Die Distraktionsosteogenese hat in den letzten Jahren im Mund-Kiefer-Gesichtsbereich das Spektrum der osteoplastischen Operationen wesentlich erweitert. Nach Zöller sind Vorteile in der simultanen Weichteildehnung, der fehlenden Entnahmemorbidität und der vaskulären Versorgung des distrahierten Segmentes zu sehen. Als Nachteil sieht Zöller die ästhetische Beeinträchtigung während der Distraktion mit T-fÖrmigen Distraktoren. Da konventionelle Distraktoren in der Regel an der Basis ansetzen, ist eine umfängliche Präparation zur Insertion ebenso wie zur Entfernung erforderlich. Demgegenüber haben Distraktionsimplantate, bei denen dentale Implantate durch vertikale Verlängerung über eine Spindel den knöchernen Höhengewinn erzielen den Vorteil des fehlenden Sekundäreingriffes, jedoch den Nachteil, daß eine sichere Verankerung der zu distrahierenden Implantatanteile oft bei individuellen Patientensituationen nicht zu realisieren ist, da die Kieferkammbreite in der jeweiligen Region meist nicht ausreicht.In recent years, distraction osteogenesis has significantly expanded the spectrum of osteoplastic operations in the oral and maxillofacial region. Erfindungsgemäß wird der Nachteil der beschriebenen Distraktoren dadurch gelöst, daß ein zylinderförmiger Distraktor, der mit einer oder mehreren Extensionen nach Art einer Mikroosteosyntheseplatte fest verbunden ist, in eine vertikale Bohrung am Rand des mobilisierten Fragments in den Knochen inseriert wird . Seine Stabilität erhält er durch eine Mikroosteosynthese an der crestalen Oberfläche des dem Distraktionssegment benachbarten Knochens. Die Distraktion erfolgt über das Aktivieren einer Spindel, die über eine Auflage eine Osteosyntheseplatte variabler Form und Größe vertikal zur Distraktorachse bewegt. Die Osteosyntheseplatte wird an der Oberfläche des zu distrahierenden Segmentes angebracht. Unter Verwendung zweier oder mehrerer Distraktoren kann bei größeren Segmenten an den Segmentenden auch mit unterschiedlicher Dimension entsprechend den Erfordernissen der anatomischen Situation distrahiert werden. Die große Variabilität bei der Oberflächenverankerung des Distraktors erlaubt die Distraktion des mobilisierten Segmentes sowohl in vertikaler als auch in einer individuell vorgegebenen anderen Raumrichtung. Die ästhetische Beeinträchtigung ist minimal, da lediglich ein stiftförmiger Aktivator die Schleimhaut durchbricht, der ggf. durch geeignete prothetische Hilfsmittel zu kaschieren ist.According to the invention, the disadvantage of the distractors described is solved in that a cylindrical distractor, which is firmly connected to one or more extensions in the manner of a micro-osteo-synthesis plate, is inserted into a vertical hole in the bone at the edge of the mobilized fragment. The great variability in the surface anchoring of the distractor allows the distraction of the mobilized segment both vertically and in another individually predetermined spatial direction. The aesthetic impact is minimal, as only a pin-shaped activator breaks through the mucous membrane, which can be concealed by suitable prosthetic aids if necessary.

In Abb. 3 ist er am Übergang von stabilem Knochen (2) und dem Distraktionssegment (3) nach Art eines enossalen Implantates eingesetzt. Die Distraktionsspindel (4) weist eine Auflage (5) auf. Die Auflage hebt eine Osteosyntheseplatte (6) vertikal. Die Extension des OZAD nach Art einer Osteosyntheseplatte ist über Knochenschrauben (7) mit dem Distraktionssegment ( 3) verbunden. Bei Drehung der Spindel (4) über die transgingivale Aktivatorschraube (8) kommt es zu einer Vertikalbewegung des Distraktionssegmentes, nach Ende des Distraktionsvorganges wird die Aktivatorschraube durch gegenläufige Rotation entfernt, der OZAD wird nach Ende des Ossifizierungsprozesses entfernt. Zur einfacheren Entfernung kann er ein Gewinde an seiner Außenfläche enthalten, dessen Richtung vorzugsweise gegenläufig zum Innengewinde gerichtet sein sollte.Fig. 1 shows the surface-anchored cylindrical alveolar distractor OZAD (1) in a side view with an extension, Fig. 2 in an occlusal view in a variant with 2 osteosynthesis plates as extensions.
