发明领域
本发明涉及一种药物组合物，具体而言涉及用于治疗心血管疾病的药物组合物，其以比索洛尔和坎地沙坦为活性成分。
背景技术
比索洛尔是选择性的β1-肾上腺素能受体阻滞剂，高选择性的β1-受体阻滞剂，无内在拟交感活性和膜稳定作用，通过竞争肾上腺素β1-受体而抑制儿茶酚胺的心血管作用，结果是使心脏收缩力及收缩速度下降，减慢传导及心率，降低心肌耗氧量，提高心肌运动耐力。临床上用于治疗高血压以及心力衰竭型冠心病；用于降低血压时，其目前在临床上的用量为每天5mg，降压效果比较明显，但仍不是很理想，有明显的副作用，表现为：水肿、头痛、致痉咳。用于治疗心力衰竭型冠心病时，其目前在临床上的用量为每天5mg，用于预后治疗，效果不是很理想，有明显的副作用，表现为：水肿、头痛、致痉咳。
坎地沙坦是血管紧张素II受体拮抗剂，是最新研究开发的一类药物，临床上用于治疗高血压以及心绞痛型冠心病；各种用于降低血压时，其在临床上的用量为每天8mg。虽然全无致痉咳的不良作用，头痛及水肿较少，但仍有较为严重其他副作用，表现为背痛、上呼吸道感染和胸闷。用于用于治疗心绞痛型冠心病时，其目前在临床上的用量为每天16mg，治疗效果明显，但也具有副作用，表现为加速了心肌细胞的凋亡，降血压明显，可影响冠状动脉的灌注，引起反射心动过速，临床症状表现为胸闷，头痛，背疼等。
现有技术中尚无将比索洛尔和坎地沙坦两种药物联合使用来治疗心血管疾病的尝试或相关报道，更没有二者联用具有突出治疗效果的相关报道。
发明内容
本发明的目的是提供一种用于治疗心血管疾病的新的药物组合物，以为医患双方提供更好的选择。
本发明人通过大量研究，首次发现比索洛尔和坎地沙坦联合用药使每种成分的治疗有效剂量与单剂量用药相比都大大减少，并具有治疗心血管疾病的优越效果，主要表现为：
1、坎地沙坦降低缓激肽和P物质的积聚，减轻比索洛尔药理作用引起的血管水肿和干咳；
2、比索洛尔作用于机体后，引起机体生理变化，使坎地沙坦的拮抗作用更强，增加血流作用明显；
3、比索洛尔可抵消坎地沙坦引起的心率增加，降低心肌耗氧量，降低坎地沙坦的副作用。
4、二者联合用药增加可治疗的适应症种类，包括高血压和冠心病，尤其是目前可进行药物治疗的各种冠心病，包括心绞痛型冠心病、心肌梗塞型冠心病和心力衰竭型冠心病。
因此，本发明提供一种用于治疗高血压的药物组合物，其含有比索洛尔和坎地沙坦，二者摩尔比例为1∶1～1∶10。
在一个优选的实施方案中，比索洛尔和坎地沙坦的摩尔比例为1∶1～1∶2。
在本发明的组合物中，比索洛尔的日剂量为0.5mg～4mg，坎地沙坦的日剂量为0.8mg～5mg。
在一个优选的实施方案中，本发明的药物组合物用于治疗高血压，比索洛尔的日剂量优选为1.20mg～4mg，最优选为2.5mg；坎地沙坦的日剂量优选为2mg～5mg，最优选为3mg。
在另一个优选的实施方案中，本发明的药物组合物用于治疗冠心病，比索洛尔的日剂量优选为0.5mg～2mg，最优选为1.0mg；坎地沙坦的日剂量优选为2mg～5mg，最优选为4mg。
在另一个实施方案中，本发明的药物组合物制备成口服剂型。
在一个优选实施方案中，本发明药物组合物的口服剂型选自片剂、胶囊、丸剂、粉末、颗粒、溶液、悬液、糖浆、酏剂、茶和咀嚼剂，所述片剂优选为分散片。
在另一个实施方案中，其制备成吸入或鼻内给药剂型。
在又一实施方案中，其制备成注射剂。
本发明的上述药物组合物在治疗心血管疾病，尤其是高血压和冠心病方面具有突出的效果，表现为两种药物的治疗有效剂量更低，疗效更高，治疗谱更宽，并减少与两种药物单独使用时相关的副作用。
具体实施方式
本发明提供一种用于治疗心血管疾病的药物组合物，其含有比索洛尔和坎地沙坦，二者摩尔比例为1∶1～1∶10，优选为1∶1～1∶2。
在本发明的组合物中，比索洛尔的日剂量为0.5mg～4mg，坎地沙坦的日剂量为0.8mg～5mg。
在一个优选的实施方案中，本发明的药物组合物用于治疗高血压，比索洛尔的日剂量优选为1.20mg～4mg，最优选为2.5mg；坎地沙坦的日剂量优选为2mg～5mg，最优选为3mg。

本发明药物组合物中的比索洛尔可以其可药用盐的形式存在。比索洛尔的可药用盐包括使用许多酸所形成的药学上相容的盐，所述的酸包括但不局限于盐酸、氢溴酸、硫酸、硝酸、磷酸、马来酸、富马酸、乙酸、水杨酸、对甲苯磺酸、酒石酸、柠檬酸、甲磺酸、甲酸、琥珀酸、萘-2-磺酸、双羟萘酸、3-羟基-2-萘羧酸和苯磺酸等。比索洛尔的可药用盐也可以是碱盐，包括铵盐；碱金属盐，如钠盐和钾盐；碱土金属盐，如钙盐和镁盐；有机碱的盐，如二环己胺盐、N-甲基-D-葡萄糖胺盐；和氨基酸的盐，例如精氨酸、赖氨酸的盐。盐在水或其他合适溶剂中更易于溶解。
本发明药物组合物中的坎地沙坦可以以其酯的形式存在，优选为目前临床上使用的坎地沙坦酯(即坎地沙坦乙酯)。
可以使用一种或多种可药用载体，以传统方式配制根据本发明所述的药物组合物。本文所用术语“可药用载体”是指适合施用于人类或其他哺乳动物如狗、猫、马、牛、绵羊或山羊的一种或多种相容性固体或液体填料、稀释剂或包封物质。术语“载体”是指与活性成分组合时有利于其应用的天然或合成的有机或无机成分。所述载体应能够与本发明的制剂相互混合，并且二者之间不存在明显影响所期望药物功效或稳定性的相互作用。载体的选择和用量是本领域技术人员公知的，例如可以从Remington’s Pharmaceutical Sciences，Mack Publishing Company，Easton，Pa中找到。
本发明的药物组合物优选配制成口服给药的剂型。例如，可以通过将活性化合物和本领域人员所熟知的可药用载体组合在一起来配制。这种载体使得本发明的药物组合物配制成片剂、胶囊、粉末、颗粒、晶体、溶液、悬液、糖浆、酏剂、茶和咀嚼剂等。适当的载体包括赋形剂，如糖之类的填充物，包括乳糖、蔗糖、甘露糖和/或山梨糖醇；纤维素制剂，如玉米淀粉、小麦淀粉、水稻淀粉、土豆淀粉、明胶、西黄蓍胶、甲基纤维素、羟丙基甲基纤维素、羟甲基纤维素钠和/或聚乙烯吡咯烷酮(PVP)。如果需要，可以加入促分解试剂，如交联的聚乙烯吡咯烷酮、琼脂或藻酸或其盐，如藻酸钠盐。
糖衣丸芯提供有适当的包衣。为了达到该目的，可以使用浓缩的糖溶液，该糖溶液任选地含有阿拉伯胶、滑石粉、聚乙烯吡咯烷酮、卡波姆胶、聚乙二醇和/或二氧化钛；其溶液和适当的有机溶剂或溶剂混合物。可以往片剂或糖衣丸包衣中添加染剂或色素，以便鉴别或分析活性化合物剂量的不同组合。
胶囊包括由明胶制成的承插式胶囊，以及由明胶和塑形剂，如甘油或山梨糖醇组成的软的、密封的胶囊。承插式胶囊所含有的活性成分可以和填充物，如乳糖；粘合物，如淀粉；和/或润滑剂，如滑石粉或硬脂酸镁；任选的稳定剂相混合。在软胶囊中，活性化合物可以溶解或悬浮在适当的液体中，如溶解或悬浮在脂肪油、液体石蜡或液体聚乙二醇。另外还可以加入稳定剂。
片剂可以是以传统方式配制的片剂，优选为分散片，可以包覆适当的包衣以增加口感或延迟吸收。
咀嚼剂例如可以是有效治疗药剂存在于口香糖中的形式。可以在本领域技术人员公知的口香糖制备工艺中引入有效治疗药剂。
至于含水制剂，可以包括螯合剂、缓冲剂、抗氧化剂、等渗剂和防腐剂中的一种或多种。螯合剂包括：乙二胺四乙酸(EDTA)及其衍生物、柠檬酸及其衍生物、烟酰胺及其衍生物和去氧胆酸钠及其衍生物。
缓冲剂包括：柠檬酸、柠檬酸钠、乙酸钠、乙酸、磷酸钠和磷酸、抗坏血酸钠、酒石酸、马来酸、甘氨酸、乳酸钠、乳酸、抗坏血酸、咪唑、重碳酸钠和碳酸、琥珀酸钠和琥珀酸、组氨酸以及苯甲酸钠和苯甲酸，及其组合。
抗氧化剂包括：选自抗坏血酸衍生物、丁羟茴醚、丁羟甲苯、没食子酸烷基酯、焦亚硫酸钠、亚硫酸氢钠、连二亚硫酸钠、巯基乙醇酸钠、甲醛次硫酸氢钠、生育酚及其衍生物、硫代甘油和亚硫酸钠的抗氧化剂。优选的抗氧化剂是硫代甘油。
等渗剂包括：选自氯化钠、甘露醇、乳糖、葡萄糖、甘油和山梨糖醇的等渗剂。
可与本组合物一起使用的防腐剂包括苯甲醇、对羟基苯甲酸酯、硫柳汞、氯丁醇和苯扎氯铵等。通常，防腐剂以至多约2％重量的浓度存在于组合物中。然而，防腐剂的确切浓度将根据预期用途而变化，本领域的技术人员可以容易地确定。
作为替代方案，本发明的药物组合物还优选配制成适于吸入或鼻内给药剂型，所述剂型选自鼻腔喷雾剂、滴鼻剂、悬液和粉末。
对于吸入给药来说，可以使用适当的推进剂，如二氯二氟甲烷、三氯一氟甲烷、二氯四氟乙烷、二氧化碳或其他合适的气体，方便地将根据本发明使用的化合物通过加压包装或喷雾器以气雾方式施用。使用加压气体的情况下，可以通过提供一个阀门来确定剂量单位，以便按规定量施用。吸入器具内胶囊和明胶软片的配方包含化合物的粉末混合物和适当的粉末主剂，如乳糖或淀粉。
可用于本发明的鼻部递送体系可采取包括含水制剂、非含水制剂及其组合的多种形式。含水制剂包括例如水凝胶、水悬浮液、脂质体水分散液、含水乳液、含水微乳液及其组合。非含水制剂包括例如非水凝胶、非水悬浮液、脂质体非水分散液、非含水乳液、非含水微乳液及其组合。各种形式的鼻腔递送体系可以包括用以保持pH的缓冲剂、药学上可接受的增稠剂和润湿剂。可以选择缓冲剂的pH以最优化治疗药经鼻腔粘膜的吸收。
就非含水鼻腔配方而言，可以选择合适形式的缓冲剂，使得当将所述配方递送到哺乳动物的鼻腔中时，与例如鼻粘膜接触而在其中达到预选的pH范围。在本发明中，组合物的pH应保持为约2.0-约6.0。期望的是，组合物的pH应不会在施用时对患者的鼻粘膜产生明显刺激。
可以使用药学上可接受的增稠剂使本发明组合物的粘度保持在所期望的水平。根据本发明可用的增稠剂包括甲基纤维素、黄原胶、羧甲基纤维素、羟丙基纤维素、卡波姆、聚乙烯醇、藻酸盐、阿拉伯胶、脱乙酰壳多糖及其组合。增稠剂的浓度将取决于所选试剂和所期望的粘度。该试剂还可用在上述粉末配方中。
本发明的组合物还可包括减少或预防粘膜干燥的润湿剂，以防止其刺激。可用于本发明的合适的润湿剂包括山梨糖醇、矿物油、植物油和甘油；润肤剂；膜调理剂；甜味剂；及其组合。本组合物中润湿剂的浓度将随所选试剂而变化。
可以使用缓释系统，如包含治疗药剂的固态疏水性多聚物的半透性基质来施用化合物。已经确立了多种缓释物质，这些缓释物质是本领域的熟练技术人员公知的。
本领域的熟练技术人员能够认识到：虽然可以采用多种形式给药，但一种特定的给药途径将比其他的途径产生更直接、更有效的反应。
药物组合物可以方便地以单位剂量形式存在，可以通过药学领域所熟知的任何方法制备。这些方法都包括使治疗药剂与构成一种或多种辅助成分的载体相组合的步骤。通常，通过使治疗药剂与液体载体、微细固体载体或二者均匀、紧密结合而制备组合物。然后，根据需要使产品成型。
本发明的药物组合物可用于治疗各种心血管疾病，包括高血压和各种类型的冠心病及其并发症，尤其可用于治疗原发性高血压、心绞痛型冠心病、心肌梗塞型冠心病、心力衰竭型冠心病及其各种并发症，如脑卒中、肾脏衰竭、动脉粥样硬化等。
以下通过优选实施例具体说明本发明的各个方面和特征。本领域的技术人员应该理解，这些实施例只是用于说明目的，而不限制本发明的范围。本发明的保护范围只受权利要求书的限制。在不背离权利要求书范围的条件下。本领域的技术人员可以对本发明的各个方面进行各种修改和改进，这些修改和改进也属于本发明的保护范围。举例来说，虽然下面以分散剂为例描述本发明的配方和制备工艺，但这些配方实例不应理解为对配方配比以及剂型产生任何限制。

实施例2：复方制剂的急毒研究
受试药物：含富马酸比索洛尔3mg(相当于比索洛尔2.5mg)、坎地沙坦酯4.2mg(相当于坎地沙坦3mg)的复方分散片
高剂量：每只家兔每天给药一次，给药剂量相当药物成分5.76mg，平均2.4mg/kg，是人用量的20倍；
中剂量：每只家兔每天给药一次，给药剂量相当药物成分2.88mg，平均1.2mg/kg，是人用量的9.5倍；
低剂量：每只家兔每天给药一次，给药剂量相当药物成分1.44mg，平均1.2mg/kg，是人用量的3.8倍；
阴性对照：复方分散片的辅料空白片，其内不含药物成分，
动物：家兔，体重2.25～2.55kg，由华西医科大学临床动物试验室提供。
试验方法：20只家兔，10只雌性、10只雄性，随机分为4个小组，每组5只，将药物用适量的水溶后灌胃，每天一次观察7天，主要指标：动物的活动，进食，排便，毛皮、心率、血压、呼吸等有无异常等。
末次给药后放血处死家兔。解剖肉眼观察肝、肾、心脏的变化
试验结果：
1.全身反应：给药和观察期间，受试组和阴性对照组动物活动、进食、排便、皮毛等均无异常，体重、呼吸、心率无明显异常。
2.给药期间，受试组血压略有下降，阴性对照组无明显的变化
3.解剖观察，受试组和阴性对照组的肝、肾、心脏一切正常，不存在异常或损伤。
实施例3：复方制剂的局部用药的长期毒性研究
受试药物：同实施例2
高剂量：每只大白鼠一次给药相当药物成分1.152mg，平均4.8mg/kg，是人用量的40倍；
中剂量：每只大白鼠一次给药相当药物成分0.576mg，平均2.4mg/kg，是人用量的20倍；
低剂量：每只大白鼠一次给药相当药物成分0.288mg，平均1.2mg/kg，是人用量的10倍；
大白鼠的体重为0.236～0.264kg
三个剂量每日给药一次，连续观察30天，重复试验三次，其结果动物全身无明显的变化；脱臼处死解剖观察，肾脏、肝脏、肝功、尿常规等都无明显的变化；表明该复方制剂毒性很低，满足临床用药的要求。
实施例4：治疗高血压的临床效果研究
对120例原发性高血压患者，随机分为比索洛尔组，坎地沙坦组，比索洛尔+坎地沙坦复方胶囊组，比索洛尔+坎地沙坦复方分散片组，每组30例，进行为期8周的观察。
试验药物：
1、比索洛尔+坎地沙坦复方胶囊：富马酸比索洛尔3mg+坎地沙坦酯4.2mg，每日一次，每次一个胶囊。
2、比索洛尔+坎地沙坦复方分散片：富马酸比索洛尔3mg+坎地沙坦酯4.2mg，每日一次，每次一片。
对照药物：
3、比索洛尔片：含富马酸比索洛尔5mg，每日一次，每次一片。
4.坎地沙坦片：含坎地沙坦酯8mg，每日一次，每次一片。
血压达标指治疗后血压降低至≤135/85mmHg；降压有效指治疗后收缩压比治疗前降低≥20mmHg或/及舒张压低≥10mmHg；或收缩压≥10mmHg同时舒张压降低≥5mmHg。
各组抗高血压治疗的比较，结果如下表1所示。

从上述的临床试验结果可以看出，两个复方制剂组的治疗效果明显优于两个单剂量组，有显著的统计学差异(p＜0.05)。由此说明：复方制剂效果明显优于单剂量配方。试验组和对照组的临床疗效、总疗效用Ridit分析进行比较。用SAS6.12软件进行统计。
实施例5：治疗高血压的临床副作用研究
在治疗实验中，对于每个组的副作用，进行了统计，结果如下表2所示。

表2数据说明，将比索洛尔与坎地沙坦组合制成的复方制剂在治疗原发性高血压方面比单剂量用药效果优越，在减少副作用方面具有协同作用。
实施例6：治疗冠心病的临床效果研究
为了研究组合物的临床效果，按照配方6制备了分散片，每片含富马酸比索洛尔1.2mg(相当于比索洛尔1.0mg)，坎地沙坦酯5.6mg(相当于坎地沙坦4mg)。
全部病例均为内科患者，共90例，其中男45例，女45例，年龄40-60岁(平均51岁)，诊断符合1979年WHO命名的缺血性心脏病诊断标准，其中心绞痛患者30例，心力衰竭冠心病患者30例，伴有心肌梗塞冠心病30例。
统计学方法：采用t检验
治疗心绞痛型冠心病的试验
将30例病人随机分为两组，每组各15例，两组在年龄、心疼次数、心电图上无统计学差异，对照组为坎地沙坦组，给药单剂量16mg/天，一日一次；第二组试验组坎地沙坦与比索洛尔，给组合物制成的制剂，相当于坎地沙坦4mg/天，比索洛尔1.0mg/天，每天一次。服药方法均为口服给药。观察2周，根据下列标准判定疗效：
显效：心绞痛消失，心电图恢复正常或ST段回恢上升0.05mv以上，T波由倒置、低平转为直立。
有效：心绞痛减轻或发作次数疼痛次数减少，心电图下移ST段治疗后回升0.05mv，但未达到正常水平。
无效：心绞痛未缓解，心电图无改变。
疗效和副作用比较分别如下表3和表4所示。

从表3和表4可以看出，实验组的效果明显好于对照组，两组之间有显著的统
计学差异；在副作用方面，试验组明显低于对照组，两组之间也有显著的统计学差异。
治疗心力衰竭型冠心病的试验
30例病人随机分为两组，每组15例，两组在年龄、心疼次数、心电图上无统计学差异，对照组为比索洛尔组，给药单剂量5mg/天，一日一次；试验组为坎地沙坦与比索洛尔组，给根据上述配方6制成的制剂，相当于坎地沙坦4mg/天，比索洛尔1.0mg/天，每天一次。服药方法均为口服给药。观察如下指标：1、超声心动图无创心功能检查；测定治疗前后左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)、左室射血分数(EF)，作为观察患者心功能的指标。2、Holte检查，观察平均心率(HR)及24小时心律失常数(ARR)。给药12周后，比较治疗前后的变化，以及服药期间的副作用，结果分别见表5和表6。

从表5和表6的数据可以看出，实验组的效果明显好于对照组，两组之间有显著的统计学差异，且在副作用方面，试验组明显低于对照组，两组之间也有显著的统计学差异。
治疗心肌梗塞型冠心病的试验
30例病人随机分为两组，每组各15例，两组在年龄、心疼次数、心电图上无统计学差异，对照组为坎地沙坦，给药单剂量16mg/天，一日一次；试验组为坎地沙坦与比索洛尔，给根据上述配方6制成的片剂，相当于坎地沙坦4mg/天，比索洛尔1.0mg/天，每天一次。服药方法均为口服给药。观察2周，根据下列标准判定疗效：
有效：疼痛消失，心电图恢复正常或ST段回恢上升0.05mv以上，T波由倒置、低平转为直立，心肌梗塞症状消失。
显效：疼痛减轻或发作次数疼痛次数减少，心电图下移ST段治疗后回升0.05mv，但未达到正常水平，心肌梗塞症状未消失。
无效：疼痛未缓解，心电图无改变，心肌梗塞症状无变化。
疗效和副作用比较分别如下表7和表8所示。

从表7和表8可以看出，实验组的效果明显好于对照组，两组之间有显著的统计学差异，且在副作用方面，试验组明显低于对照组，两组之间也有显著的统计学差异。
综上所述，本发明中包含比索洛尔和坎地沙坦的药物组合物在治疗心血管疾病，尤其是高血压和冠心病方面具有突出的效果，表现为两种药物的治疗有效剂量更低，疗效更高，治疗谱更宽，并减少与两种药物单独使用时相关的副作用。
Claims (7)
1.用于治疗心血管疾病的药物组合物，其含有比索洛尔和坎地沙坦，二者摩尔比例为1∶1～1∶10。
2.权利要求1所述的药物组合物，其中比索洛尔和坎地沙坦的摩尔比例为1∶1～1∶2。
3.权利要求1或2所述的药物组合物，其中所述的心血管疾病为高血压。
4.权利要求1或2所述的药物组合物，其中所述的心血管疾病为冠心病。
5.权利要求1的药物组合物，其制备成口服剂型、吸入或鼻内给药剂型或注射剂。
6.权利要求5所述的药物组合物，其中所述的口服剂型选自片剂、胶囊、丸剂、粉末、颗粒、溶液、混悬剂、糖浆、酏剂和咀嚼剂。
7.权利要求6所述的药物组合物，其中所述的片剂为分散片。
