- HINTERGRUND DER ERFINDUNG
- 1. Bereich der Erfindung
- Die Erfindung betrifft Stents mit verbesserter Konfiguration, die spiralenförmige Gelenkverbindungen aufweisen, die sich beim Ausdehnen abwickeln, um Verstrebungsstrukturen oder Gerüststrukturen zu bilden.
- 2. Kurze Beschreibung des Standes der Technik
- Stents sind radial ausdehnbare Endoprothesen, die typischerweise intravaskuläre Implantate sind, die transluminal implantiert und radial vergrößert werden können, nachdem sie perkutan eingeführt sind. Sie werden auch in Harnwege und Gallengänge implantiert. Sie werden verwendet, um Körpergefäße zu verstärken und Restenose nach der Angioplastie im Gefäßsystem zu verhindern. Sie können selbstausdehnend sein oder durch eine innere Radialkraft ausgedehnt werden, beispielsweise wenn sie an einem Ballon angebracht sind.
- Ein Beispiel für einen Stent ist in
EP 0 421 729 gezeigt, die einen Stent mit Segmenten offenbart, die durch spiralenförmige Gelenke miteinander verbunden sind. -
WO 98/33546 - In der Vergangenheit haben Stents viele Konfigurationen angenommen und wurden aus vielen Materialien gefertigt, auch aus Metallen und Kunststoffen. Es wurden gewöhnliche Metalle wie rostfreier Stahl genauso verwendet wie Metalle mit Formgedächtnis, beispielsweise Nitinol und dergleichen. Stents wurden auch aus biologisch abbaubaren Kunststoffen hergestellt. Sie wurden aus Draht, Schlauchmaterial usw. geformt.
- ZUSAMMENFASSUNG DER ERFINDUNG
- Mit der Erfindung wird eine neue Konfiguration für Stents bereitgestellt, an die sich sämtliche verschiedenen Arten von oben bezeichneten Stents nach dem Stand der Technik anpassen lassen. Bei der neuen Konfiguration bestehen zahlreiche Vorteile. Sie beschränkt unter anderem das Zurückwickeln und erhöht die Druckbeständigkeit bei dem ausgedehnten Stent. Sie ist sowohl in nicht ausgedehntem als auch in ausgedehntem Zustand in Längsrichtung flexibel. Sie weist mehrere Ausführungsformen auf.
- Ein wichtiger Teil der neuen Konfiguration umfaßt eine spiralenförmige Konstruktion, die verbundene Elemente umfaßt, die gewickelt oder gebogen sind, und die bei Ausdehnung des Stents in einen mehr oder weniger geradegerichteten Zustand abgewunden, abgewickelt oder geradegebogen werden. Diese Konstruktionen werden im folgenden gemeinsam als Spiralen bezeichnet. Diese Konstruktionen schaffen während des Ausdehnens Bereiche mit geringer Formänderung in dem Stent. Diese Elemente können miteinander oder mit jedem radial ausdehnungsfähigen Gliedern jeder Art verbunden werden, wobei ringförmige serpentinenartige Glieder bevorzugt werden.

Sie umfaßt eine metallschlauchartige Konstruktion10 , die am besten in den3 und4 gezeigt ist, beispielsweise aus Nitinol oder rostfreiem Stahl, die zu der Konfiguration, die in der Draufsicht gemäß den1 und2 gezeigt ist, und in einer kurzen Version, die in4 gezeigt ist, geätzt oder mit Laser geschnitten wurde. Die Konfiguration besteht aus einer Reihe von serpentinenartigen ringförmigen, ausdehnbaren Elementen oder Segmenten12 , die Schleifen14 bilden, um eine radiale ringförmige Ausdehnung zu ermöglichen. Die Segmente12 können andere Konfigurationen aufweisen, jedoch wird die serpentinenartige bevorzugt. Die Elemente12 sind miteinander durch Paare von langgestreckten Gliedern13a und13b verbunden, die an einem Ende an aufeinanderfolgenden Schleifen14 eines Segments12 befestigt sind und an ihren anderen Enden mit benachbarten Paaren von langgestreckten Gliedern13a und13b verbunden sind, wie am besten ausführlich in2 zu sehen ist. Die Glieder13a und13b weisen vorzugsweise einen engeren Abstand als die Glieder12 auf und sind in der allgemein bei16 gezeigten Weise miteinander in einer spiralförmigen Anordnung verbunden. Die Spirale16 bildet eine Konstruktion, um die herum sich die Glieder13 bei einer radialen Ausdehnung von Gliedern12 abhängig von der Spiralenwicklungsrichtung im Uhrzeigersinn oder gegen den Uhrzeigersinn zu einem im wesentlichen geraden Zustand abwickeln oder abwinden können. Bei dieser Ausführungsform sind die Spiralen16 zu wechselseitig gewickelten Konstruktionen geformt, so daß sich manche in der einen Richtung und manche in der anderen Richtung abwickeln. Natürlich können die Spiralen in jeder Ausführungsform so geformt sein, daß sie sich alle in einer Richtung abwickeln, entweder im Uhrzeigersinn oder gegen den Uhrzeigersinn, und daß sie mehr oder weniger Glieder13 aufweisen können. Auch können mehr oder weniger Spiralen zwischen den Segmenten enthalten sein. Beim Abwickeln kommt es auch zu einem Geraderichtvorgang in bezug auf die Glieder13 , wie ausführlicher in Zusammenhang mit den4 –8 beschrieben ist. Aus den4 bis8 ist ersichtlich, daß die entstandene Konfiguration in einem ausgedehnten Stent mit dieser Konfiguration aus einer Mehrzahl von Zellen umfaßt ist, deren Umfang von zwei Gliedern oder Verstrebungen, die durch den Schleifenabschnitt14 des Segments12 gebildet sind, und von zwei Gliedern oder Verstrebungen13 gebildet wird. Die Zellen sind bei16 verbunden, wie am besten in8 zu sehen ist. Insbesondere bestehen die Zellen aus zwei Arten, wie in8 gezeigt ist. Ein erstes Zellenpaar ist A und B, bestehend aus einem Segment12 und zwei Verstrebungen13a für Zelle A oder13b für Zelle B. Ein zweites Zellenpaar ist C und D, bestehend aus einem nach innen gerichteten Schleifenabschnitt14 des Segments12 und einer Verstrebung13a und einer Verstrebung13b für die Zellen C und D. - Wenn ein Stent gemäß der Erfindung, beispielsweise der in den
1 –4 gezeigte, eine Ausdehnung erfährt, beispielsweise von der Ausführungsform gemäß4 , tritt er in der in5 gezeigten Weise in voll ausgedehntem Zustand auf.5 zeigt den Stent perspektivisch. - Der Abwindevorgang, den die Spiralelemente
16 beim Ausdehnen erfahren, ist am besten in den6 –8 zu sehen, die zwecks Einfachheit und Klarheit nur die stirnseitige Oberfläche des Stents zeigen. - Wenn die radiale Ausdehnung beginnt (zu sehen in
6 ), ist zu erkennen, daß die Spiralelemente16 infolge einer Zugkraft an sämtlichen Elementen13 eine Abwindung und Geraderichtung erfahren. Insbesondere erfahren die Elemente13 eine Geraderichtung, wenn es zur Ausdehnung kommt, wie in den frühen Stadien der Ausdehnung in6 zu sehen ist. - Bei weiterer (in
7 zu sehender) Ausdehnung erfahren die Spiralen16 eine weitere Abwicklung, d. h. die Elemente13 erfahren eine weitere Geraderichtung. - Schließlich werden in
8 durch wesentliche volle Ausdehnung im wesentlichen geradegerichtete Elemente13 geschaffen, die in diesem Zustand das Zurückwickeln des Stents beschränken und die Druckbeständigkeit des Stents erhöhen. -
9 zeigt eine modifizierte Ausführungsform, bei der Elemente13a und13b mit dem Segment12 am Ende von dessen Schleifen14 in Kontakt stehen. Zu dieser Ausführungsform sei auch angemerkt, daß die Spiralen16 alle in der gleichen Richtung gewunden sind. -
10 zeigt ein Beispiel, das nicht unter den Wortlaut der Ansprüche fällt, bei dem die Glieder13 mehr gebogen und weniger krummlinig sind, jedoch immer noch eine spiralartige Konfiguration16 bilden. Der Rest der Konfiguration ähnelt der gemäß9 . In10 sind langgestreckte Elemente13 vor der Ausdehnung des Stents gezeigt. Wenn der Stent ausgedehnt wird, werden die Elemente13 gegen den Uhrzeigersinn abgewunden und werden etwas geradegerichtet. Bei voller Ausdehnung werden die Elemente13 noch weiter geradegerichtet und im wesentlichen derart geradegerichtet, daß sie für Druckbeständigkeit des Stents und geringes Zurückwickeln sorgen. Die ausgedehnte Konfiguration zeigt eine Zellenkonfiguration an, die der in8 zu sehenden ähnelt. - Weitere Ausführungsformen sind in nachfolgenden Figuren mit anderen Spiralenanordnungen gezeigt. Beispielsweise zeigt die Ausführungsform gemäß den
11 –13 Spiralenanordungen16 , die in der gleichen Richtung gewunden sind, und Elemente13 , die am Ende der Schleifen14 befestigt sind, während manche benachbarte Spiralelemente zwischen Segmenten durch Glieder15 miteinander verbunden sind. -
14 zeigt, daß manche Elemente13 in einer Spirale16 mit dem Ende der Schleifen14 in Kontakt stehen und manche mit dem eigentlichen Segment12 in Kontakt stehen. Auch sind manche benachbarte Spiralelemente durch Glieder17 miteinander verbunden. -
15 , die nicht unter den Wortlaut der Ansprüche fällt, zeigt eine abgeflachte oder langgestreckte Anordnung16 , und Elemente12 sind winkelig in Bezug auf die Langsachse des Stents angeordnet. Bei vorhergehenden Ausführungsformen waren diese Elemente oder Segmente parallel zur Achse oder horizontal angeordnet. Langgestreckte Spiralen wie in13 und Spiralen gemäß vorhergehenden Figuren können miteinander gemischt werden. (Nicht gezeigt). - Bei den bereits erläuterten Ausführungsformen sind ringförmige ausdehnbare Segmente, beispielsweise die Segmente
12 , mit Spiralenanordnungen16 durchsetzt. Jedoch kann, wie in16 zu sehen ist, zumindest ein wesentlicher Abschnitt oder der gesamte Körper des Stents lediglich aus miteinander verbundenen Spiralenanordnungen16 umfaßt sein. Eigentlich kann der gesamte Körper aus Spiralen bestehen. Bei dieser Ausführungsform verbinden sich die Elemente13 zwischen Spiralen über im wesentlichen den gesamten Körper des Stents. Wahlweise können die Enden in der gezeigten Weise Segmente12 umfassen. - Die in
17 gezeigte Ausführungsform zeigt Segmente12 , die wechselweise in einander entgegengesetzten Richtungen winkelig angeordnet sind, und mit Beinen derselben von verschiedener Länge und mit Elementen13 , die an verschiedenen Stellen mit den Segmenten in Kontakt stehen, d. h. wie an dem Schleifenabschnitt14 und an dem eigentlichen Segmentabschnitt. - Die
18 und23 –28 demonstrieren Beispiele dafür, was mit dem Begriff Spirale gemeint ist. Natürlich können zusätzliche Glieder in den Spiralen enthalten sein. Die19 –22 zeigen spiralenartige Beispiele, die nicht unter den Wortlaut der Ansprüche fallen. - Der erfindungsgemäße Stent kann auch mit wechselweise angeordneten Segmenten
12 in serpentinenartiger, ringförmiger Konfiguration versehen sein, die durch Doppelreihen von miteinander verbundenen Spiralenkonfigurationen16 miteinander verbunden sind. - Die Spiralelemente
16 können an den Enden eines Stents10 enthalten sein. Zwar kann sich die Erfindung in vielen verschiedenen Formen verkörpern, jedoch sind hier spezifische bevorzugte Ausführungsformen der Erfindung im einzelnen beschrieben. Diese Beschreibung ist eine beispielhafte Umsetzung der Prinzipien der Erfindung und soll die Erfindung nicht auf die speziellen dargestellten Ausführungsformen beschränken.
Claims (17)
- Ausdehnbarer Stent in Form eines im allgemeinen rohrförmigen Körpers (
10 ), dadurch gekennzeichnet, daß die Konfiguration desselben mehrere Spiralelemente (16 ) umfaßt, wobei jedes Spiralelement aus mehreren sich um einen gemeinsamen Punkt herum windenden Gliedern (13a , b) umfaßt ist, wobei die Spiralelemente um den Körper des Stents herum angeordnet sind, wobei sich die mehreren Spiralelemente in einer kontinuierlichen und sich allmählich erweiternden Kurve um den gemeinsamen Punkt herum winden. - Stent nach Anspruch 1, wobei sich die Spiralelemente (
16 ) an den Enden des Stents befinden. - Stent nach Anspruch 1, wobei zumindest einige von den Spiralelementen (
16 ) miteinander verbunden sind. - Stent nach Anspruch 2, wobei der Stentkörper (
10 ) aus im wesentlichen allen miteinander verbundenen Spiralelementen (16 ) umfaßt ist. - Stent nach Anspruch 1, wobei zumindest einige von den Spiralelementen (
16 ) zwischen anderen Stentelementen miteinander verbunden sind. - Stent nach Anspruch 5, wobei die anderen Stentelemente ringförmige, serpentinenartige Elemente (
12 ) sind. - Stent nach Anspruch 6, wobei die serpentinenartigen Abschnitte der Elemente (
12 ) längs in bezug auf die Längsachse des Stents angeordnet sind. - Stent nach Anspruch 6, wobei die serpentinenartigen Abschnitte der Elemente (
12 ) winkelig in bezug auf die Längsachse des Stents angeordnet sind. - Stent nach Anspruch 1, mit: einer Vielzahl von längs ausgerichteten und ausdehnbaren Segmenten mit im allgemeinen zylindrischer Konfiguration (
12 ), wobei sich der Durchmesser der Segmente durch Ausdehnung vergrößert, mindestens einem von einem Segment (12 ) ausgehenden Verbinderelement (13a ) zum Anschluß an ein von einem benachbarten Segment (12 ) ausgehendes entsprechendes Element (13b ), wobei die Spiralelemente aus Verbindern ausgebildet sind, die sich um einen gemeinsamen Punkt zwischen dem benachbarten Segment (12 ) winden, wobei sich die Verbinderelemente bei Ausdehnung der benachbarten Segmente geraderichten. - Stent nach Anspruch 9, wobei zwei Verbinderelemente (
13a ) vorhanden sind, die von einem Segment (12 ) ausgehen, um sich an zwei entsprechende, von einen benachbarten Segment (12 ) ausgehende Verbinderelemente (13b ) anzuschließen. - Stent nach Anspruch 9, wobei der Geraderichtvorgang der Verbinder ein Auseinanderwickelvorgang ist.
- Stent nach Anspruch 9, wobei die ausgerichteten, ausdehnbaren Stentsegmente (
12 ) die Form von ringförmigen, serpentinenartigen Segmenten aufweisen. - Stent nach Anspruch 12, wobei die serpentinenartigen Abschnitte der Segmente (
12 ) längs in bezug auf die Längsachse des Stents angeordnet sind. - Stent nach Anspruch 12, wobei die serpentinenartigen Abschnitte der Elemente (
12 ) winkelig in bezug auf die Längsachse des Stents angeordnet sind. - Stent nach Anspruch 1, des weiteren mit: mehreren ausdehnbaren Segmenten mit im allgemeinen zylindrischer Konfiguration (
12 ); Verbinderelementen (13a ), die sich zwischen den Segmenten (12 ) erstrecken, um entsprechende Elemente (13b ) zu verbinden, die sich von einem benachbarten Segment erstrecken, wobei die Spiralelemente aus Verbindern ausgebildet sind, die sich um einen gemeinsamen Punkt herum winden und mindestens eine Spiralkonfiguration zwischen den Segmenten bilden, wobei sich die Spirale bei Ausdehnung des Stents geraderichtet. - Stent nach Anspruch 15, wobei die Verbinder mehrere Spiralelemente (
16 ) zwischen den Segmenten bilden. - Stent nach Anspruch 15, wobei beim Geraderichten die Spiralelemente (
16 ) auseinandergewickelt werden.
