Abstract
Un dispositivo urológico (800, 830, 850, 860, 900) que comprende una válvula urológica (812, 852, 861, 901), un soporte (801, 863, 902) para la válvula, siendo el soporte generalmente tubular y estando adaptado para montarlo en un aparato urinario, un elemento de retención (802, 811, 862, 903) de la vejiga para situar el dispositivo en una vejiga, y un elemento de retención (805, 831, 864, 904) uretral para prevenir la migración del dispositivo, comprendiendo la válvula una pluralidad de valvas (3, 4, 5) de la válvula, teniendo la válvula una región de coadaptación entre las valvas de la válvula, teniendo la válvula una configuración normalmente cerrada en la que las valvas de la válvula están acopladas en la región de coadaptación, y una configuración abierta en la que las valvas están separadas en la región de coadaptación para el flujo de fluido a través de la válvula, siendo la válvula automáticamente movible desde la configuración cerrada a la configuración abierta en respuesta a una presión urológica aplicada, caracterizado por que las valvas de la válvula invierten el movimiento entre la configuración cerrada y la abierta en respuesta a la presión urológica aplicada.

Las valvas 3, 4, 5 de la válvula están reforzadas en la región de coadaptación. En este caso, esto se logra mediante un engrosamiento local del material polimérico en esta región. De forma similar, el borde 2 de soporte se refuerza mediante un engrosamiento local del material polimérico.
La región de coadaptación de las valvas 3, 4, 5 de la válvula tiene una extensión axial que es típicamente de 1 a 5 mm. Esto asegura la coadaptación positiva de las valvas a lo largo de un área interfacial significativa cuando la válvula está en la configuración normalmente cerrada. El grosor de las valvas en la región de coadaptación está típicamente entre 0,1 mm y 10 mm.
La válvula 1 es una válvula de dos vías. Se requieren fuerzas diferentes para abrir la válvula desde las direcciones opuestas. La válvula 1 requiere diferentes fuerzas para abrirla en las diferentes direcciones. Variando las propiedades (tal como la densidad) del material de la válvula, la válvula se puede adaptar personalmente para aceptar presiones de colapso variables. La válvula logra esto invirtiéndose de forma controlable cuando se coloca a presión.
La válvula 1 de la invención vuelve a su posición de trabajo original después de estar abierta completamente. Esto se logra sin dañar la válvula de trabajo.
Cuando la válvula se abre mediante una presión urológica aplicada y fluye el fluido, se abren las valvas. La superficie exterior de la válvula tiene una mayor resistencia al cambio en la forma, y de este modo la fuerza requerida para abrir el cuerpo principal en esta dirección es mayor.
Las características importantes que influyen en el funcionamiento de la válvula son las piernas de las valvas que golpean entre sí. Variando la geometría y la longitud de las valvas 3, 4, 5, se puede hacer que la válvula 1 se abra en una dirección a diferentes presiones. La apertura en la dirección opuesta depende algo menos de la geometría de las valvas, y depende más de la elasticidad y densidad del material del que está hecho el dispositivo. Adicionalmente, el diámetro global y el diámetro al que se abren las valvas influyen en la fuerza de apertura en ambas direcciones.
La válvula puede ser de cualquier material polimérico biocompatible adecuado. Puede ser de un material polimérico biocompatible que tiene propiedades que permiten que la válvula funcione como se describió.
Los materiales usados para la producción de esta válvula tienen un % de alargamiento entre 50% y 3000%. El material también tiene una resistencia a la tracción de entre 0,01 y 5 MPa. Adicionalmente, el material podría tener una acción antimicrobiana para evitar la colonización cuando se encuentra in vivo. Adicionalmente, el material puede ser elástico o viscoelástico, y puede ser opcionalmente una espuma de celda abierta. La densidad del material debería de estar entre 0,1 g/cm3 a 1,5 g/cm3.
La válvula puede tener cualquier número deseado de valvas; por ejemplo, la válvula 30 ilustrada en las Figs. 21 a 26 tiene 6 valvas 251 de válvula. Estas valvas 251 están orientadas perpendicularmente a la dirección del flujo para permitir adicionalmente una mayor distensibilidad de la abertura de la válvula.
La válvula 30 es similar a la válvula descrita anteriormente, y comprende un cuerpo de válvula polimérico que tiene una región de soporte exterior proximal con un borde 32, seis valvas 33 de válvula, y una región 36 de cuerpo principal que se extiende entre el borde 32 de soporte y las valvas 33 de válvula. Las valvas 33 de válvula se extienden terminalmente en las caras 33 de los extremos distales. Las valvas tienen cada una piernas que se extienden en un ángulo incluido de 60º entre sí. Los pares de piernas adyacentes se coadaptan para cerrar el espacio entre las valvas 33 de la válvula cuando la válvula está en la posición normalmente cerrada.
La válvula 30 tiene tres configuraciones. La primera configuración es una configuración normalmente cerrada en la que las valvas 33 de válvula se coadaptan para cerrar la válvula. La segunda configuración es una configuración abierta para permitir el flujo de fluido, en la que las valvas 33 de válvula se abren de manera que los pares de piernas de valvas se abren y se separan en respuesta a una fuerza F1 para permitir el flujo a través de la válvula 30. La tercera configuración es una segunda configuración abierta en respuesta a una fuerza F2, que es sustancialmente mayor que la fuerza F2 anterógrada.
Las diversas configuraciones de la válvula 30 se ilustran en las Figs. 21 a 26. En la configuración primera o normalmente cerrada (Fig. 21), las valvas 33 de la válvula se coadaptan.
Cuando se aplica una fuerza de presión urológica F1 al cuerpo de la válvula. Esta fuerza empuja inicialmente las valvas 33 de válvula unas contra otras (Fig. 22), y, si la presión es mayor que un valor fijado, el cuerpo de válvula se invertirá como se ilustra en la Fig. 23. Cuando la válvula se abre completamente en respuesta a la fuerza F1, el cuerpo procedimiento de la válvula (y las valvas 33) se extienden hacia abajo como se ilustra en la Fig. 23. Esto permite que pase el flujo a través de la válvula. Cuando el flujo se detiene, el cuerpo principal de la válvula volverá a la configuración inicial al invertirse en respuesta a la tendencia del material polimérico a volver a la configuración normalmente cerrada con las valvas de la válvula que se extienden como se ilustra en la Fig. 21.

La degradación hidrolítica catalizada por ácidos se produce en los enlaces de uretano en los materiales poliméricos. Estos enlaces de uretano/urea son por lo tanto los “enlaces débiles” del material poliuretánico. Se deduce que la hidrofilia intrínseca del material poliuretánico afectará a la velocidad de hidrólisis a través de la modulación de la captación de agua. De este modo, tales materiales son incompatibles con el uso en un entorno gástrico (es decir, un entorno acuoso muy ácido).
De este modo, en algunas realizaciones, la presente invención proporciona un copolímero de múltiples bloques que es biomimético e hidrolíticamente estable en un entorno gástrico. Tales copolímeros de múltiples bloques son de la fórmula I:

cada R es independientemente hidrógeno, un grupo alifático de C1-20 opcionalmente sustituido, o un grupo opcionalmente sustituido seleccionado de fenilo, arilo bicíclico de 8-10 miembros, un anillo heterocíclico saturado o parcialmente insaturado, monocíclico, de 4-8 miembros, que tiene 1-2 heteroátomos seleccionados independientemente de nitrógeno, oxígeno, o azufre, o un grupo heteroarilo monocíclico de 5-6 miembros o bicíclico de 8-10 miembros que tiene 1-4 heteroátomos seleccionados independientemente de nitrógeno, oxígeno, o azufre;
cada uno de m, n y p es independientemente 2 a 100; y
cada uno de L1 y L2 es independientemente una cadena de hidrocarburo de C1-20 bivalente en la que 1-4 unidades metilénicas de la cadena de hidrocarburo se sustituyen opcional e independientemente por -O-, -S-, -N(R)-,-C(O)-, -C(O)N(R)-, -N(R)C(O)-, -SO2-, -SO2N(R)-, -N(R)SO2-, -OC(O)-, -C(O)O-, o un cicloalquileno, arileno, heterocicleno, o heteroarileno bivalentes, con la condición de que ni L1 ni L2 comprendan un resto de urea o de uretano.
2. Definiciones:
El término “alifático” o “grupo alifático”, como se usa aquí, representa un resto de hidrocarburo que puede ser de cadena lineal (es decir, sin ramificar), ramificado, o cíclico (incluyendo condensado, en puente, y policíclico espirocondensado), y puede estar completamente saturado o puede contener una o más unidades de insaturación, pero que no es aromático. Excepto que se especifique de otro modo, los grupos alifáticos contienen 1-20 átomos de carbono. En algunas realizaciones, los grupos alifáticos contienen 1-10 átomos de carbono. En otras realizaciones, los grupos alifáticos contienen 1-8 átomos de carbono. Los grupos alifáticos adecuados incluyen, pero no se limitan a, grupos alquilo, alquenilo y alquinilo, lineales o ramificados, y sus híbridos, tales como (cicloalquil)alquilo, (cicloalquenil)alquilo o (cicloalquil)alquenilo.
La expresión “alquilo inferior” se refiere a un grupo alquilo lineal o ramificado de C1-4. Los grupos alquilo inferior ejemplares son metilo, etilo, propilo, isopropilo, butilo, isobutilo, y terc-butilo.
La expresión “haloalquilo inferior” se refiere a un grupo alquilo lineal o ramificado de C1-4 que está sustituido con uno
o más átomos de halógeno.
El término “heteroátomo” significa uno o más de oxígeno, azufre, nitrógeno, fósforo, o silicio (incluyendo cualquier forma oxidada de nitrógeno, azufre, fósforo, o silicio, la forma cuaternizada de cualquier nitrógeno básico o un nitrógeno sustituible de un anillo heterocíclico, por ejemplo N (como en 3,4-dihidro-2H-pirrolilo), NH (como en pirrolidinilo) o NR+ (como en pirrolidinilo N-sustituido)).

En algunas realizaciones, cada uno de Rx y Ry es independientemente un hidroxilo protegido o una amina protegida. A un matraz separable de 100 ml equipado con un agitador mecánico, se añadieron 15 g de polidimetilsiloxano terminado en hidroxi (DMS-S14 de Gelest Inc.) junto con 5,36 g de dicloro p-xileno (de Sigma) y 0,0089 g de acetilacetonato de cobre(II) (Cu(Acac)2 de Sigma). La mezcla de reacción se puso a reflujo a 110ºC durante 5 h. En
5 este punto, se añadieron gota a gota 19,77 g de poli(propilenglicol) terminado en hidroxi (de Sigma), y la mezcla de reacción se puso entonces a reflujo durante otras 15 h. El progreso de la reacción fue seguido por RMN 1H, y el peso molecular final, determinado mediante cromatografía de permeación en gel (GPC), fue 3000 g/mol.
