注入カニューレの末端が前記伸延構成要素の末端により近くなる形態で前記伸延構成要素の前記長さに対し隣接し且つ該長さに対して全体として平行に椎体の内部まで伸びる形態とされている装置。
