Abstract
Description
The object of the present invention is a prosthesis for the femoro-patellar joint.
We already know different prostheses of the femoro-patellar joint, they generally comprise on the one hand a trochlear element called shield or casing, implanted in place of the natural trochlea after resection, and on the other hand a patellar element called button , in the form of a spherical cap and secured to the rear face of the natural patella.
The patellar button is intended to slide, during the various flexion movements extending the leg, over the trochlear shield, guided in a convex groove in the sagittal plane and concave in the transverse plane.
Document FR-A-2,521,421 discloses a shield whose curvature of the groove in the sagittal plane is in the form of a sector of a tubular torus segment, therefore of constant radius, and which has drawbacks
Indeed, during the passage from extension to flexion, the patellar button, which in extension is positioned above the edge of the trochlear shield, abuts on it with the effect, in addition to a clearly audiole click, an effect of cam favoring the formation of a game likely to lead in the long run to dislocations or sub-dislocations.
Document FR-A2.676.641 also discloses a femoro-patellar prosthesis, the upper part of the throat of the trochlear shield in its part is, in the sagittal plane, of variable radius, increasing from the middle part towards the extreme parts, but which on the other hand does not allow perfect lateral support of the patellar button in flexion valgus rotation efforts with risk of dislocation.
This is mainly due to the shallow depth in the transverse direction of the throat, a greater depth would require, due to the overall architecture of the shield, a larger bone resection, likely to harm the possible subsequent installation of a total prosthesis of the knee joint.
The object of the present invention is to remedy these drawbacks by proposing a femoropatellar protesis making it possible to avoid a cam effect and to ensure self-centering of the patellar button.
The prosthesis according to the invention is essentially characterized in that on the one hand it comprises a trochlear shield whose curvature of the throat in the sagittal plane is of variable radius whose values are increasing from the lower part to the upper part of said throat, while remaining lower than those of the bony trochlea which moreover grow faster in said upper part, so that the edge of the latter is embedded under the cortex; and in that on the other hand the bottom of said groove has a curvature in the transverse direction of constant radius at least equal to that of the patellar button, said bottom being extended laterally tangentially by two plane banks forming between them a V d 'angle between 110 and 130; and in that, on the other hand, the external bank is wider and of less steep slope than the internal bank, and extends upwards in its upper part.
The engagement of the patellar button on the trochlear shield, during the initiation of a flexion, is carried out without projection and first on the external bank, thus avoiding the risk of subluxation.
The patellar button remains permanently centered in the throat of the trochlear shield, the pressure forces are exerted, whatever the angle of flexion, perpendicular to the shield, thus avoiding any pulling or rocking stress.
On the other hand, when the patellar button moves outward, during an external rotation of the knee, there is, due to the different slopes of the internal and external banks, an increase in the rotary control torque of the knee, favoring the external rotation of the femur on the tibia, thus avoiding the dislocation of the patella.
According to an additional characteristic of the prosthesis according to the invention, the trochlear shield has in its lower part two lateral notches, the external notch being more important than the internal notch to promote the sliding of the external femoral condyle
According to another additional characteristic of the prosthesis according to the invention, the lower edge of the trochlear shield is sharp to allow its engagement in the cortex.
The advantages and characteristics of the present invention will emerge more clearly from the description which follows and which refers to the appended drawing, which represents a non-limiting embodiment thereof.

- Figure 4 shows a side view in partial section of the same implanted prosthesis, when the leg is extended.
If we refer to FIG. 1, we can see that a femoro-patellar prosthesis comprises in a known manner a trochlear shield 1 and a patellar button 2.
The trochlear casing 1 is made of metal, more particularly of chromium-cobalt-molybdenum alloy, while the patellar button 2 is made of synthetic material, for example high density polyethylene.
The patellar button 2 has a spherical cap 20 and, projecting from its rear face 21, two anchoring studs 22 of small size which facilitates possible recovery.
The rear face 21 also includes grooves 23 intended to improve the anchoring of the patellar button 2 to the ball joint by means of cement.
The housing 1 comprises in the upper part of its rear face 10 three anchoring studs 11 intended to be implanted in the bone material in place of the natural trochlea between the two condyles. This implantation can be carried out without cement, which facilitates a possible recovery by means of a total prosthesis in the event of degradation of the joint.
If we also refer to Figure 2 we can see that the lower part 13 of the casing 1 is notched laterally, and more particularly on the external side where a notch 14 promotes the sliding of the external femoral condyle and allows the association of the prosthesis according to the invention with a uni-condylar prosthesis.
The casing 1 comprises in known manner a groove 12 convex in the sagittal plane and concave in the transverse direction, however, according to FIG. 3 it can be seen that in the transverse direction the groove 12 comprises several parts, a central part 15 corresponding to the bottom of the groove 20, of constant radius at least equal to that of the spherical cap 20 of the patellar button 2, and two flat banks 16 and 17, extending tangentially to the central part 15, and forming a V with an angle between 110 and 130.
The external bank 17 is wider than the internal bank 16, and, as can be seen in FIGS. 1 and 2, in the upper part 18 of the shield 1, its end 19 extends higher.
With reference to FIG. 4, which shows a shield 1 implanted in the femur 3 of an extended leg and a patellar button implanted in a ball joint 4, it can be seen that the groove 12 of the shield 1 is, in the sagittal direction, of curvature of variable radius, the value of which increases from the lower part 13 to the upper part 18, while remaining less than that of the radius of curvature of the natural trochlea 30 shown in broken lines, the difference in radius may even be increasing in the upper part 18
The difference in curvature between the shield 1 and the natural trochlea 3 allows the shield 1 to be embedded and to bury the upper edge 180 of the latter under the cortex.
These elements associated with the ascent 19 of the outer bank 17 promote the engagement and guiding of the patellar button 2 during the transition from extension to flexion, by eliminating the cam effect and the risks of subluxation.
On the other hand, in complete bending the button 2 escapes from the shield 1, also, in order to avoid a projection when returning to the extension, the lower edge 130 of the shield 1 is bevelled to allow its insertion into the bone material.
