Verschiedene kathetergeführte Methoden zur Gefäßrekanalisierung stehen heute zur Verfugung: Ballondilatation, Atherektomie, Stents, Laserkatheter, hydrodynamische Katheter. Ballondilatation und Atherektomie sind für weiches Gewebsmaterial weniger gut geeignet; Laserkatheter und hydrodynamische Verfahren sind zur intravasculären Entfernung von z.B.BIutgerinnseln besser geeignet, allerdings sind diese Instrumente komplex, wenig anwenderfreundlich und sehr teuer. (z.B.: HYDROLYSER, Possis Ine -CLOTBUSTER, Microvena Ine - SET CATHETER, Fitmedica Ine - TEC DEVICE, IVT Ine - EXCISER CATHETER, IVM Ine u.a.)Various catheter-guided methods for vascular recanalization are available today: balloon dilatation, atherectomy, stents, laser catheters, hydrodynamic catheters. Balloon dilatation and atherectomy are less suitable for soft tissue material; laser catheters and hydrodynamic procedures are better suited for the intravascular removal of blood clots, for example, but these instruments are complex, not very user-friendly and very expensive. Die Gestaltung der Katheterspitze erlaubt derzeit lediglich eine antegrade Aktion des Saug/Schneidemechanismus, nicht aber eine seitliche Funktion, d.h. Entfernung von Material, das seitlich vom Katheter oder an der Gefäßwand liegt. Falls das Endloch durch Material verstopft und damit der rotierende Schneidemechanismus blockiert ist, kann durch Nutzung der seitlichen Öffnungen die Funktion wiederhergestellt werden. In Gefässen von einem Durchmesser, der grosser ist als der Durchmesser des Katheters, kann eine zusätzliche seitliche Funktionsfähigkeit die Effektivität der Entfernung von endoluminalem Material (Blugerinnsel, weiche Gefäßverengungen) erheblich erhöhen.Only the exciser catheter (Fig. i) combines the removal of material, e.g. from vessels by suction, with a simple cutting mechanism in a single disposable catheter without an external energy source. The design of the catheter tip currently only allows an antegrade action of the suction/cutting mechanism, but not a lateral function, i.e. removal of material that is located to the side of the catheter or on the vessel wall. If the end hole is blocked by material and the rotating cutting mechanism is blocked, the function can be restored by using the side openings. In vessels with a diameter that is larger than the diameter of the catheter, additional lateral functionality can significantly increase the effectiveness of removing endoluminal material (blood clots, soft vascular constrictions).
