【課題】インプラント周囲感染の診断方法または病態判定方法を提供すること。【解決手段】体液中のミエロペルオキシダーゼ量またはミエロペルオキシダーゼ活性を測定対象にして、インプラント周囲感染の診断または病態判定を行う。【選択図】なし
Description
本発明は、インプラント周囲感染の診断方法または病態判定方法に関する。
現在のところ有用性が報告されているインプラント周囲感染の主な診断マーカーは、白血球数、ＰＭＮ％、ＣＲＰ、ａｌｐｈａ−ｄｅｆｅｎｓｉｎ、白血球エステラーゼ、ＩＬ−６、ＩＬ−８、細菌培養などが挙げられる（非特許文献１）。ミエロペルオキシダーゼについては、ヒトの尿路感染の診断マーカーとして尿中ミエロペルオキシダーゼが報告されている（非特許文献２）ほか、ヒトの創部感染の診断マーカーとして創部のミエロペルオキシダーゼ活性が報告されており（非特許文献３）、さらに馬の化膿性関節炎の診断マーカーとして、関節液中のミエロペルオキシダーゼが報告されている（非特許文献４）。しかし、ヒトのインプラント周囲感染の診断マーカーとしては過去に報告が無い。

本明細書においてインプラントとは体内埋め込み式器具を指す。インプラント周囲感染は術後の最も重症な合併症の一つである。インプラント周囲感染は、早期に診断できればインプラントを温存し治療可能であるとされており、迅速で精度の高い診断が求められる。しかし従来から診断に利用されている検査法は、診断精度に難点があり、そのためインプラント周囲感染は診断に難渋する症例が多い。
本発明は、従来の測定対象に代わる新規な成分を測定することによって、インプラント周囲感染の診断方法または病態判定方法を提供することを目的とする。
上述の目的を達成するために、本発明では、体液中のミエロペルオキシダーゼ量またはミエロペルオキシダーゼ活性を測定対象にしている。ミエロペルオキシダーゼは好中球の顆粒内成分である。体液中のミエロペルオキシダーゼ量またはミエロペルオキシダーゼ活性を測定するには、酵素免疫法、ラテックス凝集反応や免疫クロマトグラム法など日常多用されている免疫学的測定法や酵素反応を利用した測定法を用いることができる。本発明者はミエロペルオキシダーゼ量またはミエロペルオキシダーゼ活性を測定することでインプラント周囲感染の診断または病態判定を可能とした。
体液中のミエロペルオキシダーゼ量またはミエロペルオキシダーゼ活性を測定することによって、従来技術と比し高い精度でインプラント周囲感染の診断と病態判定が可能となる。
実施例として酵素免疫測定法（ＥＬＩＳＡ法）による測定方法を示す。
インプラント周囲感染が疑われる患者の体液を採取し、遠心分離する（３０００ｒｐｍ，５ｍｉｎ）。上清中のミエロペルオキシダーゼ量をＥＬＩＳＡ ｋｉｔ（Ｒ ＆ Ｄ Ｓｙｓｔｅｍｓ，Ｍｉｎｎｅａｐｏｌｉｓ，ＵＳＡ）を用いて測定する。
体液中のミエロペルオキシダーゼ量がカットオフ値（例えば１０，０００ｎｇ／ｍＬ）以上であれば感染と判定する。
Claims (2)
- 体液中のミエロペルオキシダーゼを測定対象にすることを特徴とするインプラント周囲感染の診断方法または病態判定方法。
- 酵素免疫法やラテックス凝集反応、免疫クロマトグラフ法などの免疫学的測定法を用いたミエロペルオキシダーゼ測定および酵素反応を用いたミエロペルオキシダーゼ活性測定を特徴とする請求項１記載のインプラント周囲感染の診断または病態判定に用いるキット。
