Abstract
一种治疗乙肝的中成药——乙肝转阴散,该药组分中既包含有依照传统中医理论治疗乙肝常用的五味子、丹参、赤芍、当归、黄芪、茯苓、白术、三七等中药,又包含有经现代医学研究证明具有抑制、清除乙肝病毒功能的蝉翼藤、叶下珠、紫金牛、瓜子金、橄榄,能纠正微循环障碍、防治肝纤维化的炮甲珠、水蛭、虻虫、全虫等类药物。经临床验证,该药治疗急慢性乙肝总有效率达84.64%,且服用方便,无任何毒副作用。
Description
本发明涉及一种治疗乙型病毒性肝炎(以下简称乙肝)的中成药——乙肝转阴散。
乙肝发病率高，传染性强，流行面广，人们一旦感染上病毒，很难清除，往往反复发作，转化成慢性乙肝，久治不愈易发展成肝硬化甚至肝癌，严重危害人们健康。对于乙肝病毒的清除、乙肝的彻底根治，目前国际国内均无理想的特效药。如西药首选药物干扰素，短期转阴率仅20％左右，且复发率高、毒副作用大、价格昂贵，大多数患者难于承受。祖国传统中医中药对乙肝病有一定疗效，但因乙肝病疗程长、中药汤剂服用不便，病人难于坚持，因而近几年市面上出现了各种各样的中成药，但因乙肝病患者病情复杂多变，寒热、虚实、差异较大，同一种药物，对不同病情病人不一定适用，且某种药物服用较久，易于寒热偏激，或苦寒过伤脾胃阳气，或温燥耗损精血津液，甚至违反中医虚虚实实之戒。而且大多数中成药纯从传统中医经验出发，未顾及运用现代医学研究成果，因而，能彻底清除病毒(俗称转阴)、对乙肝有显著疗效的药物不多。
本发明的目的在于提供一种既符合传统中医治肝实脾调肾、行气、活血、解毒的治疗原则，又运用了现代西医病毒学、免疫学、自由基学、元素医学研究成果，有较好清除病毒(转阴)功能、对彻底根治乙肝效果好的中成药——乙肝转阴散。
为达上述目的，本发明之“乙肝转阴散”除包含依据传统中医理论而经常使用的中药成份外，还包含经现代医学证明，有较好抑制、清除病毒，提高机体免疫功能等作用的中药组份。具体组份有：
A、健脾益气类药物：黄芪、生晒参、白术；
B、化痰除湿和胃类药物：法夏、茯苓、柿蒂；
C、调肝养血类药物：当归、丹参、赤芍；
D、平补肝肾类药物：制首乌、桑寄生；
E、行气活血类药物：香附、郁金；
F、搜络逐瘀类药物：水蛭、虻虫、全虫；
G、清热、利湿、解毒类药物：田基黄、蒲公英、猪胆汁；
H、能抑制、清除乙肝病毒类药物：叶下珠、蝉翼藤、紫金牛、瓜子金、橄榄；
I、能调节人体免疫功能类药物：黄芪、当归、灵芝、三七；
J、能降低转氨酶活性类药物：五味子、生地；
K、能治疗高胆红质血症类药物：田基黄、大血藤、赤芍；
L、能改善蛋白质代谢类药物：生晒参、白术、当归；
M、能纠正微循环障碍、防治肝纤维化类药物：炮甲珠、水蛭、虻虫、全虫；
N、含锌高、能纠正人体铜、锌含量比(Cu/Zn)类药物：香附、蒲公英、田基黄。
各组份含量(以生药重量份数计)为：

制作方法：
将首乌、香附、甲珠按传统中药加工方法分别预制成：制首乌、醋香附、炮甲珠，然后将一部份中药按传统方法浓煎取汁，一部份中药洗净烘干研细，将两部分药物与猪胆汁混合烘干研细为极细粉末经消毒处理后即可分装，制成冲剂、胶囊、糖衣片、蜜丸、水丸、丹剂等型中成药。
本发明之治疗乙肝的中成药——乙肝转阴散主要发明人周道红(柳州市中医院肝胆病专家、主任医师、世界医药研究中心研究员，国家级名老中医李昌源教授的第一个硕士研究生和学术经验继承人)根据自己从事中西医医务工作30余年的临床经验和心得体会，在查阅大量古、今中外文献，总结前人经验基础上，对李昌源教授治疗肝病的经验方药进行反复筛选，并加入经现代药理和临床研究证实对抑制和清除乙肝病毒、调整人体免疫机能、恢复肝脏形态和功能，防治肝纤维化和癌变确有疗效的中药组份，经科学加工炮制而成，因而具有以下优点：
1、既具备中医辩证论治特色，又吸收现代医学科研成果，治疗效果好，临床运用总有效率达84.64％。
(1)具备中医辩证论治特色：
祖国医学认为，乙肝的病因初乃湿热邪毒侵袭，久则必挟瘀血痰湿。其病发于肝，及于脾，系于肾。由于肾为肝之母，脾之本，故在乙肝的发病及转归中起着极为重要的作用。发病之前，多因肾中精气不足，以致肝脾皆虚，易受湿热邪毒侵害而发生乙肝；发病之后，肝郁脾虚，久必归肾，每多肝肾阴虚、脾肾阳虚之证；病至后期，气滞血瘀，痰湿内生，缠绵反复，久治不愈。本发明之乙肝转阴散用黄芪、生晒参、白术健脾益气，法夏、茯苓、柿蒂化痰除湿和胃，当归、丹参、赤芍调肝养血，制首乌、桑寄生平补肝肾，香附、郁金行气活血，水蛭、虻虫搜络逐瘀，田基黄、蒲公英。猪胆汁清热利湿解毒，全药组方审因论诒，理法方药相扣，攻不伤正，补不碍邪，扶正、祛邪相得益彰，故疗效颇为满意。
(2)吸收现代医学科研成果：
现代医学认为，乙肝是由于HBV病毒感染，启动人体一系列免疫反应，造成肝细胞变性坏死，肝功能损伤，微量元素代谢紊乱，微循环障碍，进而导致肝纤维化甚至癌变。本发明乙肝转阴散用叶下珠、蝉翼藤、紫金牛、瓜子金、橄榄等抑制和清除乙肝病毒，黄芪、当归、三七、灵芝等调节人体免疫功能，五味子、生地降低转氨酶活性，田基黄、大血藤、赤芍治疗高胆红质血症，生晒参、白术、当归改善蛋白质代谢，恢复A/G正常比值，炮甲珠、水蛭、全虫、虻虫纠正微循环障碍，防治肝纤维化。
元素医学研究发现：多数急慢性乙肝患者血锌降低，血铜升高，Cu/Zn比值上升，不仅使肝脏多种酶的合成受到障碍，蛋白质代谢异常，而且促进脂质过氧化损伤，免疫功能紊乱而加重肝脏病变，导致慢性化和纤维化。本发明乙肝转阴散中香附子、蒲公英、田基黄等药富含锌、锰等微量元素，可补充乙肝患者锌、锰之不足，恢复Cu/Zn比值的正常水平。从而促进乙肝病人全面康复。
经发明人临床运用实践，也证实本发明之乙肝转阴散确有显著疗效。
据不完全的统计，自92年初至97年2月28日经用本发明乙肝转阴散治疗的521例乙肝患者(包括慢迁肝、慢活肝、急性乙肝及无症状乙肝病毒携带者)中，HBsAg阳性518例，转阴79例，转阴率15.25％；HBeAg阳性329例，转阴211例，转阴率64.13％；谷丙转氨酶(ALT，即GPT)异常192例，治疗1个月后恢复正常152例，占79.17；谷草转氨酶(AST，即GOT)异常143例，治疗1个月后恢复正常118例，占82.52％。
521例乙肝患者中，显效(病毒复制指标HBeAg、HBcAb-IgM、PCR-HBV转阴一项以上、肝功能恢复正常，症状、体征消失)413例，占79.21％；有效(HBVM转阴一项以上，肝功能基本恢复正常，症状、体征改善)28名，占5.37％；无效(HBVM无变化，肝功能、症状、体征无改善)80例，占15.36％，总有效率为84.64％。
实施例一
一种治疗乙肝的中成药——乙肝转阴散。
按附表实施例一所列比例，取丹参、赤芍、当归、桑寄生等14味中药加水浓煎，将两次煎出液混合浓缩，得浓煎汁I；取新鲜猪胆汁加入上述浓煎汁中，混合均匀得混合液II；取黄芪、生晒参、白术之类中药精选去杂、粗洗后烘干，研成细粉III；将混合液II与细粉III搅拌混匀，烘干后研成极细粉未IV消毒即制成本发明之成药乙肝转阴散。上述药粉IV可直接为散剂冲水服用，也可制成胶囊、糖衣片、蜜丸、水丸、丹剂等剂型，方便患者服用。
一般用量、服法：每次服5-10克(以药粉IV量计)，每日3次，每个疗程3个月，连服1-3个疗程。
实施例二——实施例十
其制作方法同实施例一，所不同之处在于根据患者不同病情按附表一所例范围，各味药取的重量份数不一，制成对各种病情针对性更强的乙肝转阴散，用量服法相同。
附典型病例：
(1)谢某某，女，25岁，柳州市火电公司干部。患慢性迁延性乙肝6年。96年4月22日初诊，呈大三阳(即HBsAg、HBeAg、HBcAb三项阳性)，服乙肝转阴散1疗程后，抗-HBe出现，与大三阳并存；继服1疗程后，HBsAg、HBeAg一齐消失，抗-HBs出现而痊愈。
(2)林某某，男，48岁，台北市商人，患乙肝4年。96年8月3日初诊，呈小三阳(即HBsAg、HBeAb、HBcAb三项阳性)，携1疗程乙肝转阴散回台湾服用。95年11月2日由厦门打电话告知：96年10月27日在台北某医院复查HBsAg、HBeAb、HBcAb全部转阴，医生、患者均不敢相信，于10月29日另换一家医院复查，仍全部为阴性。
(3)刘某某，男，45岁，柳州市起重机厂干部，患慢性活动性乙肝2年，曾用干扰素治疗3月无效。96年10月22日初诊时为大三阳，GPT88u，B超示左肾积水，左输尿管上段扩张，经乙肝转阴散治疗1+月，96年12月7日复查HBsAg、HBeAg一齐转阴，仅抗-HBe、抗-HBc两项阳性，GPT恢复正常，左肾积水消失，左输尿管形态恢复正常。
(4)罗某，男，8岁，柳州市人事局干部子女，无症状病毒携带，96年5月16日查单项HBsAg阳性，服乙肝转阴散1疗程，于96年8月21日复查，HBsAg转阴，抗-BBs出现。2月后再复查，疗效巩固，未反跳。
(5)廖某某，男38岁，贵州省六盘水市个体户，患慢性活动性黄疸型乙型肝炎1年，92年6月28日初诊时为小三阳，GPT120u，II16u，TTT10u，TP72.25，A37.2g/L，G35.3g/l。予乙肝转阴散口服，92年7月28日复查GPT、II、TTT均恢复正常，A升为49.8g/L，G降为25.5g/L；92年8月28日复查HBsAg转阴，仅抗-HBe、抗-HBc两项阳性，继续服乙肝转阴散以巩固疗效；93年1月15日复查HBVM全部转阴；后每半年复查一次，追访至95年11月，未见复发。
(6)杨某某，女，52岁，贵阳中医学院职工，92年2月16日初诊为急性黄疸型乙肝，HBsAg、HBeAg、HBcAb-IgM三项阳性，GPT110u，TTT12u，TBIL26.8umol/L，DBIL8.4umol/L。予乙肝转阴散口服。92年3月12日复查HBeAg、HBcAb-IgM转阴，抗-HBe、抗-HBs转阳，HBsAg仍为阳性，GPT、TTT、TBIL、DBIL全部恢复正常。继服至4月22日复查，HBsAg、抗-HBe转阴，仅抗-HBs阳性。追访5年，至97年2月28日复查，抗-HBs阳性，疗效稳定，未见复发。

法 夏 2-10 茯 苓 4-20 柿 蒂 2-15
当 归 2-20 丹 参 10-50 赤 芍 10-50
制首乌 10-40 桑寄生 10-30 醋香附 3-20
郁 金 5-30 水 蛭 1-10 虻 虫 1-5
全 虫 1-5 田基黄 10-30 蒲公英 10-50
叶下珠 20-90 蝉翼藤 10-30 紫金牛 3-20
瓜子金 3-20 橄 榄 3-30 灵 芝 3-20
