Abstract

【発明の詳細な説明】
【０００１】 本発明は、肩補綴具組立体であって、中央管状ブッシングを設けられているプ
レートによって形成されている、関節窩に固定する部材を伴うメタ関節窩エレメ
ントと、メタ関節窩エレメント上に嵌められるべく構成されている関節窩球とを
具備しているタイプのものに関する。関節窩球は、この関節窩球に対して自由に
回転可能である軸方向ねじを設けられている。このねじは、メタ関節窩エレメン
トのブッシング内へ螺入させられるべく構成されている。関節窩球は、それと結
び付いている上腕部補綴具の凹状カップと協働する凸状関節面を有している。
【０００２】 本発明は、また、補綴具組立体の部分を形成している補綴具を提供する。
【０００３】 このタイプの補綴具は、例えば、Ｍｅｄｉｎｏｖの名前で出願されたフランス
特許第８９/１３３６６(２ ６５２ ４９８)号から、知られている。
【０００４】 三角筋、棘上筋、棘下筋、肩胛骨下筋及び小円筋から成る肩の筋の組により、
回旋腱板が構成されている、ということは、知られている。広範囲の断裂が腱板
に起こると、三角筋のみが、残り、そして、それは、肩の関節が正しく機能する
のを可能にするには不十分であり、このことは、肩補綴具が埋め込まれることを
必要とする。
【０００５】 補綴具であって、複数のねじによって肩胛骨の関節窩にメタ関節窩エレメント
が締着され且つ凸状関節面を有している関節窩球をそのメタ関節窩エレメントが
受容するものは、知られており、その関節窩球は、メタ関節窩エレメントの周辺
にねじ込まれる。
【０００６】 不幸なことに、長時間に亘って使用されると、関節窩球はねじが緩む傾向にあ
る、ということが、見出されている。これは、メタ関節窩エレメントの下で関節
窩球が傾くことを引き起こし、ねじ山を潰す。補綴具に生じたその損傷は、それ
を取り替えるべく、外科医が補綴具全体を取り外すことを必要とするに十分であ
り得る。
【０００７】 この問題を解決すべく、関節窩球とメタ関節窩エレメントとの対面している接
触面を凸凹にするという提案が、先ず、なされた。この技術は、不十分であると
判明している。
【０００８】 次いで、相補的な雄円錐支持面及び雌円錐支持面であって、それぞれ、メタ関
節窩エレメント及び関節窩球に作られているものの構成と、関節窩球のねじとを
結び付けるという提案が、なされた。この場合、関節窩球とメタ関節窩エレメン
トとを具備しているシステムをロックし又はロックを外すべくねじが駆動される
ところの方向の関数として、２つの円錐支持面は、互いに他方から離れ又は互い
に他方に向かって移動させられ得る。この方法で構成されている肩補綴具は、Ｍ
ｅｄｉｎｏｖの名前でのフランス特許第９５/０９３９５(２ ７３７ １０７)号
に記載されている。その補綴具は、満足を与えている。
【０００９】 しかしながら、ねじの頭部は、ねじの締め付けの最後に外科医がその最後であ
ることを適切に感じ得ない場合には、メタ関節窩エレメントを通過し得る。何故
ならば、特別の保持手段が、設けられていないからである。そのような状況下で
は、メタ関節窩エレメントは、最早、関節窩に固定されていない。
【００１０】 メタ関節窩エレメントへのアクセスを外科医に与えることにおける別の問題が
、存在し、これは、関節窩球をメタ関節窩エレメント上の所定の場所に正しく置
くことの困難さを生じる。患者は、手術の間、半分座った位置にあり、このため
、外科医は、メタ関節窩エレメントに、突き出ている位置から接近しなければな
らず、この場合、メタ関節窩エレメントは、言わば、肩の組織及び筋肉によって
画成されている竪穴の底にある。メタ関節窩エレメントへのアクセスは、限定さ
れており且つ比較的困難であり、不十分な可視性を伴っており、この不十分な可
視性は、メタ関節窩エレメントのブッシングの軸上に関節窩球のねじが正確にあ
るかどうかを確認するのを不可能にする。
【００１１】 それは、外科医が関節窩球を不正確に位置させ得、もって、メタ関節窩エレメ
ントにおける対応する穴の軸上に関節窩球の軸方向ねじが正確にはないというこ
とを、結果的にもたらす。そのような状況下においては、関節窩球のねじにおけ
る雄ねじ部とメタ関節窩エレメントの中央の穴における雌ねじ部とが正しく係合
させられていない時に、ねじの締め付けが、開始される。その誤った挿入に外科
医が気付いた時には、既に、ねじ部は、補綴具全体が取り替えられることを必要
とするに十分に損傷を受けている。２つの補綴具が使用されなければならず、従
って、装置のコストをかなり上昇させるだけでなく、手術に必要な時間が、倍に
なる。
【００１２】 本発明の目的は、これらの種々の欠点を排除するような態様で構成されている
関節窩補綴具を提供することである。
【００１３】 本発明によれば、肩補綴具は、関節窩球を案内する手段であって、ブッシング
の軸上での、メタ関節窩エレメント上へのねじの正しい配置を容易にすべく構成
されているものを備えている。
【００１４】 本発明の実施形態においては、前記案内手段は、管状ブッシング内を摺動させ
られ得るべく構成されているピンを具備しており、且つ、関節窩球のねじは、縦
軸方向チャンネルであって、ねじの両端部において開口しており且つそのチャン
ネル内へピンが挿入されるのを可能にするものを提供しており、もって、ピンは
、ねじが管状ブッシング内へ挿入されるまで、そのピンに沿ってねじが摺動する
際に関節窩球を案内すべく機能する。
【００１５】 メタ関節窩エレメントの中央ブッシング内へのピンの予備的な挿入は、メタ関
節窩エレメントの中央ブッシングの軸上への関節窩球の完全な案内をもたらすこ
とを可能にする。次いで、関節窩球は、ねじ部を傷付けるというリスクなく、関
節窩ブッシング内へ正しく螺入させられ得、これにより、第２の補綴具に頼るこ
とを必要とするというリスクが、殆ど完全に排除される。
【００１６】 本発明の別の特徴によると、メタ関節窩エレメントを関節窩に固定する手段は
、ねじであり、且つ、ねじの締め付けの最後に各ねじがメタ関節窩エレメントに
当接するのを保証する手段が、設けられている。
【００１７】 本発明の実施形態においては、当接手段は、少なくとも１つのねじの頭部の周
りに形成されているカラーであって、ねじ頭部の雄ねじ部に対して半径方向に突
出しているものと、雌ねじを螺刻されている穴の縁部の周りに設けられている対
応環状支持肩部であって、メタ関節窩エレメント内へのねじの挿入のためのもの
とを具備している。
【００１８】 本発明の別の有り得る特徴によると、当接手段は、凸状外面を呈している少な
くとも１つのねじの頭部を具備しており、その凸状外面は、前記ねじをメタ関節
窩エレメント内に挿入するための穴の壁部に形成されている相補的凹状座部に当
接すべく構成されている。
【００１９】 そして、これらの構成は、ねじの締め付けの最後にねじがメタ関節窩エレメン
トを通過するというリスクを避け、これにより、メタ関節窩エレメントを関節窩
に固定することの安全性を保証する。
【００２０】 本発明の他の特徴及び利点は、非限定的な例としての実施形態を示している添
付図面を参照しての、以下の記載から明らかになる。
【００２１】 図１に示されている肩補綴具の全体は、肩胛骨４の関節窩３へ固定されている
関節窩インプラント１と、上腕骨６の骨髄管５内に固定されている上腕骨インプ
ラント２とから成っている。
【００２２】 関節窩インプラント１は、プレート１３(図２〜図６)によって形成されている
メタ関節窩エレメント７と、このメタ関節窩エレメント７上に嵌められるべく構
成されている関節窩球９とを具備しており、そのメタ関節窩エレメント７は、そ
れを関節窩３に固定するための複数のねじ８を備えており、それらのねじのうち
の２つのねじが、図１において認められ得る。
【００２３】 関節窩球９は、関節面１１を提供しており、この関節面１１は、それと結び付
いている上腕骨補綴具２のカップ１２の相補的な面と協働すべく、この実施形態
においては凸面である。この実施形態においては、カップ１２は、凸面１１と相
補的な凹面を有している。
【００２４】 プレート１３は、好適に、円錐状であり、軸Ｘ−Ｘを対称軸とする回転対称で
あり且つ中央ブッシング１４を設けられており、この中央ブッシング１４は、好
適に、プレート１３と一体に形成されている。中央ブッシング１４は、プレート
１３から突出していると共に、メタ関節窩エレメント７を骨に固定するための円
形リブ１５を有している。ブッシング１４は、軸方向の内腔１６を提供し、この
内腔１６は、その長さの一部分に雌ねじ部１７を有している。この雌ねじ部１７
には、滑らかなゾーンを構成している内腔４９が、続いている。
【００２５】 プレート１３は、４つの穴を開けられており、これらの穴は、メタ関節窩エレ
メント７の関節窩固定ねじを通すためのものである。これらの穴１８のうちの２
つの穴は、直径方向において互いに反対側にあると共に、雌ねじ部１９を提供し
ている。それらの軸は、軸Ｘ−Ｘを対称軸として対称的に発散している。他の２
つの穴２１は、直径方向において互いに反対側にあると共に、好適に９０°に等
しい角間隔だけ穴１８からずらされている。螺刻されていない穴２１は、壁２２
によって画成されており、これらの壁２２は、相補的な凸面２３用の凹状支持座
部を構成しており、それらの凸面２３は、メタ関節窩エレメント７を関節窩３に
固定するための骨固定ねじ２５の頭部２４を画定している(図３)。
【００２６】 座部２２は、これらの座部における所定の場所にねじ２５が置かれると、中央
のブッシング１４に対して且つそれを対称軸として対称的にそれらのねじが発散
するような態様で、向きを定められている。例えば六角形のソケットである、ソ
ケット２６が、頭部２４に形成されており、もって、ねじ２５は、対応する手段
(図示せず)によって締め付けられ得る。
【００２７】 穴１９は、発散しているねじ２７であって、各々が頭部２８とねじ込軸部２９
とによって構成されているものを受容し得る。頭部２８は、雌ねじ部１９内へ螺
合するに適している雄ねじ部３１と、頭部２８の周りに形成されている端部カラ
ー３２とを有している。カラー３２は、雌ねじ部３１に対して半径方向に突出し
ていると共に、対応する角張った肩部３３であって、ねじ２７が挿入されるとこ
ろの穴１８の縁部の周りに配設されているものによって支持され得る。
【００２８】 カラー３２は、関節窩３内でのねじの締め付けの最後における、ねじ２７用の
当接手段を構成しており、このカラー３２がそれと結び付いている肩部３３と当
接するや否や、カラー３２は、ねじ２７がそれ以上移動させられるのを防止する
。同様に、ねじ２５が関節窩３内へ螺入させられ、十分遠くにまでねじ２５が係
合させられて頭部２４の面２３が凹状座部２２と接触するや否や、凹状座部２２
は、それらと結び付いているねじ２５に対して、それらをメタ関節窩エレメント
７内に停止させるための当接部として作用する。
【００２９】 この構成は、ねじの締め付けの最後にねじ２５及び２７がメタ関節窩エレメン
ト７を通過するのを、防止する。
【００３０】 関節窩球９は、実質的に半球の部品で作られており、その球面１１は、カップ
１２の対応関節面と相補的である。関節窩球９は、その内部に、円錐形の凹部３
４を備えており、この凹部３４は、軸Ｘ−Ｘを対称軸とする回転対称であると共
に、プレート１３がその中に挿入されるのを可能にするような寸法に定められて
いる。そして、軸Ｘ−Ｘを対称軸とする回転対称である関節窩球は、メタ関節窩
エレメント７上に嵌められる。
【００３１】 関節窩球９は、軸方向ねじ３５を備えており、この軸方向ねじ３５は、関節窩
球９に対して自由に回転することができる。ねじ３５の軸部３６は、その長さの
一部分に雄ねじ部１０を備えており、そして、それは、ブッシング１４の雌ねじ
部１７内へ螺入させられ得る。ねじ３５は、ハウジング３８内に挿入されるに適
している頭部３７を有しており、そのハウジング３８は、軸Ｘ−Ｘを対称軸とす
る回転対称であると共に、軸方向のオリフィス３９を介して面１１の極キャップ

され、そのハウジング４３は、軸Ｘ−Ｘ上でハウジング３８と同軸である。
【００３３】 縦軸方向のチャンネル４５が、ねじ３５の中に、そのねじの両端部において、
即ち、ソケット４１内の一方の端部において且つオリフィス３９内において、開
口している状態で配設されている。補綴具組立体は、また、関節窩球９を案内す
る手段であって、メタ関節窩エレメント７上への組み込みを容易にすべく構成さ
れているものをも具備している。図示されている実施形態においては、それらの
案内手段は、ピン４６を具備しており、このピン４６は、多少の可撓性を好適に
有しており、先ずブッシング１４の軸方向の内腔１６内へ摺動させられ、次にね
じ３５の縦方向のチャンネル４５を貫通して摺動させられるべく、構成されてい
る(図６)。ピン４６は、好適に、金属で作られており、そして、その径は、ねじ
３５におけるチャンネル４５の径よりも好都合に僅かだけ小さい。
【００３４】 ねじ３５は、滑らかな自由端部４８を有しており、この自由端部４８の先端部
は、チューブ状のブッシング１４の入口内への挿入を容易にすべく、面取りされ
ている。ねじ３５用の案内手段は、更に、ブッシング１４の入口に配設されてい
る滑らかな円筒状のゾーン４９であって、雌ねじ部１７がそれに続くものを具備
している。面取りされた先端部５１は、好適に、環状の面５２を有しており、こ
の面５２の縦断面は、直線的(即ち、円錐面)であり、そして、その面５２には、
好ましくは球形の丸みによって構成されている、丸みを付された環状の面５３が
、先端部５１の自由端部まで続いている。円錐形のセクション５２は、反対側の
端部において、滑らかなゾーン５０に合流しており、そのゾーン５０には、雄ね
じ部１０が、続いている。
【００３５】 ピン４６は、外科医により、以下のようにして使用される。
【００３６】 先ず、メタ関節窩エレメント７が、ブッシング１４を強く押し付けることによ
り且つねじ２５,２７を締め付けることにより、関節窩３に固定される。その後
、外科医は、ピン４６の一方の端部を、ブッシング１４の内腔１６内に挿入する
。カニューレを備えているねじ回しを用いて、外科医は、肩の組織及び筋肉の中
に形成されている凹部の入口へ、関節窩球９を供給し、この場合、凹部の底部は
、メタ関節窩エレメント７によって構成されており、ピン４６の端部は、軸方向
のチャンネル４５内へ挿入されている。次いで、外科医は、ねじ３５の自由端部
４８が滑らかな入口４９内に係合するまで、関節窩球９をピン４６に沿って摺動
させる(図６)。面取りされ且つ丸みを付されている先端部５１は、滑らかなゾー
ン４９の故に雌ねじ部１７を傷付けることなく且つ滑らかなゾーン５０の故に雄
ねじ部１０を傷付けることなく、ねじ３５をブッシング１４内へ挿入するのを、
容易にする。ねじ３５がブッシング１４の軸Ｘ−Ｘ上に正しく位置するや否や、
外科医は、関節窩球９がメタ関節窩エレメント７と衝突するまで、ねじ３５を締
め付け(図３)、次いで、ねじ回しとピンとを回収する。
【００３７】 円錐形のゾーン５２の丸みを付された面５３の構成、滑らかなゾーン５０の構
成、及びブッシング１４の滑らかな入口４９の構成は、案内ピン４６との組み合
わせで(図５及び図６)又はそれを用いることなく、ブッシング１４内へのねじ３
５の正しい案内をもたらすべく構成され得る。そのような状況下では、ねじ３５
は、縦方向のカニューレ１６を有し得、又は、それは、中実であり得る。それに
も拘らず、内腔１６、ピン４６、滑らかなゾーン５０を備えている端部４８、及
びブッシング１４の入口ゾーン４９の組み合わせが、メタ関節窩エレメント７の
軸Ｘ−Ｘ上の関節窩９の正しい位置までの、その関節窩球用の最も満足の行く案
内を提供する。
【００３８】 換言すると、以下の変型態様が、可能である。
【００３９】 −ねじ３５は、カニューレを有し得、即ち、軸方向のチャンネル１６を提供し
得、且つ、案内ピン４６と共に使用され得、この場合、ねじ３５の雄ねじ部とブ
ッシング１４の雌ねじ部との間に、オフセットが、存在しない。そのような状況
下では、雄ねじ部１０は、殆どねじ３５の自由端部まで延在し、そして、滑らか
なゾーン４９は、追加の雌ねじ部１７によって置き換えられる。
【００４０】 −ねじ３５は、カニューレを有しておらず、従って、軸方向のチャンネル１６
を提供しておらず、しかし、雄ねじ部１０及び雌ねじ部１７は、図面に示されて

【００４２】 ねじ３５の端部４８は、異なる輪郭を有し得、例えば、それは、参照数字５２
によって指示されているような、単純な円錐面であってもよく、あるいは、それ
は、単なる適切なサイズの丸みを付された端部面取り部であってもよい。
【００４３】 上述した関節窩補綴具１に対する適応症は、本質的に、外傷性障害、上腕骨と
肩胛骨との間の関節症、リューマチ様多発性関節炎である。概して、本発明の肩
補綴具は、回旋腱板の異常に対して適応される。

  1. 【請求項１】 肩補綴具組立体であって、 中央管状ブッシング(１４)を設けられているプレート(１３)によって形成され
    ているメタ関節窩エレメント(７)であって、それを関節窩(３)に固定する部材(
    ２５,２７)を伴っているものと、 メタ関節窩エレメント上に嵌められるべく構成されている関節窩球(９)であっ
    て、前記ブッシング内へ螺入させられ得る軸方向ねじ(３５)を備えているものと
    、 を具備しており、 関節窩球は、それと結び付いている上腕骨補綴具(２)のカップ(１２)と協働す
    る関節面(１１)を提供している、 ものにおいて、 それが、関節窩球を案内する手段(１６,４５,４６；５１,５０,４９・・・)で
    あって、ブッシング(１４)の軸(Ｘ−Ｘ)上での、メタ関節窩エレメント上へのね
    じ(３５)の正しい配置を容易にすべく構成されているものを備えている、 ことを特徴とする補綴具組立体。
  2. 【請求項２】 前記案内手段が、管状ブッシング(１４)内へ摺動させられ得
    るべく構成されているピン(４６)を具備しており、且つ、関節窩球(９)のねじ(
    ３５)が、縦軸方向チャンネル(４５)であって、そのねじの両端部において開口
    しており且つピンが前記チャンネル内へ挿入されるのを可能にするものを提供し
    ており、もって、ピンは、ねじが管状ブッシング内へ挿入されるまで、そのピン
    に沿ってねじが摺動する際に関節窩球を案内すべく機能することを特徴とする請
    求項１の補綴具組立体。
  3. 【請求項３】 ピン(３５)の軸方向チャンネル(４５)が、一方の端部におい
    て、ねじの頭部(３７)に設けられており且つカニューレを有しているねじ回しと
    協働するのに適しているソケット(４１)内へ開口していることを特徴とする請求
    項２の補綴具組立体。
  4. 【請求項４】 前記案内手段が、関節窩球(９)のねじ(３５)の滑らかな自由
    端部(４８)と、ブッシングへの前記入口に設けられており且つ雌ねじ部(１７)に
    続いている滑らかなゾーン(４９)とを具備していることを特徴とする請求項１〜
    請求項３のいずれか一項の補綴具組立体。
  5. 【請求項５】 自由端部(４８)が、円錐環状面(５２)を画定している、面取
    りされた先端部(５１)を有しており、その円錐環状面(５２)には、好ましくは球

    りに形成されているカラー(３２)であって、ねじ頭部の雄ねじ部(３１)に対して
    半径方向に突出しているものと、雌ねじを螺刻されている穴(１８)の縁部の周り
    に設けられている対応環状支持肩部(３３)であって、メタ関節窩エレメント(７)
    内へのねじの挿入のためのものとを具備していることを特徴とする請求項６の補
    綴具組立体。
  8. 【請求項８】 当接手段が、凸状外面(２３)を呈している少なくとも１つの
    ねじ(２５)の頭部(２４)を具備しており、その凸状外面(２３)は、前記ねじをメ
    タ関節窩エレメント(７)内に挿入するための穴(２１)の壁部に形成されている相
    補的凹状座部(２２)に当接すべく構成されていることを特徴とする請求項６又は
    請求項７の補綴具組立体。
  9. 【請求項９】 関節窩球(９)の軸方向ねじ(３５)をメタ関節窩エレメント(
    ７)の管状ブッシング(１４)内に案内する手段(１６,４５,１７,４９,５０,５１,
    ５２,５３)を備えている、請求項１〜請求項８のいずれか一項の補綴具組立体の
    部分を形成している肩補綴具(１)。
  10. 【請求項１０】 前記案内手段が、丸みを付された面(５２,５３)を呈して
    いる先端部(５１)を有しているねじ(３５)の一方の端部(４８)と、その先端部と
    ねじ(３５)の雄ねじ部(１０)との間の滑らかなゾーン(５０)と、ブッシング(１
    ４)内のチャンネル(１６)への入口を構成している滑らかなゾーン(４９)とを具
    備していることを特徴とする請求項９の補綴具。
  11. 【請求項１１】 縦方向チャンネル(４５)が、関節窩球(９)の軸方向ねじ(
    ３５)に設けられており、そのチャンネルは、関節窩球をメタ関節窩エレメント(
    ７)へ案内する案内ピン(４６)を受容し得、メタ関節窩エレメントのブッシング(
