Description

PROTHESE DE HANCHE
L'invention concerne une prothèse de hanche.
Les prothèses totales de hanche se composent d'une partie fémorale et d'une partie cotyloidienne.
L'invention concerne la partie fémorale.
La pièce fémorale de la prothèse de hanche se compose habituellement d'un noyau long plongeant dans la diaphyse fémorale, se terminant à sa partie supérieure, par un col de forme tronconique, et d'une pièce sphérique, ou tête, qui vient s'emboîter sur le col tronconique.
Les pièces fémorale et cotyloïdienne des prothèses totales de hanche sont de taille variée, adaptable au volume du cotyle et au volume de la diaphyse fémorale, variables suivant les patients.
Outre ce problème d'adaptation des pièces prothétiques au volume osseux du patient, sont à prendre en considération
- le réglage de la longueur du membre inférieur,
- le réglage de l'écartement transverse du membre inférieur, ou latéralisation,
- le réglage de l'anteversion du col fémoral.
Les deux premiers paramètres sont å l'heure actuelle réglés :
- pour la longueur du membre inférieur par l'enfoncement plus ou moins important de la pièce fémorale dans la diaphyse fémorale et par l'utilisation de tête sphérique dont l'enfoncement peut être varié sur le col tronconique,
- pour la latéralisation de la diaphyse par le même procédé de tête sphérique plus ou moins enfoncée sur le col fémoral.
Il reste cependant un problème qui n'est pas résolu, celui de l'anteversion, c'est-à-dire de l'orientation du col par rapport au diamètre bicondylien.
Cette anteversion est habituellement réglée en faisant effectuer à l'ensemble de la pièce prothétique fémorale un mouvement de rotation de telle façon que, dans un plan horizontal, l'axe du col prothétique fasse avec le diamètre bi-condylien frontal, un angle compris en général entre 1~ et 3~
Mais faire effectuer à la pièce prothétique fémorale une rotation dans la diaphyse fémorale modifie l'adaptation de la prothèse au fût médullaire.
Le but de la présente invention est de proposer une prothèse de hanche qui permette de régler à la fois les problèmes de longueur du membre inférieur, de latéralisation et d'anteversion.
L'invention a pour objet une prothèse de hanche, du type comportant un noyau fémoral plongeant dans la diaphyse fémorale et une tête sphérique s'articulant avec le cotyle et dont la position est réglable sur le col de la prothèse, par enfoncement varié pour régler la longueur du col fémoral pour régler la longueur du col fémoral, caractérisée en ce qu'elle comporte de plus une pièce intermédiaire , disposée entre le noyau fémoral et la sphère, le noyau fémoral portant à sa partie supérieure horizontale un pivot tronconique sur lequel vient se placer ladite pièce intermédiaire pour assurer par pivotement le réglage de l'anteversion.
Selon d'autres caractéristiques de l'invention
- la pièce intermédiaire est de hauteur variable en fonction du patient et des conditions opératoires locales, le choix de ladite pièce intermédiaire assurant le réglage de la longueur du membre inférieur.
- la pièce intermédiaire est fixée au pivot du noyau par l'intermédiaire d'une vis qui est bloquée après réglage de l'anteversion.

Sur la figure 1, on voit un fémur 1 équipé d'une prothèse selon l'invention. Cette prothèse comporte un noyau 2 logé dans la diaphyse fémorale, une pièce intermédiaire 3 et une tête sphérique (4?.
Le noyau 2 est destiné à être mis en place dans la diaphyse fémorale, il est d'une forme générale pouvant servir pour pratiquement toutes les prothèses. Il porte à sa partie supérieure un tronc de cône 5 jouant le rôle de pivot pour la pièce intermédiaire 3. Ce pivot est taraudé pour recevoir une vis 6 de fixation de la pièce intermédiaire 3.
La pièce intermédiaire 3 vient s'adapter sur le noyau par l'intermédiaire du pivot 5. Elle porte un col 7 constitué par un cône morse sur lequel vient se placer la sphère 4, Pour régler la longueur du membre inférieur, on dispose de plusieurs pièces intermédiaires de différentes hauteurs qui s'adaptent sur le noyau fémoral. Pour le réglage de la longueur du col et de la latéralisation, on procède de la manière connue en réglant l'enfoncement de la sphère 4 sur le col 7. Enfin, pour le réglage de l'anteversion, on fait pivoter la pièce intermédiaire 3 sur le pivot 5 du noyau, on la fixe par vissage de la vis 6 et on bloque cette vis.
La figure 2 représente en trait plein une position de la pièce intermédiaire 3 correspondant à la position habituelle du col des prothèses connues, et en trait interrompu, deux positions de la pièce intermédiaire 3 correspondant à deux réglages opposés de l'anteversion.
Ainsi, grâce à la disposition de la pièce intermédiaire dans la prothèse selon l'invention, on assure le réglage de la longueur du membre inférieur, de la longueur du col fémoral, de la latéralisation du membre inférieur et de l'anteversion.

Claims (3)

REVENDICATIONS
1. - Prothèse de hanche, du type comportant un noyau fémoral plongeant dans la diaphyse fémorale et une tête spérique s'articulant avec le cotyle et dont la position est réglable sur le col de la prothèse, par enfoncement varié pour régler la longueur du col fémoral, caractérisée en ce qu'elle comporte de plus une pièce intermédiaire (3), disposée entre le noyau fémoral (2) et la sphère (4), le noyau fémoral (2 > portant à sa partie supérieure horizontale un pivot tronconique (5 > sur lequel vient se placer ladite pièce intermédiaire (3) pour assurer par pivotement le réglage de l'anteversion.
2. - Prothèse selon la revendication 1, caractérisée en ce que la pièce intermédiaire (3) est de hauteur variable en fonction du patient et des conditions opératoires locales, le choix de ladite pièce intermédiaire assurant le réglage de la longueur du membre inférieur.
3. - Prothèse selon la revendication 1, caractérisée en ce que la pièce intermédiaire (3) est fixée au pivot (5 > du noyau (2) par l'intermédiaire' d'une vis (6) qui est bloquée après réglage de 1' anteversion.
