一种对多种症状具有功效的中药组合物及其衍生物和制品
技术领域
本发明涉及中医药技术领域，特别是一种脱胎于四逆汤的多功效中药组合物及其衍生中药组合物。
技术背景
《伤寒论》中记载有四逆汤：“甘草二两(炙)干姜一两半,附子一枚(生用，去皮，破八片)，上三味以水三升，煮取一升二合，去滓，分温再服”。用于回阳救逆，主治少阴病阴寒内盛证。关于四逆汤的适应症，《伤寒论》中有如下记载：
“伤寒，医下之，续得下利清谷不止，身疼痛者，急当救里；后身疼痛，清便自调者，急当救表。救里，宜四逆汤；救表，宜桂枝汤；(91)”。
“病发热头痛，脉反沉，若不差，身体疼痛，当救其里，四逆汤方。(92)”
“脉浮而迟，表热里寒，下利清谷者，四逆汤主之。(225)”
“自利不渴者，属太阴，以其脏有寒故也，当温之，宜服四逆辈(277)”

根据东汉《伤寒论》，其主要用伤寒、霍乱、泄泻，也是后世历代运用四逆汤的主流病种，根据张成东的《四逆汤古代运用的文献研究》记载的研究结果，古代文献记载四逆汤运用涉及158个症状，其中出现频率较高的前10个症状依次是：手足厥冷、汗多、泄泻、腹痛、下利清谷、上吐下利、心烦、恶寒、干呕、恶心，进一步归并为手足厥冷、恶寒、汗多、腹痛、吐利五大症，反映出四逆汤证的核心症征。四逆汤运用涉及症状出现率较高的还有胸闷，便下脓血，喘息和咳嗽症，涉及胸阳不振，寒伤肠络，肾不纳气和寒饮犯肺等病机，是源于《伤寒论》主治的发展。
四逆汤的派生配方层出不穷，其派生配方运用涉及药味共计162种，涉及20类药物，其中以与温补药配伍最为多见，例如人参、白术、肉桂、茯苓、半夏、当归、陈皮、厚朴、芍药、细辛，归属健脾益气、温里散寒、益阴养血、理气化痰四类。但是四逆汤原方配伍在东汉用生附子、干姜，晋唐以降至明则倾向于炮附子替用生附子，干姜与炮干姜互用，但至清朝则有回归之早期的主用生附子和干姜的倾向，历朝四逆汤中均用炙甘草。
可以看出，四逆汤组方精妙，法度严谨，为历代医家所推崇，是出自《伤寒论》治疗少阴寒化证的经典名方，随着临床的发展和用方经验的积累，该方已被广泛于临床各科，成为现代古方新用的重要方剂之一，国内已经有学者用现代科学方法和设备，从物质基础角度去探索和阐明其组方原理及科学内涵，揭示各药味在方中的作用。由其派生的白通汤、白通加猪胆汁汤、通脉四逆汤、茯苓四逆汤等，为中医治疗心肾虚、脾肾阳虚、真寒假热证的系列要方。根据已知派生的系列药方可以推测，还可以对四逆汤进行进一步挖掘，开拓新的派生配方以及其更宽的适应症。
发明内容
本发明提供一种药物组合物及其制品，其脱胎于经典方四逆汤，但其组方含义，适用症范围完全不同于四逆汤。
本发明提供的技术方案如下：
一种对多种症状具有功效的中药组合物，其特征在于,由2-6重量份的原料药A，0-1.5重量份的原料药B和1重量份的原料药C组成；
所述原料药A为炙甘草、生甘草、大枣和/或薏米；
所述原料药B为干姜或炮姜；
所述原料药C为制附子；
所述多种症状包括所述多种症状包括冠心病、高脂血症、心律失常、心力衰竭、高血压病、Ⅱ型糖尿病、心脏神经官能症、皮肤病、月经病、疼痛性疾病、便秘、失眠、颈椎、腰椎病、感冒、慢性支气管炎、人乳头瘤病毒感染、甲亢、神经性震颤、遗精、阳痿、小儿多动症、抑郁症、更年期综合征、慢性结肠炎、慢性胃炎、反流性食道炎、肿瘤康复、周围神经病、慢性泌尿性感染、过敏性疾病、带状疱疹、脑血管病、血液病。

优选地，还包括对所述制附子先煎煮至少15分钟，然后再加入其余原料药进行煎煮。
任一所述的中药制品的制备方法，其特征在于，包括如下步骤：对所述制附子先煎煮至少15分钟，然后加入其余原料药；煎煮所加水量比例为为每剂药每次300-800毫升，每次煎煮20-30分钟，加水2-3次，去渣合并煎煮液。
本发明提供的中药组合物，药名取为“归一饮”，主要原料药由炙甘草和/或大枣、干姜、制附子以2-3:1-1.5:1的比例组成。其脱胎于四逆汤(“甘草二两(炙)干姜一两半,附子一枚”)，但组方原理和用意完全不同于四逆汤。而本发明中药组合物或其制品(归一饮)将四逆汤的生附子换成了制附子，且用小量，用意不在祛寒邪，而是取少火生气之意，而且制附子辛散中又有固敛之意。而少火生气不是只为了补肾阳，是为了修复圆心，修复圆心不只是恢复脾胃之气，而是为了修复后天元气。因此本发明中药组合物(归一饮)是以炙甘草、大枣和/或炒薏米为君药，且剂量最大，干姜连接制附子和甘草，连接天根与圆心，虽为佐使，但其作用不可或缺；制附子少火生气，启动生长之机，是为臣。按本领域李可老先生的经验，生附子的药效是制附子的两到三倍，即生附子一枚一般有15-20克，药效上约相当于制附子40-60克，这也是李可老先生的著作中记载的用大剂量制附子的理论基础之一，因此要起到《伤寒论》四逆汤祛少阴阴寒、回阳救逆的作用，换作采用制附子的话，四逆汤的配比则为制附子40-60g、干姜24g、炙甘草32g(按汉代的一两等于现代的15.625g算)，可以看出，制附子反而是最大剂量的，是君药。而本发明归一饮中制附子的剂量是最小的，仅仅是臣药，这说明本发明的归一饮的方剂用意与四逆汤完全不同。
本发明是把四逆汤中的生附子换为了制附子，这时就已经不能再称其为四逆汤，因为中药组合物(归一饮)的主要作用不只是回阳救逆。本领域技术人员知道，生附子和制附子的作用并不完全一样。仲景凡取回阳救逆之功则用生附子，如四逆汤、通脉四逆汤、白通汤、茯苓四逆汤、干姜附子汤等等；而制附子则多用于温阳补肾、温经逐湿，如真武汤、附子汤、桂枝附子汤等等。从中药炮制上讲，一般是熟品偏补，生品偏泻；生品多偏散，熟品多偏固敛。如生地黄和熟地黄，《本草纲目》说“地黄生则大寒，而凉血，血热者需用之，熟则微温，而补肾，血衰者需用之”。再如生甘草和炙甘草、生白术和炒白术、生白芍和炒白芍、生首乌与熟首乌等等。因此相对于生附子，制附子多有固阳﹑攝阳之功，这也是后世用之引火归元的原因之一。而即使是制附子，因其剂量不同，用意也不同，如温经逐湿多用两枚甚至三枚，如附子汤、桂枝附子汤；温阳补肾，多用一枚如真武汤、桂枝加附子汤，这是少火生气之意。
从适应症方面，从古书记载看出，四逆汤主要用于回阳救逆，主治少阴病阴寒内盛证，归并为手足厥冷、恶寒、汗多、腹痛、吐利五大症。而本发明的归一饮适应症比四逆汤更广泛，临床观察本发明的中药组合物的核心原料药组方适合诸多病症如冠心病、高脂血症、心律失常、心力衰竭、高血压病、Ⅱ型糖尿病、心脏神经官能症、皮肤病、月经病、疼痛性疾病、便秘、失眠、颈椎、腰椎病、感冒、慢性支气管炎、人乳头瘤病毒感染、甲亢、神经性震颤、遗精、阳痿、小儿多动症、抑郁症、更年期综合征、慢性结肠炎、慢性胃炎、反流性食道炎、肿瘤康复、周围神经病、慢性泌尿性感染、过敏性疾病、带状疱疹、脑血管病、血液病等。仅只不适合于肾之储备不够的患者、阴虚阳亢的患者，另外，对于全身性的热毒炽盛，尤其热入营血的症候也不适合。即本发明提供的多功效中药组合物本身，脱胎于四逆汤，但适应症比四逆汤更广泛，药效温和安全，属于除了严重阴虚的患者以及单纯实热证患者之外，所有人都可以服用的保健制品。
本发明的中药组合物中，炙甘草、大枣、炒薏米可以互换或者混合使用，特别是对于炙甘草有禁忌的患者，可以完全采用大枣或炒薏米，这使得本发明的中药组合物(归一饮)的适应范围更为广泛和安全。临床实践案例证明，本发明多效中药组合物可以与多种常用原料药组合，得到对具体适应症更具有针对性和更优的配方，根据临床实验例，每种配方的效果观察验证数据至少来自于10例病例，有些上百例，具体如下：
加荆芥和防风，对皮疹的疗效更好，疗程2-3周；
加红曲对高脂血症效果更优，疗程2-3个月；
加三棱和莪术对冠心病效果更优，疗程3-6个月；
加泽泻和泽兰对心力衰竭疗效更好，疗程3－10个月；
加郁金对心脏神经官能症效果更好，疗程；
加巴戟天，对抑郁症疗效更好，疗程2-3个月；
加枸杞子和淫羊藿对Ⅱ型糖尿病(2-3个月)、失眠(1-2疗程个月)效果更好。
加巴戟天和淫羊藿，对Ⅱ型糖尿病效果更好，疗程2-3月；
加葛根，对颈椎病效果突出，疗程2-3周
加杜仲和牛膝，对腰椎病疗效更好，疗程2-3周；

根据临床使用案例，本发明的上述多效中药组合物还可以与藿香和佩兰；沙参和麦冬；黄芪；白术；半夏；肉苁蓉；丁香和柿蒂；茯苓和泽泻
辛夷和苍耳子；旋复花和代赭石；合欢花；黄芪、女贞子和补骨脂
或红花和益母草的组合得到具有不同侧重疗效的方剂；
本发明的中药组合物，都需要煎煮后才能服用。因此本发明的中药组合物的制品，在其制备方法中，都包括对方剂中的原料药进行煎煮的步骤。
在进一步优选的方案中，对制附子进行先煎煮，至少15分钟，优选至少30分钟，最好1个小时。然后再加入其余两味原料药进行煎煮制得汤剂。
本发明请求保护的中药组合物的制品，无论做成什么剂型，都可以采用本领域常规技术手段来现实，包括采用一些常规的辅料来做成各种所需的剂型。
在做成中成药的实施方案中，优选做成口服汤剂、散剂、颗粒剂、丸剂或外用剂型，但不限于此。
本发明的所有方案中采用的原料药在一般中药房都可以买到，其中，部分原料药简介如下：
炙甘草：(RADIX GLYCYRRHIZAE PREPARATA)，系豆科植物，炙甘草是用蜜烘制的甘草，俗称炮制，就是将蜂蜜置锅中炼成中蜜，改用文火加生甘草片拌炒均匀，3～5min出锅，置烤房或烘箱60～C烘至不粘手时取出，放凉即可。甘草，学名：Glycyrrhiza uralensisFisch，豆科、甘草属多年生草本，药用部位是根及根茎，药材性状根呈圆柱形，长25～100厘米，直径0.6～3.5厘米。炙甘草汤具有益气滋阴，通阳复脉治疗功效。
大枣：具有滋阴补阳，补血之功
干姜：为姜科植物姜Zingiber oj-jicinale Rosc.的干燥根茎。具有温中散寒，回阳通脉，温肺化饮的功效。
制附子：为毛茛科植物乌头Aconitum carmichaeli Debx.的子根，除去母根、须根及泥沙，经盐渍、水漂、煮、炒等法炮制后降低毒性。具有回阳救逆，补火助阳，逐风寒湿邪的功效，用于亡阳虚脱，肢冷脉微，阳痿，宫冷，心腹冷痛，虚寒吐泻，阴寒水肿，阳虚外感，寒湿痹痛。
炒薏米：薏米，拉丁文名：Coix chinensis Tod.又叫：薏苡仁、苡仁、六谷子，为禾本科植物薏苡的种仁，炒薏米为炒熟的薏米。
荆芥：别名：香荆荠、线荠、四棱杆蒿、假苏，是唇形科、荆芥属多年生植物。入药用其干燥茎叶和花穗。
防风：伞形科植物防风的根。
红曲：是以籼米为原料，采用现代生物工程技术分离出优质的红曲霉菌(monascuspurpureus)经液体深层发酵精制而成，是一种纯天然、安全性高、有益于人体健康的食品添加剂
三棱：为黑三棱科植物黑三棱Sparganium stoloni erum,Buch.–Ham.的干燥块茎。
莪术；为姜科植物蓬莪术Curcuma phaeocaulis Val.、广西莪术CurcumaKwangsiensis S.G.Lee et C.F.Liang或温郁金Curcuma wenyujin Y.H.Chen et C.Ling的干燥根茎
泽泻：Alisma plantago-aquatica Linn.多年生水生或沼生草本。
泽兰：是唇形科植物地瓜儿苗或毛叶地瓜儿苗的干燥地上部。
郁金：为姜科植物温郁金Curcuma rcenyujin Y，H.Chenet C.Ling、姜黄CurcumalongaL.、广西莪术Curcumakwangsiensis S.G.Lee et C.F.Liang或蓬莪术Curcuma phaeocaulis Val.的干燥块根。
巴戟天：学名：Morinda officinalis How，为双子叶植物茜草科巴戟天的干燥根。
淫羊藿：学名：Epimedium brevicornu Maxim.多年生草本植物全草。
葛根：为豆科植物野葛的干燥根
藿香：属唇形目，唇形科多年生草本植物，又名：合香、苍告、山茴香全草。
佩兰：拉丁学名Eupatorium aponic Turcz.为菊科植物佩兰(兰草)，以全草入药。
沙参：拉丁学名：Adenophora stricta Miq.〕桔梗科沙参属多年生草本植物，根部入药。
麦冬：麦冬(学名：Ophiopogon aponicas(Linn.F.)Ker-Gawl.)百合科沿阶草属多年生常绿草本植物的块根。
柿蒂：为柿树科植物柿Diospyros kaki Thunb.的干燥宿萼
茯苓：为多孔菌科真菌茯苓Poria cocos(Schw.)Wolf的干燥菌核。
茯神：为白茯苓中心抱有细松根者。
辛夷：木兰花的花蕾。
苍耳子：属菊科、苍耳属一年生草本菊科植物苍耳的带总苞的果实。
旋复花：菊科旋复花，又名：金沸草、金钱花、滴滴金、盗瘐、夏菊、戴椹，干燥花朵入药。
代赭石：氧化物类矿物赤铁矿的矿石。
地榆：拉丁学名：Sanguisorba officinalis L.是蔷薇科地榆属多年生草本植物的根部。
槐角：豆科植物槐(Sophora japonica L.)的干燥花及花蕾、成熟果实。
延胡索；延胡索(Corydalis yanhusuo W.T.Wang ex Z.Y.Su et C.Y.Wu)，又名：延胡、玄胡索、元胡索、元胡等。是罂粟科、紫堇属多年生草本植物的块茎。
红花：红花，(拉丁学名：Carthamus tinctorius L.)，别名：红蓝花、刺红花，菊科、红花属植物，干燥的管状花入药。
土茯苓：为百合科植物光叶菝葜的干燥根茎
紫苑：学名：Aster tataricus L.f.，别名：青苑、紫倩、小辫等；菊科紫菀属，多年生草本的根状茎。
款冬：系菊科款冬属植物款冬(Tussilago farfara L.)的花蕾。
麻黄：麻黄科植物草麻黄、中麻黄或木贼麻黄的干燥草质茎
射干；为Belamcanda chinensis(L.)Redouté多年生草本的根状茎。
红参；是参的熟用品，其加工方法是经过浸润、清洗、分选、蒸制、晾晒、烘干等工序加工而成。
具体实施方式
以下通过具体实施方式说明本发明的中药组合物及其制品的制备方法和临床功效。
实施例1-89归一饮的配方及制备
表1实施例1-89的归一饮的配方
汤剂制备步骤如下：
(1)按表1所列的各配方准备原料药
(2)每剂药每次300-800毫升，文火，对制附子先煎煮至少30分钟；
(3)加入其余两种药材煎煮提取；
(4)再加水1-2次，合并煎煮液得口服归一饮汤剂。
注：本发明中提到一剂药时，以实施例1为例，其一剂药相当于是以“炙甘草20g、干姜15g、制附子10g”为原料药制备得到的口服制品。其余实施例以此类推。
本发明的配方中，干姜可用炮姜替代。
临床实验例：
记录实施例1-89的药物进行临床用药观察；
统计结果显示，实施例1-16的中药组合物对诸多病症具有显著的疗效，临床观察统计如下表2：

表2中1-16配方中各味药的重量份数值范围之间各个数值点在门诊治疗的处方中都有使用，例如炙甘草2-6重量份，表示2重量份，2.1重量份，2.2重量份，2.3重量份，2.4重量份，2.5重量份，2.6重量份，2.7重量份…5.5重量份，5.6重量份，5.7重量份,5.8重量份,5.9重量份，6重量份都使用过；
干姜1-1.5重量份表示1重量份、1.1重量份，1.2重量份，1.3重量份，1.4重量份1.5重量份都开过处方；
其余原料的数值范围也如此，在若干门诊治疗过程中，根据症状的程度不同，采用过该范围内的不同数值。
在发明人的进一步实践中，发现在实施例1-16的多功效中药组合物基础上，针对具体不同的症状选择性添加不同的辅助性原料药，药效更为显著和具有针对性，部分临床观察病例治愈统计如下表3：

综上，本发明提供的多功效中药组合物本身，脱胎于四逆汤，但适应症比四逆汤更广泛，药效温和安全，是属于除了严重阴虚的患者以及单纯实热证患者之外所有人都可以服用的具有保健功效的中药组合物；另一方面，在该多功效中药组合物基础上，添加适量的对应症状的原料药，可以得到疗效显著的针对不同病症的针对性药物组方。

Claims (10)

1.一种对多种症状具有功效的中药组合物，其特征在于,由2-6重量份的原料药A，0-1.5重量份的原料药B和1重量份的原料药C组成；
所述原料药A为炙甘草、生甘草、大枣和/或薏米；
所述原料药B为干姜或炮姜；
所述原料药C为制附子；
所述多种症状包括冠心病、高脂血症、心律失常、心力衰竭、高血压病、Ⅱ型糖尿病、心脏神经官能症、皮肤病、月经病、疼痛性疾病、便秘、失眠、颈椎、腰椎病、感冒、慢性支气管炎、人乳头瘤病毒感染、甲亢、神经性震颤、遗精、阳痿、小儿多动症、抑郁症、更年期综合征、慢性结肠炎、慢性胃炎、反流性食道炎、肿瘤康复、周围神经病、慢性泌尿性感染、过敏性疾病、带状疱疹、脑血管病、血液病。
2.根据权利要求1所述的中药组合物，其特征在于，由2重量份的原料药A，1-1.5重量份的原料药B和1重量份的原料药C。
3.根据权利要求1所述的中药组合物，其特征在于，由3-6重量份的原料药A，1-1.5重量份的原料药B和1重量份的原料药C。
4.一种中药组合物，是在权利要求1-3任一所述的中药组合物中添加以下任一组辅助原料药而得：
荆芥和防风；
红曲；
三棱和莪术；
泽泻和泽兰；
郁金；
巴戟天；
枸杞子和淫羊藿；
巴戟天和淫羊藿；
葛根；
藿香和佩兰；
沙参和麦冬；
黄芪；
白术；

肉苁蓉；
杜仲和牛膝；
丁香和柿蒂；

辛夷和苍耳子；

6.一种对多种症状具有功效的中药制品，其特征在于，由权利要求1-3任一所述的对多种症状具有功效的中药组合物或权利要求4或5所述的中药组合物为原料制成；
所述多种症状包括冠心病、高脂血症、心律失常、心力衰竭、高血压病、Ⅱ型糖尿病、心脏神经官能症、皮肤病、月经病、疼痛性疾病、便秘、失眠、颈椎、腰椎病、感冒、慢性支气管炎、人乳头瘤病毒感染、甲亢、神经性震颤、遗精、阳痿、小儿多动症、抑郁症、更年期综合征、慢性结肠炎、慢性胃炎、反流性食道炎、肿瘤康复、周围神经病、慢性泌尿性感染、过敏性疾病、带状疱疹、脑血管病、血液病。
7.根据权利要求6所述的中药制品，其特征在于，剂型为口服汤剂、散剂、颗粒剂、丸剂、片剂、胶囊、提取液、外用剂型。
8.根据权利要求6或7所述的中药制品，其特征在于，还包含用于制备所需剂型的药学上可接受的辅料。
9.权利要求6-8任一所述的中药制品的制备方法，其特征在于，包括对所述原料药进行煎煮制得中药水提取液。
10.根据权利要求9所述的制备方法，其特征在于，包括对所述制附子先煎煮至少15分钟，然后再加入其余原料药进行煎煮。
