Abstract
The invention relates to the technical field of traditional Chinese medicines, in particular to a traditional Chinese medicine enema composition for treating ulcerative colitis and a preparation method and application thereof. The composition comprises the following raw materials: radix Pulsatillae, cortex Fraxini, herba Sedi, herba Patriniae, Coptidis rhizoma, Saviae Miltiorrhizae radix, radix Angelicae sinensis, radix aucklandiae, radix Paeoniae alba, rhizoma Bletillae, radix Sangusorbae, indigo naturalis and Catechu. The traditional Chinese medicine enema composition has strong pain relieving and bleeding stopping effects, can effectively promote intestinal mucosa healing, improve symptoms such as abdominal pain, tenesmus, hematochezia and the like, has quick and definite curative effect, treats both symptoms and root causes, is not easy to relapse after stopping administration, and has low price.

Technical Field
The invention relates to the technical field of traditional Chinese medicines, in particular to a traditional Chinese medicine enema composition for treating ulcerative colitis and a preparation method and application thereof.
Background
Ulcerative Colitis (UC) is a chronic nonspecific intestinal inflammatory disease with undefined etiology, and has clinical manifestations of abdominal pain, diarrhea, tenesmus, mucopurulent bloody stool, and pathological changes affecting rectum and colon. The disease incidence rate rises year by year, and the disease is often delayed, not cured and repeatedly attacks, which brings long-term pain to patients. Western medicines are commonly used for clinical treatment by an aminosalicylic acid preparation, corticosteroid and immunosuppressant, but the curative effect is not ideal, dependence is easy to generate, relapse is easy to occur, and the price is high.
In recent years, the traditional Chinese medicine has prominent advantages in preventing and treating UC. The traditional Chinese medicine enema is one of the external treatment methods of the traditional Chinese medicine, accords with the treatment view of the syndrome differentiation of the traditional Chinese medicine, inherits the traditional rectal administration method of the traditional Chinese medicine, leads the medicine to directly reach the focus, and exerts certain curative effect in the clinical treatment of UC. At present, the traditional Chinese medicine enema products which are mainly adopted clinically comprise tin powder, new rehabilitation liquid and the like, which are beneficial to intestinal mucosa healing, but are easy to relapse after stopping medicine, and the prognosis can not be improved.
Disclosure of Invention
Aiming at the problems that the traditional Chinese medicine enema product is easy to relapse after stopping taking the medicine and can not improve the prognosis, the invention provides the traditional Chinese medicine enema composition for treating ulcerative colitis.
The invention also provides a preparation method of the traditional Chinese medicine enema composition for treating ulcerative colitis.
The invention also provides application of the traditional Chinese medicine enema composition for treating ulcerative colitis in preparation of an enema medicine for treating ulcerative colitis.
In order to achieve the purpose of the invention, the embodiment of the invention adopts the following technical scheme:
a traditional Chinese medicine enema composition for treating ulcerative colitis comprises the following raw materials in parts by weight: 9-15 parts of Chinese pulsatilla root, 6-12 parts of cortex fraxini, 15-30 parts of sedum sarmentosum, 6-15 parts of patrinia, 6-10 parts of coptis chinensis, 10-15 parts of salvia miltiorrhiza, 6-12 parts of angelica sinensis, 6-10 parts of elecampane, 6-15 parts of white paeony root, 6-15 parts of rhizoma bletillae, 9-15 parts of sanguisorba officinalis, 3-8 parts of indigo naturalis and 4-8 parts of catechu.
The traditional Chinese medicine enema composition has strong pain-relieving and bleeding-stopping effects, can effectively promote intestinal mucosa healing, can quickly relieve symptoms such as abdominal pain, tenesmus, hematochezia and the like caused by UC, has quick and definite curative effect, treats both symptoms and root causes, is not easy to relapse after stopping administration, is an effective enema formula for treating UC, and has low price.
In the traditional Chinese medicines of the formula:
the Chinese pulsatilla root is bitter in taste and cold in nature; clearing away heat and toxic material, cooling blood and stopping dysentery;
cortex Fraxini is bitter and astringent in taste and cold in nature; clearing heat, eliminating dampness, astringing and stopping dysentery;
sedum sarmentosum is sweet and light in flavor, slightly sour and slightly cold in nature; promoting diuresis, removing jaundice, clearing away heat and toxic materials;
herba Patriniae is pungent and bitter in flavor and slightly cold in nature; clearing away heat and toxic materials, eliminating carbuncle, expelling pus, removing blood stasis, and relieving pain;
coptis root, rhizoma Coptidis is bitter in taste and cold in nature; clearing heat, eliminating dampness, purging pathogenic fire and removing toxic substances;
salvia miltiorrhiza is bitter in taste and slightly cold in nature; promoting blood circulation, regulating menstruation, removing blood stasis, relieving pain, cooling blood, resolving carbuncle, relieving restlessness, and tranquilizing mind;
chinese angelica root, radix Angelicae sinensis is sweet and pungent in flavor and warm in nature; enriching blood, regulating menstruation, promoting blood circulation, relieving pain, and loosening bowel to relieve constipation;
aucklandia root, radix aucklandiae is pungent and bitter in flavor and warm in nature; promoting qi circulation, relieving pain, invigorating spleen, and promoting digestion;
white peony root is bitter and sour in taste and slightly cold in nature; nourishing blood, astringing yin, softening liver, relieving pain, and suppressing liver yang;
rhizoma Bletillae is bitter, sweet, astringent and cold in nature; astringe to stop bleeding, resolve swelling and promote tissue regeneration;
sanguisorba root is bitter, sour and astringent in taste and slightly cold in nature; cool blood to stop bleeding, remove toxicity and heal wound;
indigo naturalis is salty in taste and cold in nature; clearing away heat and toxic material, cooling blood, removing speckle, clearing liver-fire, and arresting convulsion;
catechu is bitter and astringent in taste and slightly cold in nature; promoting blood circulation to stop pain, stopping bleeding and promoting granulation, astringing dampness and healing sore, clearing lung-heat and eliminating phlegm.
In the traditional Chinese medicine enema composition, the stringy stonecrop herb has the effects of dissolving turbidity and detoxifying, eliminating carbuncle and eliminating swelling, the Chinese pulsatilla root mainly enters the large intestine to dissolve turbidity and detoxify, and cooling blood and stopping dysentery, the stringy stonecrop herb and the Chinese pulsatilla root can eliminate the pathogenesis of turbid toxin and can also eliminate carbuncle and stop dysentery, and the stringy stonecrop herb and the Chinese pulsatilla root are monarch drugs together. The stringy stonecrop herb has the effects of dissolving turbidity and detoxifying, has the effect of relieving diarrhea with astringents in the composition, and can enable the traditional Chinese medicine enema composition to have better effect of relieving diarrhea compared with other medicines with the effects of dissolving turbidity and detoxifying.
The patrinia, the ash bark and the coptis are bitter and cold, clear heat and detoxify, and are used for strengthening the capacity of the monarch drug in dissolving turbidity and detoxifying in the traditional Chinese medicine enema composition; bai Shao excels at relieving diarrhea and abdominal pain with tenesmus, relieving spasm and pain, Dang Gui moves blood and regulates blood, and the two herbs are indicated for "moving blood and then draining pus and healing itself"; mu Xiang moves qi and removes stagnation, and regulating qi can remove qi by itself. The six medicines are used as ministerial medicines.
The five medicines have certain treatment effects on bellyache, hematochezia, diarrhea and mucous membrane injury caused by ulcerative colitis and are used as adjuvant medicines together.
The medicines are combined to clear turbid toxin, regulate qi and blood, relieve various symptoms and heal mucous membrane, so that the condition of relapse after medicine withdrawal can be effectively avoided.
Preferably, the traditional Chinese medicine enema composition comprises the following raw materials in parts by weight: 11-13 parts of Chinese pulsatilla root, 8-10 parts of cortex fraxini, 20-30 parts of stringy stonecrop herb, 8-13 parts of dahurian patrinia herb, 8-10 parts of coptis root, 11-13 parts of salvia miltiorrhiza, 8-11 parts of Chinese angelica, 8-10 parts of costus root, 8-13 parts of white paeony root, 8-13 parts of bletilla, 10-13 parts of garden burnet root, 5-7 parts of indigo naturalis and 5-7 parts of catechu.

The embodiment of the invention also provides a preparation method of the traditional Chinese medicine enema composition for treating ulcerative colitis, which specifically comprises the following operations: weighing the raw materials according to the proportion of the traditional Chinese medicine enema composition, mixing, decocting with water, and filtering to obtain the traditional Chinese medicine enema composition.
The preparation method is simple to operate, and the obtained finished product can be directly subjected to enema or further concentrated or diluted according to clinical actual requirements.
Preferably, the decoction is carried out for 1-2 times, each time lasts for 1-3 hours, and the water consumption for each time is 6-8 times of the total mass of the raw materials of the traditional Chinese medicine enema composition.
The embodiment of the invention also provides another preparation method of the traditional Chinese medicine enema composition for treating ulcerative colitis, which specifically comprises the following operations: weighing the raw materials according to the proportion of the traditional Chinese medicine enema composition, decocting the raw materials with water, filtering, concentrating, drying and mixing to obtain the traditional Chinese medicine enema composition.
The product obtained by the preparation method is a dry product, is convenient to store and transport, and can be dissolved into water to a required concentration in clinical use.
Preferably, the decocting time is 1-3 hours, and the amount of water is 6-8 times of the mass of each raw material.
The embodiment of the invention also provides application of the traditional Chinese medicine enema composition for treating ulcerative colitis in preparation of an enema medicine for treating ulcerative colitis. The traditional Chinese medicine composition is prepared into enema medicines and is administrated by rectal dropping method, so that the effects of relieving diarrhea with astringents and relieving abdominal pain can be achieved, and the relapse condition after stopping the medicine can be reduced.

Example 1
The embodiment of the invention provides a traditional Chinese medicine enema composition for treating ulcerative colitis, which comprises the following raw materials in parts by weight:
12 parts of Chinese pulsatilla root, 9 parts of cortex fraxini, 30 parts of sedum sarmentosum, 12 parts of patrinia, 9 parts of coptis, 12 parts of salvia miltiorrhiza, 10 parts of angelica, 9 parts of elecampane, 12 parts of white paeony root, 12 parts of rhizoma bletillae, 12 parts of sanguisorba, 6 parts of indigo naturalis and 6 parts of catechu.
The preparation method comprises the following steps:
mixing the above materials, placing into a standard Chinese medicinal decoction machine, adding 1200 parts of water, decocting for 2 hr, filtering, decocting the residue with 1000 parts of water for 1 hr, filtering, and mixing filtrates.
The application method comprises rectal dripping for about 300ml each time.
Example 2
The embodiment of the invention provides a traditional Chinese medicine enema composition for treating ulcerative colitis, which comprises the following raw materials in parts by weight:
12 parts of Chinese pulsatilla root, 9 parts of cortex fraxini, 30 parts of sedum sarmentosum, 12 parts of patrinia, 9 parts of coptis, 12 parts of salvia miltiorrhiza, 10 parts of angelica, 9 parts of elecampane, 12 parts of white paeony root, 12 parts of rhizoma bletillae, 12 parts of sanguisorba, 6 parts of indigo naturalis and 6 parts of catechu.
The preparation method comprises the following steps:
decocting the above materials with water 7.5 times the mass of each for 2 hr, filtering, decocting the residue with 6.5 times of water for 1 hr, filtering, mixing filtrates, concentrating, and drying to obtain water-decocted extract. Mixing the water decoction extracts of the above materials.
The composition is dissolved in water to give a solution of 0.5g per ml of the raw materials, and is administered by rectal dripping with a dosage of about 300ml per time.
Example 3
The embodiment of the invention provides a traditional Chinese medicine enema composition for treating ulcerative colitis, which comprises the following raw materials in parts by weight:
11 parts of Chinese pulsatilla root, 8 parts of cortex fraxini, 20 parts of sedum sarmentosum, 8 parts of patrinia, 8 parts of coptis, 11 parts of salvia miltiorrhiza, 8 parts of angelica, 8 parts of elecampane, 8 parts of white peony root, 8 parts of bletilla, 10 parts of sanguisorba, 5 parts of indigo naturalis and 5 parts of catechu.
The preparation method comprises the following steps:
mixing the above materials, placing into a standard Chinese medicinal decoction machine, adding 944 parts of water, decocting for 2 hr, filtering, decocting the residue with 708 parts of water for 1 hr, filtering, and mixing filtrates.

The embodiment of the invention provides a traditional Chinese medicine enema composition for treating ulcerative colitis, which comprises the following raw materials in parts by weight:
13 parts of Chinese pulsatilla root, 10 parts of cortex fraxini, 30 parts of sedum sarmentosum, 13 parts of patrinia, 10 parts of coptis, 13 parts of salvia miltiorrhiza, 11 parts of angelica, 10 parts of elecampane, 13 parts of white paeony root, 13 parts of rhizoma bletillae, 13 parts of sanguisorba, 7 parts of indigo naturalis and 7 parts of catechu.
The preparation method comprises the following steps:
decocting the above materials with water 7.5 times the mass of each for 2 hr, filtering, decocting the residue with 6.5 times of water for 1 hr, filtering, mixing filtrates, concentrating, and drying to obtain water-decocted extract. Mixing the water decoction extracts of the above materials.

decocting the above raw materials with water 8 times the mass of each raw material for 2 hr, filtering, decocting the residue with water 6 times the mass of each raw material for 1 hr, filtering, mixing filtrates, concentrating, and drying to obtain water-decocted extract. Comparative example 1
The comparative example provides a traditional Chinese medicine enema composition for treating ulcerative colitis, and on the basis of example 1, sedum sarmentosum is replaced by oldenlandia diffusa which has similar efficacy with sedum sarmentosum and can dissolve turbidity and detoxify, and the traditional Chinese medicine enema composition specifically comprises the following raw materials in parts by weight:
12 parts of Chinese pulsatilla root, 9 parts of cortex fraxini, 30 parts of spreading hedyotis herb, 12 parts of dahurian patrinia herb, 9 parts of coptis root, 12 parts of salvia miltiorrhiza, 10 parts of angelica, 9 parts of costustoot, 12 parts of white paeony root, 12 parts of common bletilla pseudobulb, 12 parts of garden burnet root, 6 parts of natural indigo and 6 parts of catechu.
The preparation method is the same as example 1.
Comparative example 2
The comparative example provides a traditional Chinese medicine enema composition for treating ulcerative colitis, and on the basis of example 1, sedum sarmentosum is replaced by glabrous greenbrier rhizome which has similar efficacy with sedum sarmentosum and can dissolve turbidity and detoxify, and the traditional Chinese medicine enema composition specifically comprises the following raw materials in parts by weight:
12 parts of Chinese pulsatilla root, 9 parts of ash bark, 30 parts of glabrous greenbrier rhizome, 12 parts of dahurian patrinia herb, 9 parts of golden thread, 12 parts of danshen root, 10 parts of Chinese angelica, 9 parts of costustoot, 12 parts of white paeony root, 12 parts of common bletilla pseudobulb, 12 parts of garden burnet root, 6 parts of natural indigo and 6 parts of catechu.
The preparation method is the same as example 1.
Effect example 1
The present effect example provides the pharmacodynamic animal experiments of examples 1 to 6 and comparative examples 1 and 2.
1. Experimental animals: 3 months old SPF grade SD rats, female and male halves, using TNBs/ethanol solution enema method to induce UC rat model.
2. Test drugs:
example set: preparing the product obtained in the embodiment 1-6 into a solution with each ml corresponding to 0.5g of raw materials;
comparative example group: preparing the products obtained in the comparative examples 1 and 2 into a solution of 0.5g of raw materials per ml;
model group: physiological saline.
3. Selecting the dosage: the dosage of each group of rats is 10ml/kg, and the administration is performed by rectal instillation for 1 time per day and is continuously performed for 14 days.
4. The number of diarrhea and mucopurulent bloody stool in each group of rats was observed and recorded on days 1, 3, 7 and 14, and the results are shown in Table 1.

As can be seen from the results in Table 1, the treatment effects were achieved in both the example groups and the comparative example groups, and the treatment effects in each example group were superior to those in each comparative example group.
Effect example 2
The effect example provides the statistical result of the clinical application effect of the traditional Chinese medicine enema composition for treating ulcerative colitis in the embodiment 1 of the invention.
1. Counting objects: 90 patients are randomly selected in the department of gastroenterology outpatient service and inpatient service of traditional Chinese medicine institute in Hebei province in 2014-2019. The conventional traditional Chinese and western medicine combined treatment scheme with consistent treatment scheme is adopted in 90 cases, wherein 50 cases simultaneously use the traditional Chinese medicine enema composition of the embodiment 1, and 40 cases are not enema. 50 patients with the enema had mucopurulent bloody stool, 47 of the patients had abdominal pain symptoms, and 48 of the patients had diarrhea. The unperfumed 40 patients were also associated with mucopurulent bloody stool, 38 of which had abdominal pain symptoms and 37 with diarrhea.
From the treatment date, patients are followed up in 3 months, 6 months and 1 year respectively, and 45 patients who use the traditional Chinese medicine enema composition of the embodiment 1 of the invention have relieved symptoms of mucopurulent bloody stool, and the remission rate is 90.00%; among 47 cases accompanied by abdominal pain symptoms, 45 cases accompanied with abdominal pain symptoms are relieved, and the relieving rate is 95.74 percent; the 48 cases accompanied with diarrhea symptoms all obtain different degrees of relief, and the relief rate is 100 percent. Of 40 patients who did not undergo enema, 33 patients had relieved symptoms of mucopurulent bloody stool, with a remission rate of 82.50%; among 38 cases accompanied by abdominal pain symptoms, 32 cases with abdominal pain symptoms are relieved, and the relieving rate is 84.21%; among 37 cases with diarrhea symptoms, 34 cases were relieved with a remission rate of 91.89%. The results are shown in tables 2 and 3.

Case 1:
tian Yi, Nu, 47 years old, Hebei province balance water man, company staff. In 2015, the symptom is repeated and difficult to cure due to the diagnosis of ulcerative colitis by hematochezia enteroscopy, and the traditional Chinese medicine enema composition in the embodiment 1 of the invention is applied for enema for 30 days (1 time in 1 day), and the disease does not relapse after the enema is applied for 2 years.
Case 2
Lidingji, male, 21 years old, Chengdu, Hebei province, college students.
