Description
技术领域
本发明涉及用于帮助人体二尖瓣功能修复和/或重建的医疗装置及方法。更具体来说，本发明涉及一种通过替换原生心脏瓣膜功能以用于治疗二尖瓣返流的医疗装置。
背景技术
人的心脏有四个腔室，四个瓣膜。心脏瓣膜控制血液流动的方向。全功能的心脏瓣膜确保心动周期中正常的血液循环得以维持。当心脏瓣膜的瓣叶因疾病不能充分闭合(吻合)时会引起心脏瓣膜返流或泄露，这些疾病包括，先天性瓣膜闭合不全，腱索撕裂，腱索拉伸，左心室扩大，乳头肌破损，以及由感染、退行性变化、瓣叶钙化、瓣环拉伸或乳头肌间距变大等引起的瓣膜结构性损害。不管何种原因，瓣膜返流会干扰心脏功能，因为它允许血液在错误方向上的回流通过瓣膜。依据返流程度的不同，这种疾病不仅会影响心脏功能甚至会引起心脏的几何形状改变。除此之外，心脏几何形状异常也是返流的原因，并且这两个过程可“共同”加速心脏功能异常化。心脏返流带来的直接后果是正向心脏输出量的减少。依据泄漏的严重性，心脏输送充足的血液流进身体的其他部分的功效将被破坏。
二尖瓣是双瓣(两个瓣叶)式瓣膜，它位于左心房(LA)和左心室(LV)之间。在心脏舒张时，由于血液填充(预加载)，左心房压力增强，正常功能的二尖瓣此时打开。随着心房压力增加并超过左心室压力，二尖瓣打开，帮助被动血流进入左心室。随着心房收缩舒张结束，使得残余血液从左心房流入左心室。心房收缩结束后二尖瓣关闭，以防止血流逆转从左心室回流至左心房。人类二尖瓣的开口面积通常是4到6平方厘米。有两个瓣叶，前叶和后叶，共同覆盖二尖瓣的开口。二尖瓣的开口由一种纤维环包围，这个环称为二尖瓣环。两个瓣叶圆周地附接至二尖瓣环，并且在心动周期过程中像环形铰链一样打开和关闭。在一个具有正常功能的二尖瓣中，瓣叶被腱索连接到左心室乳头肌上。当左心室收缩时，心室内压力迫使二尖瓣关闭，腱索保证两个瓣叶吻合(以防止两个瓣叶脱垂进入左心房，引起二尖瓣关闭不全)，并防止瓣膜开合方向错误(从而防止血液回流到左心房)。
目前，标准的心脏瓣膜关闭不全的治疗方法包括外科手术修复/治疗和血管内夹合手术。标准外科手术修复或更换过程需要开胸手术，并需要体外循环和心脏停跳。外科手术是侵入性的，死亡、中风、大出血、呼吸困难、肾脏以及其他并发症的风险很高，使得许多患者无法接受这种手术治疗。
近年来，部分器械厂商开发了血管内夹合技术。在这种方法中，由生物相容性材料制成的夹子被插入到两个瓣叶之间的心脏瓣膜，通过夹住两个瓣叶的中间部分(主要是A2和P2瓣叶)，以防止瓣叶的脱垂。然而，这种技术的不足之处也很明显，如操作过程中定位难度较大，植入不当时拆除难度大，心脏瓣膜返流的再次复发，在一个手术中需要植入多个夹子以及严格的病例选择等。
综上，存在着对开发新型医疗装置以治疗二尖瓣返流的巨大需求。现有的医疗器械并不能够完全满足临床需求。本发明的目的是给医生提供一种新型装置及方法，它可避免创伤性外科手术，是一种可以通过基于导管的，治疗二尖瓣返流的低侵入性的装置和手术方法。
发明内容
本发明的一个目的是提供一种能够有效地固定在人的二尖瓣环的位置，同时无需使用倒刺或钩等刺穿天然组织的二尖瓣置换装置。
本发明的另一目的是提供一种在人类二尖瓣置换手术中，在装置完全展开前可以在瓣环处调整该装置位置的方法。
本发明的另一目的是提供一种新颖的瓣叶结构，可提供更有效的瓣膜和血流控制。

具体实施方式
下面的详细描述是本发明当前的最佳实施方式。该描述不应被视为具有限制意义，其仅仅是为了说明本发明的实施例的一般原理。本发明的范围由所附的权利要求限定。
本技术涉及一种治疗二尖瓣返流的医疗装置，并包括在人体心脏内进行定位和锚定该装置的方法。该装置包含一个心房端部分和心室端部分。装置的心房端部分紧密放置在二尖瓣环区域并起到封合作用，以防止装置周围的区域产生泄露(血液从左心室回流至左心房)。该装置的心室端部分包含瓣膜主体和锚定特征。锚定特征可部分或完全由织物或组织覆盖，达到封合效果，防止泄漏。瓣膜主体包含组织瓣叶和瓣叶支撑结构。在正常心动周期，通过瓣膜和瓣叶打开和关闭来调节左心房和左心室之间的血液流动的方向和量。锚定特征的功能是维持装置在体内的适当位置，以防止在心动周期时可能的位置迁移。锚定特征通过与原生瓣叶，和/或瓣环，和/或其他瓣下结构间的相互作用提供了锚定效果。锚定特征的一种设计是采用生物相容性的胶黏剂/胶水将装置与原生瓣膜和/或心脏结构结合以保持瓣膜假体的位置。
在使用中，该装置将使用经皮导管的方法来进入二尖瓣区域，并且与内部瓣膜结构和瓣下结构相互作用，以恢复二尖瓣的功能。此外，该装置可通过外科手术或其他微创手术植入。该装置可以植入至人体的心脏或脉管系统管腔内，从而改善，替换，和/或重建原生二尖瓣及瓣叶的功能。
本发明还涉及一种锚定特征(夹子设计)，其利用原生瓣叶和/或瓣环，和/或其它瓣下结构，以提供一种锚定效果，从而在心动周期中保持该装置稳定在适当位置上。一旦该装置被放置在二尖瓣位置时，装置上的锚定特征通过与原生瓣叶，和/或瓣环，和/或其它瓣下结构的接合/相互作用，防止装置在心动周期中发生迁移。该装置的心房端部分也可以通过与瓣环和心脏的心房部分相互作用起到额外的锚定作用，装置的心房端部分与瓣环上心脏心房部分可以紧密接触。
本发明还提供了一种新颖的瓣叶设计。瓣叶结构可包含1到6片瓣叶，瓣叶可以一起缝合在瓣叶支撑结构内形成伞状轮廓，在心动周期中可以张开和闭合以调节血流。在心脏收缩期(心脏收缩)，伞状瓣叶可以张开到更大轮廓以关闭二尖瓣，所以不存在血液从左心室回流到左心房的问题。在心脏舒张期(心脏舒张)，伞状瓣叶可以关闭到一个较小的轮廓以打开二尖瓣，使血液能从左心房流入到左心室。这种新颖的瓣叶设计的优点包括：(i)在心脏周期期间瓣叶不会轴向压缩/挤压支撑结构，从而提高了支撑结构的疲劳强度；(ii)瓣叶结合更紧密，支撑结构上的瓣叶和瓣裙紧密结合，最大限度地减少潜在的瓣膜心央处泄漏，(iii)瓣叶中心部位没有“自由边端”，瓣叶间没有吻合作用。这个特点非常重要，因为瓣膜支撑结构的任何形变或变形将会引起小幅度的瓣心漏问题。
本发明的二尖瓣置换装置可以被压缩至低轮廓并装载到输送系统中，然后通过非侵入性的医疗方法，如通过经心尖使用输送导管输送到目标位置，或经股动脉或经房间隔方式输送。一旦到达目标植入部位，二尖瓣置换装置可从输送系统中释放，并可以通过球囊扩张(用于球囊扩张的支撑结构)，或装置弹性释放(用于自扩张的支撑结构)扩张到其正常(扩张后)尺寸大小。
在本发明的二尖瓣置换装置的瓣叶可由处理过的动物的组织/心包制成，或由生物相容的薄壁金属材料(如不锈钢，钴-铬合金，镍钛合金，钽和钛等)制成，或由生物相容的聚合物材料(如聚异戊二烯，聚丁二烯和它们的共聚物，氯丁橡胶和丁腈橡胶，聚氨酯弹性体，硅橡胶，氟弹性体和氟硅橡胶，聚酯，和聚四氟乙烯等)制成。瓣叶还可以设置有药物或生物制剂涂层以提高性能，如防止血栓的形成，并促进内皮化覆盖。二尖瓣装置上的瓣叶可同时经表层处理或涂层处理，以防止钙化。如果需要的话，在瓣膜主体和瓣叶的支撑结构上也可以覆盖织物和组织的组合层。例如，上部分可以覆盖织物层，而下部可以覆盖组织层，反之亦然。装置的心房端部分和瓣叶支撑结构主体也可部分或全部覆盖织物或组织，以改进密封效果和愈合效果。装置上的锚定部分可以部分或完全地覆盖织物，或组织，以促进组织的生长，防止瓣周漏(PVL)，并减少对周围心脏内部结构的潜在破坏。
瓣叶可通过机械交织，缝线缝制，化学，物理或粘接方法被集成到瓣叶支撑结构中。瓣叶也可以由支撑结构的元件形成。例如，瓣叶支撑结构和瓣叶可直接由聚合物或金属材料直接模制或一起形成。瓣叶也可以通过气相沉积法，溅射法，回流法，浸渍法，浇铸，挤出工艺或其他用于将两种或多种材料相连的方式制成。
组织瓣叶也可以进行药物或其他生物剂涂层处理，以防止在心脏形成血块。抗钙化材料也可涂布或放置在表面上，以防止瓣叶钙化。
图1-5展示了根据本发明的二尖瓣装置20的第一实施例。该装置20具有心房凸缘22，瓣环支撑24，颈部26将心房凸缘22连接到瓣环支撑24，以及作为瓣叶支撑结构的瓣膜主体28。各个部分由支架杆形成，所述支架杆限定出多个蜂窝结构(如图所示，或者也可以称之为网眼结构)，从而构成了一个蜂窝式矩阵结构。
心房凸缘22，瓣环支撑24和瓣膜主体28可由镍钛超弹性材料或不锈钢，钴铬合金，钛及其合金，以及其他可球囊扩张的生物相容材料制成。其他聚合物类的生物相容性材料也可用于制造装置20的这些组件，在使用中，装置20可以折叠或者压缩至输送系统中，并通过导管输送至二尖瓣的位置(例如，经股动脉或经心尖输送)。一旦到达二尖瓣位置时，装置20可以从输送系统释放，并定位在二尖瓣瓣环区。心房凸缘22可放置在原生二尖瓣瓣环处或之上，心房凸缘22的一部分延伸至左心房内部。参见图5B。心房凸缘22表面积可以等于或大于二尖瓣环面积。在使用中，心房凸缘22可以覆盖有生物相容性聚合物织物材料，组织或其他生物相容的材料，以提供围绕装置20的密封效果，并促进组织生长和加快愈合。
瓣环支撑24起到锚定作用，并且可以与瓣环，原生瓣叶，以及其他内部心脏结构或瓣下结构相互作用，以提供所需的锚定效果。参见图5B。除了由瓣环支撑24提供的锚定作用外，由心房凸缘22和瓣环支撑24产生的“夹合效应”还可以帮助装置20能自对准并在心动周期中抵抗潜在的迁移。装置20从输送系统释放时，装置20的部件将按顺序从输送系统中释放出来。例如，在经心尖输送时，心房凸缘22将从输送系统中首先释放，然后释放瓣环支撑24。相反地，经股动脉(或经房间隔)输送时，瓣环支撑24将首先释放出来，然后释放心房凸缘22。上述手术过程可在X-射线和/或经食道超声(TEE)，心脏内超声(ICE)等的指导下进行。
图4和图5A展示了装置20的每个组件的典型尺寸或几何形状范围。心房凸缘22可以是圆形的轮廓或非正圆形轮廓。若心房凸缘22为圆形轮廓，所述心房端部分的直径可以在20mm至70mm的范围内。如果心房凸缘22为非正圆形轮廓，长轴可在20mm至70mm的范围内，短轴可以在15mm至65mm范围内。此外，心房凸缘22的高度H1可以在0.5mm至20mm的范围内。在心房凸缘22的上心房端，形成心房凸缘22的每一个蜂窝结构有峰部和谷部，并在每一个峰部处具有圆形的非创伤性头端34。心房凸缘22相对于瓣膜主体28的轴线的角θ1可以在0到150度的范围内。心房凸缘22可以完全或部分地覆盖织物，或组织材料，或者组织和织物材料的组合。
瓣膜主体28的高度H3在2mm至30mm的范围内。瓣膜主体28的靠近心房侧的端部可以更高并高出心房凸缘22的上方，以减少瓣膜主体28在心室内部的长度。瓣膜主体28的横截面轮廓可以是一个完整的圆形形状或是一个非正圆形状。当瓣膜主体28为一个完整的圆形轮廓时，其直径可以在15mm至50mm的范围内。当瓣膜主体28为非全圆周轮廓时，其长轴线可在15mm至50mm范围内，短轴可以在10mm至45mm范围内。瓣膜主体28还可以具有沿其高度可变的轮廓。例如，瓣膜主体28的心房凸缘22附近的部分可以具有椭圆形的轮廓或其他非正圆形的形状，而在瓣膜主体28的远离心房凸缘22部分可以具有正圆形轮廓。组织瓣叶可以完全集成到瓣膜主体28的圆形部分，或同时包围圆形和非圆形部分。瓣膜主体28可以完全或部分地覆盖有织物，或组织材料，或者组织和织物材料的组合。例如，瓣膜主体28的上部可以覆盖织物，并在瓣膜主体28的下部可以覆盖组织，或反之亦然。在使用中，织物材料和组织可以先被缝/连接在一起，或单独地缝合/连接到瓣膜主体28上。瓣膜主体28可沿一个表面(即，内表面或外表面)或沿着两侧(即，内表面或外表面)被覆盖。在瓣膜主体28底部，形成瓣膜主体28的每一个蜂窝结构具有峰部和谷部，并在其底端处具有圆形的非创伤性头端38。
一个可选的较小直径的颈部26作为心房凸缘22至瓣环支撑24的过渡部分。当装置20处于展开状态时，颈部26从心房凸缘22径向向内延伸，形成一个U形的颈部26。颈部26的横截面轮廓可以是一个完整的圆形形状或非正圆形。当颈部26具有正圆形轮廓，其直径可以在15mm至50mm的范围内。当颈部26具有非正圆形轮廓时，长轴可以在15mm至50mm的范围内，短轴可以在10mm至45mm的范围。
颈部26然后径向向外延伸至瓣环支撑24，它包括U形节段29的环，所述U形节段29与间隔的倒V形突片30的环相交替。颈部26实际上径向向外过渡至U形节段29的环，其先径向向外延伸之后再径向向内延伸至瓣膜主体28。倒V形突片30自瓣膜主体28径向向外延伸，并具有大体垂直向上的弯曲以形成环绕颈部26的环。倒V形突片30的数量范围从1到20。倒V形突片30组成的环的横截面轮廓可以是一个完整的圆形形状或非正圆形。当为正圆形时，其直径范围为15mm至70mm。当其为非正圆形时，其长轴线可在自15mm至70mm的范围内，短轴可以在10mm至65mm的范围内。倒V形突片30的连接点是延展点32，其功能是接触或压靠心脏的二尖瓣区域的瓣环或原生瓣叶，以确保装置20在瓣环处稳固放置，从而可以作为锚定特征。每个突片30的高度H4的范围从0.5mm到10mm。突片30可以全部或部分地被组织或织物材料覆盖。例如，延展点32可不被覆盖，而瓣环支撑24的其余部分可以被覆盖组织或织物材料。使用织物除了提供额外的密封效果以防止瓣周漏(PVL)之外，还可促进组织生长，并使装置20在瓣环处固定更牢固。
因此，U形节段29的环的直径大于所述瓣膜主体28和颈部26的直径，但小于心房凸缘22的直径。同样地，突片30的环的直径大于瓣膜主体28和颈部26的直径，但小于心房凸缘22的直径。突片30和U形节段29可以布置为彼此交替以形成共同的环，并且环的直径也相当。
两个U形尾端36可以在瓣膜主体28的端部处从瓣膜主体28延伸。即使图中展示了两个尾端36的设计，但在装置20中也可以仅设计有一个尾端36，或三个或更多个尾端36。如图5A最佳实施例所示，每个尾端36可以通过从瓣膜主体28的两个下尖端38延伸以在U形底端连接而形成，其用来允许缝合线或其它线弦被连接于此，以使缝合线或线弦可以用来在装置20释放过程中调整其位置。尾端36也可以帮助实现输送系统的一些功能，如(1)瓣膜装载(将瓣膜装入输送系统保护鞘)；及(2)在心脏瓣环处帮助瓣膜定位。关于瓣膜定位，在装置20展开期间，在其从输送系统被最终释放之前，尾端36可以帮助调节装置20的位置和/或角度。尾端36的另一优点是在装置20被完全展开和从输送系统释放前，装置20已经可以正常地开始运行(部分地或完全地)，所以这给医生更多的时间来调整位置，并在装置20最终与输送系统断开之间可以暂停它。每个尾端36的长度范围可以为5mm至25mm。尾端可以是预成形并被弯曲成特定形状，从而在需要的情况下，尾端36可以延伸为不同于瓣膜主体28限定的圆形轮廓。例如，尾端36可以从瓣膜主体28的末端悬挂，并且朝着瓣膜主体28的内腔向内弯曲。
如图5B所示，当装置20在应用于原生二尖瓣位置的情况下被展开时，突片30展开并“坐”在瓣环处或瓣环上方，心房凸缘22的一部分延伸至左心房内部。心房凸缘22(从上面)和突片30(从下面)的相互作用提供了一个“夹合效应”可以有效的固定装置20在理想的位置上。当装置20展开时，U形节段29可刚好定位在瓣环下方，以提供一个额外的封合效果。心房内的支架杆/蜂窝空间可完全或部分地，或完全不被纤维和/或组织材料覆盖。通过使心房凸缘22在心房端内局部或完全不覆盖设计，可最小化干扰正向血流。该装置20的组织瓣叶48结合到瓣膜主体28上，以取代原生二尖瓣瓣叶的功能。如图7A和7B所示，原生瓣叶被定位在瓣膜主体28外表面处，或者相邻于瓣膜主体28。
图6A-6B展示了装置20的示例性的激光切割结构。该装置20可以是由金属或聚合物管材激光切割，以到达图6A中所示的结构。该切割结构将经过形状处理，微喷砂处理，和电抛光工艺，以达到如图1-5B所示的所需的轮廓或形状。每个支架杆50的宽度可在0.2mm至1.5mm范围内，且每个支架杆50的厚度范围可以从0.2mm至0.75mm。每个蜂窝结构的长度可以是在从2mm至20mm的范围内。沿着装置20的长度方向环的数量范围可以从2到20。沿着装置20的圆周蜂窝结构的数量范围可以从2到20。可选择地，该装置20也可由平坦的片材制造，然后轧制成所需的形状。该装置20可以使用多种输送方法来输送。例如，可以使用经心尖输送的方法，心房凸缘22可以首先展开并为输送过程提供触觉反馈。瓣环支撑24(即突片30)将被随后释放并展开，最后完成装置20的植入。在经股动脉/房间隔输送过程中，突片30可以先展开并为输送过程提供触觉反馈。心房凸缘22将被随后释放并展开，最后完成装置20的植入。在输送过程中，突片30可朝向瓣膜主体28向内弯曲(向上和向内，或向下和向内)。当突片30被释放/展开后，其可以张开并压在瓣环或原生瓣叶上。
在使用中，装置20可以被压缩成便于输送的较小轮廓，并在达到目标植入位点后可以释放和展开。压缩后的装置外径可以小于48Fr，在所述的临床应用中典型的范围为15至40Fr。
图7A展示了组装后的二尖瓣置换装置顶视图，其包括与上述的二尖瓣装置20相结合的瓣叶48。图7B展示了图7A的组件的底视图。图7A和7B展示了处于打开位置的瓣叶48，故自左心室至左心房的血液回流已被阻止。吻合区域为瓣叶48和在瓣膜主体28上沿装置20内腔的瓣裙部分。类似地，图8A和8B分别为图7A和7B中组装后二尖瓣装置的顶视图和底视图，展示了处于闭合位置的瓣叶48，故血液能从左心房流入到左心室。如图7A-8B及图9A-11中展示的最佳示例，本发明还提供了一种新颖的瓣叶结构。瓣叶48的的结构实质上与原生瓣叶结构恰恰相反，瓣叶48向内打开并通过向外张开接触瓣膜主体28的内表面实现闭合。瓣叶48以允许瓣叶可自由向内闭合的方式附接装置20，从而允许正向血流通过瓣膜装置。组织瓣叶的高度(深度)取决于心脏的解剖结构，可以从2-30mm不等。
图7A-11展示了瓣叶结构的最佳实施例，并且该实施例展示了使用三个瓣叶48A，48B和48C沿三个纵向缝合线72，74和76缝合后的整体结构。缝合线72，74，76沿瓣叶48A，48B和48C外边缘延伸，并在瓣叶结构与瓣膜主体28的连接处形成汇合线。瓣叶48A，48B和48C沿其边缘缝到构成瓣膜主体28的支架杆50上。在瓣叶48A，48B和48C顶端(心房端)，径向缝合线82，84和86自纵向缝合线72，74和76朝着中心点88分别延伸，在中心点88处设置有平端部60。三个内缝合线52，54和56从顶点58开始缝合(在顶点58处瓣叶48A，48B，48C的心房边缘压握在一起以形成中央平端部60)，从而使得缝合线52，54，56的起始点偏离为缝合线82，84，86。每个缝合线52，54，56分别向缝合线72，74和76延伸较短距离，并分别与缝合线72，74和76合并。这样的缝合线设置使三个瓣叶48A，48B和48C可形成用于其瓣膜结构的伞状构造，瓣叶48A，48B，48C在心室压力下不会张开至顶部(心房)缝合线82，84，86，以防止瓣叶翻转过来，从而尽可能地减少泄漏。由瓣叶48A，48B，48C和顶部(心房)的缝合线82，84，86限定的圆拱形顶板之间的距离在0.25mm和10mm范围内。装置20内的瓣叶结构可以使用多个瓣叶48制成(如上所述和如图展示)或单瓣叶48。在仅含单一瓣叶的情况下，单瓣叶48可以折叠并缝合到图7A-11所示的形状上。
因此，本发明的新颖的瓣叶设计采用反向瓣叶动作来调节左心房和左心室之间的血液流动。这种反向或“伞状”或“球囊状”瓣叶的设计提供了更好的密封/吻合效果，并且通过消除作用在使用传统瓣叶设计的瓣膜主体28上的收缩/压缩力或形变而提高了瓣膜主体28的疲劳性能。
图12-14展示了根据本发明的二尖瓣装置20的第二实施例，它包括夹子80的设计。图12-14中的装置20也有心房凸缘22，瓣环支撑24，连接心房凸缘22与瓣环支撑24的颈部26，以及作为瓣叶的支撑结构的瓣膜主体28，所有这些部件都可使用图1-5中的相同元件。每个组件亦由支架杆形成，且支架杆限定出蜂窝结构(如图所示，或者也可以称之为网眼结构)从而构成蜂窝式矩阵。
两个实施例之间的区别是增加了夹子80的环，夹子80在瓣环支撑24垂直下方间隔的位置围绕瓣膜主体28以相间隔的方式布置。这些夹子80基本呈L形，每一夹子80可从瓣膜主体28的任一支架杆垂直伸出，然后在径向方向上水平伸出，之后在尖端处形成短弯。如图15所示，在装置20展开后，夹子80的作用是夹合或固定原生瓣叶的一部分。这个夹合功能提高了装置20在二尖瓣瓣环区域的牢固性。同样心房凸缘22和突片30的夹合作用亦会起到效果，但夹子80可以提供更好的牢固性。高度H2为颈部26和瓣环支撑体24的合并高度，并且也可以改变，以适应夹子80，范围可以从0到10mm。
一旦装置20被植入，心房凸缘22，瓣膜主体28及装置自身的锚定机制(心房凸缘和突片30的夹合效果，夹子80的额外效果)，或装置20与原生瓣叶或其他内部心脏结构(或其它瓣下结构)产生的相互作用将保持装置20在理想的位置上。在心室收缩期中，当由瓣叶48和瓣膜主体28组成的瓣膜是关闭状态时，自左心室的压力会产生一个向上作用力，试图将装置20向上推向心房。这就是为什么一个可靠的和充分的锚定机制是必要的，其可以保证装置20中在植入后能够保持在展开位置的牢固性。例如，心脏收缩期间，组织瓣叶48将关闭，血液会沿主动脉瓣泵送至主动脉中。与此同时，原生瓣叶沿瓣膜主体28外表面(环绕的)向上移动(向内)，并将密封/关闭二尖瓣防止瓣周漏发生。装置20具有的锚固特征可以接合原生瓣叶和其他内部心脏结构，以防止装置20被向上推。在心脏舒张时，装置20上的组织瓣叶48会变成一个较小的轮廓，让血液流过，并充盈左心室。组织瓣叶48可通过血液流动，心脏压力，以及心动周期中支撑结构的心动脉冲运动等综合效果来运行(打开或闭合)。
除了锚定机制(瓣环支撑24和突片30)的锚定效应，心室收缩期中原生瓣叶施加到瓣膜主体28上的压力通过在装置20上施加夹紧力效果也可以帮助防止装置20向上移动到心房内。这是一种动态锚定机制，并仅在心室收缩期起到效果，在此阶段，装置20在承受的最高抬升力的作用下试图将装置20推向心房方向。这个附加的动态锚定效有助于保持装置20的合适位置，并降低锚固力及其作用在原生心脏解剖结构上的持续时间。随着时间的推移，组织生长/修复将连接/融合原生瓣叶到瓣膜主体28上。
本发明的装置20的主要优势/新颖性包括以下内容：
1.原生瓣叶和其他内部瓣膜和瓣下结构将被保留；

7.装置20可以在植入后可依据左心室尺寸/体积变化自动调整其大小/形状；
8.该装置20可以具有可变的轮廓，以帮助在心室收缩时形成密封效果，并且还通过与原生瓣叶的相互作用提供了夹合效果，以帮助保持在心室收缩期装置位置的牢固性；例如，装置20在心房凸缘22和瓣膜主体28间可以具有一个“过渡区”。“过渡区”的轮廓可以不同于装置20的其他部分。一个示例为，“过渡区”可以为一个椭圆形形状，而不是一个正圆形轮廓。过渡区可以具有一个长轴和一个短轴，长轴可以沿着“接合处对接合处”的方向，而另一个轴比长轴短。过渡区的椭圆形轮廓可以帮助建立在接合区更优良的密封效果。过渡区也可包括颈部26，以增加“密封”的表面积。这同时也为装置20提供了一个动态的锚定效应，它在心室收缩时起到作用，在此阶段最高举力作用于装置20上；
9.装置20具有使用原生瓣叶和腱索，以提供密封效果和锚定效应的设计；
10.装置20可外科手术植入或通过微创手术，如经心尖，经房间隔和经股动脉的微创手术；
11.在瓣膜主体28的尾端36让医生在释放过程中有足够的时间来调整瓣膜位置/角度以达到理想的瓣膜性能；并且
12.输送系统的内轴可以设计和制造为：在瓣膜释放展开的过程中，内轴可移动。例如，在输送过程中，一旦装置20的大部分从输送系统释放/展开后，装置20上的瓣48将开始随心脏跳动开始运行。此时，输送系统的内轴仍然可以在装置20的腔内，并可能影响装置20上的一个瓣叶的运动，在这种情况下，在输送系统内轴可以从递送系统的近端手柄端松开并向近侧拉回，这样内轴将撤出装置的内腔。因此，所有装置20上的瓣叶48均可以自由运动。这意味着当尾端36仍与输送系统相连是，装置20可实现更好的瓣膜功能。换句话说，装置20上瓣叶48在释放展开过程中即可发挥作用，这会给医生带来更多的时间来调整瓣膜位置/角度以获得最佳性能。
除了上述的固定机制，胶黏剂/界面也可用于固定装置20中在原生二尖瓣处的位置。一个例子是使用生物相容的胶水/粘合剂来连接/固定/保持装置20在二尖瓣的位置。在使用中，生物相容性粘合剂/胶水可以涂覆在装置20的外表面，如沿着瓣膜主体28和心房凸缘22的外表面或装置20的任何与原生二尖瓣结构接触的表面，如瓣环，高于或与瓣环齐平的心房表面，原生瓣叶，心脏肌肉，和其他心脏瓣膜和/或瓣下结构，以保持植入后装置20的位置。生物试剂也可以加入到生物相容的粘合剂/胶水中，以促进愈合和组织的生长。
粘合剂/胶水可以具有快速反应的性质，并可在接触血液后即刻与原生二尖瓣结构形成结合。它也可以通过热或温度激发；热或温度可以通过电解加热，或射频加热，或超声能量，或磁能，或微波能量，或血液温度本身，或化学反应生成/控制，并作用到装置20的部分或整个结构上。
粘合剂/胶水也可以具有缓慢反应的性质，并可在接触血液后一段时间与原生二尖瓣结构形成结合。需要形成结合的时间可以从1秒变化至2小时，或从1秒至28小时等。
粘合剂/胶水还可以具有可控反应的性质，并可在接触血液后以一种可控的方式与原生二尖瓣结构形成结合。这个概念的一个例子是在装置20的粘合剂/胶水层/材料层之上应用其他生物相容性材料的顶部涂层(或多层)。其他生物相容性材料的顶层可以通过使用能量，加热，化学反应，机械处理，或者磁处理以可控方式被除去或溶解，以确保顶部涂层下部的粘合剂/胶水可以有效地与天然二尖瓣结构形成结合。如本文所用，“受控制的方式”是指需要形成结合的时间可以从1秒至48小时。
虽然上面的描述参考了本发明的具体实施例，但可以理解，在不脱离本发明的精神下可以作出许多修改。所附权利要求旨在覆盖这些修改以保证其将落入本发明的真实范围和精神内。
Claims (17)
1.适用于在人体心脏二尖瓣位置展开的二尖瓣置换装置，包括：
心房凸缘，该心房凸缘形成所述装置的心房端；
瓣膜主体，该瓣膜主体形成所述装置的心室端；
瓣环支撑，该瓣环支撑连接所述心房凸缘和所述瓣膜主体，该瓣环支撑包括从其径向延伸的突片的环；
其中，所述心房凸缘的直径大于瓣膜主体的直径，而所述瓣环支撑的直径介于心房凸缘与瓣膜主体的直径之间；
其中，所述瓣环支撑还包括径向向外延伸并与突片交替的U形节段的环。
2.根据权利要求1所述的装置，还包括位于所述瓣膜主体的瓣叶结构。
3.根据权利要求1所述的装置，还包括从所述瓣膜主体的心室端延伸的至少一个尾端。
4.根据权利要求1所述的装置，还包括位于所述心房凸缘和U形节段的环之间的颈部。
5.根据权利要求1所述的装置，其中所述心房凸缘，瓣环支撑和瓣膜主体的一个或多个的至少一部分覆盖有组织。
6.根据权利要求1所述的装置，其中所述心房凸缘，瓣环支撑和瓣膜主体的一个或多个的至少一部分覆盖有织物。
7.根据权利要求1所述的装置，其中所述心房凸缘，瓣环支撑和瓣膜主体的一个或多个的至少一部分覆盖有组织和织物。
8.根据权利要求7所述的装置，其中，覆盖物可以使用组织或人工编织物，或组织和织物的混合的覆盖。
9.根据权利要求1所述的装置，其中所述心房凸缘，瓣环支撑和瓣膜主体的一个或多个的至少一部分涂有生物相容的粘接剂。
10.适用于在人体心脏二尖瓣位置展开的二尖瓣置换装置，包括：
心房凸缘，该心房凸缘形成所述装置的心房端；
瓣膜主体，该瓣膜主体形成所述装置的心室端；
瓣环支撑，该瓣环支撑连接所述心房凸缘和所述瓣膜主体，该瓣环支撑包括从其径向延伸的突片的环；
其中，所述心房凸缘的直径大于瓣膜主体的直径，而所述瓣环支撑的直径介于心房凸缘与瓣膜主体的直径之间；
其中，所述装置还包括从所述瓣环支撑上延伸的至少一个夹子。
11.根据权利要求10所述的装置，还包括位于所述瓣膜主体的瓣叶结构。
12.根据权利要求10所述的装置，还包括从所述瓣膜主体的心室端延伸的至少一个尾端。
13.根据权利要求10所述的装置，其中所述心房凸缘，瓣环支撑和瓣膜主体的一个或多个的至少一部分覆盖有组织。
14.根据权利要求10所述的装置，其中所述心房凸缘，瓣环支撑和瓣膜主体的一个或多个的至少一部分覆盖有织物。
15.根据权利要求10所述的装置，其中所述心房凸缘，瓣环支撑和瓣膜主体的一个或多个的至少一部分覆盖有组织和织物。
