Diffusion-weighted magnetic resonance imaging (MRI) has shown increased sensitivity in detecting brain white matter disease compared to traditional T2-weighted MRI. Diffusion-weighted imaging (DWI) can quantitatively assess the microstructural integrity of white matter using the average apparent diffusion coefficient (ADC(av)), a measure of the extent to which water molecules move freely within tissue. On the basis of numerous studies suggesting white matter disease in bipolar patients, particularly patients with more severe illness, this study aimed to test the utility of DWI in assessing the white matter integrity of bipolar patients with severe illness. The existing MRI scans of eight bipolar patients and eight age-matched controls with neurological illness were examined retrospectively. ADC(av) values for pixels within white matter regions of interest (ROIs) were calculated and used to plot ADC(av) frequency histograms for each ROI. Mean ADC(av) values for the two groups were then compared by ANCOVA. The bipolar mean ADC(av) (0.855 +/- 0.051 x 10(-3) mm2/s) for combined white matter ROIs significantly exceeded that of controls (0.799 +/- 0.046 x 10(-3) mm2/s), while covarying for age (F = 4.47, df = 3, p = 0.025). This is the first report of an elevated ADC(av) in the white matter of a group of patients with bipolar disorder. In this group of patients with severe illness, increased white matter ADC(av) suggests microstructural changes consistent with decreased white matter integrity. DWI may be an additional, useful tool to assess white matter abnormalities in bipolar disorder.

The contribution of vascular risk factors to Alzheimer-vascular spectrum dementias is increasingly being recognized. We provide an overview of recent literature on this subject. Overweight and obesity as well as underweight during midlife predict cognitive decline and dementia later in life. Hypertension during midlife is also associated with dementia later in life and the association is stronger for untreated hypertension. Calcium channel blockers, angiotensin converting enzyme inhibitors, and angiotensin-1 receptor-blockers may be particularly beneficial in diminishing the risk of dementia associated with hypertension. Studies have fairly consistently shown that type 2 diabetes is a risk factor for dementia. Episodes of hypoglycemia add to this risk. Regular physical exercise during any point in the lifespan protects against cognitive decline and dementia. Most benefit is realized with physical exercise during early and midlife. Dyslipidemia also increases the risk of dementia but the findings are less consistent. Findings on the possible benefit of lipid-lowering agents (statins) are conflicting. Earlier studies identified smoking as protective of dementia but recent better designed studies have consistently shown that smoking increases the risk of dementia. The association of vascular risk factors with dementia is more robust for vascular dementia than Alzheimer's disease. Heterogeneity of studies and lack of trials specifically designed to assess cognition as an endpoint make firm conclusions difficult. But considering the expected global burden of dementia and projected attributable risk of vascular risk factors to it, there is sufficient evidence to promote vascular risk factor reduction strategies as dementia prevention interventions.

The Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-V) Workgroup is considering significant changes for the group of disorders currently classified as delirium, dementia, amnestic disorders, and other cognitive disorders.1 Under the proposed changes, the diagnostic category of delirium will be retained whereas the remainder will be replaced by the categories of mild and major neurocognitive disorders. Mild neurocognitive disorders would be characterized by mild cognitive decline (1–2 standard deviations below appropriate means on formal testing) not sufficient to interfere with independence with or without compensatory strategies. In contrast, major neurocognitive disorder would denote significant cognitive decline (>2 standard deviations below appropriate norms on formal testing) that is accompanied by functional impairment. We believe this “mild” versus “major” distinction would pose serious practical and clinical challenges. The progression of neurocognitive disorders is on a continuum of minimal to marked symptoms.2 The suggested distinction will raise expectation both among clinicians and patients to get neuropsychological testing resulting in overburdening already limited resources. Brief cognitive assessment tools such as the Mini-Mental State Examination or Clock Drawing Test which are commonly used in clinical practice lack sensitivity to make the suggested distinction.3 More comprehensive testing would improve sensitivity but would not be routinely possible. Assessment for the absence or presence of functional impairment would be even harder due to lack of objective, standardized tools, and what may or may not be considered as a compensatory strategy. Further complicating such an assessment would be possible psychiatric comorbidity, for example, depression, which could adversely affect both cognition and functioning. Thus, even if comprehensive neuropsychological testing is available, the determination of whether a patient has mild versus major neurocognitive disorder would largely remain a subjective, clinical decision with significant variation across practices and patients. Inclusion of the mild cognitive disorder category is an attempt to identify persons with predementia syndromes. However, only a fraction of patients with mild cognitive impairment develop dementia.4 Methods that attempt to identify individuals at higher risk of developing dementia have not proven reliable.5 Most individuals diagnosed with mild neurocognitive disorder would thus be worried about developing a serious condition that they may never develop. The DSM-V Workgroup6 has acknowledged the limitations of neuropsychological testing to differentiate between mild and major forms of neurocognitive disorders but did not comment upon the potential clinical and public health implications we have noted earlier. Although the changes suggested by the DSM-V Workgroup are in the right direction, a simpler approach is needed to avoid clinical and public confusion, to prevent undesired “labeling,” and to limit unnecessary burden and cost to the health system. As argued by others,7 we favor retention of the term “dementia” in DSM-V as an alternative to “major neurocognitive disorders.” Neurocognitive deficits that do not meet this threshold could be followed as “neurocognitive disorder NOS” until a clearer picture evolves. The research implication of the distinction between “mild” and “major” neurocognitive disorders could be addressed through having a separate set of “research diagnostic criteria.”

Alhamdulillah, I have now started working on adding notes (grammar-related, not interpretations) to the PDFs of the color-coded, word by word grammar and translation of the Quran I have shared previously. The previous PDFs were compiled using pages from the Corpus Quran website. In this new compilation, I will be adding root words and their respective derivatives. I will be adding a few other grammatical notes here and there as well. Once you have a look at the PDF, you will understand what I mean.

This effort is a part of my broader effort to facilitate learning of the Quran. At this stage, I am working on three sets of documents: 1) the one available at this link, 2) a concise handbook of basic Quranic Arabic, and 3) a basic Quranic Arabic dictionary. These three sets of documents will provide the material needed to help read/understand the Quran. I am using various online sources and books for this effort. For this set of documents, I would like to cite the Quranic Dictionary by Shaikh Lutf ur Rahman and Mutalia Quran by Jahanzeb Nadeem and Shaikh Lutf ur Rahman.

Alhamdulillah, reading and trying to understand the Quran has been an area of my interest all my life. Over the years, my interest progressed from reading the translations of the Quran and browsing through a few Tafaseer selectively to trying to understand the Quran word-by-word and developing an intellectual sense of its main themes. During my pursuit that is ongoing I noted that the dictionaries of the Quranic root words either to brief or too elaborate. So, I decided to put together one myself one that was neither too simple nor too elaborate. It is a work in progress. So far, I have completed the root words of the first Arabic alphabet, Alif. InshaAllah, I plan to upload the PDFs of the root words by alphabet to the Archive.org website as I complete them. Once all the alphabets are done, I will create a complete dictionary of the root words and upload it as well. Further details about the dictionary provided in its introduction section.

This book has been written with a concern for the problem of irrational trends and practices in therapeutics. It focuses on the applied aspects of therapeutics, an understanding of which can facilitate physicians to approach and intervene in a clinical situation in a rational, scientific and logical manner.

'The book has been divided into three sections. The first section provides the fundamentals of rational therapeutics. In it are discussed how a patient should be approached and what factors should be taken care of to make a therapeutic decision as rational as possible. Effective communication i's an important factor for an effective therapeutic
relationship; the section, therefore, starts with a discussion on the p{ocess of communication. The next few chapters discuss what factors are to be taken care of when planning a therapeutic intervention, how to remain up-to-date- about therapeutics, and how to write a prescription to make it maximally useful. Post-prescription-writing factors like observing the patient for desired and undesired effects of the treatment
and monitoring him for compliance are overlooked by physicians in many instances and their significance in rational therapeutics is discussed in the last chapter of this section.

Section two of the book aims at providing physicians an insight into the factors that influence their therapeutic judgment and decision-making. Because -medical school training provides the knowledge and skills on the basis of which physicians practice therapeutics, the first chapter of the section emphasizes the significance of medical school training. In the absence of any formal ongoing training of physicians, and in view of the difficult access to unbiased and objective information on therapeutics, the pharmaceutical manufacturers bear a strong influence on physicians. The effects of this influence are discounted in the second chapter of the section. The last chapter of this section describes various patient and doctor factors that influence phvsicians' clinical approach. Governmental and regulatory factors also have a significant influence on the way therapeutics is practiced in a country, but because of
the complexity of these factors and my limited awareness about them, they have not been discussed in the current edition of this book.

The last section of the book highlights the commonly observed irrational trends in the use of some common drugs. In addition to recognizing the irrational tend.s, guidelines on their rational use have also been provided.  Drug categories that are discussed include antibiotics, psychotropics, nonsteroidal anti-inflammatory drugs, antiepileptics, and vitamins. Drugs used for the relief of gastrointestinal symptoms are also among the
commonly misused group of drugs but, due to reasons, they could not be included in this edition of the book. The last chapter of this section focuses on the group of drugs that have been considered as "undesired" by experts and generally their use is not recommended.
