Abstract
Prótesis inversa del hombro para la articulación de un húmero (12) en una escápula (54) de un hombro que tiene una cavidad glenoide (15), que comprende un elemento de articulación (19), por lo menos en parte convexo, que incluye unos medios para el soporte y la sujeción (14, 48) de la prótesis en dicha cavidad glenoide (15), y que se puede articular con un elemento de articulación (20) de unión, por lo menos parcialmente cóncavo, adecuado para poderse asociar con la parte superior de dicho húmero (12), elemento de articulación (19) que comprende un elemento (47) prácticamente hemisférico y un elemento de implante (48) de tipo pasador a base de elementos distintos y recíprocamente separables, con el fin de que se puedan montar secuencialmente con respecto a dicha cavidad glenoide (1S), caracterizada porque dicho elemento de implante de tipo pasador (48) comprende una primera parte cónica (48a) para su acoplamiento con un alojamiento cónico correspondiente (50) realizado en dicho elemento dearticulación hemisférico (47), y una segunda parte cónica (48b) que presenta un cono respecto de la citada primera parte cónica (48a) y que se puede sujetar en un orificio cónico (26) realizado en un elemento de soporte (14) que se puede fijar a dicha cavidad glenoide.
Description
Prótesis inversa para la articulación del
hombro.
La presente invención se refiere a una prótesis
de hombro para la articulación de la cabeza del húmero con
respecto a la escápula correspondiente. Para ser más exactos, la
prótesis de hombro según la invención está configurada como prótesis
inversa, donde se asocia una cabeza con forma convexa a la cavidad
glenoide y se articula en un alojamiento cóncavo artificial
asociado con la parte superior del húmero.
Se conocen prótesis inversas del hombro para la
articulación de una esfera unida a la escápula en una cavidad
correspondiente, obtenida en la parte superior del húmero, que
reproduce inversamente la anatomía gleno-humeral.
Normalmente, las prótesis inversas se utilizan en el caso de
degeneración muscular grave del hombro, particularmente de los
músculos del manguito de los rotadores. La degeneración de dichos
músculos origina una acción dominante por parte del deltoides, que
tiende a llevar el húmero hacia arriba, haciendo por consiguiente
que la cabeza del hueso humeral o la posible cabeza artificial se
mueva hacia arriba, con el riesgo de que pueda tropezar contra el
saliente que prolonga la espina de la escápula, conocido con el
nombre de acromion.
Partiendo de la idea de eliminar la cabeza
humeral, las prótesis inversas comprenden, como elementos asociados
con el húmero, un asiento humeral de forma cóncava que incluye un
eje de sujeción correspondiente implantado en el húmero.
El alojamiento humeral define la articulación
para un cuerpo esférico unido a la cavidad glenoide, provisto de un
pasador para la sujeción directa en la cavidad glenoide o a un
soporte previamente sujeto a dicha cavidad glenoide.
Una de las desventajas de las prótesis inversas
conocidas es que el pasador de sujeción del cuerpo esférico antes
citado resulta difícil de insertar en la cavidad glenoide o dentro
del elemento de soporte correspondiente, previamente aplicado,
debido al espacio extremadamente limitado de que se dispone; esto
constituye un obstáculo para el posicionado preciso y un límite
para las posibilidades de trabajo del cirujano.

de unos medios en forma de pasador, que guían la esfera glenoide
hasta un elemento metaglenoide sobre el cual se tiene que fijar la
esfera.
Otra de las desventajas de las prótesis inversas
conocidas es que la posición del cuerpo esférico no se puede
orientar o trasladar en la forma deseada respecto del tabique de la
cavidad glenoide donde se ha sujetado el pasador
correspondiente.
Uno de los objetos de la invención es obtener
una prótesis para la articulación del hombro donde la implantación
de una prótesis inversa permitirá montar, de forma fácil y precisa,
el cuerpo esférico en la cavidad glenoide, facilitando la labor del
cirujano.
Otro de los objetos de la invención es obtener
una prótesis de hombro en una configuración inversa, donde sea
posible elegir la orientación y la posición del cuerpo esférico con
respecto a su eje de implantación en la cavidad glenoide, debido a
la presencia de espacios no regulares entre la cavidad glenoide y la
cabeza del húmero.
El solicitante ha diseñado, comprobado y
realizado la presente invención para lograr estos objetivos y otras
ventajas y superar los inconvenientes del estado de la técnica.
La presente invención se expone y caracteriza en
la reivindicación principal, mientras que las subreivindicaciones
describen otras características innovadoras de la invención.
Una prótesis inversa de hombro comprende un
elemento de articulación por lo menos parcialmente convexo,
asociado con unos medios de implante correspondientes, normalmente
un pasador que define un eje de implantación, para sujetar a la
cavidad glenoide, y que permite articular con un elemento de
articulación correspondiente, por lo menos en parte cóncavo,
asociado con la parte superior del húmero. Según la invención, el
elemento de articulación asociado con la cavidad glenoide y los
medios de implantación de tipo pasador, correspondientes, están
constituidos por elementos diferentes y que se puede quitar
recíprocamente, con el fin de poderlos montar secuencialmente con
respecto a la cavidad glenoide.
Gracias a ello, según la primera característica,
el elemento de articulación y el medio de implantación de tipo
pasador pueden ser también de materiales diferentes; el elemento de
articulación puede ser por ejemplo de cerámica o de metal, como por
ejemplo una aleación a base de cobalto, mientras que el elemento de
implante de tipo pasador puede ser de un metal diferente, como por
ejemplo una aleación a base de titanio.
Utilizando un material cerámico o una aleación a
base de cobalto se mejora la resistencia al desgaste del elevento
convexo, que funciona como el elemento de articulación, mientras
que la utilización de una aleación a base de titanio garantiza la
buena integración del hueso así como la debida y necesaria
resistencia del anclaje.
Además, el tamaño de los elementos también se
pueden elegir en cada caso en función del espacio disponible en la
cavidad glenoide de la escápula del paciente.
Según otra característica, la utilización de un
pasador distinto/separado del elemento de articulación convexo
correspondiente permite al cirujano durante la etapa de
implantación, orientar el pasador en la forma deseada en el espacio
con respecto a su eje de inserción, sin ningún tipo de limitación
debida a la falta de espacio disponible.
En una etapa ulterior, después de haber anclado
el pasador en el asiento óseo del glenoide o a un elemento de
soporte correspondiente, el elemento de articulación convexo se
puede asociar, con la orientación deseada, con el elemento de
implante de tipo pasador correspondiente.
El acoplamiento del elemento de articulación y
el medio de implante de tipo pasador correspondiente puede ser de
cualquier tipo, por ejemplo, acoplamiento cónico, por medio de
tornillo, con elementos auxiliares como remaches o pasadores u
otros.
Según otra característica de la invención, el
elemento de articulación tiene una superficie convexa con por lo
menos un eje de simetría, y el medio de implante de tipo pasador se
puede colocar en una posición excéntrica o que forme ángulo con
respecto a dicho eje de simetría. La excentricidad o angulación del
medio de implante con respecto al elemento de articulación permite
trasladar u orientar el elemento de articulación mismo con respecto
a la cavidad glenoide, especialmente cuando el tejido óseo del
glenoide no está sano y es necesario mover el punto de
implantación.
Según una variante, si la prótesis según la
invención comprende un elemento de soporte previamente sujeto a la
cavidad glenoide, el acoplamiento del medio de implante de tipo
pasador y el elemento de soporte solidario con el asiento óseo es de
tipo cónico.
Según una variante, el medio de implante de tipo
pasador se puede atornillar por lo menos parcialmente dentro del
elemento de soporte glenoide.
Estas y otras características de la presente
invención se podrán apreciar claramente en la siguiente descripción
de una forma preferida de realización que se da como ejemplo no
restrictivo con referencia a las figuras adjuntas donde:
- La figura 1 es una sección longitudinal de una
prótesis de tipo inverso según la invención.
- La figura 2 muestra un detalle despiezado de
la prótesis de la figura 1.
- La figura 3 muestra una variante de la figura
2.
Con referencia a las figuras adjuntas, el número
10 corresponde a una prótesis inversa según la presente invención,
que se puede implantar en un hombro con el fin de permitir la
articulación de un húmero 12 en una cavidad glenoide 15 de una
escápula 54, que solo se muestra en parte.
La prótesis 10 comprende un elemento de soporte
14 o elemento glenoide, que es anclado primero al glenoide 15 y es
de metal o de plástico según una variante.
El elemento de soporte 14 (figura 2) comprende
un eje 22 y una placa 24, coaxial con el eje 22 y que tiene una
superficie exterior prácticamente cóncava 25 o según una variante,
prácticamente plana.
Se ancla a dicho elemento de soporte 14, según
se describe en lo que sigue, un elemento de articulación convexa o
esférica, solidario con el glenoides 15.
El elemento de soporte 14 presenta también un
orificio central 26, en este caso con una sección cónica, que es
pasante, y con variaciones en diámetro, pasa a través de la placa
24 y el eje 22; el elemento 14 también tiene una pluralidad de
orificios 28, dispuestos periféricamente con respecto al orificio
central 26, para la inserción de tornillos de fijación 30 para
sujetar el elemento de soporte 14 al tabique del asiento óseo.
El elemento de soporte 14 se implanta forzando
el eje 22 por interferencia, atornillando o cementándolo en un
asiento correspondiente realizado en el glenoides 15, y después se
atornillan en el mismo los tornillos de sujeción 30.
Según una variante, que no se muestra en las
figuras, el eje 22 es dividido longitudinalmente en dos partes
idénticas por razones de elasticidad y fijación estable.
Se aplica un cuerpo esférico o cabeza glenoide
19 sobre el elemento de soporte 14, que permite la articulación de
un elemento de articulación cóncavo solidario con el húmero 12.
Para ser más exactos, el elemento de
articulación solidario con el húmero 12 comprende un eje de anclaje
38, que se puede insertar dentro del húmero 12 por percusión o
atornillando y que se puede cementar o no al hueso.
En este caso, se asocia una copa humeral (figura
1) al eje 38.
Para ser más exactos, se dispone un cuerpo
humeral de soporte correspondiente 33 para sujetar la copa humeral
24.
La cabeza glenoide 19 (figuras 1 y 2) comprende
en este caso una cabeza hemisférica 47, con un eje de simetría Y, y
un pasador de acoplamiento 48 que se puede insertar y acoplar, en
este caso, mediante un acoplamiento cónico como se describe con más
detalle en lo que sigue, en el orificio central 26 del elemento de
soporte glenoide 14.
Según una variante, que no se muestra en las
figuras, el pasador 48 está completamente roscado y se puede
atornillar en un orificio central 26, cuyo interior ha sido
correspondientemente roscado.
Según una característica de la presente
invención, el pasador 48 es un componente separado con respecto a
la cabeza hemisférica 47. De este modo el pasador 48 se puede montar
por separado en una primera etapa de trabajo sobre el elemento de
soporte 14 y ulteriormente, en una segunda etapa, se puede sujetar
fácilmente sobre el pasador 48 la cabeza hemisférica 47.
El hecho de que el pasador 48 y la cabeza
hemisférica 47 están constituidas por varias piezas, facilita, en
primer lugar, la inserción y la colocación de la cabeza glenoide 19
sobre el elemento de soporte 14, ya que permite, aunque el espacio
disponible sea muy limitado, que el cirujano oriente el pasador 48
en la forma deseada en el espacio con respecto a su eje de
implantación.
En segundo lugar, el pasador 48 y la cabeza
hemisférica 47 pueden ser de materiales diferentes.
El pasador 48 puede ser de una aleación de
titanio para garantizar la debida resistencia y una suficiente
integración del hueso mientras que la cabeza hemisférica 47 es de
una aleación a base de
cobalto-cromo-molibdeno o de
material cerámico, con el fin de mejorar la resistencia al desgaste
de la prótesis 10. Para ser más exactos, el material cerámico se
utiliza para aquellos casos en los que los pacientes son alérgicos a
ciertos elementos contenidos en las aleaciones a base de
cobalto.
La cabeza hemisférica 47, en este caso, es hueca
en su interior, por razones de peso así como para no interferir en
las partes de la cabeza de los tornillos de sujeción 30.
El pasador 48 (figura 2) comprende una primer
parte cónica 48a, en la que se ha realizado una cavidad interior
roscada 49, y una segunda parte cónica 48b que se puede insertar en
el orificio central 26 del elemento de soporte 14 y que tiene un
cono opuesto con respecto a la primera parte 48a.
El pasador 48 puede tener, en una realización,
una longitud superior a la del eje 22 y comprende también, en la
realización que aquí se muestra, una parte terminal roscada 48c que
se puede atornillar dentro de la parte terminal del eje 22 y dentro
de la pared ósea de la escápula 54 con el fin de sujetar el pasador
(48) de forma definitiva, cuando se monta y se sujeta dentro del
orificio 26 del elemento de soporte 14 (figura 2).
Sobre la cara interna de la cabeza hemisférica
47 se realiza un cono de acoplamiento 50 solidario con la misma y
que se extiende hacia el exterior, en cuyo interior se ha realizado
una cavidad pasante 52 abierta al exterior.
El cono 50 sirve para implantar la cabeza 47
sobre la parte cónica correspondiente 48a del pasador 48, después
de que este último se ha sujetado sobre el elemento de soporte
14.
Alternativamente, el pasador 48 y la cabeza 47
se pueden montar previamente y sujetarse al elemento de soporte 14
atornillando la parte 48c.
En una última etapa, la cabeza hemisférica 47 se
sujeta al pasador 48 por medio de un tornillo de fijación de
seguridad 52 que se inserta a través de la cavidad abierta 52 y se
atornilla dentro de la cavidad roscada 48 de la primera parte
cónica 48a.
En la realización mostrada en la figura 2, el
eje principal de simetría Y de la cabeza hemisférica 47 coincide
con un eje X del cono 50 al acoplar con el pasador 48.
Según la variante mostrada en la figura 3, una
cabeza glenoide 119 comprende una cabeza hemisférica 147, que tiene
una superficie convexa simétrica con respecto a un eje Y, y que
está acoplada por medio de un cono correspondiente 150 con un
pasador 48 que tiene un eje X de implantación, excéntrico
con
respecto al eje de simetría Y.

La excentricidad del cono de acoplamiento 150
permite trasladar la cabeza hemisférica 147 en sentido
frontal-posterior o lateral-medial
con respecto al glenoides 15, o modificar su orientación con
respecto al eje 22. Esto permite corregir posibles espacios no
simétricos, especialmente en el caso de que la pared ósea del
glenoides 15 no esté sana y el eje 22 se tenga que implantar en una
posición ligeramente trasladada con respecto a la zona en la que la
cabeza glenoide 19 se articula con la copa humeral 20.
No obstante, es evidente que se pueden realizar
modificaciones y/o adiciones a la prótesis universal 10 descrita
hasta ahora, sin apartarse del ámbito de la presente invención.
Por ejemplo, el eje X del cono de acoplamiento
150 puede formar ángulo con respecto al eje de
simetría Y.

Claims (10)
1. Prótesis inversa del hombro para la
articulación de un húmero (12) en una escápula (54) de un hombro
que tiene una cavidad glenoide (15), que comprende un elemento de
articulación (19), por lo menos en parte convexo, que incluye unos
medios para el soporte y la sujeción (14, 48) de la prótesis en
dicha cavidad glenoide (15), y que se puede articular con un
elemento de articulación (20) de unión, por lo menos parcialmente
cóncavo, adecuado para poderse asociar con la parte superior de
dicho húmero (12), elemento de articulación (19) que comprende un
elemento (47) prácticamente hemisférico y un elemento de implante
(48) de tipo pasador a base de elementos distintos y recíprocamente
separables, con el fin de que se puedan montar secuencialmente con
respecto a dicha cavidad glenoide (1S), caracterizada porque
dicho elemento de implante de tipo pasador (48) comprende una
primera parte cónica (48a) para su acoplamiento con un alojamiento
cónico correspondiente (50) realizado en dicho elemento de
articulación hemisférico (47), y una segunda parte cónica (48b) que
presenta un cono respecto de la citada primera parte cónica (48a) y
que se puede sujetar en un orificio cónico (26) realizado en un
elemento de soporte (14) que se puede fijar a dicha cavidad
glenoide.
2. Prótesis de hombro según la reivindicación 1,
caracterizada porque dicho alojamiento cónico (50) se
extiende hasta un lado exterior convexo de dicho elemento
prácticamente hemisférico (47), definiendo una cavidad pasante
(52).
3. Prótesis de hombro según cualquiera de las
reivindicaciones anteriores, caracterizada porque en el
citado elemento de implante de tipo pasador (48) se ha realizado una
cavidad roscada (49) para atornillar un tornillo de seguridad (43)
con el fin de sujetar el elemento de implante de tipo pasador (48)
a dicho elemento prácticamente hemisférico (47).
4. Prótesis de hombro según cualquiera de las
reivindicaciones anteriores, caracterizada porque dicho
elemento de implante de tipo pasador (48) tiene una parte terminal
roscada (48c), que se puede atornillar dentro de la citada cavidad
glenoide
(15).

5. Prótesis de hombro según cualquiera de las
reivindicaciones anteriores, caracterizada porque dicho
elemento prácticamente hemisférico (47) y el citado elemento de
implante de tipo pasador (48) son de materiales diferentes.
6. Prótesis de hombro según la reivindicación
5, caracterizada porque el elemento prácticamente hemisférico
(47) es de material cerámico o de metal y el elemento de implante
de tipo pasador (48) es de metal.
7. Prótesis de hombro según cualquiera de las
reivindicaciones anteriores, donde la superficie convexa del
elemento prácticamente hemisférico (47) es simétrica por lo menos
con respecto a un eje (Y), caracterizada porque el elemento
de implante de tipo pasador (48) mencionado se puede colocar en
posición excéntrica con respecto al mencionado eje (Y) de simetría
del mencionado elemento prácticamente hemisférico (48).
8. Prótesis de hombro según cualquiera de las
reivindicaciones anteriores, donde la superficie convexa del
elemento prácticamente hemisférico (47) es simétrica por lo menos
con respecto a un eje (Y), caracterizada porque dicho
elemento de implante de tipo pasador (48) se puede colocar a lo
largo de un eje (X) inclinado con respecto al eje (Y) de simetría
de dicho elemento de articulación (47).
9. Prótesis de hombro según la reivindicación 1,
caracterizada porque el alojamiento cónico mencionado (50)
está descentrado y/o forma ángulo con respecto a un eje (Y) de
simetría del elemento prácticamente hemisférico mencionado
(47).
10. Prótesis de hombro según la reivindicación
1, caracterizada porque por lo menos una parte terminal (48c)
de dicho elemento de implante de tipo pasador (48) se puede sujetar
atornillando a una parte correspondiente del citado orificio cónico
(26).
