Description
Claims (34)
- シースと関連して使用するための拡張器であって、
遠位端及び近位端を画定する、細長い本体と、
前記本体内に位置付けられる内腔であって、前記本体の前記遠位端において遠位開口部を画定する、内腔と、
前記内腔から前記拡張器の側壁を通して前記拡張器の長さの一部分に沿って延在する、スロットと、を備える、拡張器。 - 前記スロットが、前記拡張器の前記近位端から前記拡張器の前記遠位端に延在する前記拡張器の長さの大部分に沿って延在する、請求項1に記載の拡張器。
- 前記拡張器が、ショアDスケール上で50超のデュロメータ硬度を有する材料で形成される、請求項1に記載の拡張器。
- 前記拡張器が、前記シースの遠位端を越えて少なくとも10cm延在するように構成される、請求項1に記載の拡張器。
- 前記拡張器が、ショアDスケール上で45以下のデュロメータ硬度を有する材料で形成される、請求項4に記載の拡張器。
- 前記拡張器が、該拡張器の前記遠位端に隣接した先細でない円筒部分を含む、請求項4に記載の拡張器。
- 前記先細でない円筒部分が、少なくとも3cmの長さである、請求項6に記載の拡張器。
- 前記先細でない円筒部分が、0.025cm(0.010インチ)未満の側壁の厚さを有する、請求項6に記載の拡張器。
- 前記拡張器が、前記先細でない円筒部分に隣接した先細の部分を形成する、請求項6に記載の拡張器。
- 前記先細の部分が、少なくとも10cmの長さである、請求項9に記載の拡張器。
- 前記拡張器が、放射線不透過性材料で形成される、請求項1に記載の拡張器。
- 前記拡張器が、該拡張器の前記遠位端に隣接した放射線不透過性マーカーバンドを含む、請求項1に記載の拡張器。
- 潤滑性コーティングを更に備える、請求項1に記載の拡張器。

前記拡張器が、ショアDスケール上で45以下のデュロメータ硬度を有する材料で形成される、拡張器。 - 前記先細でない円筒部分が、少なくとも3cmの長さである、請求項14に記載の拡張器。
- 前記拡張器が、放射線不透過性材料で形成される、請求項14に記載の拡張器。
- 前記拡張器が、該拡張器の前記遠位端に隣接した放射線不透過性マーカーバンドを含む、請求項14に記載の拡張器。
- 潤滑性コーティングを更に備える、請求項14に記載の拡張器。

前記シースアセンブリは、
細長いシースと、進入拡張器と、追跡拡張器と、を備え、
前記進入拡張器は、
遠位端及び近位端を画定する、細長い本体と、
前記本体内に位置付けられる内腔であって、前記本体の前記遠位端において遠位開口部を画定する、内腔と、
前記内腔から前記進入拡張器の側壁を通して前記進入拡張器の長さの一部分に沿って延在する、スロットと、
を備え、
前記追跡拡張器は、
遠位端及び近位端を画定する、細長い本体と、
前記本体内に位置付けられる内腔であって、前記本体の前記遠位端において遠位開口部を画定する、内腔と、
前記内腔から前記追跡拡張器の側壁を通して前記追跡拡張器の長さの一部分に沿って延在する、スロットと、
前記追跡拡張器の前記遠位端に隣接した先細でない円筒部分と、
を備える、シースアセンブリ。 - 前記シースアセンブリは、第3の拡張器を更に備え、
前記第3の拡張器は、
遠位端及び近位端を画定する、細長い本体と、
前記本体内に位置付けられる内腔であって、前記本体の前記遠位端において遠位開口部を画定する、内腔と、
を備え、
前記第3の拡張器が、該第3の拡張器の前記遠位端に隣接した先細でない円筒部分と、前記先細でない円筒部分に隣接した先細の部分と、を備え、
前記第3の拡張器が、ショアDスケール上で45以下のデュロメータ硬度を有する材料で形成される、請求項19に記載のシースアセンブリ。 - 患者の脈管構造内にシースを配置する方法であって、
進入拡張器及びシースを患者の前記脈管構造の中に挿入することであって、前記進入拡張器が、側壁内に第1のスロットを備え、前記進入拡張器が、ガイドワイヤ上に位置付けられる、挿入することと、
前記進入拡張器を除去することと、
側壁内に第2のスロットを備える追跡拡張器を、前記ガイドワイヤを除去せずに前記シースの中に挿入することと、
前記追跡拡張器及びシースを、前記脈管構造の中に前進させることと、を含む、方法。 - 前記ガイドワイヤが、前記進入拡張器から前記第1のスロットを介して部分的に除去される、請求項21に記載の方法。
- 前記ガイドワイヤが、前記追跡拡張器から前記第2のスロットを介して部分的に除去される、請求項21に記載の方法。
- 患者の頸動脈の中への頸動脈シースの配置のために設計される迅速交換拡張器であって、前記拡張器が、前記頸動脈シースの前記遠位端に対して遠位に位置する、その全長の中央に位置する円形内腔を有し、前記頸動脈シース内に位置する、前記拡張器の長さの大部分の前記拡張器の外面に延在するスロットを有する、迅速交換拡張器。
- 前記拡張器が、前記患者の皮膚を通して前記患者の鼠径部の大腿動脈の中に進入するために明示的に設計される進入拡張器であり、50超のショアDデュロメータを持つ比較的硬いプラスチック材料から形成される、請求項1に記載の迅速交換拡張器。
- 前記拡張器が、頸動脈狭窄を通して配置されたガイドワイヤ上を、前記ガイドワイヤを前記狭窄から引き出すことなく容易に前進するように明示的に設計される追跡拡張器であり、少なくとも10cmの前記頸動脈シースの末端を越えて延在する長さを有し、前記拡張器の前記プラスチック材料が、45以下のショアDデュロメータを持ち、比較的柔らかい、請求項1に記載の迅速交換拡張器。
- 前記拡張器の前記遠位部分が、少なくとも3cmの長さを有する薄壁円筒管であり、前記頸動脈シースの前記遠位端に対して遠位にある前記拡張器の残りの部分が、略先細の区分であり、その最大直径が、前記頸動脈シースの前記遠位端に位置し、前記残りの部分の前記遠位端が、前記薄壁円筒管と同一の直径を有し、前記拡張器の前記先細の区分の長さが、少なくとも10cmである、請求項4に記載の迅速交換追跡拡張器。
- 前記拡張器が、前記拡張器を放射線不透過性にするように、タングステン等の金属粒子で充填されるプラスチックから形成される、請求項1に記載の迅速交換拡張器。
- 前記拡張器が、該拡張器の前記遠位端、又はその付近に位置する放射線不透過性マーカーバンドを有する、請求項1に記載の迅速交換拡張器。
- 頸動脈内の狭窄を通して配置されたガイドワイヤ上を前進するように設計される追跡拡張器であって、頸動脈シースをヒト患者の頸動脈の中に前進させるようにも設計され、前記拡張器の前記遠位部分が、少なくとも3cmの長さを有する薄壁円筒管であり、前記頸動脈シースの前記遠位端に対して遠位にある前記拡張器の残りの部分が、略先細の区分を有し、その最大直径が、前記頸動脈シースの前記遠位端に位置し、前記残りの部分の前記遠位端が、前記薄壁円筒管と同一の外径を有し、前記拡張器の前記先細の区分の長さが、少なくとも5cmであり、前記追跡拡張器が、前記拡張器の全長に延在する中心内腔を有し、それにより、前記拡張器をオーバーザワイヤ(over-the-wire)拡張器にし、前記拡張器がまた、45以下のショアDデュロメータを有する比較的柔らかいプラスチック材料から形成される、追跡拡張器。
- 前記拡張器が、前記拡張器を放射線不透過性にするように、タングステン等の金属粒子で充填されるプラスチックから形成される、請求項7に記載の追跡拡張器。
- 前記拡張器が、該拡張器の前記遠位端、又はその付近に位置する放射線不透過性マーカーバンドを有する、請求項7に記載の追跡拡張器。
- 前記拡張器が、蛇行性動脈を通過するその能力を強化するように、潤滑性コーティングを有する、請求項7に記載の追跡拡張器。
- 血管造影、及び/又は血管形成、及び/又はステント留置を受ける予定の頸動脈の中に頸動脈シースを配置するための方法であって、
1− 迅速交換進入拡張器を頸動脈シース内に配置し、患者の皮膚を通して、前記患者の大腿動脈の中に前記組み合わせを皮下挿入する工程と、
2− 前記進入拡張器を除去し、それを追跡拡張器と置換する工程と、
3− 前記追跡拡張器及び前記頸動脈シースを、前記追跡拡張器の前記遠位端が前記頸動脈の中に完全に前進するまで、頸動脈の中に以前に配置されたガイドワイヤ上を前進させる工程と、
4− 前記頸動脈シースを、その遠位端が処置される前記頸動脈の中に位置するまで、前記追跡拡張器上を前進させる工程と、
を含む、方法。
