Claims (63)
- 眼内で眼科処置を実施するための装置であって、
手持ち部分と、
該手持ち部分の遠位端領域から延びる細長い部材であって、管腔と、細長い部材の遠位端領域の近くにある開口とを含む細長い部材と、
該手持ち部分の内部に位置し、該細長い部材の開口と流体連通している吸引ポンプとを備え、
該吸引ポンプは、
・長手方向軸に沿って延びており、複数のローブカムを有するカムシャフトと、
・長手方向軸に対して平行に延びるチューブと、
・該カムシャフトのカムによって駆動され、長手方向軸に対して垂直な面内で移動して該チューブを順次圧縮する複数のカムフォロワと、を含む、
装置。 - チューブは、カムシャフトの両側に位置決めされた一対の周辺チューブに分割する近位流路を備え、
一対の周辺チューブは、ポンプマニホールドと遠位側で合流して、細長い部材の管腔と流体連通する遠位流路を形成する、請求項1に記載の装置。 - 一対の周辺チューブを順次圧縮する複数のカムフォロワは、実質的に非パルス性の吸引を生成する、請求項2に記載の装置。
- 実質的に非パルス性の吸引は、約3cc/分未満である、請求項3に記載の装置。
- 実質的に非パルス性の吸引は、3cc/分~約10cc/分の範囲である、請求項3に記載の装置。
- 実質的に非パルス性の吸引は、10cc/分より大きく、約25cc/分以下である、請求項3に記載の装置。
- 細長い部材は、手持ち部分に対して往復運動で移動可能である、請求項1~6のいずれかに記載の装置。
- 細長い部材は、レンズ断片化、レンズ乳化または前部硝子体切除のために構成されている、請求項1~6のいずれかに記載の装置。
- 細長い部材は、約1.25mmの最大断面直径を有する、請求項1~6のいずれかに記載の装置。
- 細長い部材は、約3mm以下の最大断面直径を有する、請求項1~6のいずれかに記載の装置。
- 細長い部材は、20ゲージ~27ゲージの範囲である硝子体切除プローブである、請求項1~6のいずれかに記載の装置。
- 細長い部材は、手持ち部分の内部に固定的に接続された外側チューブ内で往復摺動するように構成される、請求項1~6のいずれかに記載の装置。
- 細長い部材の遠位先端が、刃先を形成する、請求項12に記載の装置。
- 外側チューブは、側壁を貫通する開口を備える、請求項13に記載の装置。
- 細長い部材はさらに、外側チューブと協調して2つの切込みを形成するように構成された側方開口をその遠位端領域の近くに備える、請求項14に記載の装置。
- 細長い部材は、吸引ポンプと流体連通する真空マニホールドのチャンバ内に位置決めされた近位開口を備える、請求項1~6のいずれかに記載の装置。
- 細長い部材は、300cpmで振動するように構成される、請求項1~6のいずれかに記載の装置。
- 細長い部材は、500~600cpmで振動するように構成される、請求項1~6のいずれかに記載の装置。
- 細長い部材は、約5000cpmまでで振動するように構成される、請求項1~6のいずれかに記載の装置。
- 細長い部材は、約7500cpmまでで振動するように構成される、請求項1~6のいずれかに記載の装置。
- ポンプからの入力を増幅して、硝子体切除用に構成された細長い部材の出力を達成するように構成されたギアボックスをさらに備える、請求項1~6のいずれかに記載の装置。
- ギアボックスは、遊星ギアボックスである、請求項21に記載の装置。
- カムシャフトと動作可能に接続するように構成された駆動機構をさらに備え、該駆動機構は、カムシャフトを回転するように構成される、請求項1~6のいずれかに記載の装置。
- 駆動機構からの入力を上昇させるためのギア装置をさらに備える、請求項23に記載の装置。
- 吸引ポンプのカムシャフトは、約15cc/分~30cc/分の吸引能力を送給するために一定の速度で回転する、請求項24に記載の装置。
- 駆動機構からの入力は、約140RPMであり、
ギア装置は、少なくとも約3:1、4:1、5:1、6:1、約30:1までの比率を有する、請求項24に記載の装置。 - カムシャフトを回転するように構成された駆動機構は、細長い部材の振動も駆動する、請求項23に記載の装置。
- カムシャフトは、カムシャフトの回転運動を細長い部材の往復直線運動に変換するように構成されたカッターアセンブリと動作可能に連結する、請求項27に記載の装置。
- カッターアセンブリは、ランプカムと、カッター戻りスプリングとを備える、請求項28に記載の装置。
- 細長い部材の近位運動は、ランプカムの関数であり、
細長い部材の遠位運動は、カッター戻りスプリングの関数である、請求項29に記載の装置。 - カッターアセンブリは、並進磁石ディスクと、回転磁石ディスクとを備える、請求項28に記載の装置。
- 並進磁石ディスクは、1つ以上の磁石を含み、
回転磁石ディスクは、1つ以上の磁石を含む、請求項31に記載の装置。 - 並進磁石ディスクおよび回転磁石ディスクの1つ以上の磁石は、局所磁界を生成し、回転磁石ディスクの1つ以上の磁石は、並進磁石ディスクの1つ以上の磁石と整列したり、整列から外れたりして回転する、請求項32に記載の装置。
- 並進磁石ディスクは、カッタースプラインに接続され、該カッタースプラインは、細長い部材に接続され、その結果、該並進磁石ディスク、該カッタースプラインおよび該細長い部材は全て軸方向に一緒に並進する、請求項33に記載の装置。
- 並進磁石ディスク、カッタースプラインおよび細長い部材は、カムシャフトの回転に起因して長手方向軸に沿って軸方向にある距離を双方向に移動するように構成される、請求項34に記載の装置。
- 回転磁石ディスクの1つ以上の磁石と並進磁石ディスクの1つ以上の磁石とのアライメントは、磁気吸引または磁気反発による直線運動を生じさせる、請求項32に記載の装置。
- 回転磁石ディスクは、磁石の極が交互配列するように互いに相対的に配向した複数の磁石を備える、請求項32に記載の装置。
- 並進磁石ディスクに対する回転磁石ディスクの回転により、前記複数の磁石の極が並進磁石ディスクの1つ以上の磁石とともに交互配列の反発および吸引を生じさせ、細長い部材の振動を生じさせる、請求項32に記載の装置。
- 1つ以上の磁石を含む第2回転磁石ディスクをさらに備え、該第2回転磁石ディスクは、並進磁石ディスクに対して遠位側に位置決めされる、請求項32に記載の装置。
- 第1および第2回転磁石ディスクの前記1つ以上の磁石と並進磁石ディスクの1つ以上の磁石とのアライメントにより、切削チューブの振動を生じさせる、請求項39に記載の装置。
- 第2回転磁石ディスクの1つ以上の磁石は、並進磁石ディスクの1つ以上の磁石の反発を生じさせるように配置された極を有し、
第1回転磁石ディスクの1つ以上の磁石は、吸引を生じさせるように配置された極を有し、
反発および吸引は、並進磁石ディスクを第1および第2回転磁石ディスクの間で直線的に移動させるように付勢するより強い磁界を生成する、請求項40に記載の装置。 - 第2回転磁石ディスクは、遠位側のハードストップを形成し、
第1回転磁石ディスクは、並進磁石ディスクのための近位側のハードストップを形成する、請求項41に記載の装置。 - 細長い部材の振動からカムシャフトの回転を物理的に切り離すように構成されたカッタースイッチをさらに備える、請求項1~6のいずれかに記載の装置。
- カッタースイッチは、手持ち部分での入力の起動時に前記細長い部材が振動するか否かを制御する、請求項43に記載の装置。
- カッタースイッチは、カッターアセンブリの回転中に細長い部材の振動をロックする、請求項43に記載の装置。
- 手持ち部分の入力に機能的に接続された抽気バルブをさらに備え、該抽気バルブは、細長い部材の管腔を経由して吸引ポンプの有効流量を調節するように構成される、請求項1~6のいずれかに記載の装置。
- 前記入力は、押圧されるように構成されたトリガである、請求項46に記載の装置。
- 抽気バルブの開放形態は、吸引ポンプによって発生したほぼ全ての吸引を、細長い部材の管腔から離れるように吸い上げる、請求項47に記載の装置。
- 抽気バルブの閉鎖形態は、吸引ポンプによって発生した全吸引を細長い部材の管腔を通過するように方向付けする、請求項47に記載の装置。
- 抽気バルブは、流体チャネル内の弁座に対して移動可能な螺旋状従動ニードルを有するニードルバルブである、請求項47に記載の装置。
- ニードルバルブの開放形態は、吸引ポンプを灌流流体ラインに接続し、その結果、細長い部材の管腔を通る最小吸引をもたらす、請求項50に記載の装置。
- 休止状態から離れるトリガの第1移動量が、吸引ポンプを起動し、ニードルバルブは開放状態のままであり、そのため吸引ポンプからの吸引は、切削チューブの管腔を通って送給されない、請求項51に記載の装置。
- 休止状態から離れるトリガの第2移動量が、ニードルを弁座に向けて付勢する、請求項52に記載の装置。
- 第1移動量は、ゼロより大きく、トリガストロークの約20%未満である、請求項52に記載の装置。
- 第2移動量は、トリガストロークの少なくとも20%である、請求項53に記載の装置。

眼内で第1眼科処置を実施するための第1作動部分であって、該ドライバ部分の回転可能なカプラと動作可能に接続するように構成された第1作動部分とを備え、
該第1作動部分は、
・第1作動部分の遠位端領域から延びる第1細長い部材であって、管腔および、該第1細長い部材の遠位端領域の近くにある開口を含む第1細長い部材と、
・該第1作動部分の内部にあり、該第1細長い部材の遠位端領域の近くにある開口と流体連通しているピストンポンプであって、パルス吸引を発生するように構成されたピストンポンプとを備え、
前記キットはさらに、眼内で第2眼科処置を実施するために、ドライバ部分の回転可能なカプラと動作可能に接続するように構成された第2作動部分を備え、
該第2作動部分は、
・第2作動部分の遠位端領域から延びる第2細長い部材であって、管腔および、該第2細長い部材の遠位端領域の近くにある開口を含む第2細長い部材と、
・該第2作動部分の内部にあり、該第2細長い部材の開口と流体連通しているリニア蠕動ポンプであって、円滑な流れ吸引を発生するように構成されたリニア蠕動ポンプとを備える、
キット。 - ピストンポンプは、複数のピストンと、回転可能なカプラを介してモータによって回転可能な回転カムアセンブリとを備え、
該回転カムアセンブリの回転により、複数のピストンは、第1細長い部材の管腔内に不連続な負圧のパルスを発生する、請求項56に記載のキット。 - ピストンポンプは、約100cc/分までの体積吸引を発生するように構成される、請求項56に記載のキット。
- リニア蠕動ポンプは、長手方向軸に沿って延びる、複数のローブカムを有するカムシャフトと、
長手方向軸に対して平行に延びるチューブと、
カムシャフトのカムによって駆動される複数のカムフォロワであって、長手方向軸に対して垂直な面内で移動してチューブを順次圧縮する複数のカムフォロワと、を備える、請求項56に記載のキット。 - 複数のカムフォロワは、前記チューブを順次圧縮して、実質的に非パルス性の吸引を生成する、請求項58に記載のキット。
- 実質的に非パルス性の吸引は、約10cc/分までである、請求項60に記載のキット。
- 前記モータは、第1細長い部材および第2細長い部材の往復運動を駆動するように構成される、請求項56に記載のキット。
- 第1細長い部材は、レンズ断片化のために構成され、
第2細長い部材は、前部硝子体切除のために構成される、請求項56に記載のキット。
