Abstract

The brace comprises a first bone plate 1 with a row of holes, a number of bone screws 4 each with a shoulder to receive the plate at a spacing from the bone and a threaded head portion 6 which projects through the holes and receives a threaded locking member 8, each locking member itself having a shoulder and a projecting threaded head portion 11 to locate and secure a second bone plate 13 generally parallel to but spaced from the first. The brace may be used for internal or external osteosynthesis. <IMAGE>

Description

Bone Brace for osteosynthesis The invention relates to a bone brace for osteosynthesis.
To fit the broken bone sections of a bone fracture self tightening bone plates with a high axial and flexural stiffness are employed, that are fastened to the bone by means of bone screws. This application of bone fracture healing exhibits a number of advantages. However the high contact pressure along the bone weakens the bone shell and damages the bone skin. Furthermore an insufficient mineralisation of the bone shell has been observed under the bone plate. The healing of the bone and the imminent improved stability and increased bone substance only started after removal of the solid bone plate.
To overcome these disadvantages a tolerable osteosynthesis plate according to DE OS 31 18 344 was designed, that is sufficiently rigid to ensure a fixation of the bone in the early healing stages of the bone fracture, but is not too rigid to lead to a deterioration of the bone in the later healing stages. The bone plate is designed as a hollow body with a biologically inert polymeric material placed in the metal zones, which prevents ingrowing of the bone.
According to CH-PS 668 A74 an osteosynthetic pressure plate is known which shows a widening cross section profile with increasing distance from the bone. This produces a reduced application area on the bone with retention of the pressure plate's rigidity, so that a smaller part of the bone surface is covered and an increased circulation in the bone is achieved. Both solutions need sophisticated manufacturing, and the contact between bone surface and boneplate is nonetheless present even if to a lesser extent. In PL-PS 145 631 and DD-PS 258 361 a bone brace comprising of a bone plate and bone screws is shown. The bone screws with bone thread have, instead of a screwhead, a square neck and a threaded extension. The bone screws are screwed into the bone parts by means of a square spanner such that the neck has a spacing from the bone skin.However, the necks have to be aligned such that they engage with the bone plate, that has a groove on its bone facing side, on application.
The bone plate is fixed with nuts which are screwed on to the threaded extensions. Due to the design of the neck, adjustment only is possible in four steps per turn. Since bone screws haves a high pitch it is hardly possible to align the necks in one plane. Upon attachment of the bone plate, tensions are encountered which put additional pressure on the position of the screws in the bones. In addition to that the stability of bone plates normal dimensions is not sufficient, especially not in the treatment of fractures of the lower extremites.
The aim of the invention is the development of a bone brace for internal and external osteosynthesis, where the bone plate is arranged with a spacing to the bone surface ensuring a sufficient axial rigidity of the bone plate during the early healing stages and a tensionless tightening of the bone screws is achieved.
The invention achieves this aim, in that one or more bone plates are arranged with a pacing from the first bone plate by means of distance pieces. The distance pieces are provided with an internal thread which can be brought into engagement with the threaded extension of the bone screw and at the opposite end are provided with a support surface and another threaded extension. The bone screws have preferably a circular collar. The bone screws beneath their collar, the distance pieces, and the nuts, that are capable of being screwed on to the threaded extensions, are provided with a polygonal member, particularly a hexagonal shaft, with identical dimension.
Particularly for fractures of the lower extremities, the bone portions are fixed by means of the first bone plate with suitable longitudinal dimensions in that the bone screws are screwed in first and their collars aligned..
On to them the bone plate is laid and braced with the distance pieces that are screwed on the threaded extensions. For stabilisation an additional bone plate, that can differ in its longitudinal dimension from the first bone plate, is laid on the distance pieces and braced with them by means of nuts that are screwed on the threaded extensions of the distance pieces. It is however possible to fix a part of the bone screws on the first bone plate by means of the nuts.
The advantage of the invention is that the bone brace exhibits sufficiently high rigidity in the early stages of healing especially with the treatment of fractures of the lower extremities.
The invention is described with reference to an exemplary embodiment and with the accompanying drawing.
The bone brace comprises a bone plate (1) with a row of oval conical holes (2) and groove (3) on its bone facing side. The bone fragments are fixed on to it at a spacing from the bone surface by mean of bone screws (4). The bone screw (4) has an annular collar (5) and a threaded extension (6) instead of a screw head. A polygonal member (7) for screwing in bone screw (4) which serves as support for the bone plate (1) is arranged beneath collar (5). Boneplate (1) is fixed to the bonescrews (4) by means of distance pieces (8) on its bone facing side.
Distance piece (8) is axially symmetrical, its bone plate facing end is shaped complementary to the screwhead of the bonescrew and is provided with an internal thread (9) which can be brought into engagement with the threaded extension (6) of the bone screw (4). At the opposite end it is shaped to act as a support surface (10) on which a threaded extension (11) of identical dimensions to threaded extension (6) is arranged. Beneath the threaded extension (11) the diameter of distance piece (8) equals the diameter of collar (5) of bonescrew (4). Distance piece (8) has a polygonal member (12) for tightening. On to the threaded extensions (11) a second bone plate (13) of identical design is placed.
Bone plate (13) is fixed on the distance pieces (8) by means of a nut (14) which on the bone plate (13) facing side is shaped like the screw head of a bone screw and, which on the opposite side provides a polygonal member.
Preferably the polygonal member (7) of the bone screw (4), the polygonal member (12) of the distance piece (8) and the polygonal member of the nut (14) are designed as a hexagonal shaft with identical dimensions.

Claims (5)

Patent Claim
1. A bone brace for osteosynthesis comprising a bone plate with a row of holes which is held at a spacing from the bone and engages over the broken bone sections, bone screws with a square neck and a threaded extension together, with nuts rounded towards the boneplate and provided with a square head at the opposite side, characterised in that one or more bone plates (13) are arranged spaced from the bone plate (1) by distance pieces (8), and that the distance pieces (8) are provided with an internal thread (9) which can be brought into engagement with the threaded extension (6) of the bone screw (4), and which at the opposite end are provided with a support surface (10) and a threaded extension (11).
2. A bone brace according to claim 1, characterised in that the bone screw (4) has a ring-like collar (5).
3. Bone brace according to claims 1 and 2, characterised in that the bone screw (4) beneath its neck (5), the distance piece (8), and the nut (14) are provided with a polygonal member, particularly a hexagonal shaft, with identical dimensions.

A bone brace for osteosynthesis comprising a first bone plate (1) with a row of holes, a number of;tne screws (#4) each with a shoulder to receive the plate (1) at a spacing from the bone and a threaded head portion (6) which projects through the holes and receives a threaded locking member (8), wherein the locking members (8) each have a shoulder and a projecting threaded head portion (11) to locate and loc - second bone plate (13) generally parallel to but spaced from the first (1).
5. A bone brace for osteosynthesis substantially as herein described with reference to the accompanying drawings.
