    Introduction: Cisplatin is a well-known chemotherapeutic agent used in many cancer treatments. Several antioxidant agents are used for diminishing the toxic side effects of the cisplatin therapy. Alpha-lipoic acid (α-LA) and oxytocin (OT) are antioxidant agents that can be used in toxicity. Our aim is to investigate the effect of these antioxidants in cisplatin-induced ototoxicity in tissue level. Materials and Methods: Forty Wistar albino rats divided into five groups as control, cisplatin, cisplatin + intraperitoneal (IP) OT, cisplatin + intratympanic (IT) OT, and cisplatin + IT α-LA. The drug administration is applied for 4 days, and at the end of the procedure, the cochleas are harvested. After tissue preparation, GSTP1 levels are investigated and the intensity of the reaction is scored as negative (−), weak (1+), moderate (2+), or strong (3+). Results: Group 4 has a moderate staining which can be interpreted as high immunoreaction. When we compare with Group 1, this staining difference is statistically significant (P < 0.02). When we observe the Group 3, we cannot detect any difference with Group 1 in immunoreactivity. Conclusion: α-LA and OT are antioxidants effective against cisplatin ototoxicity. The expression of GSTP1 isozyme is increased in antioxidant-treated groups. Increased levels of these isozymes proved the increased healing response in tissue levels. Antioxidant agents can be used for adverse effects during cisplatin treatment. IT route is effective as IP systemic route.

    Bu çalışmada, parotis kaynaklı tümörlerin anatomik ve histopatolojik dağılımının yanı sıra tedavi yaklaşımın-dan elde ettiğimiz sonuçlar sunuldu. Dr. Lütfi Kırdar Kartal Eğitim ve Araştırma Hastanesi 2. KBB kliniğinde 2001 ile 2008 tarihleri arasında parotis cerrahisi uygulanan 52 hastanın kayıtları geriye dönük olarak gözden geçirildi. Yaş, cinsiyet, klinik belirti ve bulgular, histopatolojik tanı, komplikasyonlar, tümör evresi ve prognoz gibi bilgiler değerlendirildi.     Patients who underwent surgery for thyroid masses were evaluated with regard to clinical symptoms, diagnostic and treatment methods, and the results of treatment. The study included 92 patients (72 women, 20 men; mean age 45 years; range 16 to 71 years). Clinical diagnoses were based on findings of ultrasonography, thyroid function tests, thyroid scintigraphy, and fine-needle aspiration biopsy. Operations performed were near-total thyroidectomy (n=40), hemithyroidectomy (n=25), bilateral subtotal thyroidectomy (n=16), and total thyroidectomy (n=11). Patients with carcinoma associated with neck lymph node metastasis also underwent lateral, anterolateral, modified radical, or radical neck dissections.     The disease extention in nasal polyps was compared by using the mitotic activity rates and the computed tomography scores. This study was conducted on 19 nasal polyposis patients (8 males, 11 females; mean age 40.0+/-13.7 years; range 20 to 63 years). The preoperative computed tomography records of the patients were evaluated according to the Lund-Mackay grading system. The polyp tissues of the same patients were stained with the Ki-67 antigen for immunohistochemical evaluation. The correlation between the radiologic results and the Ki-67 values was compared by means of the Spearman's correlation test. The mean computed tomography score was observed as 14.3+/-4.7 (range 7-24). The mean Ki-67 score resulting from the immunohistochemical staining was calculated as 24.3+/-18.5 (range 3.3-73.5%).     A thirty-five-year-old woman had an asymptomatic, slow growing mass that pushed the right tonsilla palatina and was pulsatile in the oropharyngeal region. Magnetic resonance imaging showed a vascular mass at the carotid bifurcation, causing displacement of the external and internal carotid arteries. The diagnosis of a carotid body tumor was confirmed by magnetic resonance angiography. Surgery included a subadventitial dissection at the carotid bifurcation, preserving the hypoglossal and vagal nerves. The patient developed loss of function of the tenth cranial nerve postoperatively. After a year follow-up period, there was no change in the function of the tenth cranial nerve.

With the “Fishtail” technique, it was observed that the intercommissural distance and interincisaldistance could be increased by more than 10 mm and 5 mm, respectively, while preserving the specific structure of the mouth commissure [2]. According to conventional surgical techniques, the “fishtail” technique is aesthetically and functionally acceptable, cheap and practical.

    Head-and-neck region tumors are mostly seen on the larynx. The risk factors of this region may increase the secondary tumors. Nasal squamous cell carcinoma (SCC) can be detected after larynx SCCs, a second primary tumor. Second primary tumors are difficult to manage. Total rhinectomy is the choice of treatment modality, but rehabilitation is necessary. We present a secondary nasal SCC treated with total rhinectomy. A 67-year-old patient, who was treated for larynx SCC by total laryngectomy 3 years ago, was admitted to the outpatient clinic with a complaint of nasal swelling and pain. The patient was diagnosed as nasal SCC. Total rhinectomy was performed, and silicon nasal prosthesis was applied after 3 months of surgery. Total rhinectomy can be rehabilitated via silicon nasal prosthesis. This was cost-effective, safe, less time-consuming, and functional. Using prosthesis has given us better chance to detect recurrences. We encourage surgeons to use prosthesis for rehabilitation.

They can be seen in any part of the body but the most common area in the neck region is posterior triangle. They usually present as asymptomatic masses. There are various ways of treatment, but the most common treatment modality is surgical resection.     A 66-year-old man presented with progressive unilateral nasal obstruction. He had noted worsening left nasal blockage for several months, which was abruptly relieved with the expression of a polypoid mass during a sneeze. He froze the specimen in plastic wrap and made an otolaryngology appointment. He was evaluated 1 week later. Physical examination revealed a healthy appearing man with no obvious nasal obstruction. Results of the ENT examination, including nasal endoscopy, were normal. The polypoid tissue brought by the patient was sent for pathologic evaluation, which revealed adenoid cystic carcinoma. A repeat nasal endoscopy revealed a left posterior septal lesion just anterior to the choana and opposite the middle turbinate (Fig 1). Magnetic resonance imaging did not show any cranial nerve or brain involvement. The patient was taken to the operating room. An endoscopic biopsy of the lesion visualized in the office confirmed a diagnosis of adenoid cystic carcinoma, and the lesion and posterior septum were resected. The margins were controlled by frozen section. The patient was discharged on the same day and at 9 months has no evidence of disease.

Süperior orbital fis-sür veya kavernöz sinüste yerleşen granülamatöz lezyon sebebiyle bu sendrom kraniyal sinir paralizilerine yol açar.     Objective: To analyze the exposure of the jugular foramen afforded by the fallopian bridge technique. Method: The jugular foramen exposure was obtained using the jugular foramen approach combined with the fallopian bridge technique. We applied this technique using 10 temporal bone specimens at a tertiary referral center. The exposure was assessed by means of depth of the dissection field and two separate dissection spaces that were created anteriorly and posteriorly to the facial nerve. Anteroposterior and superoinferior dimensions of the anterior dissection space were 4.4 1.03 mm and 6.3 2.49 mm, respectively. The superoinferior and anteropos-terior dimensions of the posterior dissection space were 11.7 2.08 mm and 8.0 2.95 mm, respectively. The depth of the first dissection field was 8.2 2.11 mm.     OBJECTIVES We investigated the relationship between the size of concha bullosa (CB) detected by coronal paranasal sinus computed tomography (CT) and the frequency of sinonasal symptoms. PATIENTS AND METHODS The study included 60 patients (39 females, 21 males; mean age 38.3 years; range 17 to 67 years) who had CB on CT scans and underwent endoscopic sinus surgery for symptoms of chronic sinusitis, nasal obstruction and/or headache. The patients were evaluated in three groups according to the ratio of the cross-section area of CB to that of the nasal cavity (NC); hence, group 1, 0-14%; group 2, 15-29%; and group 3, = or >30%. The relationship of CB/NC with clinical symptoms and nasal pathologies were examined. RESULTS The presenting complaints were sinusitis in 28 (46.7%), nasal obstruction in 56 (93.3%), and headache in 36 (60%) patients. As patients with foreign bodies in the paranasal sinus are not commonly seen in otolaryngological practice, especially glass particles in the frontal sinus, and papers dealing with this issue are rare in the English language literature, we present our case and a review of the literature.

    This retrospective, population-based study reviewed skin prick test (SPT) results against various allergens of 1,552 patients with allergic rhinitis (AR) in the district of Kartal, Istanbul, Turkey. The skin prick tests yielded a positive result in 946 patients (60.9%). Seven hundred and forty-give (48%) patients had perennial AR, 558 (36%) perennial AR with seasonal exacerbations and 249 (16%) had seasonal AR. The prevalence of AR was highest in the age of 21-40 years with 48.7%. The allergen group with the highest SPT positivity was pollens at 44.3%, followed by molds at 38.4%, mites at 35.1%, and epithelia-insect at 30.8%. The strongest single allergen was Altemaria altemata with 33.3%, followed by Dermatophagoides farinae with 29.3%, Dermatophagoides pteronyssinus with 25.3% and a mix of four cereals (barley, maize, oat, wheat) with 25.2%.     OBJECTIVES The aim of this study was to compare the usefulness of nasal smear eosinophilia with skin prick test for the diagnosis of allergic rhinitis and to determine the degree of correlation between the tests. PATIENTS AND METHODS Forty-one patients (16 males, 25 females; mean age 35.17; range 14 to 56 years) with a clinical history suggestive of nasal allergy and fifteen controls (7 males, 8 females; mean age 27.6; range 16 to 40 years) without any history of allergy were enrolled in this study. Skin sensitivity tests were applied in both groups. Nasal smear was examined by light microscopy. RESULTS A positive skin test reaction was demonstrated in 73.2% of the study group and in 20% of the controls, whereas 73.2% of the patients and 13.3% of the controls demonstrated significant nasal smear eosinophilia. Surgical treatment of paranasal sinus mucoceles includes endoscopic approach or external approach. The authors report a patient of frontal mucocele who presented with a history of progressive unilateral protrusion. Computed tomography scan revealed a large mucocele of the frontal sinus with orbital extension on the same side. He was successfully treated with endoscopic marsupialization without any serious complications.

    Non Hodgkin lymphomas correspond to 25 % of all head and neck cancers. These rare tumors only include less than 5 % of malign tumors in parotid region. Differential diagnosis of these tumors cover many malign and benign tumors of the parotid gland. Definite diagnosis depends on sufficient tissue material of parotidectomy specimen. Treatment modality is surgical removal of the lesion with or without additional radiation and chemotherapy depending on the stage of the tumor. Prognosis is better than other forms of the B-cell lymphoma. We present a 54 year old woman who suffered from progressively and slowly growing mass on parotid region, without any inflammatory disease or chronic infection, diagnosed with mucosa associated lymphoid tissue lymphoma of the parotid gland.     Cisplatin is a potent chemotherapeutic drug with serious side effects such as ototoxicity which is characterized by irreversible, bilateral, progressive sensorineural hearing loss. Oxytocin, which is a well-known hormone secreting during pregnancy, has antioxidant and antiinflammatory effect. Our study aims to test and compare the effect of intratympanic (IT) and intraperitoneal (IP) oxytocin on cisplatin ototoxicity with DPOAE. A total of 24 Wistar albino rats were randomly divided into four groups: Group 1 received 0.1-0.3 ml IT saline + IP saline solutions for 4 days (n = 6), Group 2 received cumulative dose of 20 mg/kg IP cisplatin divided into two equal doses in first and second days of experiment + 0.1-0.3 ml IT saline for 4 days, Group 3 received same dose of cisplatin as Group 2 + 0.1-0.3 ml IT oxytocin for 4 days, and Group 4 received same dose of cisplatin as Groups 2 and 3 + IP oxytocin with dose of 1 mg/kg. DPOAE was performed prior to procedure and at the end of the experiment on day 5. Group 2 showed severe ototoxic effect of cisplatin according to DPOAE result (p < 0.05). When compared with Group 2, DPOAE amplitude reductions were smaller in Group 3 (3.2, 3.8, 4.5, 6.3 and 7.6 kHz) (p < 0.05) and Group 4 which is statistically significant in 5.4, 6.3 and 7.6 kHz (p < 0.05). When Group 3 and Group 4 were compared, reductions were smaller in 2.7 and 3.2 kHz in Group 3 (p < 0.05). In this study, we showed the protective effect of IT and IP oxytocin on cisplatin ototoxicity. We suggest oxytocin in cisplatin ototoxicity, especially via IT route even with high-dose cisplatin.

    Dermoid and epidermoid cysts are rare cysts of the head and neck region, which should be considered in differential diagnosis of sublingual mass, constituting 0.01 % of all oral cavity cysts. There are suspected theories regarding the basis of the pathology. 30-year-old male referred to our clinic with a chief complaint of sublingual mass which had been present for many years but the lesion enlarged over the past 3 years. MRI examination revealed a giant cystic mass which was 3.6 × 3.9 mm in diameter and medially located at sublingual region. Intraoral approach was preferred for surgery and no recurrence or complaints were detected during follow-up period. When a cystic mass is detected on the floor of oral cavity, we must consider dermoid cysts for differential diagnosis. Surgery is the only treatment.     Aim: To compare the protective effect of dexamethasone and lactate against cisplatin-induced ototoxicity. Materials and methods: Thirty Wistar rats were randomly divided into 4 groups. After the rats were sedated with intraperitoneal (IP) ketamine hydrochloride (50 mg/kg) and xylazine (7.5 mg/kg), baseline ABRs (auditory brainstem evoked responses) were measured in response to clicks and tone pips of 4, 8, 12, and 16 kHz. After auditory thresholds were determined, the animals received drug administration as follows: Group 1 (n: 6) received intratympanic (IT) saline (0.9% NaCl) solution, Group 2 (n: 8) IP cisplatin (20 mg/kg) alone, Group 3 (n: 8) IT dexamethasone (0.1-0.3 mL), and Group 4 (n: 8) IT lactated Ringer's (LR) solution (0.1-0.3 mL) followed after 30 min by 20 mg/kg cisplatin.     To evaluate the protective effect of betahistine on noise-induced hearing loss (NIHL) using scanning electron microscopy (SEM) and distortion product otoacoustic emission (DPOAE). A total of 8 adult albino guinea pigs were used in this study (study group: n=4 and control group: n=4). DPOAE measurements of both groups were performed before the procedure. Two hours before noise exposure, 0.9% NaCl solution was injected perorally to the control group and betahistine was administered through a peroral catheter to the study group. Both groups were then exposed to 105-dB sound pressure level (SPL) 4-kHz frequency-based narrow-band noise for 2 h per day for 5 days. DPOAE measurements were performed again on the 6th day and cochleae were dissected and examined by SEM on the 7(th) day. Regarding the results of DPOAE, NIHL was observed in both groups on the 6th day (p<0.05). Loss, flattening, and fusion, which are findings of permanent hearing loss, were determined in the stereocilia of the inner and outer hair cells by SEM. These findings were evaluated as signs of permanent increase in the threshold. When DPOAE measurements and SEM results were evaluated in the study group, no significant difference was observed in NIHL compared with the control group (p>0.05). In our study, it was observed that simultaneous administration of betahistine during noise had no protective effect on permanent increase in the threshold. However, further studies on noise and long-term use of betahistine can be performed.

    This retrospective, population-based study reviewed skin prick test (SPT) results against various allergens of 1,552 patients with allergic rhinitis (AR) in the district of Kartal, Istanbul, Turkey. The skin prick tests yielded a positive result in 946 patients (60.9%). Seven hundred and forty-give (48%) patients had perennial AR, 558 (36%) perennial AR with seasonal exacerbations and 249 (16%) had seasonal AR. The prevalence of AR was highest in the age of 21-40 years with 48.7%. The allergen group with the highest SPT positivity was pollens at 44.3%, followed by molds at 38.4%, mites at 35.1%, and epithelia-insect at 30.8%. The strongest single allergen was Altemaria altemata with 33.3%, followed by Dermatophagoides farinae with 29.3%, Dermatophagoides pteronyssinus with 25.3% and a mix of four cereals (barley, maize, oat, wheat) with 25.2%. In this case study one patient was diagnosed and treated successfully with complete surgical resection of a laryngeal granular cell tumor that was originated from the left arytenoid region that very rare location. There is no evidence of recurrence 2 years after surgery. Granular cell tumors should be considered in the differential diagnosis of laryngeal masses, particularly in the posterior glottis.

    The disease extention in nasal polyps was compared by using the mitotic activity rates and the computed tomography scores. This study was conducted on 19 nasal polyposis patients (8 males, 11 females; mean age 40.0+/-13.7 years; range 20 to 63 years). The preoperative computed tomography records of the patients were evaluated according to the Lund-Mackay grading system. The polyp tissues of the same patients were stained with the Ki-67 antigen for immunohistochemical evaluation. The correlation between the radiologic results and the Ki-67 values was compared by means of the Spearman's correlation test. The mean computed tomography score was observed as 14.3+/-4.7 (range 7-24). The mean Ki-67 score resulting from the immunohistochemical staining was calculated as 24.3+/-18.5 (range 3.3-73.5%). The study included 92 patients (72 women, 20 men; mean age 45 years; range 16 to 71 years). Clinical diagnoses were based on findings of ultrasonography, thyroid function tests, thyroid scintigraphy, and fine-needle aspiration biopsy. Operations performed were near-total thyroidectomy (n=40), hemithyroidectomy (n=25), bilateral subtotal thyroidectomy (n=16), and total thyroidectomy (n=11).
