Abstract
La présente invention concerne une prothèse de disque intervertébral, comprenant au moins trois pièces dont un premier plateau, dit plateau supérieur (1), un deuxième plateau, dit plateau inférieur (2), et un noyau (3), la surface supérieure (31) du noyau (3) étant en contact avec au moins une partie (10) de la surface inférieure du plateau supérieur (1) et la surface inférieure (32) du noyau (3) étant en contact avec au moins une partie de la surface supérieure du plateau inférieur (2), et le plateau inférieur (2) étant mobile au moins par rapport au noyau (3), caractérisée en ce que des moyens de coopération existent entre le plateau inférieur (2) et le noyau (3), de façon à limiter ou supprimer les mouvements en translation du noyau (3) par rapport au plateau inférieur (2), selon un axe sensiblement parallèle au plateau inférieur (2), et à limiter ou supprimer les mouvements en rotation du noyau (3) par rapport au plateau inférieur (2), autour d'un axe sensiblement perpendiculaire au plateau inférieur (2), les plans passant par les plateaux supérieur (1) et inférieur (2) formant un angle sensiblement constant.
Description
Prothèse de disque intervertébral La présente invention concerne une
prothèse de disque intervertébral, destinée à être substituée aux disques fibro-cartilagineux assurant la liaison 5 entre les vertèbres de la colonne vertébrale, en particulier au niveau du
rachis cervical.
Il est connu dans l'art antérieur divers types de prothèses. Une partie de ces prothèses, soit parce qu'elles sont en matériau compressible, soit parce qu'elles permettent un mouvement trop important entre elles des 10 différentes pièces constituant la prothèse, peuvent entraîner relativement facilement l'éjection d'au moins une partie de la prothèse vers l'extérieur des
vertèbres, ce qui n'est pas souhaitable pour le patient.
La présente invention a pour but de pallier certains inconvénients de l'art antérieur en proposant une prothèse de disque intervertébral simple et 15 qui permette de limiter les mouvements entre elles des différentes pièces
constituant la prothèse.
Ce but est atteint par une prothèse de disque intervertébral comprenant au moins trois pièces dont un premier plateau, dit plateau supérieur, un deuxième plateau, dit plateau inférieur, et un noyau, la surface 20 supérieure du noyau étant en contact avec au moins une partie de la surface inférieure du plateau supérieur et la surface inférieure du noyau étant en contact avec au moins une partie de la surface supérieure du plateau inférieur, et le plateau inférieur étant mobile au moins par rapport au noyau, caractérisée en ce que des moyens de coopération existent entre le plateau 25 inférieur et le noyau, de façon à limiter ou supprimer les mouvements en translation du noyau par rapport au plateau inférieur, selon un axe sensiblement parallèle au plateau inférieur, et à limiter ou supprimer les mouvements en rotation du noyau par rapport au plateau inférieur, autour d'un axe sensiblement perpendiculaire au plateau inférieur, les plans passant 30 par les plateaux supérieur et inférieur formant un angle sensiblement constant. Selon une autre particularité, le plateau inférieur comporte des
moyens mâles coopérant avec des moyens femelles du noyau.

Selon une autre particularité, l'angle est obtenu par le fait que le noyau
forme un angle aigu dans le sens antéro-postérieur.
Selon une autre particularité, les mêmes plateaux sont assemblables
avec des noyaux d'épaisseurs différentes.
Selon une autre particularité, l'angle entre les plateaux supérieur et 10 inférieur est compris entre 0 et 150.
Selon une autre particularité, les mouvements du noyau par rapport aux plateaux supérieur et/ou inférieur donnent une liberté de mouvement au patient et permettent, en même temps, de rattraper les défauts de
positionnement de la prothèse.
Selon une autre particularité, au moins une partie de la surface inférieure du plateau supérieur est concave et complémentaire de la surface
supérieure du noyau.
Selon une autre particularité, les dimensions de chaque moyen mâle
sont légèrement inférieures à celles de chaque moyen femelle de façon à 20 permettre un léger débattement entre le noyau et le plateau inférieur.
Selon une autre particularité, les dimensions de chaque moyen mâle sont sensiblement les mêmes que celles de chaque moyen femelle de façon
à empêcher tout débattement entre le noyau et le plateau inférieur.
Selon une autre particularité, les moyens mâles du plateau inférieur 25 sont deux ergots recourbés vers l'intérieur de la prothèse et situés en vis-àvis l'un de l'autre sur deux bords de la prothèse, et en ce que les moyens
femelles du noyau sont deux évidements.
Selon une autre particularité, au moins un des ergots est remplacé par
un plot muni d'un perçage sur lequel est fixée une patte à l'aide d'une 30 goupille pénétrant dans le perçage.
Selon une autre particularité, les moyens mâles du plateau inférieur sont deux tétons situés à proximité du centre du plateau inférieur, et en ce
que les moyens femelles du noyau sont deux puits.
Selon une autre particularité, les moyens mâles du plateau inférieur s sont deux parois situées en vis-à-vis l'une de l'autre à proximité de deux bords de la prothèse, et en ce que les moyens femelles du noyau sont des évidements. Selon une autre particularité, les moyens mâles du plateau inférieur
sont une nervure située au centre de la prothèse, et en ce que les moyens io femelles du noyau sont une rainure.
Selon une autre particularité, le noyau est en polyéthylène.
Selon une autre particularité, le plateau inférieur comprend une ou plusieurs ouvertures à proximité de son côté antérieur, prévues pour
accueillir des moyens d'ancrage de la prothèse dans une vertèbre.
Selon une autre particularité, l'ouverture du plateau inférieur est rectangulaire, et en ce que le moyen d'ancrage est constitué d'un corps,
faisant avec le plateau inférieur un angle aigu, et d'une tête.
Selon une autre particularité, les ouvertures du plateau inférieur sont
circulaires, et en ce que les moyens d'ancrage ont la forme d'un clou.
Selon une autre particularité, la prothèse étant ancrée dans une vertèbre, le plateau inférieur est pris en sandwich entre la tête des moyens
d'ancrage et ladite vertèbre.
Selon une autre particularité, le plateau supérieur est bombé sur au
moins une partie de sa surface supérieure pour s'adapter à la forme des 25 vertèbres.

La prothèse de disque intervertébral selon l'invention est constituée d'un plateau supérieur (1) qui s'articule par rapport à un plateau inférieur (2) par l'intermédiaire d'un noyau (3), comme cela est visible en particulier sur les figures 4a, 4b, 5a et 6a. Un avantage de la prothèse selon l'invention est s qu'elle comprend des pièces simples qui peuvent être dimensionnées pour
que la prothèse soit posée sur le rachis cervical.
Le plateau supérieur (1), visible en particulier sur les figures la et 1 b, est légèrement concave sur au moins une partie (10) de sa surface inférieure, de façon à s'adapter à la surface supérieure légèrement convexe io du noyau (3). La surface supérieure du noyau (3) est complémentaire de la partie concave (10) du plateau supérieur (1), ce qui permet le mouvement
entre le plateau supérieur (1) et le noyau (3).
Dans une variante de réalisation, une partie de la surface supérieure du plateau supérieur (1) est bombée, comme cela est représenté sur les 15 figures 10a et 10b, afin d'adapter de mieux s'adapter à la vertèbre sur laquelle la prothèse est destinée à être posée, le dessous des vertèbres étant en creux. La partie bombée du plateau supérieur (1) est alors située dans la partie antérieure du plateau supérieur, comme cela est visible en
particulier sur la figure 1 Ob.
Le plateau inférieur (2) est sensiblement plan. En effet, sa surface inférieure n'a pas besoin d'être bombée ou en creux puisque le dessus des vertèbres est sensiblement plat. Dans le mode de réalisation des figures 2a, 2b, 7a et 8a, le plateau inférieur (2) comprend deux ergots (20) situés en visà-vis l'un de l'autre sur deux bords (21, 22) sensiblement parallèles du 25 plateau inférieur (2). Chaque ergot (20) est recourbé vers l'intérieur de la prothèse et peut ainsi pénétrer dans des évidements (30) situés sur le noyau (3). Le noyau (3), visible en particulier sur les figures 3a et 3b, comporte une surface inférieure sensiblement plane, prévue pour s'adapter sur le plateau inférieur (2). Le noyau (3) est de faible épaisseur (par exemple 3 mm) pour 30 une prothèse cervicale ou d'épaisseur plus importante (par exemple 15 mm)
pour une prothèse lombaire.
Dans le mode de réalisation des figures 3a, 3b, 4a et 4b, les dimensions de chaque évidement (30) du noyau (3) sont légèrement supérieures à celles de chaque ergot (20) du plateau inférieur (2) de façon à limiter le débattement du noyau (3) par rapport au plateau inférieur (2), à la 5 fois en translation suivant un axe sensiblement parallèle au plateau inférieur (2), et en rotation autour d'un axe sensiblement perpendiculaire au plateau inférieur (2). Le mouvement entre le plateau supérieur (1) et le noyau (3), ainsi que le débattement du noyau (3) par rapport au plateau inférieur (2) permettent ainsi au patient de bouger, et, éventuellement, de rattraper les 10 défauts de positionnement de la prothèse. Ce débattement présente également l'avantage d'empêcher une usure prématurée due aux contraintes
appliquées sur la prothèse.
Dans le mode de réalisation des figures 5a et 5b, les dimensions de chaque évidement (30) du noyau (3) sont sensiblement les mêmes que 15 celles de chaque ergot (20) du plateau inférieur (2), de façon à empêcher un quelconque débattement du noyau (3) par rapport au plateau inférieur (2), à la fois en translation et en rotation. Dans ce dernier cas, le seul mouvement de la prothèse autorisé est celui du plateau supérieur (1) par rapport au
noyau (3).
Dans le mode de réalisation des figures 9a et 9b, un des ergots (20)
est remplacé par un plot muni d'un perçage (200). Une patte (23) vient fixer sur le plot grâce à par une goupille (24) pénétrant dans le perçage (200).
Dans une variante de réalisation, les deux ergots sont remplacés par un plot
auquel vient se fixer une patte (23).
Dans le mode de réalisation des figures 6a, 6b, 6c, 6d et 6e, le plateau inférieur (2) ne comporte pas d'ergots (20) mais deux tétons (25) situés à proximité du centre du plateau inférieur (2). Le noyau (3), par complémentarité, ne comporte alors pas d'évidements (30), mais deux puits (35) sous sa surface inférieure. Les dimensions des tétons (25) du plateau 30 inférieur (2) et des puits (35) du noyau (3) sont telles que, dans la variante représentée sur les figures 6b et 6c, un léger débattement en translation et en rotation est permis, et dans la variante représentée sur les figures 6d et
6e, aucun débattement n'est permis.
Dans un autre mode de réalisation, non représenté, le plateau inférieur (2) comporte une nervure sur sa surface supérieure et pas d'ergots 5 (20) ni de tétons (25). Le noyau (3), par complémentarité, comporte une rainure sous sa surface inférieure. Les dimensions de la nervure du plateau inférieur et de la rainure du noyau sont telles que, dans une variante, un léger débattement en translation et en rotation est permis, et dans une autre
variante, aucun débattement n'est permis.
Dans un autre mode de réalisation non représenté, le plateau inférieur (2) comporte, à la place des ergots (20), deux parois, disposées en vis-à- vis l'une de l'autre, à proximité de deux bords (21, 22) sensiblement parallèles du plateau inférieur, mais plus vers l'intérieur de la prothèse que les ergots (20). Le noyau (3) comporte des évidements complémentaires aux parois. 15 Les dimensions de chaque évidement du noyau de ce mode de réalisation sont, soit légèrement supérieures, soit sensiblement les mêmes que celles de chaque paroi du plateau inférieur, de façon à permettre ou non un léger
débattement en translation et en rotation.
Dans encore un autre mode de réalisation non représenté, les 20 éléments femelles sont situés sur le plateau inférieur et les éléments mâles
sur le noyau.
La prothèse de disque intervertébral selon l'invention permet en particulier de corriger les défauts de lordose, et de remettre de la lordose au rachis, par exemple cervical. La présence d'un angle aigu dans le sens 25 antéro-postérieur (F, figure 4b), entre le plateau supérieur (1) et le plateau
inférieur (2) de la prothèse est donc nécessaire. Par exemple, cet angle est compris entre 0 et 150. Pour ajuster l'angle nécessaire en fonction du patient, il suffit de choisir un noyau (3) avec un angle approprié entre le plan moyen représentant sa surface supérieure et le plan passant par sa surface 30 inférieure.
Dans le cas o les éléments femelles sont situés sur le plateau inférieur et les éléments mâles sur le noyau, le noyau lordosant, par le fait qu'il forme un angle aigu dans le sens antéro-postérieur, peut alors être rendu solidaire du plateau par une saillie pénétrant dans une cavité ou
ouverture du plateau inférieur.
L'inclinaison des prothèses connues dans l'art antérieur est obtenue, 5 soit par la forme du plateau supérieur, lorsque le noyau est plat, soit par la position du plateau supérieur par rapport au noyau, lorsque ce dernier est bombé. Par rapport au premier cas de l'art antérieur cité ici, l'usinage de la prothèse de la présente invention est plus économique car le noyau est composé d'un matériau moins cher (par exemple, polyéthylène) que celui 1o composant les plateaux. Par rapport au second cas de l'art antérieur cité ici le noyau de la présente invention ne risque pas s'éjecter hors de la prothèse puisque l'angle entre les plateaux est sensiblement constant lorsque la
prothèse est en place.
Si le chirurgien souhaite une lordose déterminée pour un patient, il 15 choisira un noyau (3) qui ne puisse avoir aucun débattement par rapport au plateau inférieur (2). En revanche, s'il souhaite seulement que la lordose reste dans une plage de valeurs, il choisira un noyau qui puisse avoir un léger débattement en translation et en rotation par rapport au plateau
inférieur (2).
La prothèse de disque intervertébral selon l'invention peut, dans une variante de réalisation, représentée sur les figures 7a, 7b, 8a et 8b, être ancrée dans la colonne vertébrale pour empêcher la prothèse de migrer sous l'effet de la résultante transversale de la force exercée par la colonne vertébrale sur la prothèse en place, qui est d'autant plus importante que la 25 lordose est importante. Dans ce cas, le plateau inférieur (2) comprend une ou plusieurs ouvertures (28, 29) situées à proximité du côté postérieur de la
prothèse, permettant d'accueillir des moyens d'ancrage (4, 5).
Ainsi, dans le cas des figures 7a et 7b, l'ouverture (28) du plateau inférieur (2) est rectangulaire et le moyen d'ancrage (4) est constitué d'un 30 corps (40) et d'une tête (41). Les dimensions de la tête (41) sont légèrement supérieures à celles de l'ouverture (28) du plateau inférieur (2), de façon à ce que, une fois le moyen d'ancrage (4) en place dans une vertèbre, le plateau inférieur (2) soit pris en sandwich entre la tête (41) du moyen d'ancrage (4) et ladite vertèbre. Un angle, inférieur ou égal à 900, est compris entre le corps
(40) du moyen d'ancrage (4) et le plateau inférieur (2).
Dans le cas des figures 8a et 8b, deux ouvertures circulaires (29) sont 5 comprises dans le plateau inférieur (2) et les moyens d'ancrage (5) ont la forme de clous, avec une tête de dimensions supérieures à celles des ouvertures (29) pour permettre de prendre en sandwich le plateau inférieur (2) entre la tête des moyens d'ancrage (5) et la vertèbre sur laquelle est
ancrée la prothèse.
Il doit être évident pour les personnes versées dans l'art que la
présente invention permet des modes de réalisation sous de nombreuses autres formes spécifiques sans l'éloigner du domaine d'application de l'invention comme revendiqué. Par conséquent, les présents modes de réalisation doivent être considérés à titre d'illustration, mais peuvent être is modifiés dans le domaine défini par la portée des revendications jointes, et
l'invention ne doit pas être limitée aux détails donnés ci-dessus.
Claims (20)
1. Prothèse de disque intervertébral comprenant au moins trois pièces dont un premier plateau, dit plateau supérieur (1), un deuxième plateau, dit 5 plateau inférieur (2), et un noyau (3), la surface supérieure du noyau (3) étant en contact avec au moins une partie (10) de la surface inférieure du plateau supérieur (1) et la surface inférieure du noyau (3) étant en contact avec au moins une partie de la surface supérieure du plateau inférieur (2), et le plateau supérieur (1) étant mobile au moins par rapport au noyau (3), i0 caractérisé en ce que des moyens de coopération existent entre le plateau inférieur (2) et le noyau (3), de façon à limiter ou supprimer les mouvements en translation du noyau (3) par rapport au plateau inférieur (2) selon un axe sensiblement parallèle au plateau inférieur (2), et à limiter ou supprimer les mouvements en rotation du noyau (3) par rapport au plateau inférieur (2), 15 autour d'un axe sensiblement perpendiculaire au plateau inférieur (2), les plans passant par les plateaux supérieur (1) et inférieur (2) formant un angle
sensiblement constant.
2. Prothèse de disque intervertébral selon la revendication 1,
caractérisée en ce que le plateau inférieur (2) comporte des moyens mâles 20 coopérant avec des moyens femelles du noyau (3).
3. Prothèse de disque intervertébral selon la revendication 1, caractérisée en ce que le plateau inférieur (2) comporte des moyens femelles
coopérant avec des moyens mâles du noyau (3).
4. Prothèse de disque intervertébral selon une des revendications 1 à 25 3, caractérisée en ce que l'angle est obtenu par le fait que le noyau (3) forme
un angle aigu dans le sens antéro-postérieur (F).
5. Prothèse de disque intervertébral selon la revendication 4, caractérisée en ce que les mêmes plateaux (1, 2) sont assemblables avec
des noyaux (3) d'épaisseurs différentes.
6. Prothèse de disque intervertébral selon une des revendications 4
ou 5, caractérisée en ce que l'angle entre les plateaux supérieur (1) et
inférieur (2) est compris entre 0 et 150.
7. Prothèse de disque intervertébral selon une des revendications 1 à
6, caractérisée en ce que le noyau (3) est mobile par rapport aux plateaux supérieur (1) et/ou inférieur (2), ce qui permet de rattraper les défauts de positionnement des trois pièces (1, 2, 3) de la prothèse les unes par rapport aux autres.
8. Prothèse de disque intervertébral selon une des revendications 1 à
7, caractérisée en ce qu'au moins une partie (10) de la surface inférieure du plateau supérieur (1) est concave et complémentaire de la surface
supérieure (31) du noyau (3).
9. Prothèse de disque intervertébral selon une des revendications 1 à
8, caractérisée en ce que les dimensions de chaque moyen mâle sont légèrement inférieures à celles de chaque moyen femelle de façon à
permettre un léger débattement entre le noyau (3) et le plateau inférieur (2).
10. Prothèse de disque intervertébral selon une des revendications 1 à 15 8, caractérisée en ce que les dimensions de chaque moyen mâle sont
sensiblement les mêmes que celles de chaque moyen femelle de façon à
empêcher tout débattement entre le noyau (3) et le plateau inférieur (2).
11. Prothèse de disque intervertébral selon une des revendications 2
et 4 à 10, caractérisée en ce que les moyens mâles du plateau inférieur (2) 20 sont deux ergots (20) recourbés vers l'intérieur de la prothèse et situés en vis-à-vis l'un de l'autre sur deux bords (21, 22) de la prothèse, et en ce que
les moyens femelles du noyau (3) sont deux évidements (30).
12. Prothèse de disque intervertébral selon la revendication 11, caractérisée en ce qu'au moins un des ergots (20) est remplacé par un plot 25 muni d'un perçage (200) sur lequel est fixée une patte (23) à l'aide d'une
goupille (24) pénétrant dans le perçage (200).
13. Prothèse de disque intervertébral selon une des revendications 2
et 4 à 10, caractérisée en ce que les moyens mâles du plateau inférieur (2) sont deux tétons (25) situés à proximité du centre du plateau inférieur (2), et 30 en ce que les moyens femelles du noyau (3) sont deux puits (35).
14. Prothèse de disque intervertébral selon une des revendications 2
et 4 à 10, caractérisée en ce que les moyens mâles du plateau inférieur (2) sont deux parois situées en vis-à-vis l'une de l'autre à proximité de deux bords (21, 22) de la prothèse, et en ce que les moyens femelles du noyau (3)
sont des évidements.
15. Prothèse de disque intervertébral selon une des revendications 2 5 et 4 à 10, caractérisée en ce que les moyens mâles du plateau inférieur (2) sont une nervure située au centre de la prothèse, et en ce que les moyens
femelles du noyau (3) sont une rainure.
16. Prothèse de disque intervertébral selon une des revendications 1 à
, caractérisée en ce que le noyau (3) est en polyéthylène.
17. Prothèse de disque intervertébral selon une des revendications 1 à
16, caractérisée en ce que le plateau inférieur (2) comprend une ou plusieurs ouvertures (28, 29) à proximité de son côté antérieur, prévues pour accueillir
des moyens d'ancrage (4, 5) de la prothèse dans une vertèbre.
18. Prothèse de disque intervertébral selon la revendication 17, 15 caractérisée en ce que l'ouverture (28) du plateau inférieur (2) est rectangulaire, et en ce que le moyen d'ancrage (4) est constitué d'un corps
(40), faisant avec le plateau inférieur (2) un angle aigu, et d'une tête (41).
19. Prothèse de disque intervertébral selon la revendication 17,
caractérisée en ce que les ouvertures (29) du plateau inférieur (2) sont 20 circulaires, et en ce que les moyens d'ancrage (5) ont la forme d'un clou.
20. Prothèse de disque intervertébral selon une des revendications 1 à
19, caractérisée en ce que le plateau supérieur (1) est bombé sur au moins
une partie de sa surface supérieure pour s'adapter à la forme des vertèbres.
