Abstract
Description
Claims (21)
- 心臓弁を修復する弁輪形成装置(1)であって、心臓弁は、弁輪と、心房面(16)及び心室面(17)を有する一対の弁尖(15)を含む弁組織を有しており、
弁輪形成装置は、第1支持部材(5)と第2支持部材(6)とを備え、前記第1及び第2支持部材は、シザーリンク18を形成するよう重なり合うとともに、前記弁尖を把持するよう構成される、弁輪形成装置。 - 前記第1及び第2支持部材は、それぞれ実質的に細長い又は円形状の連続体として形成されており、当該連続体は、前記第1及び第2支持部材の端部に、腱索の周囲をガイドする開口を備える、請求項1に記載の弁輪形成装置。
- 前記第1及び第2支持部材は、実質的に全ての腱索を包囲するよう構成される、請求項1又は請求項2に記載の弁輪形成装置。
- 前記第1支持部材及び前記第2支持部材は、互いが対称な位置に配置される、請求項1〜請求項3の何れかに記載の弁輪形成装置。
- 前記第1支持部材及び前記第2支持部材は、互いが非対称な位置に配置される、請求項1〜請求項3の何れかに記載の弁輪形成装置。
- 前記第1支持部材及び/又は前記第2支持部材が角度付けされている、請求項1〜請求項5の何れかに記載の弁輪形成装置。
- 前記第1支持部材及び/又は前記第2支持部材が、その両端部に傾斜部(19,19'、20,20')を備えている、請求項1〜請求項6の何れかに記載の弁輪形成装置。
- 前記シザーリンク18は、前記第1及び第2支持部材の周囲に配置される少なくとも1つの接続要素(7)を備え、前記第1及び第2支持部材は、前記少なくとも1つの接続要素により接続可能とされる、請求項1〜請求項7の何れかに記載の弁輪形成装置。
- 前記接続要素は、ヒンジ又はピボットジョイント(21,21')を備えている、請求項1〜請求項8の何れかに記載の弁輪形成装置。
- 前記第1及び第2支持部材は、前記少なくとも1つの接続要素7により解除可能に接続される、請求項1〜請求項9の何れかに記載の弁輪形成装置。
- 前記少なくとも1つの接続要素は、前記第1及び第2支持部材を互いの角度が一定となるよう固定する固定部材を備えている、請求項8〜請求項10の何れかに記載の弁輪形成装置。
- 前記第1支持部材及び前記第2支持部材は、これらの支持部材の中央領域に凹部を備え、当該凹部は、対向し且つ前記シザーリンクの中心ピボット点を形成するよう配置される、請求項1〜請求項11の何れかに記載の弁輪形成装置。
- 前記第1支持部材及び前記第2支持部材は、変化する厚さを有する、請求項1〜請求項12の何れかに記載の弁輪形成装置。
- 前記第1支持部材は、実質完全に第2支持部材と重なり合う、請求項1〜請求項13の何れかに記載の弁輪形成装置。
- 前記第1支持部材5及び前記第2支持部材6は、緩んで拡張した形状と、カテーテルを介して送達するため断面が縮小された圧縮形状とを取るよう、形状記憶材料により形成される、請求項1〜請求項14の何れかに記載の弁輪形成装置。
- 一対の弁尖及び交連を有する心臓弁に第1支持部材(5)及び第2支持部材(6)を含む弁輪形成装置を配置する方法(200)であって、
前記心臓へのアクセスを提供するステップ(201)と、
前記第1支持部材を前記交連間に延在し前記心臓弁を貫くよう配置するステップ(202)と、
前記第2支持部材を前記交連間に延在し前記心臓弁を貫くよう配置するステップ(203)と、
前記心臓弁の組織を把持するため前記第1支持部材及び前記第2支持部材を固定する固定ステップ(208)とを備え、
前記第1支持部材の近位端は、第1弁尖の心房面と当接するよう構成され、前記第1支持部材の遠位端は、第2弁尖の心室面と当接するよう構成され、
前記第2支持部材の近位端は、前記第2弁尖の心房面と当接するよう構成され、前記第2支持部材の遠位端は、前記第1弁尖の心室面と当接するよう構成される、方法。 - 前記固定ステップは、前記第1支持部材の近位端を第2支持部材の遠位端にくっつけて、これらの間にある心臓弁の対向する両側の組織を挟持するステップ209を備える、請求項16に記載の方法。
- 前記第1及び第2支持部材は、交連において当該第1及び第2支持部材の間にシザーリンク18を形成するため、重ね合わせるステップ(207)を備える、請求項16又は請求項17に記載の方法。

- 前記第1及び第2支持部材をカテーテルを介して患者の体外から前記心臓内に別々に配置するステップ205と、前記第1及び第2支持部材を重ね合わせるステップ206とを備える、請求項16〜請求項18の何れかに記載の方法。
- 前記心臓への送達は、経心尖アプローチ又は経大腿アプローチの経カテーテル法により行われるか、心房壁を介して外科的に行われるか、あるいは、カニューレ又はカテーテルを介して行われる、請求項16〜請求項20の何れかに記載の方法。
